Objective: To investigate the incidence and independent risk factors of adverse events (AEs) associated with therapeutic plasma exchange (TPE) in patients undergoing allogeneic hematopoietic stem cell transplantation (allo-HSCT), and to provide evidence to support the safe use of TPE in clinical practice. Methods: This retrospective study included 167 allo-HSCT recipients who underwent a total of 385 TPE procedures between March 2022 and December 2023. Patients were divided into two groups according to the occurrence of TPE-related AEs. Baseline characteristics, plasma exchange parameters, and clinical features were collected. Univariate analysis and multivariable Firth logistic regression were performed to identify independent risk factors for AEs. Results: A total of 385 TPE procedures were performed in 167 allo-HSCT patients, and 33 patients (19.8%) experienced TPE-related AEs. Among the 33 patients who experienced AEs, allergic reactions were the most common (n=18, 54.5%), followed by blood pressure fluctuations (n=11, 33.3%) and hypocalcemia (n=4, 12.1%). Most AEs were classified as grade II (n=23, 69.7%), and no grade IV (fatal) events were observed. Univariate analysis demonstrated a significant difference in the incidence of AEs among different hematocrit (HCT) categories (<20%, 20%-30%, and>30%) (Fisher's exact test, P=0.022). Although the overall difference was statistically significant, none of the pairwise comparisons remained significant after Bonferroni correction (adjusted P>0.0167). Multivariable Firth logistic regression identified higher HCT levels, analyzed as a continuous variable (OR=1.07, 95% CI: 1.01-1.13, P=0.020), and TPE performed for cytokine release syndrome (CRS) (OR=9.21, 95% CI: 1.25-78.00, P=0.030) as potential independent risk factors for TPE-related AEs. Receiver operating characteristic (ROC) curve analysis showed that HCT had an AUC of 0.67 (95% CI: 0.57-0.77) for predicting AEs, with an optimal cutoff value of 24.5%, a sensitivity of 60.6%, and a specificity of 75.4%. Using this cutoff, the AEs rate was significantly higher in patients with HCT≥24.5% than in those with HCT<24.5% (37.7% vs 11.4%, P<0.001). Conclusion: A-mong allo-HSCT patients receiving TPE, allergic reactions were the predominant AEs. Higher HCT levels and TPE performed for CRS may be associated with an increased risk of AEs. HCT≥24.5% may help identify patients requiring intensified monitoring, although its discriminatory ability is moderate (AUC=0.67) and it should not be used as a standalone criterion for clinical decision-making. Findings regarding the CRS subgroup require validation in larger cohorts. Optimizing extracorporeal circuit priming and strengthening peri-procedural monitoring may help improve the safety of TPE.

Objective To explore the differences in the incidence and severity of diarrhea between patients receiving propylene glycol-melphalan (PG-MEL) or propylene glycol-free-melphalan (PGF-MEL) pretreatment during autologous hematopoietic stem cell transplantation (AHSCT). Methods The clinical data of 30 patients who underwent AHSCT with high-dose melphalan (HDM) pretreatment at the Department of Hematology, Zhongshan Hospital (Xiamen), Fudan University, from January 2024 to November 2025 were retrospectively analyzed. Patients were divided into the PG-MEL group (n=15) and the PGF-MEL group (n=15) according to the formulation. The incidence, onset time, maximum daily frequency, duration, type, and severity of diarrhea were compared between the two groups. Oral mucositis, vomiting, infection, fever, and other related adverse events, as well as hematopoietic recovery, were also observed. Results The overall incidence of diarrhea was 93.3% in both groups. There was no significant difference in the onset time of diarrhea between the two groups ([4.0±2.0] d vs [3.0±2.0] d). Compared with the PG-MEL group, the PGF-MEL group had a higher maximum daily frequency of diarrhea (8 [6,14] times vs 6 [4,6] times, P=0.009), a higher proportion of grade 3–4 diarrhea (57.1% vs 14.3%, P=0.046), and a longer duration of diarrhea ([8.0±4.0] d vs [5.0±2.0] d, P=0.033). No statistically significant differences were observed between the two groups in the incidence of other adverse events such as oral mucositis, vomiting, infection, fever, or neutrophil and platelet engraftment times. Conclusions PG-MEL and PGF-MEL showed the same overall incidence of diarrhea in AHSCT. However, the PGF-MEL group experienced more severe diarrhea and longer duration. There were no significant differences between the two formulations in other adverse reactions or short-term transplant outcomes.

Chinese Journal of Lung Cancer 2025;28(4):319-324
Non-small cell lung cancer (NSCLC), a leading cause of cancer-related deaths worldwide, remains a significant clinical challenge despite advances in immune checkpoint inhibitors therapy, with drug resistance persisting as a major obstacle. Palmitoylation, a critical post-translational modification (PTM) primarily catalyzed by palmitoyltransferases of the zinc finger DHHC-type (ZDHHC), has recently demonstrated important implications in NSCLC. This review aims to elucidate the mechanisms and clinical potential of ZDHHC-mediated protein palmitoylation in NSCLC progression and immune escape.

Epilepsy is a chronic neurological disorder affecting ~65 million individuals worldwide. Abnormal synaptic plasticity is one of the most important pathological features of this condition. We investigated how ubiquitin-specific peptidase 47 (USP47) influences synaptic plasticity and its link to epilepsy. We found that USP47 enhanced excitatory postsynaptic transmission and increased the density of total dendritic spines and the proportion of mature dendritic spines. Furthermore, USP47 inhibited the degradation of the ubiquitinated α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid receptor (AMPAR) subunit glutamate receptor 1 (GluR1), which is associated with synaptic plasticity. In addition, elevated levels of USP47 were found in epileptic mice, and USP47 knockdown reduced the frequency and duration of seizure-like events and alleviated epileptic seizures. To summarize, we present a new mechanism whereby USP47 regulates excitatory postsynaptic plasticity through the inhibition of ubiquitinated GluR1 degradation. Modulating USP47 may offer a potential approach for controlling seizures and modifying disease progression in future therapeutic strategies.
