OBJECTIVE To explore the molecular characteristics of small colony variants(SCVs)of Staphylococcus aureus isolates so as to provide theoretical bases for clinical control of the persistent and recurrent infections in-duced by the SCVs.METHODS The clinical blood specimens that were collected from the First Affiliated Hospital of Traditional Chinese Medicine University of Guangzhou were cultured to acquire 1 strain of SCV S.aureus S2.The colonial morphology,growth rate,catalase,plasma-coagulase,production capability of biofilm and antimi-crobial susceptibility of the strain SCVs were observed,the growth curve and autolysis curve were drawn,the whole genome sequencing was performed for the S2,the phylogenetic evolution of the genome and the single nu-cleotide polymorphism(SNP)were analyzed.RESULTS The S2 was identified as S.aureus,with the catalase and plasma-coagulase tested positive.The strain was resistant to various types of antibiotics,including macrolides like erythromycin and azithromycin,fluroquinolones such as levofloxacin and ciprofloxacin,and penicillin.The forma-tion of pigment was reduced,the formation of hemolytic rings was low;the growth rate of the strain was slower than that of ATCC 25923(quality control strain)and ZC1(the normal control strain),the autolytic activity of the strain was higher than that of the ATCC 25923 and ZC1,and the biofilm production of the strain was remarkably less than that of the ATCC 29213(P＜0.05).The whole genome sequencing demonstrated that the molecular typ-ing of the S2 was ST239-t030 clone,which harbored a variety of drug resistance genes and virulence genes,and the menB gene had missense mutation.CONCLUSION The formation of SCVs may be associated with the inhibited synthesis of menadione caused by the missense mutation of menB gene.

Objective To assess the efficacy of an innovative operational management model in enhancing the refined operational management of a public hospital.Methods An innovative operational management model,"One Body,Two Wings and Three Drives",was developed,which involved establishing a systematic operational management system,strengthening per-formance and cost control,and reinforcing the supporting roles of discipline construction,scientific and technological innovation,and smart hospital initiatives.This comprehensive approach aimed to systematically promote hospital operational management re-forms and improve overall efficiency and quality.Results After using this model,the hospital presented continuous improve-ments in operational efficiency and medical quality,with key performance indicators trending positively.Over the past three years,the average annual growth rate of outpatient and emergency service visits reached 6.6％,inpatient service visits increased by 5.7％,and the Case Mix Index(CMI)rose by 0.22 over two consecutive years.Conclusion This model is highly systemat-ic,practical,and policy-compatible,providing a replicable path for the high-quality development of public hospitals.

Liver Cancer is a prevalent malignant tumor worldwide,with various treatment options available. Among these, ablation therapy holds a significant role in liver cancer treatment due to its minimally invasive nature and lower complication rate. This article reviews the indications and contraindications of liver cancer ablation,the basic principles of different ablation techniques,and their advantages and limitations in clinical applications for liver cancer. Each ablation technique possesses unique characteristics regarding therapeutic efficacy,application scope,and complication profiles,necessitating the selection of the most appropriate approach tailored to the patient′s specific condition and tumor attributes. Furthermore,this article also discusses the potential role of ablation therapy in multidisciplinary treatment,highlighting its synergistic application with liver transplantation,interventional therapy,and immunotargeted therapy to significantly improve outcomes for unresectable liver cancer. Specifically,ablation therapy can induce an anti-tumor immune response by locally destroying the tumor,offering a potential application prospect for combining ablation with immunotherapy. Looking forward,with advances in nanotechnology,artificial intelligence,and image-guided techniques,ablation therapy is expected to progress towards higher precision,personalization,and safety,offering optimized treatment options for liver cancer patients.

OBJECTIVE To analyze the biological characteristics and genomic features of highly lytic bacteriopha-ges isolated from sewage in tropical hospitals and provide references for the hospital-associated infection prevention and control of multidrug-resistant Acinetobacter baumannii(MDR-Ab).METHODS With MDR-Ab as the host bacterium,bacteriophages were isolated from sewage.Transmission electron microscopy was employed to observe their morphology,and determine the optimal multiplicity of infection(MOI),one-step growth curve and environ-mental stability.Whole-genome sequencing and bioinformatics analysis were conducted to annotate functional genes and construct a phylogenetic tree.RESULTS A virulent bacteriophage,HN_Aba_01,was isolated.Elec-tron microscopy revealed that it belonged to the Myoviridae family,with a head diameter of 50 nm and a tail length of 90 nm.This bacteriophage exhibited strong lytic activity,with an optimal MOI of 0.000 000 1,a latent period of 5 minutes and a lysis yield of 15 PFU/cell.It remained stable at temperatures ranging from 4 ℃ to 60 ℃and pH values from 3 to 10.Genomic analysis identified 85 ORFs,including lyase,perforin and depolymerase genes.It shared 98.12％identity with the bacteriophage AbP2 genome(reference genome)and was classified into the Obolenskvirus genus.CONCLUSIONS The bacteriophage isolated from tropical hospital sewage,with high lyt-ic activity and good environmental adaptability,can be used for the hospital-associated infection prevention and control of MDR-Ab.

The cerebellum is heavily connected with other brain regions, sub-serving not only motor but also nonmotor functions. Genetic mutations leading to cerebellar dysfunction are associated with mental diseases, but cerebellar outputs have not been systematically studied in this context. Here, we present three dimensional distributions of 50,168 target neurons of cerebellar nuclei (CN) from wild-type mice and Nlgn3R451C mutant mice, a mouse model for autism. Our results derived from 36 target nuclei show that the projections from CN to thalamus, midbrain and brainstem are differentially affected by Nlgn3R451C mutation. Importantly, Nlgn3R451C mutation altered the innervation power of CN→zona incerta (ZI) pathway, and chemogenetic inhibition of a neuronal subpopulation in the ZI that receives inputs from the CN rescues social defects in Nlgn3R451C mice. Our study highlights potential role of cerebellar outputs in the pathogenesis of autism and provides potential new therapeutic strategy for this disease.

The high-quality development requirements for public hospitals,national examination orientations,and DIP medical insurance payment reforms present significant challenges to the refined management of public hospitals.Therefore,it is essential to enhance the operational management of these institutions.This paper aims to develop an operation evaluation index system for clinical departments using the Delphi method for assessing the efficiency of resource input and output across various clinical departments.It provides a scientific basis for decision-making regarding resource allocation,transformation towards re-fined management,and the enhancement of operational guidance for departments.
