1.Predictive value of serum microRNA-19a-3p,microRNA-146a and 25-hydroxyvitamin D3 for prognosis of children with respiratory syncytial virus bronchiolitis

Objective To evaluate the predictive value of serum microRNA-19a-3p(miR-19a-3p),microRNA-146a(miR-146a)and 25-hydroxyvitamin D3[25-(OH)D3]for the prognosis of children with respiratory syncytial virus(RSV)bronchiolitis.Methods A total of 202 children with RSV bronchiolitis were enrolled as disease group,followed up for one year after discharge,and divided into good prognosis group(n=55)and poor prognosis group(n=147).Additionally,180 healthy chil-dren matched by gender and age were selected as control group.Serum levels of miR-19a-3p,miR-146a and 25-(OH)D3 were measured in both the disease and control groups.The clinical data and the levels of serum miR-19a-3p,miR-146a and25-(OH)D3 of children in the poor prognosis group and the good prognosis group were compared.The correlations of serum miR-19a-3p,miR-146a and 25-(OH)D3 with the prognosis of children and their predictive value for poor prognosis of chil-dren were analyzed.Multivariate Logistic regression analysis was used to screen the related influen-cing factors of poor prognosis in children.Results The proportion of smoking among family mem-bers,the expression levels of serum miR-19a-3p and miR-146 in the poor prognosis group were sig-nificantly higher than those in the good prognosis group,and the level of 25-(OH)D3 was signifi-cantly lower than that in the good prognosis group(P<0.05).The expression levels of serum miR-19a-3p and miR-146a were positively correlated with the poor prognosis of the children(r=0.408,0.332,P<0.001),and the level of serum 25-(OH)D3 was negatively correlated with the poor prognosis of the children(r=-0.386,P<0.001).The results of receiver operating characteristic(ROC)curve analysis showed that the area under the curve(AUC)of serum miR-19a-3p,miR-146a and 25-(OH)D3 for predicting poor prognosis in children was 0.726,0.817 and 0.834,re-spectively.The AUC of the combined prediction of the three was 0.938,which was higher than that of the single prediction(Z=5.515,3.879,3.860,P<0.001),and the sensitivity and specificity were 83.64%and 90.48%,respectively.The miR-19a-3p,miR-146a,25-(OH)D3 and smoking among family members were all influencing factors for the poor prognosis of children with RSV bron-chiolitis(P<0.05).Conclusion Serum miR-19a-3p,miR-146a and 25-(OH)D3 all have cer-tain predictive values for the prognosis of children with RSV bronchiolitis,and the combined predic-tive value of the three is higher.
2.Progress of application of fully immersive virtual reality technology in limb function rehabilitation of stroke patients

Stroke is characterized by a high disability rate. After stroke, patients will have different degrees of limb dysfunction, which seriously affects their quality of life. In recent years, fully immersive virtual technology has been gradually applied at home and abroad to promote limb function rehabilitation of stroke patients. This paper reviews the technical components, application forms, application effects, prospects and challenges of fully immersive virtual reality technology in limb function rehabilitation of stroke patients, in order to provide theoretical basis for the subsequent application of fully immersive virtual reality technology to improve the level of limb function rehabilitation of stroke patients.
3.Comparative analysis of social skills and problem behaviors between children and adolescents with ASD and ADHD

Objective:To explore the characteristics of social skills and problem behaviors of children and adolescents with autism spectrum disorder (ASD) and attention deficit hyperactivity disorder (ADHD), as well as the association with core symptoms.Methods:A total of 409 patients aged 5-18 years old with ASD or ADHD in the outpatient department of Nanjing Brain Hospital from 2023 to 2024, and 344 children and adolescents with typical development(TD) were recruited.All participants were matched in a ratio of 1∶1∶1 (ASD∶ADHD∶TD) according to gender and age, and 97 participants were included in each group for analysis.The Chinese version of the social skills improvement system rating scales(SSIS-RS-C) was used to evaluate social skills and problem behaviors, and autistic child behavior checklist(ABC), childhood autism rating scale(CARS), the Chinese version of the social communication questionnaire(SCQ) and the Chinese version of Swanson, Nolan, and Pelham, version Ⅳ scale-parent form(SNAP-Ⅳ) were used to evaluate the core symptoms of ASD and ADHD, respectively. SPSS 26.0 software was used to perform variance, Chi-square test, Spearman correlation analysis and multivariate Logistic regression analysis.Results:The social skills score of ASD group was lower than ADHD group ((61.53±24.26) vs (80.89±15.19), P<0.05), while the problem behavior score of ASD group was higher than ADHD group ((38.82±11.92) vs (34.00±12.45), P<0.05). In ASD group, the scores of ABC, CARS and SCQ were negatively correlated with the score of social skills ( r=-0.26--0.55, P<0.05). In ADHD group, the total score and each subscale of SNAP-Ⅳ were positively correlated with the score of problem behavior ( r=0.25-0.65, P<0.05). Multivariate Logistic regression analysis showed that empathy was a negative influencing factor of ASD ( B=-0.246, OR=0.782, P<0.05), and hyperactivity/inattention was a positive influencing factor of ASD ( B=0.589, OR=1.802, P<0.01), while only hyperactivity/inattention was a positive influencing factor of ADHD( B=0.779, OR=2.180, P<0.01). Conclusion:Children and adolescents with ASD and ADHD both have defects in social skills and problem behaviors, and these defects are associated with the core characteristics of their respective diseases.

Methods:
A total of 24 AIS patients and 25 healthy controls were recruited. The limits of stability (LOS) test were conducted to assess balance function, while surface electromyography was used to measure the activity of core muscles, including the internal oblique, external oblique, and multifidus. Diaphragm thickness was measured using ultrasound during different postural tasks. Center of pressure (COP) displacement and trunk inclination distance were also recorded during the LOS test.
Results:
AIS patients showed significantly greater activation of superficial core muscles, such as the internal and external oblique muscles, compared to the control group (p < 0.05). Diaphragm activation was lower in AIS patients during balance tasks (p < 0.01). Although no significant difference was observed in COP displacement between the groups, trunk inclination was significantly greater in the AIS group during certain tasks (p < 0.05).
Conclusion
These findings suggest distinct postural control patterns in AIS patients, highlighting the importance of targeted interventions to improve balance and core muscle function in this population.

6.Evaluation of Anti-osteoporosis Activity and Hepatotoxicity of Xianling Gubao Based on Zebrafish Model

ObjectiveTo investigate the association and translational mechanism between the hepatotoxicity of Xianling Gubao (XLGB) and its treatment of osteoporosis based on a zebrafish model. MethodsZebrafish were randomly selected four days after fertilization (4 dpf) and exposed to different concentrations of XLGB (0.7,0.35 mg·L-1) for 96 h. At the endpoint of the exposure, the mortality rates of zebrafish in the treatment groups of different concentrations were counted, and the "dose-toxicity" curves were plotted. The 10% sublethal concentration (LC10) was calculated. The liver area, acridine orange staining, and pathological tissue sections of transgenic zebrafish [CZ16 (gz15Tg.Tg (fabp 10a: ds Red; ela31: EGFP)] were used as indicators to confirm the hepatic damage caused by the sublethal concentration of XLGB. By using the prednisolone (PNSL)-induced osteoporosis model of zebrafish, the anti-osteoporosis activity of XLGB was evaluated by using the area of skull stained by alizarin red and the cumulative optical density value as indicators. Then, the toxicity difference of XLGB on the liver of zebrafish in healthy and osteoporotic states was compared, and the mechanism of the translational action of the toxicity of XLGB was predicted based on network pharmacology and real-time polymerase chain reaction(Real-time PCR). ResultsThe LC10 of XLGB on zebrafish (8 dpf) was 0.7 mg·L-1. Compared with the blank group, the sublethal concentration (LC10=0.7 mg·L-1, 1/2 LC10=0.35 mg·L-1) of XLGB induced an increase in the number of apoptosis of hepatocytes in a dose-dependent manner, and the tissue arrangement of the liver was disordered and loose. The vacuoles were obvious, and the fluorescence area of the liver was significantly reduced (P<0.01). Compared with the blank group, the mineralized area and cumulative optical density value of zebrafish skull in the PNSL model group were significantly reduced (P<0.01), and those in the 0.7,0.35 mg·L-1 XLGB treatment group were significantly increased compared with the model group (P<0.01). Most importantly, 0.7 mg·L-1 XLGB had no significant effect on the liver of zebrafish in the osteoporosis disease model compared with the blank group. The results of network pharmacology and real-time PCR experiments showed that the toxic transformation of XLGB might be related to the differences in the expression levels of key targets, such as tumor protein 53 (TP53), cysteine aspartic acid specific protease-3(Caspase-3), interleukin(IL)-6, and alkaline phosphatase(ALP) in different organismal states. ConclusionUnder certain conditions, XLGB has hepatotoxicity in normal zebrafish, but under osteoporotic conditions, XLGB not only exerts significant anti-osteoporosis activity but also alleviates hepatotoxicity significantly, which provides a reference for the safe clinical use of XLGB and real evidence for the theories of traditional Chinese medicine of attacking poison with poison and of treating disease with corresponding drugs without damage to the body.

10.Clinical features of ulcerative colitis in patients with small intestinal bacterial overgrowth

Objective:To explore the relationship between intestinal bacterial overgrowth (SIBO) and ulcerative colitis (UC).Methods:From December 2023 to June 2024, 85 patients with UC from the Gastroenterology Department of the Luquan branch of the Second Hospital of Hebei Medical University were enrolled. The lactulose hydrogen-methane breath test was performed to assess the prevalence of SIBO. Clinical data, including basic information, clinical manifestations, endoscopic manifestations, inflammatory indicators, current medication regimen, and past medical history, were collected. Furthermore, the body mass index (BMI), modified Mayo score, and patient-reported outcome (PRO2) score were calculated to evaluate disease activity in each patient. The Student′s t-test, Chi-square test, non-parametric test, and multiple logistic regression were used to analyze the data and explore the relationship between SIBO and UC. Results:The incidence of abdominal pain and bloating in patients who were SIBO positive with UC was higher than in those who were SIBO negative [abdominal pain: 50.0%(10/20) vs. 23.1%(15/65), χ2=5.34, P=0.021; abdominal distension: 40.0% (8/20) vs. 13.8% (9/65), χ2=5.01, P=0.025]; the difference was statistically significant ( P<0.05). Patients who were SIBO positive with UC were more likely to develop hypoproteinemia and anemia than those who were SIBO negative [hypoproteinemia: 50.0% (10/20) vs. 15.4% (10/65), χ2=8.35, P=0.004; anemia: 35.0% (7/20) vs. 9.2% (6/65), χ2=5.98, P=0.014]; the difference was statistically significant ( P<0.05). In the intestinal methanogen overgrowth (IMO) positive group, the number of patients with UC with 1-2 stool times/day was higher, and the distribution of stool times between the IMO positive and IMO negative groups was significantly different ( χ2=6.45, P=0.040). Furthermore, combined hypoproteinemia and anemia were risk factors for SIBO in patients with UC (hypoproteinemia OR=4.331, 95% CI 1.117-16.799, P=0.034; anemia OR=5.515, 95% CI 1.231-24.700, P=0.026). Conclusions:We observed a clinical overlap between SIBO and UC. SIBO could be targeted to optimize the treatment of patients with UC in the future.
