1.Effect of neuromuscular electrical stimulation on quadriceps muscle strength and walking for patients after anterior cruciate ligament reconstruction

ObjectiveTo investigate the effect of neuromuscular electrical stimulation (NMES) on quadriceps muscle strength and walking for patients after anterior cruciate ligament reconstruction (ACLR). MethodsThirty-four patients after ACLR were selected at Beijing Rehabilitation Hospital of Capital Medical University from July, 2022 to October, 2023, and randomly divided into control group (n = 17) and experimental group (n = 17). Both groups received routine rehabilitation and functional training, and the experimental group received NMES during the functional training, while the control group received sham NMES, for eight weeks. Quadriceps peak torque-to-weight ratio, single-leg support phase and plantar impulses during walking were measured before and after intervention. ResultsTwo cases in the control group and three in the experimental group dropped down. Quadriceps peak torque-to-weight ratio improved in both groups after intervention (|t| > 17.578, P < 0.001), and improved more in the experimental group than in the control group (t = 4.714, P < 0.001); while the affected single-leg support phase and the affected/unaffected single-leg support phase ratio improved in both groups (|t| > 16.882, P < 0.001), and improved more in the experimental group than in the control group (t > 3.234, P < 0.01); and plantar impulses of all zones optimized in both groups (t > 9.221, P < 0.001), and were better in the experimental group than in the control group(|t| > 2.852, P < 0.01). ConclusionNMES may further improve quadriceps muscle strength, plantar pressure distribution during walking and single-leg support in patients after ACLR.
2.Effects of resveratrol on cGAS-STING signaling pathway in fibroblast-like synoviocytes of patients with rheumatoid arthritis

Objective :
To investigate the effects of resveratrol(Res) on fibroblast-like synoviocytes(FLS) in patients with rheumatoid arthritis(RA), and to explore the possible mechanism of Res inhibiting the release of inflammatory factors from FLS.
Methods :
FLS from RA patients were culturedin vitroand treated with different concentrations of Res(0, 20, 40, 80, 160, 320 μmol/L). The viability of FLS cells was detected by CCK-8 assay after 12 and 24 h. The contents of inflammatory factor interleukin-6(IL-6) and tumor necrosis factor-α(TNF-α) in cell supernatant were detected by ELISA. The expression levels of cyclic guanosine monophosphate-adenosine monophosphate synthase(cGAS) and stimulator of interferon gene(STING) were measured by Western blot; After lentivirus infection with FLS caused the cells to overexpress cGAS, the cells were divided into Control group(blank control), cGAS group(cGAS overexpression), Res+cGAS group(Res 160 μmol/L+cGAS overexpression) and Res group(Res 160 μmol/L). The expression level of STING protein in cells of each group was determined by Western blot, the viability of FLS cells in each group was detected by CCK-8, and the contents of inflammatory factor IL-6 and TNF-α in the supernatant of cells of each group were detected by ELISA method.
Results :
The results of CCK-8 experiment showed that under 40, 80, 160 μmol/L Res treatment, FLS viability decreased significantly after 24 h compared with blank control group(P<0.01). ELISA results showed that the contents of IL-6 and TNF-α in cell supernatant were also significantly decreased after treatment with Res of 40, 80 and 160 μmol/L(P<0.01). Meanwhile, Western blot results showed that Res could significantly decrease the protein expression levels of STING and cGAS in FLS cells after treatment of 40, 80 and 160 μmol/L(P<0.05,P<0.01). Compared with the Control group, the expression level of STING protein in FLS increased after overexpression of cGAS(P<0.05); compared with the Res group, the content of inflammatory factors in the supernatant of FLS and the expression level of STING protein in FLS significantly increased after overexpression of cGAS(P<0.01,P<0.05).
Conclusion
The appropriate concentration of Res can inhibit the release of inflammatory cytokines in FLS cells, which may be related to the blocking of cGAS-STING signaling pathway.
3.New treatment methods for complex urinary stones: Needle-perc assisted endoscopic surgery

Complex urinary stones have been the focus and difficulty of clinical treatment.Traditional multi-channel percutaneous nephrolithotomy (PCNL) has a high stone-free rate and efficacy in the treatment of multiple or large volume kidney stones,but has risk of serious complications such as bleeding and injury.Retrograde intrarenal surgery (RIRS) has been preferred in the treatment of renal stones with a diameter of less than 2 cm due to its characteristics of small trauma and fast recovery.However,it is difficult to treat small calyx stones with poor anatomy,lower renal calyx stones with small IPA,and diverticulum stones.Needle-perc assisted endoscopic surgery (NAES) includes the following modes:①Needle-perc plus PCNL,which can effectively reduce the number of channels,reduce surgical risks,and protect kidney function while ensuring a high stone-free rate; ②Needle-perc plus RIRS,which can significantly improve the success rate and stone-free rate without increasing kidney damage,thus having significant advantages in dealing with complex urinary stones.In order to popularize and promote this technique,this essay will review the relevant domestic and foreign literature and the clinical experience of our center,focusing on the indications,usage specifications,operative skills and precautions of NAES.
4.Research progress of radiomics in urolithiasis

As a new quantitative analysis method of radiology image data, radiomics has been widely used in the diagnosis and treatment evaluation of various diseases especially malignant tumors, promoting the development of individualization and precision in disease diagnosis and treatment. In urolithiasis, radiomics is mainly used in the differential diagnosis of ureteral calculi, preoperative prediction of different calculus compositions, and prediction of efficacy of various treatment modalities. This paper introduced the basic workflow of radiomics, and reviewed its application progress in urolithiasis.
5.Application of AI-MR in the planning of PCNL for special types of complex upper urinary stones

Objective To evaluate the application value of artificial intelligence mixed reality(AI-MR)technology in the planning of ultrasound-guided percutaneous nephrolithotomy(PCNL)for special types of complex upper urinary stones.Methods The prospective single-center,single-arm clinical study involved 15 patients with complex upper urinary stones undergoing ultrasound-guided PCNL during Aug.2022 and May 2023,including 9 male and 6 female,3 cases of pelvic ectopic kidney stones,5 cases of horseshoe kidney stones,3 cases of renal stones combined with spinal deformity,and 4 cases of transplant kidney stones.Based on preoperative computed tomography urography(CTU)data,digital three-dimensional reconstruction was performed,and AI-MR was used to project surgery-related three-dimensional images in real space to obtain"perspective"information of the surgical area.This facilitated preoperative design and planning,including target calyx,number of channels,and auxiliary measures.The compliance of target calyx and number of channels,stone clearance rate,total operation time,time required to establish the percutaneous renal channel,decrease in hemoglobin level,surgical complications,and postoperative hospital stay were analyzed.Results All 15 patients underwent preoperative planning using AI-MR and successfully completed one-stage ultrasound-guided PCNL.Based on the preoperative planning,we utilized S-PCNL alone or combined with Needle-perc or antegrade/retrograde FURS/RIRS.Among all patients,4 underwent single-channel S-PCNL,3 multi-channel S-PCNL,and 8 S-PCNL combined with Needle-perc or FURS.The compliance of target calyx and number of channels was 86.7%,the one-stage stone clearance rate was 80.0%,the average time for establishing the channel was(2.3±0.3)minutes,the average total operation time was(61.5±12.2)minutes,the mean decrease in hemoglobin level was(9.6±1.2)g/L,and the average postoperative hospital stay was(4.6±0.5)days.There were no Clavien-Dindo grade ≥ Ⅱ complications,such as blood transfusion,organ injury,or urosepsis.Conclusion Before surgery,AI-MR can be used to quantitatively analyze imaging data for patients with special types of complex upper urinary stones,which can achieve three-dimensional fluoroscopy effects,formulate surgical plans,optimize puncture paths,effectively avoid the risk of damage to surrounding organs,reduce complications,shorten treatment cycle and improve the first-stage stone clearance rate.
6.Effect of soft brace wearing on plantar dynamics in patients with chronic ankle instability

Objective To investigate the effect of daily soft brace wearing on plantar dynamics during walking in patients with chronic ankle instability(CAI). Methods A total of 52 patients with unilateral chronic ankle instability(CAI)in Beijing Rehabilitation Hospital from February,2021 to January,2023 were randomly divided into control group(n=26)and experimental group(n=26).Both groups underwent an eight-week exercise training program.The control group wore placebo brace dur-ing daily activities,while the experimental group wore soft ankle brace.Plantar dynamic parameters were mea-sured using a pressure plate system during walking,including peak plantar pressure and plantar impulse before and after intervention. Results Six participants dropped out in the control group and five in the experimental group,resulting in a final inclu-sion of 41 participants.After intervention,there was no significant difference in peak plantar pressure and im-pulse on the affected side in the control group among different areas(P>0.05).In the experimental group,the peak pressure and impulse in the heel medial,heel lateral and forefoot medial areas increased(|t|>4.192,P<0.001),while the peak pressure and impulse in the midfoot and lateral forefoot areas decreased(t>2.984,P<0.05);the peak pressure and impulse in the heel medial,heel lateral and forefoot medial areas were higher in the experimental group than in the control group(|t|>2.126,P<0.05),and the peak pressure and impulse were low-er in the midfoot and forefoot lateral areas(t>2.133,P<0.05). Conclusion Wearing a soft brace during daily activities may optimize the distribution of peak plantar pressure and plan-tar impulse on the affected side in patients with CAI,which may prevent recurrence of sprains.
7.The Safety and Feasibility of Simultaneous Bilateral Adrenal Vein Sampling via Basilic Vein Approach:the First Report

Objectives:To evaluate the safety and feasibility of simultaneous bilateral adrenal vein sampling(AVS)via the basilic vein approach. Methods:21 consecutive patients with primary aldosteronism(PA)who underwent simultaneous bilateral AVS via the basilic vein in Fuwai Hospital between July 2023 and November 2023 were enrolled in this study.The puncture site,catheter used in AVS,operation time,fluoroscopy time,contrast agent dosages,success rate of bilateral sampling,adverse events,and complications were recorded and analyzed.Successful sampling was determined by a selectivity index(cortisol in the adrenal vein/cortisol in inferior vena cava)greater than or equal to 2. Results:The average age of 21 patients was(49.3±7.7)years,with 13 male patients.The first 5F sheath was successfully inserted into the right basilic vein in all patients,the second 5F sheath insertion failed in two patients and switched to the ipsilateral cephalic vein approach.The 5F MPA1 catheter was inserted into the right adrenal vein and the 5F TIG catheter into the left adrenal vein in all patients.Operation time was 17.50(12.00,22.00)min,fluoroscopy time was 5.90(4.75,10.55)min,and contrast agent dosage was 25.00(25.00,35.00)ml.Bilateral AVS was successful in all patients.Two patients experienced adverse events,one case was catheter entanglement,which resulted in 5F TIG catheter slipped from adrenal vein,and another case was vascular spasm.No complications were recorded. Conclusions:Simultaneous bilateral AVS via basilic vein approach is safe and feasible in most PA patients,further researches with larger patient cohort are needed to validate the results from this study.
8.Comparative analysis of operative timing of laparoscopic cholecystectomy for acute calculous cholecystitis

Objective To determine the appropriate time to perform laparoscopic cholecystectomy(LC)for acute calculous cholecystitis.Methods The clinical data of 125 patients with acute calculous cholecystitis who underwent LC between Jan.2018 and Dec.2021 were retrospectively analyzed.According to the interval time from symptom onset to operation,all patients were divided into 2 groups:immediate group(43 cases)who underwent LC within 24 hours and delayed group(82 cases)who underwent LC in 25-72 h.The operation time,intraoperative blood loss,conversion to laparotomy,postoperative complications,postoperative hospitalization days and hospitalization expenses between the two groups were compared.Results The operation time,intraoperative blood loss of immediate group were lower than those of delayed group[50(45,65)min vs 65(55,95)min,10(10,20)ml vs 20(10,30)ml,P<0.05].There were no significant differences between the two groups with respect to incision infection,bile leak and biliary duct injury(4.7%vs13.4%,2.3%vs 2.4%,2.3%vs 1.2%,P>0.05),but the total rate of postoperative complications in immediate group reduced significantly(9.3%vs 29.3%,P<0.05).No mortality occurred in either group.Both of postoperative hospitalization days and direct medical costs reduced in immediate group[5(5,7)d vs 7(6,7)d,6 503(6 231,7 749)yuan vs 7 056(6 448,9 105)yuan,P<0.05].Conclusion LC for the operable patient with acute calculous cholecystitis,during 24 h from onset of symptom,significantly reduced the operation time,intraoperative blood loss and postoperative hospital stay without increasing the rate of bile leak and biliary duct injury.
9.Analysis on clinical characteristics and pathological characteristics of 6 cases of congenital liver fibrosis

Objective To explore the clinical features,imaging and pathological characteristics of the patients with congenital liver fibrosis(CHF).Methods The medical case data of 6 patients with pathological-ly diagnosed CHF in this hospital from January 2011 to June 2021 were retrospectively analyzed,and the clini-cal menifastations,laboratory indicators,imaging characteristics,pathological manifestations,treatment and outcomes were summarized.Results The clinical classification in 6 cases of CHF was mainly portal hyperten-sion(66.67%),and the most common clinical manifestations and signs were hepatosplenomegaly(83.33%),melena(50.00%)and abdominal distension(33.33%).Total bil irubin was normal,and only 2 cases(33.33%)were mildly elevated.The prothrombin time was in the normal range in 4 cases(66.67%),and mild abnormality in 2 cases(33.33%).Imaging showed abnormal liver morphology in 6 cases,spleen enlarge-ment in 5 cases(83.33%),portal vein widening in 3 cases(50.00%),diffuse dilatation of intrahepatic bile ducts in 2 cases(33.33%),and complicating Caroli disease in 1 case(16.67%).The pathological manifesta-tions were normal liver parenchymal cells,periportal fibrosis,and manifold-manifold bridging-like fibrosis.Six cases received the conventional liver protection therapy,3 cases underwent splenectomy+pericardia vascular dissection,the median follow-up time was 68.2 months,1 case died due to liver failure,and 5 cases were rela-tively stable.Conclusion The patients with hepatic disease whose portal hypertension is inconsistent to the degree of hepatic function damage,especially those complicating polycystic kidney disease should perfect the liver puncture pathological examination and genetic testing to clarify the diagnosis,and conduct the genetic counseling and intervention treatment as soon as possible.

Image navigation technology, artificial intelligence, and machine learning algorithms were further applied in the field of stone diagnosis and treatment.
