1.Forensic clinical examination of personal injury caused by dexamethasone poisoning

Objective To analyze the forensic clinical appraisal of personal injury caused by dexamethasone poisoning and provide reference for similar cases.Methods A retrospective analysis was conducted on 16 individuals exposed to dexamethasone-adulterated drinking water in a poisoning case.The age,gender,adverse reactions,and clinical manifestations of the appraised individuals were examined based on interview records and clinical medical histories.Results Among the 16 appraised individuals,7 were male and 9 were female,aged 29-50 years.The primary clinical manifestations involved the nervous,respiratory,digestive,and muscular systems,as well as the skin and other organs.After discontinuation of the"hormone water,"5 individuals experienced adverse clinical outcomes.Conclusion Forensic clinical examination in dexamethasone poisoning cases should be performed after the completion of clinical treatment,with particular attention to damage in specific organs.The appraisal must comprehensively consider poisoning duration,dosage,treatment measures,detoxification process,and individual variability.Such cases warrant close attention in judicial practice,and relevant authorities should improve appraisal standards to better meet forensic needs.
2.Characteristics of pulmonary function alterations in pediatric patients with Mycoplasma pneumoniae and effect of inhaled glucocorticoids during recovery phase on their pulmonary functions

Objective:To discuss the characteristics of pulmonary function changes in the pediatric patients with Mycoplasma pneumoniae pneumonia(MPP),the effect of inhaled corticosteroids during the recovery phase on pulmonary function,and the improvement effects of different aerosolized medications on pulmonary function,and to clarify the significance of inhaled corticosteroids in the treatment of MPP pediatric patients during the recovery phase.Methods:A retrospective study was conducted.Sixty-nine MPP children who received inhaled corticosteroids after discharge were selected as treatment group.According to the different medications used after discharge,they were divided into steroid group(receiving inhaled corticosteroids alone,n=42)and combination group(receiving inhaled corticosteroids combined with inhaled long-acting bronchodilators,n=27).Additionally,30 children who did not receive aerosol therapy after discharge during the same period were selected as control group.The general data of the pediatric patients in various groups were collected.The pulmonary function parameters,including forced vital capacity(FVC),forced expiratory volume in the first second(FEV1),ratio of forced expiratory volume in 1 second to forced vital capacity(FEV1/FVC),peak expiratory flow(PEF),and maximal mid-expiratory flow(MMEF),were detected using pulmonary function equipment at the time of entering the recovery phase and 1 month after entering the recovery phase.The changes in pulmonary function of the pediatric patients between control group and treatment group were analyzed.Results:The pulmonary ventilation dysfunction in the MPP pediatric patients was mainly restrictive,followed by mixed.The pulmonary function could be normal or only show small airway dysfunction in some children.In both teatment and control groups,the second pulmonary function parameters including FVC,FEV1,PEF,maximal expiratory flow at 25%of forced vital capacity(MEF25),maximal expiratory flow at 50%of forced vital capacity(MEF50),maximal expiratory flow at 75%of forced vital capacity(MEF75),and MMEF of the pediatric patients were significantly increased compared with the first measurement(P<0.05).Compared with control group,the increase in FEV1/VC at the second measurement of the pediatric patients in treatment group was not significant(P>0.05).The differences in pulmonary function parameters showed an increasing trend in treatment group and control group.The differences in MEF25 and MMEF of the pediatric patients in the treatment group were higher than those in control group(P<0.05).The differences in pulmonary function parameters of the pediatic patients in both steroid group and the combination group showed an overall increasing trend.The differences in FVC,FEV1,FEV1/VC,MEF25,MEF50,and MMEF of the pediatric patients in steroid group were slightly higher than those in combination group.Compared with steroid group,the differences in PEF and MEF75 of the pediatric patients in combination group were slightly higher,but the differences were not statistically significant(P>0.05).The differences in MEF25,MEF50,and MMEF of the pediatric patients in steroid group were higher than those in control group(P<0.05).Conclusion:Both large and small airway functions can be affected in the pediatric patients with MPP,with restrictive ventilation dysfunction being predominant.Airway function can improve during the disease recovery phase.Inhaled corticosteroids during the recovery phase may have a positive therapeutic significance in promoting the recovery of pulmonary function,especially small airway function.The bronchodilators showed no significant effect on improving the pulmonary function during the recovery phase.Therefore,inhaled corticosteroids alone during the recovery phase may promote the recovery of pulmonary function in children with MPP.

BACKGROUND:Research results related to medical magnesium alloy materials have increased year by year.However,the literature in the field of medical magnesium alloy materials has not yet passed the systematic evaluation of measurement.OBJECTIVE:To present the research status of medical magnesium alloy materials and explore the research trends and hotspots of this field.METHODS:The core data set of Web of Science was used as the literature source to search the relevant researches on medical magnesium alloy materials from 2014 to 2024.The knowledge map was generated by using CiteSpace software with authors,countries,institutions,journals,keywords,and co-citations as nodes for visualization analysis.RESULTS AND CONCLUSION:(1)The number of papers on medical magnesium alloy materials has been growing steadily in the past decade.China has the largest number of articles,and is the backbone of the field,but the quality of articles and international influence need to be further improved.The Chinese Academy of Sciences and Peking University are the pioneering institutions in this field,constructing a cooperative network covering many well-known institutions at home and abroad.The journal BIOACT MATER has the greatest intensity of outbreaks,and the impact of journal in the field will be further expanded in the future.(2)The microstructure,degradation behavior,biocompatibility,mechanical properties and corrosion resistance of medical magnesium alloy materials have been hot research topics in recent years.Alloying technology,as a key way to give medical alloys diversified characteristics,has become a research focus for researchers to explore.(3)Comprehensively applying various modification means to initially regulate the degradation mode of the material and to obtain the appropriate degradation rate may be a future research trend.In the future,magnesium alloy products should be customized for specific application scenarios,under the guidance of the concept of"precise bio-adaptation,"realizing the transformation from single material design to overall design of structure and function.

7.Cellular and cytokine immune responses during varicella zoster virus reactivation of herpes zoster infection

Objective To study changes in immune cells and cytokines during the reactivation stage of varicella-zoster virus(VZV)in patients with herpes zoster.Methods A total of 50 patients with herpes zoster and 30 healthy individuals were selected from Xi'an Ninth Hospital between May 2022 and October 2022.Flow cytometry was used to detect the proportion of peripheral blood CD3+cells,CD4+cells,CD8+T cells,B cells and NK cells,as well as levels of cytokines IL-2,IFN-γ,IL-10 and IL-6.We analyzed the immune mechanism of VZV reactivation stage in herpes zoster patients.Results Compared with the healthy control group,the proportion of CD3+cells and CD4+T cells in herpes zoster patients decreased significantly;the proportion of NK cells significantly increased;the levels of IFN-γ,IL-10 and IL-6 significantly increased;the proportion of CD8+T cells,B cells and IL-2 content showed an increasing trend,but there was no significant difference.In addition,the severity of neurological involvement in herpes zoster patients might affect changes in cytokine levels.Conclusion During the reactivation period of VZV,changes in the proportion of immune cells and cytokine expression levels are closely related to the occurrence and development of herpes zoster.
8.Research on the model construction of medical consumables replenishment strategy and inventory control strategy based on supply-process-distribution mode

Objective:To build a medical consumables demand prediction model to predict the demand for medical consumables,and to reduce the occurrence of emergency replenishment and shortage of medical consumables.Methods:The medical consumables demand prediction model was established using support vector regression(SVR)algorithm,the replenishment strategy and inventory control strategy optimization model was established using multiple population genetic algorithm(MPGA)to build a medical consumables demand forecasting model,and the replenishment strategy and inventory control strategy were automatically generated.The relevant data of supply-processing-distribution(SPD)mode introduced by Benxi Central Hospital for medical consumables management from January 2019 to May 2023 were selected for model training,model validation test(test set)and model application prediction(application set),respectively.The average absolute prediction error,peak prediction error and trough of prediction error of the model were evaluated,and the average ratio of consumption per day to inventory cost,the average number of urgent orders per month,the average number of out of stock per month,the average number of non-urgent orders per month and the decreasing range of the index weekly average were compared.Results:There was no statistically significant difference in the average absolute prediction error between the model test set and the application set(P>0.05).The average absolute prediction error of the model was 0.033 5±0.024 5,the peak prediction error was 0.071 7 and the trough of prediction error was-0.0090.After the application of the model,the ratio of average daily consumption to inventory cost,the average number of monthly emergency order frequency,the average number of shortage of stock,and the average number of monthly non-emergency orders were(0.457 5±0.060 3),(23.95±6.04),(5.58±2.17),and(20.68±2.77),respectively.The ratio of daily average consumption to inventory cost was higher than that before the application,the average number of monthly emergency order,shortage of stock,and monthly non-emergency order were lower than those before the application,the difference was statistically significant(F=371.912,88.486,124.472,142.138,P<0.000).After the application of the model,the weekly average inventory amount decreased by 43.66%,and the average number of emergency replenishment decreased by 53.76%,the average number of shortages of stock decreased by 76.95%,and the average shortage of stock normal replenishments decreased by 34.41%.Conclusion:The medical consumables demand prediction model can predict the demand for medical consumables,optimize replenishment and inventory control strategies,reduce the cost of medical consumables inventory,reduce occurrence of emergency replenishment and shortages of stock,and reduce the number of normal replenishment.

Objective The diagnosis of nasal fractures poses challenges in forensic clinical evaluation.This study aims to develop and enhance an artificial intelligence-based model for nasal fracture recognition,evaluate its performance,and provide assistance and support for forensic clinical identification.Methods Multi-center nasal CT images were selected and screened according to the consensus standards set by Chinese experts in nasal CT examination and diagnosis.A recognition model was constructed,followed by external verification and evaluation.Additionally,the diagnostic capabilities of qualified appraisers/doctors with different professional titles(primary,intermediate,and senior)were compared with the performance of the intelligent recognition model.The accuracy,sensitivity,specificity),and negative predictive value(NP)of the intelligent recognition model were comprehensively evaluated.Results The intelligent recognition model exhibited high diagnostic efficiency and stability.It improved the diagnostic accuracy of radiologists and appraisers in detecting nasal fractures while effectively bridging the gap between inexperienced doctors/appraisers and experienced ones.Conclusion The intelligent recognition model for nasal fractures can assist appraisers in enhancing their ability to locate such fractures on CT images and improve work efficiency while enhancing appraisal opinions'accuracy and scientificity.
10.Knowledge map of artificial intelligence applied in forensic medicine

Objective To conduct a comprehensive visual analysis of the application of Artificial Intelligence(AI)in forensic medicine using bibliometric tools so as to create knowledge maps of cooperation network,research hotspots,important findings,and potential future trends in this field.Methods The Web of Science(WoSCC)was utilized as the primary data source,search formula incorporating AI and forensic medicine as core subject words was constructed,resulting in a dataset comprising 2 287 literature records.Vosviewer,Citespace,and Bibliometrix were employed for analyzing various aspects such as cooperation network,keyword co-occurrence networks,clustering dynamics,clusters,centrality degree and thematic strategic coordinate charts.These analyses facilitated the creation of corresponding visual maps.Results The collaboration among authors still requires further strengthening;however significant groups have emerged among institutions and countries.Research hotspots and important findings predominantly revolve around algorithmic applications.Furthermore,"identification"related research appears to become a prominent future research trend.Conclusion By employing bibliometric analysis techniques on the application of artificial intelligence in forensic medicine domain,this study successfully elucidats cooperation networks,research hotspots,important findings,future research directions,and provides objective support through empirical evidence for related studies.
