BACKGROUND:
The role of inflammation in the development of gestational diabetes mellitus (GDM) has recently become a focus of research. The systemic immune-inflammation index (SII) and systemic inflammation response index (SIRI), novel indices, reflect the body's chronic immune-inflammatory state. This study aimed to investigate the associations between the SII or SIRI and GDM.
METHODS:
A prospective birth cohort study was conducted at Beijing Obstetrics and Gynecology Hospital from February 2018 to December 2020, recruiting participants in their first trimester of pregnancy. Baseline SII and SIRI values were derived from routine clinical blood results, calculated as follows: SII = neutrophil (Neut) count × platelet (PLT) count/lymphocyte (Lymph) count, SIRI = Neut count × monocyte (Mono) count/Lymph count, with participants being grouped by quartiles of their SII or SIRI values. Participants were followed up for GDM with a 75-g, 2-h oral glucose tolerance test (OGTT) at 24-28 weeks of gestation using the glucose thresholds of the International Association of Diabetes and Pregnancy Study Groups (IADPSG). Logistic regression was used to analyze the odds ratios (ORs) (95% confidence intervals [CIs]) for the the associations between SII, SIRI, and the risk of GDM.
RESULTS:
Among the 28,124 women included in the study, the average age was 31.8 ± 3.8 years, and 15.76% (4432/28,124) developed GDM. Higher SII and SIRI quartiles were correlated with increased GDM rates, with rates ranging from 12.26% (862/7031) in the lowest quartile to 20.10% (1413/7031) in the highest quartile for the SII ( Ptrend <0.001) and 11.92-19.31% for the SIRI ( Ptrend <0.001). The ORs (95% CIs) of the second, third, and fourth SII quartiles were 1.09 (0.98-1.21), 1.21 (1.09-1.34), and 1.39 (1.26-1.54), respectively. The SIRI findings paralleled the SII outcomes. For the second through fourth quartiles, the ORs (95% CIs) were 1.24 (1.12-1.38), 1.41 (1.27-1.57), and 1.64 (1.48-1.82), respectively. These associations were maintained in subgroup and sensitivity analyses.
CONCLUSION
The SII and SIRI are potential independent risk factors contributing to the onset of GDM.

Efficient bacterial drug susceptibility test can help precise guidance on using antimicrobial and improve the efficiency of development of antibiotics.Traditional antimicrobial susceptibility testing(AST)methods are simple, reliable and specific, but suffer from drawbacks of time-consuming.The CCK8 reagent can be reduced by the dehydrogenase in the bacterial cells to form a water-soluble orange-yellow formazan, and the concentration of viable bacteria is proportional to the absorbance of formazan at 450 nm(OD450 nm).In this work, based on the principle of CCK8 colorimetry, the antibiotic loaded composite fiber membrane was used as the culture matrix for AST.Simple, rapid and high-throughput AST was realized by the difference in OD450 nm,which reflected the differences in viable bacteria quantity during bacterial growth. The total detection time was less than 8 h.The potential application of the method was evaluated using S.aureus,P.aeruginosa in wound simulation fluid and E.coli in artificial urine as simulated actual samples. The results of this method were in consistent with the disk diffusion test. This method provided a new idea for AST,which was helpful for the guidance of clinical medication and the development of antibiotics.

A rapid quantitative immunochromatographic assay for procalcitonin(PCT)using metal-organic frameworks modified with gold and platinum nanoparticles(MAPs)as labels was established in this work. The detection probe was prepared by conjugating MAPs with anti-PCT monoclonal antibody via an electrostatic adsorption method.Anti-PCT polyclonal antibody and sheep anti-mouse IgG were sprayed onto the nitrocellulose(NC)membrane as the test line and quality control line, respectively,to construct immunochromatographic strip for PCT quantitative detection via signal-amplification-based sandwich immunoassay. The results showed that the MAP-based immunochromatographic test had high sensitivity, high specificity, and good stability. The dynamic range for detection of PCT was 0.61 pg/mL-320 ng/mL,the detection limit was 0.25 pg/mL,and the intra-day and inter-day precision(Relative standard deviation)were less than 15%.The results of real sample analysis showed that a quite low volume of sample was required for detection of PCT in whole blood, which was of great significance for the early diagnosis, monitoring and treatment, and prognosis of inflammation.

Objective lo contrast the diagnostic efficacies of perineal and transrectal approaches for combined prostate biopsies guided via magnetic resonance imaging(MRI)and ultrasound fusion imaging.Method A retrospective analysis of clinical data from 271 patients subjected to combined prostate biopsies guided by MRI and ultrasound fusion imaging at the first people's hospital of Wuxue city and Wuhan Tongji Hospital from January 2022 to November 2023 was conducted. They were divided into two groups according to the puncture path,132 cases were transrectal and 139 cases were perineal. The differences between the detection rates of prostate cancer(PCa)and clinical significant prostate cancer(csPCa)across different prostate imaging reporting and data system(PI-RADS)scores and lesion localizations within the two puncture paths were compared. Results The detection rates of PCa in the perineal and transrectal puncture groups(68.3%vs.59.8%,P=0.145)and csPCa(64.7%vs.55.3%,P=0.112)did not exhibit statistically significant differences. However, the perineal puncture group experienced fewer complications. For patients with a PI-RADS score of 4,both PCa(77.2%vs.56.1%,P=0.027)and csPCa(71.9%vs.48.8%,P=0.02)detection rates were higher via the perineal combined biopsy),compared to the transrectal path.Additionally, for apical(PCa:84.3%vs.48.7%,P=0.001;csPCa:81.8%vs.48.7%,P=0.004)and anterior(PCa:77.1%vs.46.5%,P=0.006;csPCa:68.6%vs.44.2%,P=0.031)prostate lesions, the detection rates via the perineal combined biopsy were considerably higher than those of the transrectal path. Conclusion For patients carrying a PI-RADS score of 4 or with prostate lesions situated at the apical or anterior section, the perineal approach for biopsy sampling emerges as the most appropriate method.
