Objective To investigate the mechanism by which blue light irradiation-activated microglia mediate immune cell recruitment and exacerbate retinal damage, and to explore the role of microglial depletion in inhibiting immune infiltration and protecting the retina. Methods SPF-grade C57BL/6J mice were randomly divided into control group, blue light irradiation group, and PLX5622 pretreatment blue light irradiation group.A retinal injury model was established by continuous LED blue light irradiation for 2 days.In the PLX5622 pretreatment group, microglia were specifically depleted before blue light irradiation. After modeling,HE staining and OCT examination were used to examine retinal histomorphological changes;ERG examination was performed to evaluate retinal function;DHE staining and RT-qPCR were used to detect oxidative stress and inflammatory responses, and Iba1,CD68,CD11b immunofluorescence staining and flow cytometry were used to analyze microglial activation status and immune cell infiltration. Results After blue light irradiation, the retinal outer nuclear layer thickness was significantly reduced;ERG a-wave and b-wave amplitudes decreased;expression of oxidative stress-related genes Nrf2,Sod2,and HO-1 was upregulated;expression of inflammatory factors IL-1β,TNF-α,and ICAM-1 increased;the number of Iba1-positive microglia increased and migrated extensively to the outer nuclear layer;the proportion of CD68+cells and CD11b+immune cells was elevated;and activated microglia aggregated around blood vessels to mediate immune cell infiltration. After PLX5622 pretreatment to deplete microglia, immune cell infiltration was significantly reduced;inflammatory responses were alleviated;retinal structural damage was markedly improved, and visual function was protected. Conclusions Blue light irradiation activates microglia and promotes their migration to the injury area.Activated microglia mediate immune cell recruitment and infiltration, exacerbating retinal inflammatory damage.Microglial depletion can effectively inhibit immune infiltration, rescue retinal structure and function, and provide new therapeutic strategies for the prevention and treatment of blue light-related ocular diseases.

Objective The aim is to understand the common scenarios of non-medical use of prescription opioids(NMUPO)and analyze the potential facilitating and hindering factors in the regulatory process of NMUPO from the perspective of healthcare professionals. Methods Healthcare professionals in local hospitals were surveyed through a two-stage purposive sampling from June to August 2022 in Ningbo,China. The survey was conducted using a semi-structured questionnaire on topics, and thematic analysis were used to identify and summarise key themes and patterns. Results A total of 75 participants were included, the average age was(43.9±7.2)years,and 54(72.0%)were male. The most common NMUPO scenarios involved middle-aged males pretending acute severe pain to obtain injectable opioids. The facilitating and hindering factors related to the regulation of NMUPO can be categorized into three types:institutional governance, technical support, and individual behaviors. At the institutional level, facilitating factors included strict national prescribing policies and local"narcotic drug card"systems, while barriers comprised incomplete lists of controlled substances. At the technological support level, facilitating factors included the establishment of regional health information platforms, while barriers included the lack of standardized prescription guidelines and diagnostic decision-support tools. At the individual level, facilitating factors included the public's cautious attitude toward drug misuse, while barriers included strained doctor-patient relationships. Conclusion China still faces significant challenges in addressing NMUPO and urgently needs to improve the existing regulatory system. It is recommended that reforms be carried out in areas such as pharmaceutical control mechanisms, drug treatment and rehabilitation services, preventive health education activities, and the optimized use of health information systems.

Objective:To analyze the impact of the universal coverage, integration and DRG/DIP payment method reform of Basic Medical Insurance(BMI)on the equity of the health service utilization and resource allocation. Methods:The 1998-2022 panel data on health service utilization and resources were collected for China.Health equity is measured using the Concentration Index(CI)and Theil index, and the impact of BMI reform on health equity is analyzed by BP neural network and ITSA.Results:Universal coverage, integration, and DRG/DIP payment reform of BMI had a positive effect on the number of health workers, beds, and admissions(P<0.001),and inhibited the growth of per capita hospitalization costs(P<0.05).The CI for the number of health workers and beds declined by 0.004 and 0.003 per year after universal coverage of BMI;and both still declined by 0.002 per year with a 5-year lag(P<0.05).The CI for the hospital admissions fell by 0.014 immediately after the DRG/DIP payment reform and by 0.012 per year thereafter(P<0.001).Conclusion:BMI reforms not only can contribute to improve the health service utilization and resources, but also it can also improve health equity. In addition,it has a lagged effect on improving health equity.

Objective To explore the correlation between TCM constitution and sleep status in escort troops. Methods Standardized TCM Constitution Questionnaire and Pittsburgh Sleep Quality Index (PSQI) were used to investigate escort troops. Factors of PSQI and constitution types were analyzed. Results The results of PSQI showed the number of escort troops who had bad sleeping (PSQI>7) was 64, which accounted for 46%of the escort troops, among which, 45 people (70.3%) were justified as biased constitution, while 19 were normal constitution (29.7%). The top three biased constitution in bad sleeping were:qi deficiency, phlegm-dampness and dampness-heat. PSQI factors such as sleep disorder, daytime dysfunction and total score had positive correlation with qi deficiency, yang deficiency, yin deficiency, dampness-heat and blood stasis, and negative correlation with normal constitution. Conclusion Sleep condition of escort troops has correlation with TCM constitution, which indicates that sleep status could be improved by regulating TCM constitution.

Objective To investigate the physiological and psychological symptoms of drug abusers under rehabilitation period through labor. Methods A self-designed clinical symptoms questionnaire and Eysenck Personality Questionaire (EPQ) were used to evaluate 1 004 drug abusers under rehabilitation period through labor, and the related symptoms were analyzed. Results The mainly physiological symptoms were fatigue, forgetfulness, aching pain of joints, arrhythmia and discomfort of limbs, and the mainly psychological symptoms are drug craving, insomnia and anxiety. The mean scores of P and E were higher in those who had the drug craving symptoms ( P<0.01), and the mean scores of N were higher in those who had the insomnia or anxiety symptoms ( P<0.01).Conclusion The mainly physiological symptoms of drug abusers under rehabilitation period through labor are fatigue, forgetfulness, aching pain of joints, arrhythmia and discomfort of limbs, which are probably caused by the unbalance of the nervous and internal secretive systems, and the plastisity of the nervous system. The psychological symptoms may due to the effect of morphine and the personality of the drug abusers.
