Background The decline of physical activity in the elderly due to aging may increase the risk of sarcopenia. Currently, there is a lack of evidence from large natural populations on the relationship between PA and sarcopenia. Objective To explore the relationship between PA and sarcopenia in the elderly aged 60 years and above in 10 provinces (autonomous regions) of China. Methods Data were retrieved from the 2022—2023 round of the China Development and Nutrition Health Impact Cohort. Personal basic information and PA data were collected by questionnaire survey. Skeletal muscle mass was measured by bio-electrical impedance analysis, muscle strength was measured using a grip dynamometer, and physical performance was reflected by 6-meter walk speed. 2.Correlation Between Cardiovascular Events and Traditional Chinese Medicine Syndrome in Patients with Rheumatoid Arthritis:A Cross-Sectional Study

ObjectiveTo explore the correlation between the occurrence of cardiovascular events in rheumatoid arthritis(RA) and traditional Chinese medicine(TCM) syndrome. MethodsThe cross-sectional study selected 6713 RA patients from 122 centres nationwide, in which general information such as name, gender, age, height, body weight, and course of disease were collected by completing a questionnaire; patients were classified into eight types of syndrome according to the information of their four examinations,i.e. wind-dampness obstruction syndrome, cold-dampness obstruction syndrome, dampness-heat obstruction syndrome, phlegm-stasis obstruction syndrome, stasis-blood obstructing collateral syndrome, qi-blood deficiency syndrome, liver-kidney insufficiency syndrome, and qi-yin deficiency syndrome. According to the occurrence of cardiovascular events, they were divided into the occurrence group and the non-occurrence group, and the condition assessment data and laboratory examination indexes were recorded. The test of difference between groups was used to analyse the possible risk factors for the occurrence of RA cardiovascular events, and binary logistic regression was used to analyse the correlation between TCM syndromes and RA cardiovascular events. ResultsA total of 6713 RA patients were included, including 256 cases in occurrence group and 6457 in non-occurrence group. There was no statistically significant difference between groups in terms of height, gender, insomnia, appetite, white blood cell(WBC), hemoglobin(HGB), platelets(PLT), rheumatoid factor(RF), anti-cyclic peptide containing citrulline(CCP), alanine aminotransferase(ALT), aspartate aminotransferase(AST), γ-glutamyl transpeptidase(GGT), urea creatinine(CREA), and glucose(GLU)(P>0.05). The TCM syndromes between groups showed significant statistic differences(P<0.05). Patients in occurrence group had longer disease duration, heavier body weight, and older age; more severe conditions such as disease activity(DAS-28), number of painful joints(TJC), number of swollen joints(SJC), health questionnaire scores(HAQ), visual analog scores(VAS), restlessness, and fatigue; higher blood sedimentation rate(ESR), low-density lipoprotein(LDL-C), triglyceride(TG), total cholesterol(TC), D-Dimer, and lower high-density lipoprotein(HDL-C)(P<0.05). The distribution of syndrome types showed that dampness-heat obstruction syndrome accounted for the largest proportion of patients in both groups and was higher in RA cardiovascular events. Logistic regression analysis showed that the occurrence of RA cardiovascular events was strongly associated with dampness-heat obstruction syndrome[OR=5.937, 95%CI (4.434, 7.949), P<0.001]. ConclusionThe occurrence of RA cardiovascular events were associated with TCM syndromes, and the probability of cardiovascular events in the RA patients with dampness-heat obstruction syndrome was 5.937 times higher than patients with other TCM syndromes.
3.The relationship between urinary arsenic methylation metabolic patterns and the transformation of skin keratinization and pigmentation abnormalities in population exposed to arsenic through drinking water

Objective:To study the relationship between urinary arsenic methylation metabolism patterns and skin keratinization and pigmentation abnormalities in population exposed to arsenic through drinking water.Methods:Using a cross-sectional study method, a survey on endemic arsenic poisoning was conducted among permanent residents of drinking water endemic arsenic poisoning areas in Bayannur City, Inner Mongolia Autonomous Region in 2004 (before water improvement). In 2017 (after water improvement), 71 arsenic exposed individuals were followed up as survey subjects. According to the "Diagnosis of Endemic Arsenism" (WS/T 211-2015), the clinical grading of skin injuries (skin keratinization, pigmentation abnormalities) in the survey subjects was evaluated. Urine samples were collected for detection of arsenic methylation metabolite levels by high-performance liquid chromatography inductively coupled plasma mass spectrometry and calibrated with urinary creatinine. The changes and amplitudes of urinary arsenic methylation indicators before and after water improvement were calculated and analyzed according to the outcome of skin keratinization and pigmentation abnormalities which were divided into reduced, unchanged, and added groups.Results:(1) The changes in urinary total arsenic (TAs), inorganic arsenic (iAs), monomethyl arsenic (MMA), and dimethyl arsenic (DMA) levels in different outcome groups of skin keratinization were compared, and the differences were statistically significant ( H = 9.08, 8.77, 9.28, 8.57, P < 0.05). The changes in urinary TAs, iAs, MMA, DMA levels, iAs percentage (iAs%), DMA percentage (DMA%), and primary methylation index (PMI) in different outcome groups of skin pigmentation abnormalities were compared, and the differences were statistically significant ( H = 8.04, 10.67, 8.29, 9.14, 6.30, 9.10, 7.20, P < 0.05). (2) The comparison of amplitudes in urinary TAs, iAs, MMA, and DMA levels in different outcome groups of skin keratinization showed statistically significant differences ( H = 6.92, 7.34, 6.66, 6.16, P < 0.05). The amplitudes in urinary iAs level, iAs%, DMA%, and PMI in different outcome groups of skin pigmentation abnormalities were compared, and the differences were statistically significant ( H = 7.94, 7.61, 9.95, 7.22, P < 0.05). Conclusion:The changes pattern of urinary TAs, iAs, MMA, DMA, iAs%, DMA%, and PMI in population exposed to arsenic through drinking water is related to the transformation of skin keratinization and pigmentation abnormalities.
4.Efficacy and safety of probiotics for irritable bowel syndrome : a systematic review

Irritable bowel syndrome (IBS) is a common functional gastrointestinal disorder and its symptoms are subject to the composition of the gut microbiota. However, it is not yet clear how probiotics relieve IBS, and there is no well-acknowledged standards concerning the clinical application of probiotics. This systematic review was conducted on the literature published from January 2020 to December 2023 and included 12 randomized controlled trials on probiotics in the treatment of IBS. Compared with placebo, the probiotic preparations improved the overall efficacy in IBS patients and was also well tolerated, suggesting the potential of probiotics in IBS management. Nevertheless, different probiotic strains, dosages, combinations, and dosing schedules may affect the outcomes. In the future, large-scale, multi-center, long-term follow-up studies are still needed to clarify the effective strains and methods of use and to promote the use of probiotics in the treatment of IBS.
5.Inhibitory effect of astragaloside Ⅳ on cisplatin-induced liver injury in mice and its mechanism

Objective:To investigate the inhibitory effect of astragaloside Ⅳ(AS-Ⅳ)on cisplatin(CDDP)-induced liver injury in the mice,and to elucidate its possible mechanism.Methods:Forty male C57BL/6 mice with body weights of 18-22 g were randomly divided into control group,model group,AS-Ⅳ group and adenosine 5'-monophosphate-activated protein kinase(AMPK)inhibitor(Compound C)+AS-Ⅳ group.The mice in control group and model group were gavaged with the same volume of normal saline,and the drug was administered continuously for 9 d.The mice in AS-Ⅳ group and Compound C+AS-Ⅳ group were given AS-Ⅳ aqueous solution(150 mg·kg-1·d-1),respectively.On the 6th day of experiment,the mice in Compound C+AS-Ⅳ group were intraperitoneally injected with Compound C(20 mg·kg-1),and on the 7th day,except for control group,the mice in other groups were intraperitoneally injected with 20 mg·kg-1 CDDP to establish the mouse liver injury models,and the mice were sacrificed 48 h later.Serum and liver tissues were collected,and the levels of aspartate aminotransferase(AST)and alanine aminotransferase(ALT)in the serum of the mice,as well as the activities of superoxide dismutase(SOD)and catalase(CAT)and the levels of malondialdehyde(MDA)in the liver tissue of the mice in various groups were detected by kits.The pathomorphology of liver tissue of the mice in various groups were detected by HE staining.The expression levels of glutathione peroxidase 4(GPX4),ferritin heavy chain 1(FTH1)and ferroptosis inhibitory protein 1(FSP1)proteins in liver tissue of the mice in various groups were detected by immunohistochemical staining,and the expression levels of nuclear factor-E2-related factor 2(Nrf2),heme oxygenase-1(HO-1)and AMPK proteins in liver tissue of the mice in various groups were detected by Western blotting method.Results:Compared with control group,the levels of AST and ALT in serum of the mice in model group were increased(P<0.01),the activities of SOD and CAT in the liver tissue were significantly decreased(P<0.01),and the MDA level was increased(P<0.01);compared with model group,the levels of AST and ALT in serum of the mice in AS-Ⅳ group were decreased(P<0.01),the MDA level in the liver tissue was decreased(P<0.01),and the activities of SOD and CAT were increased(P<0.01);compared with AS-Ⅳ group(P<0.01),the levels of AST and ALT in serum of the mice in Compound C+AS-Ⅳ group were increased(P<0.01),the level of MDA in liver tissue was increased(P<0.05),and the activities SOD and CAT were decreased(P<0.01).The HE staining results showed that compared with control group,the liver damage degree of the mice in model group was enhanced,the hepatocyte arrangement was disordered,and some hepatocyte edema were increased;compared with model group,the liver morphology of the mice in AS-Ⅳ group returned to normal;compared with AS-Ⅳ group,the hepatocyte arrangement of the mice in Compound C+AS-Ⅳ group was disordered and the edges were blurred.The immunohistochemistry results showed that compared with control group,the expression levels of GPX4,FTH1 and FSP1 proteins in liver tissue of the mice in model group were decreased(P<0.05);compared with model group,the expression levels of GPX4,FTH1 and FSP1 proteins in liver tissue of the mice in AS-Ⅳ group were increased(P<0.05);compared with AS-Ⅳ group,the expression levels of GPX4,FTH1 and FSP1 proteins in liver tissue of the mice in Compound C+AS-Ⅳ group were decreased(P<0.05 or P<0.01).The Western blotting results showed that compared with control group,the expression levels of Nrf2,HO-1 and AMPK proteins in liver tissue of the mice in model group were decreased(P<0.01);compared with model group,the expression levels of Nrf2,HO-1 and AMPK proteins in liver tissue of the mice in AS-Ⅳgroup were increased(P<0.01);compared with AS-Ⅳ group,the expression levels of Nrf2,HO-1 and AMPK proteins in liver tissue of the mice in Compound C+AS-Ⅳ group were decreased(P<0.01).Conclusion:AS-Ⅳ can alleviate the CDDP-induced liver injury,and its mechanism may be related to the regulation of AMPK/Nrf2/HO-1 signal pathway and ferroptosis by AS-Ⅳ.

8.Curative Effect of Tripterygium wilfordii Polyglycoside Tablets in Treatment of Rheumatoid Arthritis in Real World Based on Propensity Score Matching

ObjectiveTo evaluate the clinical efficacy and safety of Tripterygium wilfordii polyglycoside tablets (TWP) in the treatment of rheumatoid arthritis (RA) in the real world. MethodDiagnosis and treatment data of patients with RA in Chinese medicine rheumatology registration research information platform information database (CERTAIN) from January 1,2019 to January, 2024 were collected. According to the inclusion and exclusion criteria, data were screened. The included data were divided into an exposure group and a control group according to the use of TWP or not. Propensity score matching (PSM) was used in both groups to keep the baseline balanced and comparable. The disease activity score (DAS28-ESR) of 28 joints based on the erythrocyte sedimentation rate (ESR)before and after treatment was compared between the two groups, as well as health assessment questionnaire (HAQ),visual analogue scale (VAS),tender joint count (TJC),swollen joint count (SJC), patient's global assessment (PGA),evaluator's global assessment (EGA),laboratory indexes, clinical curative effect, and adverse reactions. ResultA total of 3 978 patients were included,including 929 in the exposure group and 3 049 in the control group. Before PSM,there were significant differences in demographic information,DAS28-ESR score,PGA,EGA,HAQ,VAS scores,SJC, and TJC between the two groups (P<0.05). After successful PSM matching,922 patients in the exposure group and 922 patients in the control group were included. There was no significant difference in demographic information and DAS28-ESR between the two groups before treatment,and the differences in other indexes between the two groups decreased to varying degrees. After treatment,the DAS28-ESR,PGA,EGA,HAQ,SJC,TJC,VAS scores, ESR,and IgG immune index of the two groups were significantly lower (P<0.01). Compared with those in the control group after treatment,the DAS28-ESR,PGA,EGA,HAQ,VAS scores, and ESR in the exposure group after treatment decreased more significantly (P<0.05,P<0.01). There was no significant difference in TJC in the exposure group after treatment. However, TJC in the exposure group was significantly higher than that in the control group before treatment (P<0.05). In terms of TJC reduction,the exposure group performed better than the control group. There was no significant difference in SJC and IgG between the exposure group and the control group after treatment. After treatment,the clinical symptoms of poor appetite,insomnia and many dreams,upset,fatigue,and fear of wind and cold in the two groups were improved. Except that the proportion of women in the exposure group was higher than that in the control group (P<0.01),there was no significant difference in the incidence of other adverse reactions between the two groups after treatment. ConclusionTWP to treat RA can effectively reduce DAS28-ESR,PGA,EGA,HAQ,TJC,and VAS scores and improve the general symptoms. Except for the women at childbearing age with fertility requirements that TWP is not applicable,it shows good security.
9.Evaluation of the effect of long-term full-dose cognitive digital therapy on cognitive function in patients with cognitive disorders

Objective:To evaluate the effect of long-term full-dose cognitive digital therapy on improving and maintaining cognitive function in patients with cognitive disorders, and to provide basis for nursing management of patients with cognitive disorders.Methods:This study adopted three parallel randomized control trial design. By adopting the method of continuous grouping and convenient sampling, from January 2020 to January 2023, 90 patients with cognitive disorders who were treated in the neurology outpatient department of Beijing Xuanwu Hospital of Capital Medical University were selected as the study objects. These patients were divided into the control group, outpatient group and home group with 30 patients in each group by using the random digit table method. The control group received routine nursing, patients in the outpatient group received cognitive digital therapy in clinic based on routine nursing, and patients in the home group received cognitive digital therapy at home based on routine. Before intervention, 6 and 12 months after intervention, Mini-Mental State Examination (MMSE) and Montreal Cognitive Assessment (MoCA) were compared among three groups.Results:A total of 85 patients were enrolled in three groups. There were 28 cases in the control group, including 12 males and 16 females, aged (64.68±11.26) years. There were 30 patients in the outpatient group, including 19 males and 11 females, aged (65.74 ± 8.44) years. There were 27 cases in the home group, including 14 males and 13 females, aged (66.81 ± 11.18) years. The MMSE scores of the control group before intervention, 6 and 12 months after intervention were (22.00 ± 4.82), (17.25 ± 5.15), (14.25 ± 4.54) points respectively, and those of the outpatient group were (22.26 ± 3.46), (22.93 ± 3.77), (22.30 ± 3.57) points, while those of the home group were (22.40 ± 3.72), (22.03 ± 3.76), (21.90 ± 4.08) points respectively. The analysis of variance of repeated measurements showed that the intergroup, time and interaction effects were statistically significant ( Fbetween-group=15.30, Ftime=4.10, Finteraction=20.25, all P<0.05). The total scores of MoCA in the control group were (16.50 ± 4.87), (12.64 ± 4.94), (9.89 ± 4.01) points before intervention, 6 and 12 months after intervention, (18.74 ± 4.22), (18.96 ± 4.31), (18.41 ± 4.34) points in the outpatient group, (17.20 ± 5.12), (16.93 ± 3.67), (16.93 ± 4.10) points in the home group respectively. The analysis of variance of repeated measurements showed that the intergroup, time and interaction effects were statistically significant ( Fbetween-group=16.30, Ftime=5.74, Finteraction=12.08, all P<0.05). Conclusions:Long-term adequate outpatient and in-home cognitive digital therapy can effectively improve and maintain cognitive function in patients with cognitive disorders. It optimizes the management scheme of cognitive function training and provides evidence-based basis for non-drug intervention of patients with cognitive disorders.

Objective:To discuss the protective effect of polygonatum odoratum polysaccharide(POP)on the mice with cisplatin-induced acute kidney injury(AKI),and to clarify its possible mechanism.Methods:Forty male C57BL/6 mice were randomly divided into control group,model group,POP group,and ferroptosis inducer Erastin combined with POP(Erastin+POP)group,and there were 10 mice in each group.The mice in POP group and Erastin+POP group were given intragastric administration of POP(400 mg·kg-1),and on the 7th day,the mice in model group,POP group,and Erastin+POP group were intraperitoneally injected with cisplatin(20 mg·kg-1)to establish the AKI models,the mice in control group were injected with the same volume of normal saline,and the mice in Erastin+POP group were intraperitoneally injected with Erastin(40 mg·kg-1)one day in advance(on the 6th day of the experiment).After 9 d,the mice were killed and the serum and kidney tissue were collected,and the levels of serum creatinine(Scr)and blood urea nitrogen(BUN)and the levels of malondialdehyde(MDA)and glutathione(GSH)in kidney tissue of the mice in various groups were detected by kit;HE staining was used to observe the pathomorphology of kidney tissue of the mice in various groups;the expression levels of ferroptosis suppressor protein 1(FSP1),ferritin heavy chain 1(FTH1),and glutathione peroxidase 4(GPX4)proteins in kidney tissue of the mice in various groups were detected by immunohistochemistry;Western blotting method was used to detect the expression levels of nuclear factor-E2-related factor 2(Nrf2)and heme oxygenase-1(HO-1)proteins in kidney tissue of the mice in various groups.Results:Compared with control group,the levels of Scr and BUN of the mice in model group were significantly increased(P<0.01),the level of MDA in kidney tissue was significantly increased(P<0.01),and the level of GSH was significantly decreased(P<0.01);most kidney tubules were dilated,the epithelial cells were swollen,the vacuolar degeneration and epithelial cells fell off,and the protein-like tubules could be seen in the lumen;the expression levels of FSP1,FTH1,GPX4,Nrf2,and HO-1 proteins in kidney tissue were decreased significantly(P<0.05 or P<0.01).Compared with model group,the levels of Scr and BUN of the mice in POP group were significantly decreased(P<0.01),the level of MDA in kidney tissue was significantly decreased(P<0.01),and the level of GSH was significantly increased(P<0.01);the dilatation of kidney tubular lumen,epithelial cell swelling,vacuolar degeneration,and epithelial cell exfoliation were decreased;the expression levels of FSP1,FTH1,GPX4,Nrf2,and HO-1 proteins in kidney tissue of the mice in POP group were significantly increased(P<0.05 or P<0.01).Compared with POP group,the levels of Scr and BUN of the mice in Erastin+POP group were significantly increased(P<0.01),the level of MDA in kidney tissue was increased(P<0.05),and the level of GSH was significantly decreased(P<0.01);the pathological injury of kidney tissue was aggravated obviously;the expression levels of FSP1,FTH1,GPX4,Nrf2,and HO-1 proteins in kidney tissue were significantly decreased(P<0.05 or P<0.01).Conclusion:POP can reduce the AKI in the mice induced by cisplatin,and its mechanism may be related to the inhibitory effect of POP on the ferroptosis induced by cisplatin.
