The prescription of Qidong Yixin oral liquid is derived from the experience of national medical master Ren Jixue in treating viral myocarditis (VMC). It has the functions of tonifying Qi, nourishing the heart, calming the mind, and relieving palpitations. It is used to treat VMC and angina pectoris of coronary heart disease caused by deficiency of both Qi and Yin. However, the understanding of its efficacy evidence, advantageous aspects, dosage and administration, and medication safety remains insufficient in clinical practice. Therefore, the development of the Expert Consensus on the Clinical Application of Qidong Yixin Oral Liquid (hereinafter referred to as consensus) was initiated. Consensus strictly followed the process and methods of the expert consensus on the clinical application of Chinese patent medicines of the China Association of Chinese Medicine, successively completing multiple tasks such as the consensus project initiation, determination of clinical problems, evidence search and evaluation, formation of recommendation opinions and consensus suggestions, solicitation of opinions, peer review, submission for review and release, and so on. Consensus formed a total of 10 recommendation opinions and 12 consensus suggestions, clarifying the clinical positioning, efficacy advantages, syndrome differentiation, dosage and administration, combination therapy, timing of medication, adverse reactions, contraindications, and precautions of Qidong Yixin oral liquid, indicating that it has good clinical advantages and safety in the treatment of VMC and angina pectoris of coronary heart disease, providing norms and references for physicians to safely and rationally apply Qidong Yixin oral liquid. Consensus was reviewed and approved for release by the Standardization Office of the China Association of Chinese Medicine on December 23, 2024. Standard number:GSCACM-376-2024.

Objective::To examine the impact of hypertensive disorders of pregnancy (HDP) on offspring metabolomics.Methods::We searched five databases: PubMed, Ovid Embase, MEDLINE, Web of Science, and China National Knowledge Infrastructure, and included studies that reported metabolomics among human offspring born to HDP-complicated pregnancies.Results::Database search yielded 4054 articles, and after full-text screening, ten observational studies met inclusion criteria. Half of the studies had a sample size of less than 100 and were all observational studies in preeclampsia (PE) and gestational hypertension.Neonates were the most focused group in all included studies. Offspring born to HDP-complicated pregnancies exhibited statistically significant variations in blood metabolomics compared to their counterparts, characterized by amino acids, lipids, carnitine, and others (e.g., 1α,25-(OH) 2-D). Most studies reported a significant increase in differential metabolites of offspring born to HDP-complicated pregnancies. Four studies ( n = 1109) measured lipids-related metabolites, and all consistently showed that offspring born to PE-complicated pregnancies had significantly higher concentrations than non-PE exposed offspring. Conclusion::The existing evidence suggests an intergenerational effect of HDP on offspring metabolomics. Long-term follow-up studies are needed to advance the health effects of related adverse health outcomes and inform the prevention of offspring’s health.

Adverse Drug Reactions Journal 2025;27(4):238-244
Neuroinflammatory response runs through the pathological process of epilepsy, and the regulation of proinflammatory factors such as (tumor necrosis factor-α, TNF-α), (interleukin, IL)-1β, IL-6 and Toll-like receptors 4/nuclear factor-kappa B signaling pathways may help improve epilepsy symptoms. Traditional Chinese medicines with anti-epileptic effects can be mainly divided into the following 6 categories: Pinggan Xifeng drugs (平肝息风药, liver-calming and wind-extinguishing herbs), Qingre drugs (清热药, heat-clearing drugs), Huatan drugs (化痰药, phlegm-relieving drugs), Huoxue Huayu drugs (活血化瘀药, circulation-promoting and stasis-removing drugs), Buxu drugs (补虚药, tonifying drugs), and Kaijiao drugs (开窍药, consciousness-restoring drugs). The active ingredients contained in these traditional Chinese medicines can play a therapeutic role in epilepsy treatment by regulating the release of key proinflammatory factors and inflammatory signaling pathways. Adverse reactions caused by Chinese patent medicines involve a variety of factors, which can be summarized into the process of medicinal materials preparation, the principle of traditional Chinese medicine compatibility, the accuracy of dialectical treatment, individual differences, etc. When using anti-epileptic Chinese medicine in clinical practice, it should be flexibly applied according to specific conditions to achieve the goal of precise treatment.

Objective This study sought to establish a non-alcoholic fatty liver disease(NAFLD)model in Wistar-SD hypercholesterolemia(WSHc)rats induced by a high-fat diet and to reveal the pathogenesis of NAFLD in these rats through the Srebp-1 gene. Methods After 2 weeks of dietary treatment, thirty 6-week-old WSHc rats were divided into High-fat control group,HFD+AAV no load group, and HFD+AAV group, with 10 rats in each group. The HFD+AAV no load group and HFD+AAV group were intravenously injected with a vector virus and an shRNA-containing virus, respectively.WSHc rats were fed with a normal fat diet as a normal control group.Serum levels of ALT,AST,TBIL,ALP,TBA,GLU,CHOL,and TG were measured every 2 weeks. After a further 8 weeks of feeding, the rats were euthanized and livers were excised for HE staining,Oil Red O staining,Masson staining, and Sirius red staining to observe the morphology, lipid deposition, and fibrosis of the liver tissues.RT-qPCR was performed to detect the expression of lipid metabolism-related genes namely Srebp-1,Aacs,FASN and LDLR in the livers.Furthermore, hepatocytes were isolated, cultured, and divided into a normal control group and a high-fat control group.Next, expression of the Srebp-1 gene was detected by RT-qPCR.Srebp-1 knockout(KO)hepatocytes were constructed, then TG content was detected and the lipid accumulation was observed by Oil Red O staining. Results After 10 weeks of high-fat diet treatment, serum ALT(P<0.001),ALP(P<0.001),TBA(P<0.05),GLU(P<0.001),and CHOL(P<0.001)significantly increased in WSHc rats.Abnormal lipid deposition with formation of large vacuolar lipid droplets and fibrotic lesions in livers were observed. The mRNA expression of Srebp-1 noticeably increased in WSHc rats(P<0.001).Moreover,compared with the high-fat control group, the ALT(P<0.05)and GLU(P<0.01)in the HFD+AAV group decreased, and liver lipid deposition and the formation of large vacuolar lipid droplets were alleviated.Expressions of genes such as FASN(P<0.05)and LDLR(P<0.01)were significantly upregulated.Additionally, there was a significant increase in the expression of Srebp-1 in hepatocytes of the high-fat control group(P<0.001),while after Srebp-1 gene knockout, cellular TG levels decreased and the degree of lipid droplet aggregation was reduced. Conclusions The Srebp-1 gene plays a regulatory role in hepatic lipid metabolism and deposition, modulating the expression of lipid metabolism-related genes in WSHc rats with NAFLD.In vitro experiments demonstrated that downregulation of Srebp-1 alleviates lipotoxic injury in hepatocytes, suggesting that the development of NAFLD in WSHc rats is closely associated with abnormally high expressions of the Srebp-1 gene.
