1.Study of the effect of self-perceived hearing status on depression in middle-aged and older people in the community

Objective:Hearing loss can seriously affect mental health status, and this study aims to investigate the influence of hearing health status on depressive symptoms among middle-aged and older individuals in the community.Methods:From June to December 2023, a stratified random sampling method was employed to select 1 238 community-dwelling middle-aged and elderly people aged 45 years and above from four cities (Hangzhou, Shanghai, Baoding, and Zhengzhou) as research subjects. A questionnaire survey was conducted to collect the subjects′ basic information, hearing health status [assessed by the Hearing Handicap Inventory for Adults-Screening Version (HHIA-S)], and depressive symptoms [assessed by the Geriatric Depression Scale-15 (GDS-15)]. T-tests, rank-sum tests and chi-square tests were used for univariate analysis, while, multiple linear regression and binary Logistic regression were applied to analyze the relationship between hearing health status and depressive symptoms.Results:A total of 1 183 community-dwelling middle-aged and elderly people aged 45 years and above were included in the final analysis (464 males and 719 females, aged from 45 to 96 years). The detection rate of hearing loss was 35.3%(418/1 183), while, the detection rate of depressive symptoms was 9.89%(117/1 183). Age, level of interaction with children, self-rated health, perceived loneliness, and hearing health significantly influenced depressive symptoms among older adults residing in the community ( P<0.05). Individuals with moderate to severe hearing loss ( β=2.04, 95% CI: 1.47, 2.62) exhibited higher GDS-15 scores compared to those without hearing impairment. Furthermore, after correcting for sex, age, marital status, monthly per capita family income, education, residence, smoking status, alcohol use, use of psychotropic medication (anxiolytic or depressant), number of illness, self-health assessment, and autonomy, middle-aged and older adults with mild to moderate hearing loss ( OR=2.89, 95% CI: 1.76, 4.88) and severe hearing loss ( OR=5.79, 95% CI: 3.05, 11.01) demonstrated an increased likelihood of experiencing depression. Conclusions:The degree of hearing loss in community-dwelling middle-aged and elderly individuals is closely associated with the risk of depressive symptoms. Therefore, it is imperative to enhance hearing health screening and to provide mental health support to individuals with hearing loss, in order to mitigate the onset and progression of depressive symptoms.

Objective To evaluate the value of modified percutaneous endoscopic gastrostomy(PEG)for patients with amyotrophic lateral sclerosis(ALS)and dysphagia.Methods From January 2018 to December 2023,47 patients with ALS were treated with modified PEG in our hospital.The surgery was performed under local anesthesia.The patients were placed in a semi-seated position,and the surgeon performed surgery with an ultra-fine endoscope through oral entrance to the stomach.Results The modified PEG were successfully completed in all the 47 cases.The operation time was5-20 min[mean,(10.3±1.7)min].The postoperative hospitalization lasted for2-8 d(mean,2.8 d).Postoperative complications occurred in6 cases(12.8%),including aspiration pneumonia in4 cases(8.5%)and incision infection in2 cases(4.3%).The body mass index was17.06±0.89 before PEG and 18.15±0.81 at 3 months after PEG,with significant difference(t=-10.373,P=0.000).Conclusion For ALS patients with dysphagia,modified PEG is feasible,which significantly improves nutritional status of patients.
3.Safety of percutaneous endoscopic gastrostomy in older patients with amyotrophic lateral sclerosis

Objective:A retrospective study was conducted to investigate the safety of Percutaneous Endoscopic Gastrostomy(PEG)in elderly patients with Amyotrophic Lateral Sclerosis(ALS)and to identify the factors influencing major complications.The aim is to establish a more robust foundation for optimizing both the procedure and its timing, thereby enhancing the evidence base for refining the surgical approach.Methods:We retrospectively collected clinical data from elderly ALS patients(≥60 years old)who underwent PEG at Peking University Third Hospital between January 2006 and January 2024.This dataset includes information on general health conditions, the progression of ALS, comorbidities, surgical details, and related complications.Additionally, we analyzed the postoperative complications experienced by the included patients, focusing specifically on the risk factors associated with aspiration pneumonia.Results:A total of 140 patients were included in the study, with a mean age of 68.2±5.9 years.Among these, 69 were male(49.3%)with a mean age of 68.4±6.5 years, and 71 were female(50.7%)with a mean age of 67.7±6.0 years.Successful outcomes were observed in 139 cases(99.3%).One case was not completed due to respiratory arrest caused by intraoperative aspiration.Postoperative complications occurred in 36 cases(25.7%), which included 9 cases of wound infection(6.4%), 1 case of abdominal infection(0.7%), 21 cases of aspiration pneumonia(15.0%), 1 case of local fistula leakage(0.7%), 2 cases of transient fever(1.4%), and 2 cases of death during hospitalization(1.4%).Both univariate and multivariate analyses indicated that an onset duration of ≤1 year( P=0.020)and a half-sitting position( P=0.022)significantly influenced the occurrence of aspiration pneumonia, acting as protective factors( β<0, OR<1). Conclusions:PEG is a safe method for providing enteral nutrition to elderly patients with ALS.While most complications associated with the procedure are mild and can be managed, Aspiration pneumonia remains a common and serious complication.However, early surgical intervention and the use of a semi-seated position during the procedure can help reduce the incidence of aspiration pneumonia.

7.Pretreatment of torasemide and bacterial endotoxin test methods research

Objective:To establish a bacterial endotoxin test method for torasemide.Methods:Torasemide was dissolved in dimethyl sulfoxide(DMSO),and then diluted with bacterial endotoxin test water(BET water).According to the bacterial endotoxin test method of general rule 1143 in the fourth part of the 2020 edition of the"Pharmacopoeia of the People's Republic of China,"the endotoxin in the sample was detected by gel method,dy-namic turbidity method and dynamic chromogenic method,respectively.The interference of torasemide solution and the recovery ratio of endotoxin were investigated.Results:After the sample was dissolved in DMSO and diluted with BET water,there was no precipitation.The interference test of gel method,dynamic chromogenic method and dynamic turbidity method met the requirements and could be excluded.Conclusion:After dissolution of torasemide by DMSO,it is feasible to use gel method,dynamic turbidimetric method and dynamic chromogenic method for bacterial endotoxin test.
8.Analysis of mini-CEX Scores and influencing factors after teaching"fundamentals of nursing"in the elderly service management program

Objective To investigate the current status of mini-CEX scores among students in the Elderly Service Man-agement program after completing the"Fundamentals of Nursing"course and analyze the influencing factors.Methods A total of 99 students from the Elderly Service Management program at the Wuming campus of Guangxi Medical University were selected as the study subjects.Assessment tools included a general information questionnaire,the Chinese version of the Mini Clinical E-valuation Exercise(mini-CEX),the Self-Rating Scale of Self-Directed Learning(SRSSDL),and a self-learning ability assess-ment scale.Stepwise linear regression analysis was employed to explore the factors affecting mini-CEX scores.Results The total mini-CEX score for the 99 students was 49.00(44.00,55.00).Stepwise linear regression analysis revealed that being a student leader,SRSSDL scores,self-learning ability,and teaching model were significant factors(P<0.05),explaining 56.8%of the total variance.Conclusion The clinical comprehensive ability of students in the Elderly Service Management program requires enhancement,influenced by multiple factors including teaching model,self-learning ability,and self-directed learning capacity.
9.Professor LIU Minru's Medication Rule in Treatment of Polycystic Ovary Syndrome (Intracellular Lipid Membrane Congestion Syndrome)

ObjectiveTo analyze and sum up the medication rule and the core prescription of Professor LIU Minru in the treatment of polycystic ovary syndrome (intracellular lipid membrane congestion syndrome)and explore the effect mechanism underlying the medication. MethodTwo platforms were used to carry out data mining to analyze the characteristics and rules of Professor LIU's prescriptions for the treatment of this disease. Network pharmacology was used to further clarify active ingredients in the core prescription,and a traditional Chinese medicine-active ingredient-target network was constructed,with the potential mechanism of action analyzed. ResultA total of 321 prescriptions were included in the medical records,involving 178 Chinese medicinals and 28 kinds of formula granules.The Chinese medicinals mainly act on the liver and kidney meridians, whose main tastes were sweetness,pungency,and bitterness and properties were mainly warm,mild,and slightly cold.Commonly used medicine pairs include Dioscoreae Rhizoma-Rehmanniae Radix Praeparata,Chuanxiong Rhizoma-Angelicae Sinensis Radix,Bupleuri Radix-Aurantii Fructus,and Gleditsiae Spina-Curcumae Rhizoma.The commonly used formulas are Xuefu Zhuyutang,Siwugang,Yangjing Zhongyutang,etc. The core prescription is composed of 12 Chinese medicinals such as angelica,white peony,saponaria thorn,and epimedium,containing 74 active ingredients,including quercetin,luteolin,kaempferol,fisetin,and β-sitosterol.A total of 37 key targets were found,involving phosphatidylinositol 3-kinase (PI3K)/protein kinase B (Akt),mitogen-activated protein kinase (MAPK),Janus protein tyrosine kinase (JAK)/signal transduction and activator of transcription (STAT),and other signaling pathways. ConclusionThe pathogenesis of this disease is complex.Professor LIU adheres to the pathogenesis of "kidney deficiency as the root cause,and phlegm retention as the symptom". Under the guidance of the academic ideology of "kidney Qi as the root,and protecting Yin as the foundation" and "nurturing Yin to support Yang",she takes into account Yin and Yang in kidney tonic,replenishes and consolidates the essence and blood,and relieves manifestations by dissipating mass, activating blood, and regulating Qi. She has a rigorous thinking in formulating prescriptions. The core prescription has the characteristics of comprehensive regulation by multiple components at multiple targets in multiple pathways.

Percutaneous endoscopic gastrostomy(PEG) is a common method of enteral nutrition commonly used in patients requiring long-term intestinal feeding.PEG has been widely applied in various fields, including neuro-muscular diseases, gastrointestinal disorders, malignant tumors, and chronic illnesses.Among them, Amyotrophic lateral sclerosis(ALS)is one of the important indications for PEG.ALS is a progressive neuro-muscular disease that predominantly affects middle-aged and elderly individuals, often leading to swallowing difficulties and malnutrition.Studies have shown that providing enteral nutrition through PEG can improve the prognosis and survival of ALS patients.This article aims to review the clinical application value of PEG in ALS patients.
