ObjectiveTo observe the clinical effect of modified Huanglian Wendantang on cardiovascular risk factors in patients with metabolic syndrome under the guidance of treating disease before its onset. MethodsA total of 82 patients with metabolic syndrome treated in the First Affiliated Hospital of Heilongjiang University of Chinese Medicine from July 2023 to July 2024 were selected and allocated into an observation group (41 cases) and a control group (41 cases) by the random number table method. The control group received routine treatment, and the observation group was treated with modified Huanglian Wendantang on the basis of routine treatment. Both groups were treated for 8 weeks. The therapeutic effects on TCM symptoms after treatment in the two groups were evaluated. The levels of obesity degree indicators, blood pressure indicators, glucose and lipid metabolism indicators, inflammatory factors, and vascular endothelial function indicators before and after treatment in the two groups were measured, and the treatment safety was evaluated. ResultsAfter treatment, the total response rate of TCM symptoms in the observation group was 97.56% (40/41), which was higher than that (87.80%, 36/41) in the control group (χ2=5.205, P<0.05). After treatment, both groups showed declines (P<0.05) in systolic blood pressure (SBD), diastolic blood pressure (DBP), triglyceride (TG), total cholesterol (TC), low density lipoprotein cholesterol (LDL-C), fasting blood glucose, 2-hour postprandial blood glucose (2 h PG), glycosylated hemoglobin (HbA1c), fasting insulin (FINS), Homeostatic Model Assessment for Insulin Resistance (HOMA-IR), body mass index (BMI), waist circumference (WC), waist-to-hip ratio (WHR), leptin (LEP), interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), endothelin-1 (ET-1), and inducible nitric oxide synthase (iNOS). Moreover, the declines in the observation group were more obvious than those in the control group (P<0.05, P<0.01). After treatment, both groups showed elevated levels of high density lipoprotein cholesterol (HDL-C), adiponectin (ADP), nitric oxide (NO), and endothelial nitric oxide synthase (eNOS) (P<0.05), and the above indexes in the observation group were higher than those in the control group (P<0.01). ConclusionUnder the guidance of the thought of treating disease before its onset, modified Huanglian Wendantang was used to treat patients with metabolic syndrome. The decoction improved the clinical efficacy by ameliorating IR to improve insulin sensitivity, reducing inflammation, and protecting the vascular endothelial function. It inhibits cardiovascular risk factors without inducing adverse reactions, being worthy of clinical application and promotion.

Objective:To evaluate a novel classification of sacroiliac screw bony channels based on CT axial scans proposed by the present study and a new grading system for accuracy and safety of sacroiliac screw placement. Methods:A retrospective study was conducted to analyze the data of 132 patients with pelvic fracture who had undergone percutaneous fixation of the posterior pelvic ring with sacroiliac screws at Department of Emergency Surgery, Qilu Hospital of Shandong University from January 2023 to January 2025. In this cohort, 79 cases were male and 53 ones female, with an age of (44.9±12.4) years. According to the Denis classification of sacral fractures, 69 cases were of type 1, 54 cases of type 2 and 9 cases of type 3. A total of 160 sacroiliac screws were placed in this cohort. Based on the preoperative CT axial plain scans of the sacrum, a novel CT classification of sacroiliac screw bony channels was proposed as follows: S 1 screw parallel channel (71 screws inserted), S 1 screw oblique channel (31 screws inserted), and S 2 screw parallel channel (58 screws inserted). A new grading system for accuracy and safety of sacroiliac screw placement was developed based on the position of sacroiliac screw relative to that of sacroiliac bony channel in postoperative CT axial scan: grade 1: screw completely within the bony channel; grade 2: screw incompletely within the bony channel without contacting the anterior or posterior cortex; grade 3: contacting but not penetrating the cortex; grade 4: penetrating the cortex. Six senior surgeons verified the inter-observer consistency and intra-observer repeatability of the new grading system for accuracy and safety of sacroiliac screw placement. Results:According to the new grading system for accuracy and safety of sacroiliac screw placement, of the 160 sacroiliac screws placed in the 132 patients, 122 were graded as grade 1, 19 as grade 2, 9 as grade 3, and 10 as grade 4, yielding an excellent and good rate of 88.13% (141/160), and an acceptability rate of 93.75% (150/160). The Kappa values for inter-observer consistency and intra-observer repeatability of the new grading system were 0.87 and 0.97, respectively. All the 132 patients were followed up for (18.3±3.6) months after surgery. During the follow-up period, none of the patients experienced such complications as retraction, breakage, or infection of the screws. Screw loosening occurred in some patients who were followed up for more than 1 year [5.51% (7/127), with 3 S 1 screws of grade 3 and 4 S 1 screws of grade 4]. Conclusion:The novel classification of sacroiliac screw bony channels based on CT axial scans proposed by the present study and the new grading system for accuracy and safety of sacroiliac screw placement can lead to a high acceptability rate of sacroiliac screw placement, indicating clinical practicality of the novel classification and the new grading system.

BACKGROUND:Long non-coding RNA(LncRNA)plays an important role in nervous system development and neurological diseases.Previous studies by the research team have demonstrated that human umbilical cord mesenchymal stem cells overexpressing erythropoietin(EPO-MSCs)under ischemic and hypoxic conditions have better neuroprotective functions and significantly activate the expression of LncRNA XIST.Research suggests that XIST is related to the pathogenesis of hypoxic-ischemic encephalopathy, but the role and mechanism of its regulation by EPO-MSCs in protecting ischemic-hypoxic neurons remain unclear.OBJECTIVE:To explore the new mechanism by which LncRNA XIST,in response to EPO-MSC signaling, affects the apoptosis of ischemic-hypoxic SH-SY5Y cells.METHODS:(1)SH-SY5Y cell lines with knockdown of LncRNA XIST(sh-XIST)and negative control(NC-XIST)were constructed through lentiviral transfection.Oxygen-glucose deprivation was used to induce ischemic-hypoxic injury in the cells.Transwell chambers were used to create a non-contact co-culture system with EPO-MSCs,sh-XIST,and NC-XIST ischemic-hypoxic SH-SY5Y cells.Cell proliferation ability was detected using the CCK-8 assay.Cell migration ability was assessed using the scratch assay, and cell apoptosis was measured by flow cytometry.(2)RNA-seq bioinformatics analysis was performed to screen for differentially expressed genes and pathways between sh-XIST and NC-XIST cell lines.Dual-luciferase experiments were used to verify the relationship between miR-124-3p and the target genes XIST and GRIN1.qRT-PCR was conducted to validate the expression levels of downstream miR-124-3p and GRIN1 genes.(3)miR-124-3p inhibitors and mimics were added to verify phenotypic changes in SH-SY5Y cells after ischemic-hypoxic injury and co-culture with EPO-MSCs.RESULTS AND CONCLUSION:(1)Compared with the NC-XIST group,SH-SY5Y cells in the sh-XIST group showed reduced proliferation and migration abilities and increased apoptosis after ischemic-hypoxic injury and co-culture with EPO-MSCs.(2)Dual-luciferase experiments showed that miR-124-3p interacted with the target gene XIST.SH-SY5Y cells transfected with miR-124-3p mimics and co-cultured with EPO-MSCs showed decreased apoptosis after ischemic-hypoxic injury, while SH-SY5Y cells transfected with miR-124-3p inhibitors showed increased apoptosis after co-culture with EPO-MSCs.(3)Transcriptomic sequencing and bioinformatics analysis of sh-XIST revealed significant downregulation of the neuroactive ligand-receptor pathway and the key receptor gene GRIN1 for central nervous system development.(4)Dual-luciferase experiments showed that miR-124-3p interacted with GRIN1.GRIN1 expression was significantly downregulated in the sh-XIST group after ischemic-hypoxic injury compared with the NC-XIST group. These findings indicate that LncRNA XIST promotes GRIN1 expression by upregulating miR-124-3p,thereby reducing cell apoptosis after ischemic-hypoxic injury and co-culture with EPO-MSCs and enhancing proliferation and migration.sh-XIST can block this protective function.

The increasing prevalence of the aging population, and inadequate and uneven distribution of medical resources, have led to a growing demand for telemedicine services. Gait disturbance is a primary symptom of neurological disorders such as Parkinson's disease (PD). This study proposed a novel approach for the quantitative assessment and analysis of gait disturbance from two-dimensional (2D) videos captured using smartphones. The approach used a convolutional pose machine to extract human body joints and a gait phase segmentation algorithm based on node motion characteristics to identify the gait phase. Moreover, it extracted features of the upper and lower limbs. A height ratio-based spatial feature extraction method was proposed that effectively captures spatial information. The proposed method underwent validation via error analysis, correction compensation, and accuracy verification using the motion capture system. Specifically, the proposed method achieved an extracted step length error of less than 3 cm. The proposed method underwent clinical validation, recruiting 64 patients with Parkinson's disease and 46 healthy controls of the same age group. Various gait indicators were statistically analyzed using three classic classification methods, with the random forest method achieving a classification accuracy of 91%. This method provides an objective, convenient, and intelligent solution for telemedicine focused on movement disorders in neurological diseases.

ObjectiveTo observe the effect of Jiawei Shenqi Yixin prescription on cardiovascular risk factors in the patients with heart failure with preserved ejection fraction and insulin resistance. MethodFrom January 2021 to January 2022, a total of 82 patients with heart failure with preserved ejection fraction were enrolled in the ward of the First Affiliated Hospital of Heilongjiang University of Chinese Medicine. The patients were randomly assigned into two groups ( 41 cases) and received the same basic treatment. The observation group was additionally treated with Jiawei Shenqi Yixin prescription for 8 weeks. The clinical efficacy, traditional Chinese medicine (TCM) efficacy, cardiac function indexes [NT-probrain natriuretic peptide (NT-proBNP) and 6-min walking test (6MWT)], echocardiographic parameters [left atrial volume index (LAVI), left ventricular mass index (LVMI), peak early diastolic to peak late diastolic mitral flow velocity (E/A) ratio], insulin resistance-related indexes [fasting insulin (FINS), fasting plasma glucose (FPG), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), homeostatic model assessment of insulin resistance (HOMA-IR), triglyceride-glucose index (TYG), and triglyceride to high-density lipoprotein cholesterol (TG/HDL-C) ratio], inflammatory cytokines [tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), adiponectin (ADP), and C-reactive protein (CRP)], vascular endothelial function indicators [nitric oxide (NO), endothelial nitric oxide synthase (eNOS), inducible nitric oxide synthase (iNOS), and endothelin-1 (ET-1)], and the safety of treatment were determined. In addition, Pearson correlation analysis was performed to analyze the correlations of insulin resistance, inflammatory cytokines, and vascular endothelial factors with the mitigation of heart failure. ResultIn terms of clinical efficacy, the therapy of the observation group was significantly effective in 26 patients, effective in 12 patients, ineffective in 3 patients, with the total effective rate of 92.68%, the therapy of the control group was significantly effective in 14 patients, effective in 12 patients, and ineffective in 15 patients, with the total effective rate of 63.41%. The clinical total effective rate of the observation group was higher than that of the control group (χ2=11.6, P<0.05). In terms of TCM efficacy, the therapy of the observation group was significantly effective in 26 patients, effective in 11 patients, and ineffective in 4 patients, with the total effective rate of 90.24%; the therapy of the control group was significantly effective in 9 patients, effective in 13 patients, and ineffective in 19 patients, with the total effective rate of 53.66%. The TCM total effective rate of the observation group was higher than that of the control group (χ2=8.19, P<0.05). Compared with those before treatment, the levels of NT-proBNP, LAVI, LVMI, FPG, FINS, HOMA-IR, TYG, TG/HDL-C, TNF-α, IL-6, CRP, ET-1, and iNOS in two groups declined after treatment (P<0.05), while the levels of 6MWT, E/A, ADP, NO, and eNOS elevated (P<0.05). After treatment, the observation group had lower levels of NT-proBNP, LAVI, LVMI, FPG, FINS, HOMA-IR, TYG, TG/HDL-C, TNF-α, CRP, and ET-1 (P<0.05) and higher levels of 6MWT, E/A, ADP, and NO than the control group (P<0.05). In addition, the increase in 6MWT after treatment was positively correlated with the increase in NO and the decrease in ET-1. The decrease in LVMI after treatment was positively correlated with the increase in NO and the decrease in FINS. The increase in left ventricular ejection fraction after treatment was positively correlated with the decreases in TNF-α and TYG (P<0.05). Adverse reactions were observed in neither group. ConclusionJiawei Shenqi Yixin prescription can significantly mitigate the symptoms, reduce inflammation, and improve vascular endothelial function in the patients with heart failure with preserved ejection fraction and insulin resistance, being safe without causing adverse reactions.
