1.Effect of corneal stromal lenses combined with 0.1% cyclosporine A eye drops on Mooren's ulcer

AIM:To evaluate the efficacy of corneal stromal lenses combined with 0.1% cyclosporine A eye drops in patients with Mooren's ulcer and its effect on ocular surface inflammatory factors.METHODS:In this prospective randomized controlled trial, 200 patients(272 eyes)with Mooren's ulcer were enrolled from January 2022 to January 2024. Patients were randomly assigned to receive either corneal stromal lenses alone(control group: 100 patients, 136 eyes)or combined with 0.1% cyclosporine A eye drops(observation group: 100 patients, 136 eyes). All patients were followed up for 3 mo. Clinical efficacy, visual acuity improvement, recovery time, pain score, complications, and tear levels of interleukin(IL)-1β, IL-6, and tumor necrosis factor-α(TNF-α)were compared between the two groups.RESULTS:A total of 196 patients(268 eyes)completed follow-up, with 2 cases(2 eyes)lost to follow-up in each group and a lost to follow-up rate of 2.0%. The observation group showed a higher total effective rate(95.5% vs 83.6%, P<0.05). Postoperative best corrected visual acuity(LogMAR)of the observation group was significantly better than that of the control group(0.42±0.15 vs 0.65±0.18, P<0.001). Epithelial(5.24±1.15 vs 7.86±1.43 d)and stromal recovery(12.35±2.46 vs 16.72±3.15 d)were faster in the observation group(both P<0.001). Pain scores and complication rates of the observation group were lower than those of the control group(both P<0.001). Both groups showed reduced tear IL-1β, IL-6, and TNF-α levels postoperatively, with greater reductions in the observation group(P<0.001).CONCLUSION: Corneal stromal lenses combined with 0.1% cyclosporine A eye drops provide superior outcomes for Mooren's ulcer by enhancing visual recovery, accelerating corneal healing, reducing pain, and lowering complications, potentially through inhibition of ocular surface inflammatory factors.
2.Expression of TRM cells in the lesions of imiquimod-induced models of psoriasis in mice

Objective To investigate the effect of tissue-resident memory T cells(TRM)on imiquimod-induced psoriatic-like skin lesions in mice,and to elucidate the underlying mechanisms of TRM involvement in this process.Methods Forty female BALB/c mice were procured and randomly allocated into four groups:ten in the blank control group,and thirty for the establishment of a psoriasis mouse model.Following successful modeling,the thirty mice were further randomized into three groups:the model control group,the methotrexate-treated group,and the imiquimod-treated group,with ten mice in each group.Mice in the blank control group and model control group were uniformly treated with Vaseline for intervention.The methotrexate group and the imiquimod group were treated with 62.5mg of 5%imiquimod cream.The methotrexate group was administered by gavage at a dose of 1 mg/kg,and the gavage volume of each group was 10 mL/kg.The model control group,blank group and imiquimod group were gavaged with the same volume of normal saline.Treatment was conducted over six consecutive days.Subsequently,comparisons were made across groups regarding the psoriasis area and severity index(PASI),histopathological findings,inflammatory cytokine levels,and TRM cell levels.Results(1)The imiquimod group exhibited signifi-cantly lower scores for erythema(2.54±0.32),skin thickening(2.59±0.25),and scaling(2.52±0.29)compared to the methotrexate group,model control group,and blank control group(P<0.05).Additionally,the methotrexate group demonstrated reduced scores for erythema,skin thickening,and scaling compared to the model control group(P<0.05).(2)Hematoxylin-eosin(HE)staining revealed that the epidermis in the methotrexate group became thin-ner,with fewer parakeratotic cells and increased hair follicles.Conversely,the imiquimod group displayed abnor-mal cell morphology and relatively thicker white skin after modeling.(3)The imiquimod group showed significantly lower levels of TNF-α(51.63±4.39 pg/mL),IL-1β(35.53±4.15 pg/mL),IFN-γ(23.43±3.41 pg/mL),and IL-23(15.24±2.95 pg/mL)compared to the methotrexate and model control groups(P<0.05).Similarly,the methotrexate group exhibited reduced levels of TNF-α,IL-1β,IFN-γ,and IL-23 compared to the model control group(P<0.05).(4)The imiquimod group had significantly lower levels of CD8+CD103+cells(15.39±2.31)than the methotrexate and model control groups(P<0.05).Furthermore,the methotrexate group demonstrated lower levels of CD8+CD103+cells compared to the model control group(P<0.05).Conclusion Miquimod induces heavier skin lesions,faster response,and more epidermal thickening in psoriasis like mice.CD8+CD103+TRM cells and inflammatory factors may be involved in the recurrence of psoriasis.

4.Practice of tracking and controlling of one case of Staphylococcus aureus infection in breast surgery department of a specialized cancer hospital

OBJECTIVE To investigate the causes of abnormal rising trends of isolation rate and infection of Staph-ylococcus aureus in the breast surgery department of a specialized cancer hospital.METHODS By means of pro-spective survey in combination with retrospective survey,an epidemiological survey was conducted for 5 patients with hospital-associated S.aureus infection who were treated in breast surgery department of Sun Yat-sen Univer-sity Cancer Center from Jan.2023 to Aug.2023,and the homology was observed.RESULTS The incidence rate of hospital-associated S.aureus infection(ward E 0.08%,ward F 0.38%)of the breast surgery department patients was higher during the period from Jan.2023 to Aug.2023,higher than 0.004%of the whole patients during the same period.The environmental sampling result showed that the colonized rate of S.aureus was 17.39%(4/23)and in nasal cavity 7.41%(2/27)in hands of the healthcare workers,and 28.57%in nasal cavity of logistical per-sonnel.Bundle interventions could reduce the incidence of hospital-associated methicillin-resistant S.aureus(MRSA)infection and the colonized rate in hands of the healthcare workers but had less impact on the colonized rate in the nasal cavity.The result of pulse field gel electrophoresis(PFGE)indicated that there was risk of cross transmission of colonized S.aureus between the hands of healthcare workers and the nasal cavities.The hand hy-giene,measures for contact transmission and environmental disinfection were supposed to be taken as the major links for control of the hospital-associated S.aureus infection.CONCLUSIONS The sporadic community-ac-quired infections and hospital-associated infections are the leading causes of the abnormal rise of the isolation rate and the infection rate of S.aureus in the breast surgery department.The bundle prevention and control measures can block the transmission of infections.

6.Normal ranges and Z-scores analysis of the normal fetal umbilical vein diameter, blood volume flow and normalized volume blood flow in the second and third trimesters of pregnancy

Objective:To explore the method of ultrasonography for detecting the fetal umbilical vein diameter, blood flow volume and normalized volume blood flow and establish normal reference ranges with umbilical vein diameter, blood flow volume and normalized blood flow and Z-scores for umbilical vein diameter and blood volume flow.Methods:This was a prospective study on 907 normal fetuses in the Second Xiangya Hospital, Central South University and Women and Children Healthcare Hospital of Zhuzhou from March 2019 to December 2020. The umbilical vein diameter (Duv), umbilical vein blood flow volume (Quv) and normalized volume blood flow (nQ = Quv/estimated fetal weight) of the free loop of umbilical vein (FUV) and fetal intra-abdominal umbilical vein (IUV) were collected. And the mean values and 90% confidence intervals of Duv, Quv and nQ in two segments of umbilical veins at different gestational ages were calculated. Regression analysis of Duv, Quv and nQ were performed with gestational age (GA), and the parameters of umbilical vein in different segments were compared. Finally, with gestational age (GA) as the independent variable, Z-scores of the Duv and Quv were built.Results:The mean values and 90% confidence intervals of Duv, Quv, and nQ in 858 (94.6%) normal fetal umbilical veins were successfully obtained. The Duv, Quv of the FUV and IUV increased as pregnancy progressed. The Quadratic curve of Duv and Linear curve of Quv were of the highest fitnesses, respectively( r=0.951, 0.941, 0.986, 0.982; all P<0.001). While nQ increased with GA followed by a decreased trend, and the Quadratic curve was the highest fitting curve of nQ( r=0.610, 0.611; all P<0.001). Duv-FUV was greater than Duv-IUV( P<0.001), nQ-FUV was bigger than Quv-IUV( P=0.001), and he difference was not statistically significant between Quv-FUV and Quv-IUV( P=0.133). Z-scores models of Duv and Quv were successfully established, and all Z-scores were Gaussian distribution. Conclusions:The normal ranges and Z-scores of umbilical vein parameters are useful to improve the evaluation of placental circulation and provide a strong basis for the monitoring of fetus-related diseases and the evaluation of pregnancy prognosis. The choice of FUV or IUV umbilical vein to evaluate placental circulation may depend on the actual situation in clinical application.
7.Meridian Tropism of Components in Bupleuri Radix Based on Nonalcoholic Steatohepatitis Model and Principal Component Analysis

ObjectiveTo explore the meridian tropism of components in Bupleuri Radix (Chaihu, CH) based on the model of nonalcoholic steatohepatitis (NASH) and clarify the substance basis of the meridian tropism of CH in Xiaoyaosan (XYS) by means of principal component analysis. MethodEighty SPF male C57BL/6 mice were randomly assigned into 8 groups, with 10 mice in each group. Except that the blank group was fed with the methionine choline-sufficient (MCS) diet, the other mice were fed with methionine choline-deficient (MCD) diet for 4 weeks to establish the nonalcoholic steatohepatitis (NASH) model. After the established model was confirmed by hematoxylin-eosin (HE) staining, the mice were administrated with corresponding drugs by gavage once a day for 4 weeks. Specifically, the 8 groups were XYS group (2.874 g·kg-1), XYS-CH group (2.445 g·kg-1), XYS-CH+volatile oils (Vol, 0.163 mg·kg-1) group, XYS-CH+polysaccharides (Pol, 24.067 mg·kg-1) group, XYS-CH+flavones (Fla, 2.241 mg·kg-1) group, and XYS-CH+saponins (Sap, 2.746 mg·kg-1) group. The model group and the blank group were administrated with the same volume of normal saline. After the last administration, the mice were sacrificed for the collection of blood and liver tissue. The pathological changes of liver were observed by HE staining and oil red O staining. Enzyme linked immunosorbent assay (ELISA) kits were used to determine the levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST), triglyceride (TG), total cholesterol (TC), high-density lipoprotein (HDL), and low-density lipoprotein (LDL) in serum as well as malondialdehyde (MDA), superoxide dismutase (SOD), catalase (CAT), and glutathione peroxidase (GSH-Px) in liver. SPSS Statistics 23 was used for principal component analysis and comprehensive evaluation to determine the substance basis of the meridian tropism of CH in NASH mice. ResultCompared with the blank control group, the modeling led to hepatocyte swelling, increased fat vacuoles, and appearance of inflammatory cells. Further, the modeling elevated the levels of ALT, AST, TG, TC, and LDL and lowered the HDL level in serum, and it increased the MDA level and decreased the SOD, CAT, and GSH-Px levels in liver. Compared with the model group, the administration of XYS and XYS-CH in combination with the components of CH alleviated the oxidative damage in liver (P<0.05). The comprehensive score of the pharmacological efficacy was in a descending order as follows: XYS > XYS-CH+Sap > XYS-CH+Fla > XYS-CH+Pol > XYS-CH+Vol > XYS-CH. Among the chemical components of CH, Sap had the best effect. ConclusionSap lowers the blood lipid level, regulates the abnormal lipid metabolism, and alleviates the oxidative damage of liver, which is the substance basis for CH to exert the meridian tropism in liver.
8.Predictive model for early neurological deterioration in minor ischemic stroke without reperfusion therapy

Objective To explore independent risk factors for early neurological deterioration in first-ever minor ischemic stroke without reperfusion therapy,and to achieve individualized prediction by constructing a nomogram model.Methods We enrolled first-ever minor ischemic stroke patients of non-reperfusion therapy from January 1,2017 to December 31,2019 in the Zhaoqing First People’s Hospital.The clinical and imaging data of the Neurological deterioration group(END group) and the non-END group were analyzed.The independent related factors were screened and the nomogram model was constructed.Results A total of 384 cases were enrolled,including 66 cases (17.19%) in the END group and 318 cases (82.81%) in the non-END group.Logistic regression analysis showed that diabetes mellitus,TOAST typing,albumin levels,and cerebral atherosclerosis score (CAS) were independent influencing factors (P<0.05).The nomogram model is constructed and the ROC curve is generated,and the area under the curve is 0.859[95%CI(0.758,0.864)].Model consistency can be hinted at by the calibration curve and the Hoster-Lemeshow test.Finally,the clinical decision curve indicates that the model has clinical application value.Conclusion This study explores the nomogram model for END in first-ever minor ischemic stroke without reperfusion therapy,which has some clinical application value,is composed of four major factors:diabetes mellitus,TOAST typing,albumin level and CAS.
9.Association of EphA2 gene polymorphisms with susceptibility to age-related cataract among ethnic Han Chinese from Hubei.

OBJECTIVE:
To assess the association of Eph-receptor tyrosinekinase-type A2 (EphA2) gene polymorphisms with susceptibility to age-related cataract (ARC) among ethnic Han Chinese from Hubei Province.
METHODS:
280 patients with cortical ARC and 200 healthy controls were recruited. Polymorphisms at four loci (rs3768293, rs3754334, rs477558 and rs7548209) of the EphA2 gene were detected by PCR-restriction fragment length polymorphism (PCR-RELP) assay. Allelic and genotypic frequencies of the two groups were compared.
RESULTS:
Compared with the control group, the AA genotype of rs3768293 locus was more common, while the AC genotype was much rarer (P< 0.05). No significant difference was found in allelic and genotypic frequencies of rs3754334 locus between the two groups (P> 0.05). The AA genotype of the rs477558 locus was more common in the patient group, while the AG genotype was much rarer (P< 0.05). The genotype GG of the rs7548209 locus was more common in the patient group, while the CG genotype was rarer (P< 0.05), though no significant difference in allelic or haplotypic frequencies between the two groups (P> 0.05).
CONCLUSION
Polymorphisms of rs477558, rs7548209 and rs3768293 loci of the EphA2 gene are associated with susceptibility to ARC among ethnic Han Chinese from Hubei province.

10.Expression levels of serum miR-210 and miR-375 in patients with non-small cell lung cancer

Objective To investigate the expression levels of serum miR-210and miR-375in patients with non-small cell lung cancer (NSCLC).Methods A total of 25NSCLC patients (NSCLC group) and 14healthy volunteers (control group) were enrolled in this study.The relative expression levels of 9miRNAs (miR-182, miR-126, miR-31, miR-21, miR-221, miR-200b, miR-183, miR-210and miR-375) in 6 NSCLC patients and 6healthy volunteers were measured by RT-qPCR.The dysregulated miRNAs will be selected as candidate miR-NAs.The diagnostic value were evaluated by ROC curve.Results Compared with control group, 2 (miR-210and miR-375) out of 9miRNAs were up-regulated in NSCLC group, and the differences were statistically significant (P<0.05), while the other 7miRNAs were not consistent with the reported literatures.Therefore, miR-210and miR-375were selected as candidate miRNAs.We found that the relative expression level of miR-210in the lung adenocarcinoma group was significantly different from control group (P<0.05), while there was no significant difference between the squamous cell carcinoma group and the control group (P>0.05).There was no significantly statistical difference in the relative expression level of miR-375between lung squamous cell carcinoma group, lung adenocarcinoma group and the control group (P>0.05).The AUC of serum miR-210of lung adenocarcinoma group was 0.737 5 (95%CI:0.498 3-0.976 7, P=0.091 4) with a medium diagnostic value.Conclusion MiR-210is highly expressed in the serum of patients with lung adenocarcinoma, suggesting that miR-210may be a novel tumor marker for the diagnosis of lung adenocarcinoma.The value of miR-375in the diagnosis of lung cancer still needs to be further explored.
