Objective To assess the correlation between the body mass index(BMI)and short-term prognosis in the patients with acute myocardial infarction complicating cardiogenic shock(AMI-CS).Methods A total of 262 patients with diagnosed AMI-CS hospitalized in this hospital from January 2013 to February 2022 were selected as the study subjects, and a total of 255 patients with AMI-CS were included after excluding the patients with incomplete clinical data,no coronary angiography, loss to follow-up and low BMI(<18.5 kg/m2).Among them, those with BMI of 18.5-<24.0 kg/m2 were included in the normal BMI group(n=150),and those with BMI of≥24.0 kg/m2 were included in the overweight or obese group(n=105).The baseline data, auxiliary examinations and treatment measures of the patients were collected, and the patients who did not die at the time of discharge were followed up by telephone on 30 d after AMI onset, and the survival status of the patients, whether having major adverse cardiovascular events(MACE)occurrence and the occurrence time were obtained. The correlation between BMI and endpoint events was assessed by the COX regression model. Results The proportion of complicating atrial fibrillation,BNP and urea levels in the overweight or obesity group were lower than those in the normal BMI group, the BMI and Hb levels were higher than those in the normal BMI group, and the differences were statistically significant(P<0.05).There was no statistically significant difference in the results of cardiovascular examination between the two groups(P>0.05).The use rate of β-receptor blockers in the overweight or obese group was higher than that in the normal BMI group, and the difference was statistically significant(P<0.05).On 30 d of follow-up,the all-cause mortality rate in the overweight or obese group was 39.0%,which was lower than 44.0%in the normal BMI group, but the difference was not statistically significant(P=0.430);there was no statistically signifi-cant difference in the incidence rate of each MACE event and the total incidence rate of MACE between the two groups(P>0.05).The BMI normal group served as the control, the risk of death and the occurrence risk of MACE events in the overweight or obese group had no significant increase(P>0.05).The short-term prognosis had no significant interaction between BMI and the patients in different subgroups(P>0.05).Con-clusion The short-term prognosis has no significant difference between the overweight/obese AMI-CS pa-tients and normal BMI AMI-Cs patients.

Objective:To analyze key aspects in cultural integration for cross-region specialists alliances of closed cooperation, for promoting such integration and identifying efficient operation of the practice. Methods:Quantitative and qualitative methods were used. 192 employees of primary hospitals participated in a questionnaire survey about their personal information and hospital culture self-evaluation on 18th February, 2019. Data so collected were subject to descriptive analysis. On February 21, 2019, the deans of the hospitals were invited for in-depth interviews, and focus group discussions were arranged for 7 doctors from the center hospital who had provided medical support for the primary hospital for more than half and a year, and for 5 doctors and 2 head nurses from the obstetric and neonatal department of the primary hospital respectively. The purpose is to understand the direction, key aspects, achievements and challenges of cultural integration between the two hospitals. Content analysis method was used to study recordings and interview documentation. Results:Employees of the primary hospital had a high satisfaction with indicators of organizational citizenship behavior, and the quality of medical care and team orientation, with the self-rated original scoring for the elements of hospital culture being 3.755, 3.754 and 3.698 respectively. On the other hand, they found insufficiencies in the innovation, poor orientation and incentive mechanism, with the self-rated original scoring for the elements of hospital culture being 3.469, 3.391 and 3.297 respectively. The self-rated total scoring was lower among medical technicians and those with bachelor′s degree or above, which were 3.029 and 3.202 respectively. Hospital culture integration is designed to strengthen technical guidance and care for doctors and patients, and to strengthen cooperation and support in HR training, scientific research innovation and spiritual culture construction. The key to integration is acceptance. The current roadblocks for efficient operation of this model come from medical insurance policy, material resources policy, logistics support, informationization management, personnel training and support and performance management. Conclusions:The cultural integration of cross-region specialists alliances of closed cooperation should be realized through the interactions of values, systems, behaviors and material resources dimensions. Government should play a leading and coordinating role and improve supporting measures.

OBJECTIVELow or moderate consumption of red wine has a greater benefit than the consumption of other beverages in the prevention of atherosclerosis and coronary heart disease and this is increasingly attributed to the polyphenol compounds in red wine, such as resveratrol. In the present study, we investigated the effect of resveratrol on platelet aggregation in vitro and in vivo.
METHODSPlatelet aggregation in rabbits and normal subjects was measured using Born's method.
RESULTSResveratrol, at 10 - 1000 micromol/L, significantly inhibited platelet aggregation in vitro induced by collagen, thrombin, and ADP in healthy subjects. The inhibitory effect was concentration-dependent. Hypercholesterolemia induced by high-cholesterol diet enhanced ADP-induced platelet aggregation. Resveratrol 4 mg x kg(-1) x d(-1) inhibited ADP-induced platelet aggregation in vivo despite no changes in serum lipid levels.
CONCLUSIONSResveratrol inhibits platelet aggregation both in vitro and in vivo. This may be one of the mechanisms by which resveratrol prevents atherosclerosis.

OBJECTIVETo investigate changes in the expression of sarcoplamic reticular Ca(2+)-ATPase (SERCA) and IP(3)-I receptors (IP(3)R(1)) mRNA in patients with atrial fibrillation.
METHODSThirty-eight patients with mitral stenosis undergoing open heart surgery were studied. 100 mg of atrial tissue was obtained during surgery from the right appendage and the right atrium. The amount of messenger ribonucleic acid (mRNA) amount of SERCA and IP(3)R(1) was measured by reverse transcription-polymerase chain reaction (RT-PCR) and normalized to the mRNA levels of glyceraldehyde 3-phosphate dehydrogenase (GAPDH).
RESULTSLevels of mRNA expression of SERCA in patients with AF, as compared with subjects in sinus rhythm, was lower and that of IP(3)R(1) was higher. The longer AF was sustained, the higher the levels of mRNA. There was no significant difference between right atrial free wall and right appendage.
CONCLUSIONSThe expression changes of SERCA and IP3R mRNA may correlate with the initiation or maintenance of AF.

AIM To investigate the effect of frozen recombinant staphylokinase on the hemostatic and fibrinolytic systems in healthy volunteers, in order to obtain reliable evidence for the possibility of further clinical application. METHOD r-Sak had been taken intravenously by 20 cases of healthy volunteers in different dosages (1 mg, 2.5 mg, 5 mg, 10 mg, 15 mg). The clinical hemorrhagic manifestations were observed and a set of hemostatic tests(BT, BPC, ATPP, PT, TT, Fg) and fibrinolytic tests (PL∶A,α2-PI∶A, FDP, D-D) monitored before and after injection. RESULT Four of 20 volunteers showed slight hemorrhagic tendency on mucocutaneous area (3/4 from gingivea and 2/4 at the sites of injection). It stopped spontaneously. None of them showed visceral bleeding. There were no significant changes in hemorrhagic and coagulative phases. Only 4 of them showed slight abnormal changes in D-D. It was supported that r-Sak was a highly selective fibrirolytic agent without significant influence in human hemostatic and coagulatic system. CONCLUSION The specific ranges of doseges, r-Sak is a relatively safe and well tolerated agent for healthy people. Further clinical study is still needed for the suitable dosage for clinical application.
