To explore the clinical features, systematic diagnostic process, and iron chelation therapy strategies of secondary iron overload in aceruloplasminemia (ACP). A retrospective analysis was performed on the clinical data of patients diagnosed with aceruloplasminemia in Peking Union Medical College Hospital from December 2018 to September 2025. The clinical manifestations related to iron overload, key diagnostic points, and treatment responses were systematically summarized. A total of four patients were diagnosed with ACP. All of them had middle to late onset of the disease, and all had diabetes. Among them, 3 cases presented with progressive neurological symptoms. All patients showed a characteristic pattern of abnormal iron metabolism: significantly elevated serum ferritin (range: 730-12 091 ng/mL), decreased transferrin saturation (range: 4.9%-23.4%), and significantly reduced or undetectable ceruloplasmin levels. Genetic testing confirmed pathogenic mutations in the CP gene in all patients. For diagnosis, it is necessary to take the clinical "triad"(diabetes mellitus, neurological symptoms, anemia) as clues, combine with characteristic abnormalities in iron metabolism indicators and imaging evidence, and conduct differentiation from other iron overload diseases such as hereditary hemochromatosis. In terms of treatment, all the 4 patients received iron chelation therapy, including deferoxamine, deferiprone, and deferasirox. One of the patients showed a significant decrease in serum ferritin level with improvement in neurological symptoms after treatment. Iron overload secondary to ACP has insidious manifestations, and its clinical diagnosis relies on a high degree of vigilance and systematic iron metabolism evaluation. Establishing a stepwise diagnostic process from clinical screening to genetic testing is crucial. Early initiation and implementation of individualized iron chelation therapy is a core therapeutic measure to effectively manage iron overload and delay the progression of multiple organ damage.

Immunoglobulin G4-related disease (IgG4-RD) is a rare systemic immune-mediated disorder characterized by infiltration of IgG4-positive plasma cells and multiorgan fibrosis. Among them, cases with multiple pancreatic cystic lesions as the prominent manifestation are relatively rare, and they are easily misdiagnosed as pancreatic tumors or pancreatitis in clinical practice. Herein, we report one patient with IgG4-RD admitted to Peking Union Medical College Hospital. The patient initially presented with multiple pancreatic cystic lesions and aneurysmal dilatation of the abdominal aorta, accompanied by markedly elevated serum IgG4 levels. Combined with pathological findings from submandibular gland biopsy, the diagnosis of IgG4-RD was ultimately established. After treatment with a regimen of glucocorticoids combined with cyclophosphamide, the patient′s condition improved. Meanwhile, we performed a literature review of cases with IgG4-RD-related pancreatic cystic lesions, aiming to improve clinicians′ understanding of this rare disease phenotype.
5.Advances in Clinical Features, Pathogenesis and Treatment of Acquired Trisomy 8
JOURNAL OF RARE DISEASES 2026;5(3):367-372
Acquired trisomy 8 (+8) is a numerical abnormality of somatic chromosomes. Previous understanding of +8 has largely focused on hematological diseases such as myelodysplastic syndrome. Recent studies have revealed a close association between +8 and autoinflammatory diseases. Although clinical manifestations associated with acquired +8 have been progressively elucidated, its pathogenic mechanisms remain incompletely understood. Meanwhile, the comorbidity of hematological diseases and autoinflammatory diseases poses substantial challenges to the effective management of such conditions. This article reviews advances in clinical features, pathogenesis, and treatment of acquired +8, aiming to provide references for clinical diagnosis and treatment.

Objective:To demonstrate the type of CEBPA gene mutations among patients with acute myeloid leukemia (AML), clinical characteristics, and prognostic effect on patient outcomes. Methods:Demographic data, clinical features, laboratory characteristics, and data about treatment and follow-up of 57 patients with CEBPA mutated AML diagnosed at Peking Union Medical College Hospital between April 2016 and November 2022 were collected and analyzed. Results:In total, 57 patients with CEBPA mutation accounted for 16.1% of all the 353 patients with AML, among which 28 patients had CEBPA-bZIPinf and 29 had CEBPA-other. Compared with the CEBPA-other group, the CEBPA-bZIPinf group was younger (54 vs 64 years, P=0.010), de novo AML was more common ( P=0.001), and the level of bone marrow blast was higher (68.0% vs 36.3%, P=0.001). Moreover, 24 patients from the CEBPA-bZIPinf group and 19 from the CEBPA-other group received chemotherapy. The one-course complete remission (CR) rate of the CEBPA-bZIPinf group was significantly higher than that of the CEBPA-other (87.5% vs 47.4%, P=0.010) and CEBPA-wt (87.5% vs 50.3%, P=0.002) groups. After a median follow-up of 11 months, the median OS of the CEBPA-bZIPinf group was significantly longer than that of the CEBPA-wt group (not reached vs 22.1 months, P=0.012) . Conclusion:CEBPA-bZIPinf mutated AML is a unique clinical entity, with a younger age of diagnosis, better response to chemotherapy, and better prognosis.
