1.Analysis of influencing factors for successful detoxification in patients undergoing methadone maintenance therapy

OBJECTIVE To explore the influencing factors for successful detoxification in patients undergoing methadone maintenance therapy. METHODS A retrospective selection of 161 methadone maintenance therapy patients from the South Branch of Zhongshan Second People’s Hospital (including methadone maintenance treatment sites in Shiqi District, Xiqu District, Development Zone of Zhongshan City) from January 1, 2012, to January 1, 2025, was conducted as the study object. Data collected included patients’ sociodemographic information, drug abuse history, laboratory test results, medication records, etc. Patients were divided into the unsuccessful detoxification group and the successful detoxification group based on whether methadone detoxification was achieved. Univariate, univariate Cox regression, and multivariate Cox proportional hazards regression were used for influencing factor analysis, and the Kaplan-Meier method was employed for survival analysis. RESULTS Among the 161 methadone maintenance therapy patients, 96 were in the successful detoxification group and 65 in the unsuccessful detoxification group, yielding a successful detoxification rate of 59.63%. Multivariate Cox proportional hazards regression analysis revealed that age, registered residence status, age at first drug use, and duration of drug abuse were key influencing factors for successful detoxification in methadone maintenance therapy patients (P<0.05). Specifically, the successful detoxification rate for patients with Zhongshan local registered residence was 8.364 times higher than that for patients with non-local registered residence; for every 1-year increase in patient age, the successful detoxification rate decreased by 22.7%; for every 1-year increase in age at first drug use, the successful detoxification rate rose by 33.4%; and for every 1-year increase in duration of drug abuse, the successful detoxification rate increased by 33.5%. Survival analysis showed that the successful detoxification rate in the methadone low-dose group (≤30.8 mg) was significantly higher than that in the methadone high-dose group (>30.8 mg) (P=0.015), and the successful detoxification rate in the population with Zhongshan local registered residence was significantly higher than that in those with non-local registered residence (P<0.001). CONCLUSIONS Age, registered residence status, age at first drug use, and duration of drug abuse are key influencing factors for successful detoxification in patients undergoing methadone maintenance therapy, and the last methadone dose may be associated with successful detoxification.
2.Consensus on the use of DSA-guided intrathecal drug delivery system combined with Zi Wu Liu Zhu Acupoint Therapy for cancer pain management

Objective To explore the efficacy of DSA-guided intrathecal drug delivery system combined with Zi Wu Liu Zhu Acupoint Therapy for management of cancer pain and provide reference for its standardized clinical application.Methods and Results Recommendations were formulated based on literature review and expert group discussion,and consensus was reached following expert consultation.The consensus recommendations are comprehensive,covering the entire treatment procedures from preoperative assessment and preparation,surgical operation process,postoperative management and traditional Chinese medicine treatment to individualized treatment planning.The study results showed that the treatment plans combining traditional Chinese with Western medicine effectively alleviated cancer pain,reduced the use of opioid drugs,and significantly improved the quality of life and enhanced immune function of the patients.Postoperative follow-up suggested good treatment tolerance among the patients without serious complications.Conclusion The formulated consensus is comprehensive and can provide reference for clinicians to use DSA-guided intrathecal drug delivery system combined with Zi Wu Liu Zhu Acupoint Therapy.The combined treatment has a high clinical value with a good safety profile for management of cancer pain.
3.The constituent elements, experiences, and popularization significance of the palliative care model of integrated elderly care and medical services

Under the trend of increasing aging, integrated elderly care and medical services is an important measure to optimize the supply of elderly care services and promote the good death of the elderly. Using the cooperative production theory and the classical grounded theory, a qualitative analysis was conducted on 38 cases of elderly palliative care and 25 cases of hospital-based palliative care under the integrated elderly care and medical services model from a hospital in Nanning City using Nvivo 20.0 software. This paper found that the integrated elderly care and medical services mode emphasized the deep integration of medical and elderly care services by integrating resources and improving service efficiency, to achieve the basic experience of comprehensive health care for the elderly. The promotion of these experiences has a positive significance for building a multi-agent cooperative production system, strengthening personnel training, perfecting the performance distribution mechanism, and further promoting the development of the national palliative care pilot.
4.Guangxi expert consensus on rapid comprehensive evaluation of oral dihydropyridine calcium channel blockers in the treatment of cardiovascular disease

OBJECTIVE To conduct a rapid and comprehensive evaluation of commonly used oral dihydropyridine calcium channel blockers (DHP CCBs) in Guangxi, and provide scientific basis for clinical medication and drug selection in medical institutions. METHODS Based on the actual drug use data of public medical institutions at the second level and above in Guangxi Zhuang Autonomous Region, and based on the national centralized collection catalog, commonly used oral DHP CCBs were selected. The Professional Committee of Evidence-based Pharmacy of the Guangxi Pharmaceutical Association organized relevant experts from multiple medical institutions in the region to conduct a quantitative scoring of the selected oral DHP CCBs from five key dimensions of pharmaceutical characteristics, effectiveness, safety, economy, and other attributes, by referring to the Quick Guideline for Drug Evaluation and Selection in Chinese Medical Institutions (the Second Edition), combined with the latest evidence-based medical research results, and widely soliciting suggestions from clinical and pharmaceutical experts. Finally, the Guangxi Expert Consensus on Rapid Comprehensive Evaluation of Oral Dihydropyridine Calcium Channel Blockers in the Treatment of Cardiovascular Disease was formulated. RESULTS A total of 30 commonly used oral DHP CCBs were selected. Among them, Amlodipine besylate tablets (Huizhi) demonstrated superior comprehensive performance (81.79 points), excelling particularly in pharmaceutical properties, effectiveness and other attributes. The compound scores of Amlodipine besylate tablets (Chongqing Yaoyou) and Amlodipine besylate tablets (Suzhou Dongrui) ranked second and third respectively, with scores of 81.66 and 81.60 points. CONCLUSIONS This consensus can provide guidance and decision-making support for the rational clinical application of oral DHP CCBs in Guangxi Region and the selection of drug directories in medical institutions.

BACKGROUND:
The available evidence regarding the benefits of percutaneous coronary intervention (PCI) on patients receiving dialysis with coronary artery disease (CAD) is limited and inconsistent. This study aimed to evaluate the association between PCI and clinical outcomes as compared with medical therapy alone in patients undergoing dialysis with CAD in China.
METHODS:
This multicenter, retrospective study was conducted in 30 tertiary medical centers across 12 provinces in China from January 2015 to June 2021 to include patients on dialysis with CAD. The primary outcome was major adverse cardiovascular events (MACE), defined as a composite of cardiovascular death, non-fatal myocardial infarction, and non-fatal stroke. Secondary outcomes included all-cause death, the individual components of MACE, and Bleeding Academic Research Consortium criteria types 2, 3, or 5 bleeding. Multivariable Cox proportional hazard models were used to assess the association between PCI and outcomes. Inverse probability of treatment weighting (IPTW) and propensity score matching (PSM) were performed to account for potential between-group differences.
RESULTS:
Of the 1146 patients on dialysis with significant CAD, 821 (71.6%) underwent PCI. After a median follow-up of 23.0 months, PCI was associated with a 43.0% significantly lower risk for MACE (33.9% [ n = 278] vs . 43.7% [ n = 142]; adjusted hazards ratio 0.57, 95% confidence interval 0.45-0.71), along with a slightly increased risk for bleeding outcomes that did not reach statistical significance (11.1% vs . 8.3%; adjusted hazards ratio 1.31, 95% confidence interval, 0.82-2.11). Furthermore, PCI was associated with a significant reduction in all-cause and cardiovascular mortalities. Subgroup analysis did not modify the association of PCI with patient outcomes. These primary findings were consistent across IPTW, PSM, and competing risk analyses.
CONCLUSION
This study indicated that PCI in patients on dialysis with CAD was significantly associated with lower MACE and mortality when comparing with those with medical therapy alone, albeit with a slightly increased risk for bleeding events that did not reach statistical significance.

6.TRIM25 inhibits Japanese encephalitis virus replication in U251 cells by up-regulation of the IFN-β and degrading the viral capsid protein

Objective:To investigate the inhibitory effect of tripartite motif-containing 25 (TRIM25) on the replication of Japanese encephalitis virus (JEV) in cells and its molecular mechanism.Methods:Human glioma cells (U251 cells) and Kunming mice were infected with JEV, and then the cells and brain tissue samples were collected. The transcription levels of six TRIM genes were detected by real-time PCR, and the expression of TRIM25 in cells was detected by Western blot. U251 and A549 cells overexpressed with TRIM25 and U251 cells knocked out with TRIM25 gene were constructed. Cells were infected with JEV, and the replication of JEV was detected by viral plaque assay, real-time PCR and Western blot. The interaction of TRIM25 with viral proteins was investigated by co-immunoprecipitation (Co-IP) and indirect immunofluorescence assay. The expression of IFN-β in overexpressed TRIM25 cells was detected by real-time PCR and ELISA.Results:JEV infection promoted the expression of TRIM25 in cells and mouse brain tissues. TRIM25 overexpression restricted JEV replication in U251 and A549 cells, while TRIM25 knockout enhanced JEV replication. TRIM25 overexpression upregulated the level of IFN-β in cells. TRIM25 interacted with JEV capsid protein and promoted the degradation of capsid protein.Conclusion:TRIM25 can inhibit the replication of JEV in cells by upregulating IFN-β and promoting the degradation of JEV C protein.
7.Construction of a competency-oriented psychosomatic medical cognitive ability model for medical students

Objective:To construct a competency-oriented psychosomatic medical cognitive ability model for medical students, and provide theoretical basis for practical research on improving their psychosomatic medical cognitive abilities.Methods:Based on a literature review and behavioral event interviews with 20 medical students, a 23-item questionnaire was developed to assess the cognitive abilities of medical students in psychosomatic medicine. In October 2023, a stratified random sampling method was used to select third- and fourth-year undergraduates from the affiliated clinical college of a medical university for a questionnaire survey on the Questionnaire Star platform. SPSS 22.0 software was used for item analysis, internal consistency analysis, and exploratory factor analysis. AMOS 21.0 software was employed for confirmatory factor analysis.Results:Based on the reliability and validity analysis of the questionnaire, two items were removed, and a competency-oriented psychosomatic medical cognitive ability model for medical students was constructed. The model consisted of 21 items in the three dimensions of knowledge, skill, and attitude.Conclusions:The competency-oriented psychosomatic medical cognitive ability model for medical students shows high reliability and validity in statistical analysis, thereby contributing to the cultivation of their psychosomatic medical cognitive abilities.
8.Investigating the Medication Patterns of WU's Tumor School for Anti-metastasis Through Enhancing Physical Health and Pro-moting Detoxification Based on Data Mining

[Objective]To explore the medication patterns of WU's tumor school for anti-metastasis through"enhancing health and promoting detoxification"based on data mining.[Methods]Systematically review the literature related to the prescription and medication patterns of WU Liangcun in the treatment of tumor metastasis,and summarize his clinical experience in the use of the"enhancing health and promoting detoxification"method.Complete case records of lung cancer,gastric cancer and colorectal cancer metastasis treated by SHEN Minhe,WANG Binbin and RUAN Shanming from January 1,2020 to August 31,2023 were collected.Data mining analysis was performed on the effective prescriptions by using the Traditional Chinese Medicine Inheritance Computer System(V3.5).[Results]A total of 3 023 prescriptions from SHEN Minhe were included,involving 344 types of Chinese herbs.Codonopsis Radix was frequently used,and common herb pairs included Codonopsis Radix,Poria cocos-Atractylodis Macrocephalae Rhizoma.WANG Binbin's 2 654 prescriptions involved 370 types of Chinese herbs.Paeoniae Radix Alba was frequently used,and common pairs included Coicis Semen,Fagopyri Dibotryis Rhizoma-Hedyotis diffusa Willd.RUAN Shanming's 1 182 prescriptions included 257 types of herbs.Astragali Radix was frequently used,and common pairs included Astragali Radix,Alismatis Rhizoma-Scutellariae Radix.WU's tumor school prefers to use herbs with sweet,bitter and acrid flavors,as well as neutral,cold and warm properties,avoiding excessive purgation or tonification and extreme cold or heat.They advocated for gentle tonification and gradual adjustment,as well as a balanced combination of cold and heat.In terms of meridian tropism,they focused on using herbs that entered the spleen and lung meridians,targeting the pathogenesis of spleen and stomach deficiency,which led to the accumulation of dampness and phlegm,emphasized the simultaneous regulation of the spleen and lung,to strengthen the spleen and stomach,eliminate dampness and phlegm,regulate the flow of Qi and clear cancerous toxins.[Conclusion]WU's tumor school has been passed down for three generations,following the principle of"enhancing health and promoting detoxification".The overall medication is balanced and tailored to different types of malignant tumors,demonstrating significant clinical efficacy.
9.Predictive value of refeeding syndrome and its influencing factors for 30-day intensive care unit readmission in critically ill patients

Objective:To investigate the predictive value of refeeding syndrome (RFS) and its influencing factors for 30-day intensive care unit (ICU) readmission in critically ill patients.Methods:A prospective cohort study was conducted. Critically ill patients admitted to the department of critical care medicine, department of respiratory and critical care medicine, and department of neurology at Tianjin Medical University General Hospital from January to April in 2025 were enrolled. Patients were assessed for RFS according to the American Society for Parenteral and Enteral Nutrition (ASPEN) criteria. General information within 24 hours of ICU admission was collected via the electronic medical record system. Treatment details and 30-day ICU readmission status were dynamically recorded. Participants were divided into readmission and non-readmission groups based on whether ICU readmission occurred within 30 days. Intergroup comparisons were performed to identify differences. Multivariate Logistic regression was used to analyze the relationship between RFS and its influencing factors with 30-day ICU readmission. Receiver operator characteristic curve (ROC curve) was plotted to evaluate the predictive performance of risk factors.Results:A total of 196 critically ill patients were enrolled, among whom 25 (12.76%) were readmitted to ICU within 30 days and 171 (87.24%) were not. Significant differences were observed in the readmission group compared with the non-readmission group, including significantly higher rates of nasogastric decompression, higher acute physiology and chronic health evaluation Ⅱ (APACHEⅡ) score, a higher incidence of RFS, and a longer duration of nasogastric decompression. Multivariate Logistic regression analysis showed that RFS was an independent risk factor for 30-day ICU readmission [odds ratio ( OR) = 5.756, 95% confidence interval (95% CI) was 1.603-20.670, P = 0.007]. APACHEⅡ score showed a positive correlation trend with 30-day ICU readmission ( OR = 1.057, 95% CI was 0.991-1.127, P = 0.092). ROC curve analysis showed that the combined prediction model incorporating RFS and APACHEⅡ score had an area under the ROC curve (AUC) of 0.766 (95% CI was 0.668-0.864), with a sensitivity of 88.0% and a specificity of 62.0%, which was significantly superior to a single indicator (the AUC of RFS and APACHEⅡ score was 0.639 and 0.624, respectively). Conclusions:RFS significantly increases the risk of 30-day ICU readmission in critically ill patients. A combined model incorporating RFS and APACHEⅡ score demonstrates good predictive efficacy for 30-day ICU readmission in critically ill patients.
10.Shear wave elastography for assessing acute cold exposure-induced changes in rabbit sciatic nerves

Objective:To quantitatively evaluate the changes of the sciatic nerve of rabbits following acute cold exposure and intraperitoneal lavage rewarming therapy by applying shear wave elastography(SWE)technology.Methods:Thirty healthy adult New Zealand white rabbits,weighing approximately(4.0 ± 0.2)kgResults:Body temperature comparison:the body temperature of all 3 groups decreased at T2 compared to T1,with statistically significant differences(all P<0.05). By T4,the body temperature of all groups had returned to normal,showing no significant differences compared to T1( P>0.05). The peritoneal lavage + rewarming platform group showed a faster recovery rate in body temperature during T2-T3 compared to both the rewarming platform group and the blank control group,with statistically significant differences(all P<0.05). There were no significant differences in the recovery rate between the rewarming platform group and the blank control group during T2-T3(all P>0.05). Comparison of conventional ultrasound parameters:the longitudinal width,cross-sectional area,and perimeter of the sciatic nerve showed statistically significant differences between T2 and T1(all P<0.05). No significant differences were observed between T3 and T4,or between T1 and T4(all P>0.05). Comparison of SWE parameters:T2 phase showed an increase in SWE values of the sciatic nerve,with statistically significant differences in the maximum(Emax)and average(Emean)Young's modulus compared to T1 and T4(all P<0.05). No significant differences were observed between T1 and T4(all P>0.05). Transmission electron microscopy results:under acute cold exposure,the sciatic nerve showed swelling,with vacuole formation visible in the axons. After peritoneal lavage rewarming therapy,the morphology of the sciatic nerve returned to normal. Conclusions:Following acute cold exposure,the rabbit's sciatic nerve exhibits contraction,with increased stiffness,which can be quantitatively assessed using conventional ultrasound combined with SWE. Peritoneal lavage therapy is an effective rewarming method that significantly enhances the rewarming rate.
