OBJECTIVES:
To investigate the clinical treatment outcomes and the changes of the outcomes over time in extremely preterm twins in Guangdong Province, China.
METHODS:
A retrospective analysis was performed for 269 pairs of extremely preterm twins with a gestational age of <28 weeks who were admitted to the department of neonatology in 26 grade A tertiary hospitals in Guangdong Province from January 2008 to December 2017. According to the admission time, they were divided into two groups: 2008-2012 and 2013-2017. Besides, each pair of twins was divided into the heavier infant and the lighter infant subgroups according to birth weight. The perinatal data of mothers and hospitalization data of neonates were collected. The survival rate of twins and the incidence rate of complications were compared between the 2008-2012 and 2013-2017 groups.
RESULTS:
Compared with the 2008-2012 group, the 2013-2017 group (both the heavier infant and lighter infant subgroups) had lower incidence rates of severe asphyxia and smaller head circumference at birth (P<0.05). The mortality rates of both of the twins, the heavier infant of the twins, and the lighter infant of the twins were lower in the 2013-2017 group compared with the 2008-2012 group (P<0.05). Compared with the 2008-2012 group, the 2013-2017 group (both the heavier infant and lighter infant subgroups) had lower incidence rates of pulmonary hemorrhage, patent ductus arteriosus (PDA), periventricular-intraventricular hemorrhage (P-IVH), and neonatal respiratory distress syndrome (NRDS) and a higher incidence rate of bronchopulmonary dysplasia (P<0.05).
CONCLUSIONS
There is a significant increase in the survival rate over time in extremely preterm twins with a gestational age of <28 weeks in the 26 grade A tertiary hospitals in Guangdong Province. The incidences of severe asphyxia, pulmonary hemorrhage, PDA, P-IVH, and NRDS decrease in both the heavier and lighter infants of the twins, but the incidence of bronchopulmonary dysplasia increases. With the improvement of diagnosis and treatment, the multidisciplinary collaboration between different fields of fetal medicine including prenatal diagnosis, obstetrics, and neonatology is needed in the future to jointly develop management strategies for twin pregnancy.

Objective:
To observe the dynamic impacts of shock waves on the severity of lung injury in rats with different injury distances.
Methods:
Simulate open-field shock waves; detect the biomechanical effects of explosion sources at distances of 40, 44, and 48 cm from rats; and examine the changes in the gross anatomy of the lungs, lung wet/dry weight ratio, hemoglobin concentration, blood gas analysis, and pathology.
Results:
Biomechanical parameters such as the overpressure peak and impulse were gradually attenuated with an increase in the injury distance. The lung tissue hemorrhage, edema, oxygenation index, and pathology changed more significantly for the 40 cm group than for the 44 and 48 cm groups. The overpressure peak and impulse were significantly higher for the 40 cm group than for the 44 and 48 cm groups ( < 0.05 or < 0.01). The animal mortality was significantly higher for the 40 cm group than for the other two groups (41.2% . 17.8% and 10.0%, < 0.05). The healing time of injured lung tissues for the 40 cm group was longer than those for the 44 and 48 cm groups.
Conclusions
The effects of simulated open-field shock waves on the severity of lung injuries in rats were correlated with the injury distances, the peak overpressure, and the overpressure impulse.

Alzheimer's disease(AD) is an incipient aging neurodegenerative disease, which increases rapidly along with the development trend of social aging and seriously threatens the health of the people. In the absence of effective preventive measures, it will have an enormous impact on the socio-economic and healthcare system. The study found that abnormal cell signal transduction is a key link in many diseases. Cell signal transduction theory has been widely used to clarify the essence of traditional Chinese medicine visceral image and the mechanism of traditional Chinese medicine. 'Correlation of Liver and Kidney' is one of the core plates of the theory of 'Correlation of Five Organs', which is suitable for explaining the pathogenesis of complex diseases and the correlation of multiple syndromes, and guiding the prescription of clinical syndrome. Hei Xiaoyaosan, as the first choice compound for the prevention and treatment of AD based on the theory of "Correlation of Liver and Kidney' in our team, can play the effects of prevention and treatment by soothing liver and nourishing blood, strengthening spleen and tonifying kidney, and promoting brain collaterals and dredging viscerab spirit. Based on the theory of 'Correlation of Liver and Kidney', this paper expounds the pathogenesis of AD from the perspective of traditional Chinese medicine, and puts forward the methods and ideas of the preventing and treating of AD from Ca2+-calcium/calmodulin dependent protein (CaM)/calcium/calmodulin dependent protein kinaseⅡ(CaMKⅡ)-cyclic adenosine phosphate reactive element binding protein (CREB) cell signal transduction pathway by consulting literatures and previous studies.

Collectively migrating tumor cells have been recently implicated in enhanced metastasis of epithelial malignancies. In oral squamous cell carcinoma (OSCC), v integrin is a crucial mediator of multicellular clustering and collective movement ; however, its contribution to metastatic spread remains to be addressed. According to the emerging therapeutic concept, dissociation of tumor clusters into single cells could significantly suppress metastasis-seeding ability of carcinomas. This study aimed to investigate the anti-OSCC potential of novel endostatin-derived polypeptide PEP06 as a cluster-dissociating therapeutic agent . Firstly, we found marked enrichment of v integrin in collectively invading multicellular clusters in human OSCCs. Our study revealed that metastatic progression of OSCC was associated with augmented immunostaining of v integrin in cancerous lesions. Following PEP06 treatment, cell clustering on fibronectin, migration, multicellular aggregation, anchorage-independent survival and colony formation of OSCC were significantly inhibited. Moreover, PEP06 suppressed v integrin/FAK/Src signaling in OSCC cells. PEP06-induced loss of active Src and E-cadherin from cell-cell contacts contributed to diminished collective migration of OSCC . Overall, these results suggest that PEP06 polypeptide 30 inhibiting v integrin/FAK/Src signaling and disrupting E-cadherin-based intercellular junctions possesses anti-metastatic potential in OSCC by acting as a cluster-dissociating therapeutic agent.

Objective:To analyze the attenuation of immobilization baseplate of carbon fiber for ray and the change of dose verification results for enhanced radiation field of MatriXX. And to primarily research the influence of this immobilization baseplate for the dose distribution of radiation field of radiotherapy. Methods: 20 patients with cervical cancer were enrolled in the research. The angle of gantry was 180°in the enhanced plan of immobilization field, and the corresponding verification plans were designed. When the verification plan was implemented on the Varian Clinac iX of accelerator, the two-dimensional ionization chamber array (MatriXX) was used to measure dose distributions of the plan. The immobilization baseplate of carbon fiber (ORFIT AIO) was used in the research. The situations of no baseplate (A group), one baseplate (B group), two baseplates (C group)and three baseplates (D group)between the handpiece of accelerator and Matri XX were researched, respectively. The data of the 4 groups were obtained by using software (OminiPro I'mRT), and the largest dose point and passing rate of Gamma in plane of radiation field were counted and analyzed. Results: Compared with A group, the average attenuation factors for ray on the largest dose point of B, C and D group in the plane of radiation field were (1.73±0.11)%, (3.44±0.36)%, and (5.48±0.44)%, respectively. The passing rates of Gamma in the 4 groups were (94.87±2.31)%, (99.18±0.61)%, (99.79±0.27)% and (96.24±1.99)%, respectively. And the differences of passing rate between each group and A group were statistical significance (t=9.788,t=9.261,t=2.256,P<0.05).Conclusion:The immobilization baseplate of carbon fiber has influence for the attenuation of high-energy X-rays and it can affect the dose distribution of radiation field which has passed baseplate in some degrees. And when physicists calculates the dose, they should consider the effect of immobilization baseplate of carbon fiber for dose distribution of radiation field.

Objective To explore the clinical and neuroimaging features in a Chinese family with hereditary diffuse leukoencephalopathy with neuroaxonal spheroids (HDLS) caused by mutation of the colony stimulating factor 1 receptor gene (CSF1R). Methods The proband and another patient from a HDLS pedigree were assessed respectively through standardized clinical evaluation (medical history inquiry, physical examination),neuropsychology assessment,MRI,genetic sequencing, as well as brain PET imaging with carbon11-labelled Pittsburgh compound-B(11C-PIB). Results A HDLS pedigree with three patients was recruited to this study. Apathy, memory decline, slow behavior were the first symptoms for two of the patients. Being bedridden, urinary incontinence and epilepsy were developed at the later stage. A missense mutation c. 2381T>C(p. I794T) in exon 18 of the CSF1R gene of chromosome 5 was identified in the proband. The brain DWI illustrated multiple patchy high signal in periventricular white matter and centrum semiovale which was characterized by persistence, and the corpus callosum was affected in the early stage. Conclusion The multiple patchy high signal with persistence in periventricular white matter and centrum semiovale of DWI is helpful for the early diagnosis of HDLS.
