1.Status of screening myopia among primary and middle school students in Linfen Community of Shanghai from 2019 to 2023

Objective:
To analyze the prevalence and trend of screening myopia among primary and middle school students in Linfen Community of Shanghai from 2019 to 2023, so as to provide a reference for the prevention and control of myopia from the perspective of the community.
Methods:
From 2019 to 2023, all primary(5) and middle(2) school students aged 6-15 years in Linfen Community of Shanghai were screened. Statistical analysis was performed using the Chi square test and trend Chi square test. The curve fitting model was used to fit the model of the increase rate of screening myopia among primary and middle school students in 2019, 2021 and 2023.
Results:
The overall rate of screening myopia among primary and middle school students in Linfen community from 2019 to 2023 was 55.17%. The prevalence rate of screening myopia was 79.43% in boys and 81.92% in girls in middle school, and the difference was statistically significant ( χ 2=5.71, P =0.02). In 2019, 2021, and 2023, the peak age of screening myopia among primary and middle school students in Linfen Community gradually occurred earlier, at the age of 7(12.13%), 6( 12.28 %), and 6(14.99%) years old, respectively. The growth rate of screening myopia in students aged 8-12 years in 2023 was lower than that in 2019 and 2021.
Conclusions
The screening myopia rate of primary and middle school students aged 6-15 years in Linfen Community is relatively high, with primary school girls higher than boys, and growth spurt accelerates. It is suggested that prevention and control of myopia in the community should focus on preschool children and adolescent girls.
2.Growth and intelligence development among a cohort of low birth weight infants

Objective:
To explore growth and intelligence development of low birth weight infants (LBWI) at 24 and 36 months of age, so as to provide reference for early monitoring and intervention of the development of LBWI.
Methods:
A total of 100 LBWI born and managed in Hefei Maternal and Child Health Care Institution were selected from 2012 October 1 to 2015 December 30, and 99 normal birth weight infants (NBWI) under child health management in the same sitinstitution were selected as controls. According a prospective cohort study method, and based on the establishment of a cohort and monitoring of childhood growth and development, a unified method was used to longitudinally follow up and observe the physical fitness of two groups of infants at the determined time points. The development of LBWI and NBWI at 24 and 36 months of age was surveyed using the Gesell Development Scale.
Results:
Weight, length and head circumference of LBWI children at the age of 15-36 months were significantly lower than those of NBWI children ( P <0.05). In addition, 117 children (43.98%) completed the full assessment of intelligent development scale, including 62 LBWI and 55 NBWI. The scores of Gesell in NBWI group was higher than that in LBWI group at 24 and 36 months of age, including adaptability, gross motor, fine metor skills, language and personal social functions ( t =-4.17, -3.82, -3.21 , -3.03, -2.61; -4.23, -3.16, -3.07, -3.13, -3.99, P <0.05). Multivariate linear regression analysis found that birth weight was positively correlated with adaptability, gross motor, fine motor skills, language functions at 24 and 36 months of age and personal social function at 36 months of age ( β =0.004, 0.010; 0.003, 0.008; 0.003, 0.007; 0.004, 0.009; 0.011, P <0.05).
Conclusion
The growth and development of LBWI children are significantly delayed compared to NBWI children. The scores of LBWI children are lower than those of NBWI children in all functional areas. Weight is the main factor affecting children s intellectual development. Early monitoring and intervention of low birth weight infants should be carried out to avoid or mitigate adverse consequences.
3.Application of an intervention plan based on unplanned readmission risk model in the rehabilitation of patients with acute myocardial infarction complicated with cardiogenic shock after percutaneous coronary intervention

Objective:To study and analyze the application effect of intervention plan based on unplanned readmission risk model (LACE) in the rehabilitation of patients with acute myocardial infarction (AMI) complicated with cardiac shock (CS) after percutaneous coronary intervention (PCI).Methods:Ninety-three patients with AMI complicated with CS who received PCI in Tianjin Union Medical Center from January 2019 to December 2020 were enrolled. The patients were divided into LACE intervention group ( n = 46) and routine intervention group ( n = 47) according to the different nursing intervention methods. The patients in the routine intervention group received routine interventions, including drug care, diet care, psychological care, health education and telephone follow-up, while those in the LACE intervention group were assessed for the risk of LACE, and then intervention measures were formulated according to the score of LACE index, including strengthening risk awareness, life behavior, daily life ability, self-care ability, health recovery and health needs. The follow-up period in both groups was 3 months. The changes of cardiac function, incidence of adverse cardiac events, readmission rate, self-management ability after intervention and quality of life level before and after intervention were compared between the two groups. Results:There was no significant difference in cardiac function or quality of life before intervention between the two groups. After intervention for 3 months, the cardiac function and quality of life in the two groups were improved as compared with those before intervention. The left ventricular ejection fraction (LVEF) in the LACE intervention group was significantly higher than that in the routine intervention group (0.533±0.076 vs. 0.492±0.072, P < 0.05), the left ventricular end diastolic diameter (LVEDD) was significantly lower than that in the routine intervention group (mm: 47.09±7.01 vs. 53.23±7.15, P < 0.01), and the World Health Organization Quality of Life-brief (WHOQOL-BREF) score was also significantly higher than that in the routine intervention group (66.32±6.19 vs. 55.79±7.26, P < 0.01). The scores of self-management ability in the coronary heart disease self-management scale (CSMS) after intervention in the LACE intervention group were significantly higher than those in the routine intervention group (adverse hobbies score: 17.37±3.24 vs. 14.21±2.73, symptoms score: 14.82±3.11 vs. 10.56±2.65, emotional cognition score: 16.17±2.83 vs. 12.95±2.41, first aid score: 11.85±1.94 vs. 10.62±1.56, disease knowledge score: 15.58±2.73 vs. 12.68±2.61, daily life score: 17.80±2.61 vs. 14.33±2.36, treatment compliance score: 11.47±1.84 vs. 8.56±1.23, all P < 0.01). The incidence of adverse cardiac events and readmission rate in the LACE intervention group were significantly lower than those in the routine intervention group (10.87% vs. 29.79%, 4.35% vs. 17.02%, both P < 0.05). Conclusion:The intervention plan based on LACE risk model can effectively promote postoperative rehabilitation of patients with AMI complicated with CS after PCI, and also help to improve patients' self-management ability and quality of life, which is worthy of clinical promotion and application.

Syndrome of resistance to thyroid hormone(RTH)is a rare hereditary thyroid disease with various clinical manifestations and laboratory findings. RTH could be misdiagnosed and mistreated, resulting in aggravation of the disease. We reviewed the medical records of a patient with RTH over the past six years. In addition, we provided a summary of latest progress for RTH to help the clinicians to improve the understanding of the disease.
5.Application of empowerment psychological care intervention in direct caregivers of children with epilepsy

Objective:To analyze the effects of empowerment psychological care intervention on the negative emotions, mental toughness and quality of life of the direct caregivers of children with epilepsy.Methods:A total of 150 direct caregivers of 150 children with epilepsy in the Department of Neurology, Anhui Provincial Children's Hospital from September to December 2019 were recruited, and divided in to control group ( n=74) and experimental group ( n=76) by random number table method. The control group was given conventional psychological nursing, while the experimental group was given empowerment psychological care. The negative mood, mental toughness and quality of life of the direct caregivers were compared between the two groups. Results:After intervention, the scores of Anxiety Self-Rating Scale (SAS) and Self-rating Depression Scale (SDS) of the experimental group were lower than those of the control group, the scores of the WHO Quality of Life Assessment Instrument (WHOQOL-100) and the Psychological Resilience Scale (RSA) were higher than those of the control group, and the differences were statistically significant ( P<0.05) . Conclusions:Empowerment psychological care can significantly improve the mental state of the direct caregivers of children with epilepsy and improve their mental toughness and quality of life.
6.Improvement Effects of Erythropoietin Derivative Helix B Surface Peptide on Epirubicin-induced Myocardial Injury in Rats and Its Mechanism Study

OBJECTIVE: To study the improvement effects and its mechanism of erythropoietin derivative helix B surface peptide (HBSP) on epirubicin-induced myocardial injury in rats. METHODS: SD rats were randomly divided into control group, model group, erythropoietin (EPO) group and HBSP group, with 10 rats in each group. Except for control group, other groups were given epirubicin 2.5 mg/kg intraperitoneally, once a week, for consecutive 6 weeks to induce myocardial injury model. EPO group and HBSP groups were given rhEPO 5 000 u/kg or HBSP 60 μg/kg intraperitoneally, 3 times a week (one day before medication, first day and third day after medication of epirubicin), for consecutive 6 weeks. At the beginning of the first administration, the rats were weighed at the 11th week. The cardiac function was measured by echocardiography [left ventricular end-diastolic diameter (LVIDd), ejection fraction (EF), fractional shortening (FS) and cardiac output(CO)]. The serum levels of troponin I (cTnI) and amino-terminal B-type natriuretic peptide (NT-proBNP) were determined by ELISA. mRNA expression of PI3K and Akt in myocardial tissue was detected by RT-PCR. The protein expression of p-PI3K and p-Akt in rat myocardium were detected by Western blot. RESULTS: During research period, two rats died in model group, one in EPO group and none in HBSP group. Compared with control group, body weight, the levels of EF, FS and CO were decreased significantly in model group, while the contents of LVIDd and cTnI, NT-proBNP were increased significantly; mRNA expression of PI3K and Akt as well as protein expression of p-PI3K and p-Akt were decreased significantly, above differences were statistical significant (P<0.05). Compared with model group, body weight, the levels of EF, FS and CO were increased significantly in EPO group and HBSP group, while the contents of LVIDd and cTnI, NT-proBNP were decreased significantly; mRNA expression of PI3K and Akt as well as protein expression of p-PI3K and p-Akt were increased significantly, above differences were statistical significant (P<0.05). Compared with EPO group, the contents of cTnI and NT-proBNP were decreased significantly in HBSP group, with statistical significance (P<0.05). CONCLUSIONS: HBSP can improve myocardial injury in rats as much as EPO, and has less toxity. Their mechanism may be related to the activation of PI3K/Akt signaling pathway.
7.Forensic pathological analysis on 38 cases of fatal cardiac tamponade

Objective To explore the forensic pathological characteristics and the main identification points of fatal cardiac tamponade. Methods 38 cases of fatal cardiac tamponade from department of pathology, the first affiliated hospital of chengdu university of TCM from 2005 to 2015 were reviewed and analyzed retrospectively. Results Fatal cardiac tamponade mostly occurred to men (71.1%) with an average age of 44; Bloody effusion accounted for 85% of the direct causes of death (34 cases). The most underlying causes of death were diseases (73.7%), majorly aortic dissection, coronary heart disease and malignant tumors. Seventy five percent of death occurred within 12 hours of illness. Medical behaviors were involved in 30 cases (78.9%), of which 26 cases (86.7%)were without medical malpractice. The relationship between injury and disease was involved in 15 fatal cases (39.5%). Conclusion The basic requirement for accurately completing forensic medical appraisal of fatal cardiac tamponade cases was to master the forensic pathological characteristics and the path of forensic identification.
8. Application of pegylated recombinant human granulocyte colony-stimulating factor to prevent chemotherapy-induced neutropenia in patients with lymphoma: a prospective, multicenter, open-label clinical trial

Objective:
To evaluate the efficacy and safety of pegylated recombinant human granulocyte colony-stimulating factor (PEG-rhG-CSF) in prophylaxis neutropenia after chemotherapy in patients with lymphoma.
Methods:
This was a multicenter, single arm, open, phase Ⅳ clinical trial. Included 410 patients with lymphoma received multiple cycles of chemotherapy and PEG-rhG-CSF was administrated as prophylactic. The primary endpoint was the incidence of Ⅲ/Ⅳ grade neutropenia and febrile neutropenia (FN) after each chemotherapy cycle. Meanwhile the rate of antibiotics application during the whole period of chemotherapy was observed.
Results:
①Among the 410 patients, 8 cases (1.95%) were contrary to the selected criteria, 35 cases (8.54%) lost, 19 cases (4.63%) experienced adverse events, 12 cases (2.93%) were eligible for the termination criteria, 15 cases (3.66%) develpoed disease progression or recurrence, thus the rest 321 cases (78.29%) were into the Per Protocol Set. ②During the first to fourth treatment cycles, the incidences of grade Ⅳ neutropenia after prophylactic use of PEG-rhG-CSF were 19.14% (49/256) , 12.5% (32/256) , 12.18% (24/197) , 13.61% (20/147) , respectively. The incidences of FN were 3.52% (9/256) , 0.39% (1/256) , 2.54% (5/197) , 2.04% (3/147) , respectively. After secondary prophylactic use of PEG-rhG-CSF, the incidences of Ⅳ grade neutropenia decreased from 61.54% (40/65) in the screening cycle to 16.92% (11/65) , 18.46% (12/65) and 20.75% (11/53) in 1-3 cycles, respectively. The incidences of FN decreased from 16.92% (11/65) in the screening cycle to 1.54% (1/65) , 4.62% (3/65) , 3.77% (2/53) in 1-3 cycles, respectively. ③The proportion of patients who received antibiotic therapy during the whole period of chemotherapy was 34.39% (141/410) . ④The incidence of adverse events associated with PEG-rhG-CSF was 4.63% (19/410) . The most common adverse events were bone pain[3.90% (16/410) ], fatigue (0.49%) and fever (0.24%) .
Conclusion
During the chemotherapy in patients with lymphoma, the prophylactic use of PEG-rhG-CSF could effectively reduce the incidences of grade Ⅲ/Ⅳ neutropenia and FN, which ensures that patients with lymphoma receive standard-dose chemotherapy to improve its cure rate.
9.Study on feedback of operating room specialty certified nurse theoretical training and quality improvement

Objective To explore curriculum and faculty qualification of Chongqing operating room (OR) specialty certified nurse theoretical tralning program by feedback of students, and to im-prove the quality of tralning. Method Relying on the national key project of the First Affiliated Hos-pital of Chongqing Medical University, Chongqing Health administration OR nurse specialist tralning program set additional courses related to research, teaching, patient safety and first ald tralning based on operation room specialist nurse tralning outline. 81 participants were given questionnalres issued by college of nursing after the 7th OR specialist nurse theoretical tralning, and statistics analysis was made of it. Result Effective questionnalres was100%. The overall judgment on theory tralning was high, with 92.60%of the students giving overall score above 150 points (out of 170);93.84%of students sat-isfied on teaching staff ratio while 95.06% students satisfied on course content. For the 17 items, the majority scores given by the students were ≥ 9 points (a 10-points scale), on the top of the list were item “Theoretical tralning helps to enhance professional theoretical level and guide clinical practice”“Teaching content meet needs of OR specialist nurses” “Teaching content is helpful to improve the core competence of specialist nurses” (all 96.30%, 78 out of 81), while the bottom was “Instructor create an environment which fosters student participation”(85.19%, 69 out of 81). Conclusion Chong-qing OR specialist theoretical tralning has good practical and guidance property, the content meet OR nurses needs, instructor ratio is reasonable, and students are highly satisfied with it. Instructors need to encourage students' participation in class interaction, and pay attention to improving their research capabilities.
10.The study of the changes in serum concentrations of angiogenic factors in patients with unstable angina pectoris

Objective To explore the changes in serum concentrations of angiogenic factors in patients with unstable angina pectoris.Methods A total of 60 patients with unstable angina pectoris was eligible for study and divided into diabetes group ( A group, n =20) and non-diabetes group (B group, n =40).Another 30 general-matched healthy subjects from medical examination center were enrolled as control group .Serum samples were collected , and serum concentrations of vascular endothelial growth factor (VEGF), angiopoietin-1, angiopoietin-2, angiogenin, angiostatin, basic fibroblast growth factor (bFGF), and platelet-derived growth factor-BB ( PDGF-BB) were measured by cytokine array technology and compared between the groups .Results Compared with the control group, the serum concentrations of VEGF [(325.2 ±210.1)pg/ml] and angiopoietin-2 [(3031.3 ±1865.5)pg/ml] were sig-nificantly increased in patients with unstable angina pectoris (both P <0.05).Whereas,no significant differences in serum concentra-tions of angiogenin,angiopoietin-1,angiostatin,bFGF,and PDGF-BB were detected between control group and patient groups .There were no significant differences in serum concentrations of above all 7 biomarkers between diabetes group and non-diabetes group .Con-clusions Serum concentrations of VEGF and angiopoietin-2 were increased in patients with unstable angina pectoris ,and diabetes didn't affect the increases in serum concentrations of VEGF and angiopoietin-2 caused by unstable angina pectoris .
