To systematically analyzed the reporting status of core elements in publicly available clinical practice guideline(hereafter referred to as "guideline") protocols published domestically and internationally over the past decade, identified existing problems, and provided evidence to inform the standardized writing and publication of future guideline protocols.

Objective To investigate the characteristics of executive function impairment in patients with chronic insomnia disorder and its correlation with Objective sleep structure parameters. Methods A total of 92 patients who met the DSM-5 diagnostic criteria for chronic insomnia disorder were enrolled as chronic insomnia disorder group， and 45 healthy controls were enrolled as healthy control group. All subjects underwent assessments for anxiety and depression， overnight polysomnography， and executive function， and the two groups were compared in terms of sleep parameters （including sleep latency， sleep efficiency， total sleep time， sleep stages， and the proportion of each stage） and executive function （Stroop Color-Word Test， digit span test， and trail making test）， as well as the correlation between these parameters. Results Compared with the healthy control group， the chronic insomnia disorder group had significant increases in Stroop Card C completion time， Stroop interference effect， trail making test-B completion time， and trail making test B-A time difference and significant reductions in the score of digit span backward task and the total score of digit span test （P<0.05）. The correlation analysis showed that total sleep time， sleep efficiency， NREM stage 3 duration， and the proportion of NREM stage 3 were negatively correlated with Stroop Card C completion time， Stroop interference effect， trail making test-B completion time， and trail making test B-A time difference and were positively correlated with the score of digit span backward task and the total score of digit span test （P<0.05）. Conversely， NREM stage 1 duration and the proportion of NREM stage 1 were positively correlated with Stroop Card C completion time， Stroop interference effect， trail making test-B completion time， and trail making test B-A time difference and were negatively correlated with the score of digit span backward task （P<0.05）. Conclusion Patients with chronic insomnia disorder exhibit executive function impairment in multiple dimensions including inhibitory control， working memory， and cognitive flexibility， which are associated with the reductions in sleep efficiency and deep sleep.

Background: Adrenocortical hormones, particularly 11-oxygenated androgens, are pivotal in female reproductive health and fertility. Standardized detection kits and population-specific reference intervals are lacking in China, hindering related clinical applications. Methods: A HPLC-tandem mass spectrometry (HPLC-MS/MS) pipeline was developed, rigorously validated, and applied to simultaneously quantify corticosterone, cortisone, cortisol, 18-OH cortisol, androstenedione (A4), 11β-hydroxyandrostenedione (11-OH A4), dehydroepiandrosterone, and dehydroepiandrosterone sulfate in serum samples from 455 reproductive-aged women (18–45 yrs) in Guangxi, China. Age-dependent concentration trends were analyzed, and reference intervals stratified by age (2.5th to 97.5th percentiles) were established. Correlations with body-composition metrics, ethnicity, and the menstrual cycle were investigated. Results: The HPLC-MS/MS method demonstrated high precision (intra- and inter-assay CVs < 15%), accuracy, and sensitivity. All eight hormones exhibited significant age-related declines (P < 0.001 for seven hormones; P = 0.001 for 11-OH A4). Notably, 11-OH A4 levels were significantly lower in the 35–45-yr (3.05 nmol/L) and 25–34-yr (3.09 nmol/L) age groups than in the 18–24-yr (3.57 nmol/L) age group, whereas no significant difference was observed between the 35–45-yr and 25–34-yr age groups. Weak negative correlations were observed between the body mass index and corticosterone and cortisone levels, whereas ethnicity and the menstrual cycle showed no significant associations with hormone levels. Conclusions We developed an HPLC-MS/MS-based method for simultaneously quantifying eight adrenocortical hormones, including 11-OH A4, and defined age-specific reference intervals for reproductive-aged Chinese women. These findings advance the clinical utility of adrenocortical hormones in diagnosing and managing reproductive disorders.

Objective:To investigate the effects of low-frequency neuromuscular electrical stimulation combined with exercise training on the recovery of rectus abdominis after delivery.Methods:This study used a case-control design. A total of 150 postpartum women with diastasis recti abdominis who were admitted to Women and Infants Hospital of Zhengzhou from January 2023 to March 2024 were selected for the study. The patients were divided into a treatment group ( n = 75, receiving low-frequency neuromuscular electrical stimulation combined with exercise training) and a control group ( n = 75, receiving exercise training only) based on the different treatment methods used. Both groups underwent treatment for 8 weeks. The inter-rectus distance, abdominal core muscle strength, pain levels, and thickness of the bilateral rectus abdominis muscles were compared between the two groups. Results:After treatment, the inter-rectus distances at 3 cm above the umbilicus, at the umbilicus, and 3 cm below the umbilicus in the treatment group were all smaller than those in the control group ( t = -3.25, -3.49, -4.52, all P<0.05). The time taken for the treatment group to complete the level 1 test of the eight-level abdominal bridge test was significantly longer than that in the control group [(62.00 ± 7.03) seconds vs. (35.89 ± 6.88) seconds, t = 22.99, P<0.05]. The root mean square values of the transverse abdominis muscle on both the affected side and the healthy side during the stepping test in the treatment group were greater than those in the control group [(6.70 ± 0.92) vs. (5.64 ± 0.89), t = 7.17, P<0.05]. The scores for each item on the Global Pain Scale and the total score in the treatment group were significantly lower than those in the control group (all P<0.05). The thicknesses of the rectus abdominis muscles on both sides (left side: at the umbilicus, 3 cm below the umbilicus; right side: 3 cm above the umbilicus, at the umbilicus, and 3 cm below the umbilicus) in the treatment group were greater than those in the control group (all P<0.05). Conclusions:Low-frequency neuromuscular electrical stimulation combined with exercise training can promote the recovery of rectus abdominis function, reduce the inter-rectus distance, improve core muscle strength, increase the thickness of the rectus abdominis, and relieve pain.

AIM:To compare the degree of disease simulation between the two mouse models of acute exacer-bation of chronic obstructive pulmonary disease(AECOPD)using intranasal instillation of lipopolysaccharide(LPS)and fine particulate matter(PM2.5)for 3 d based on exposure to cigarette smoke(CS)for 90 d.METHODS:Thirty-two male C57BL/6 mice were randomly divided into 4 groups:control group(n=8),CS group(n=8),CS+PM2.5 group(n=8)and CS+LPS group(n=8).The AECOPD models in CS+PM2.5 and CS+LPS groups were established by CS exposure combined with intranasal PM2.5 and LPS instillation.Lung function,lung pathology and airway goblet cell hyperplasia using histologi-cal staining were measured.To evaluate the degree of lung inflammation and mucus secretion in mice,the prorein levels of mucin 5AC(MUC5AC),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)in the bronchoalveolar lavage fluid(BALF)were detected by ELISA and total white blood cell(WBC)counts and the BALF differential cell counts(neutro-phils,macrophages,lymphocytes)were detected by Giemsa staining.RESULTS:In CS group,lung function decreased(P<0.05),and bronchial inflammation index increased(P<0.01),airway goblet cell hyperplasia and airway collagen de-position were significant(P<0.01),total WBC count and differential cell count in the BALF increased(P<0.05),MUC5AC and inflammatory factor IL-6 and TNF-α levels increased(P<0.05),compared with control group.Compared with CS group,lung function decreased(P<0.05),the bronchial inflammation index increased(P<0.01),airway goblet cell hyperplasia and airway collagen deposition were significant(P<0.01),total WBC count and differential cell count in the BALF increased(P<0.05),and MUC5AC and inflammatory factor IL-6 and TNF-α levels increased(P<0.05)in CS+PM2.5 and CS+LPS groups.Compared with CS+PM2.5 group,lung function decreased(P<0.05),the bronchial inflamma-tion index increased(P<0.01),airway goblet cell hyperplasia and airway collagen deposition were significant(P<0.01),total WBC count and differential cell count in the BALF increased(P<0.05),and MUC5AC and inflammatory factor IL-6 and TNF-α levels increased(P<0.05)in CS+LPS group.CONCLUSION:Exposure to CS combined with both intrana-sal PM2.5 and LPS instillation allowed for establishing AECOPD models in mice,and CS exposure combined with intrana-sal LPS instillation better simulated AECOPD characteristics.

The interplay between psychological pain and physical pain is one of the key issues in the research of non-suicidal self-injury (NSSI). Psychological pain, driven by negative self-perception and difficulties in emotional regulation, activates specific brain regions, such as the anterior cingulate gyrus and insula, areas that overlap with neural networks involved in physical pain. Physical pain can, in turn, activate distinct neural pathways that temporarily alleviate psychological pain. However, repeated self-injury may lead to increased pain tolerance over time, contributing to a vicious cycle. This paper reviews the potential neural network connections and interactions between psychological and physical pain in the context of NSSI behavior. It also summarizes current clinical interventions and their effectiveness in treating NSSI, aiming to offer new insights for future research and clinical practice in preventing and treating NSSI.

ObjectiveTo evaluate the clinical efficacy and safety of the traditional Chinese medicine （TCM） compound Chaiqin Xiaoyong Decoction （柴芩消痈饮， CXD） combined with Jinhuang Ointment （金黄膏， JO） in treating the nodular stage of acne mastitis of liver meridian heat accumulation type. MethodsA randomized， double-blind， placebo-controlled clinical trial was conducted. A total of 108 patients with liver meridian heat accumulation type acne mastitis in the nodular stage were randomly assigned to a treatment group and a control group， with 54 patients in each group. Both groups received topical application of JO once daily at a thickness of 3~5 mm for 8 hours， along with standard nursing care. On this basis， the treatment group received oral CXD granules， while the control group received placebo granules， administered twice daily， 3 sachets per dose， for 14 consecutive days. Clinical efficacy， TCM symptom scores， nodule size， visual analogue scale （VAS） pain scores， white blood cell （WBC） count， C-reactive protein （CRP） level， and systemic immune-inflammation index （SII） were compared. At the end of treatment， efficacy and safety indicators were evaluated. A 6-month follow-up was conducted to compare the proportion of patients undergoing surgical treatment. ResultsThe total clinical efficacy rate in the treatment group was 90.38% （47/52）， significantly higher than 32.00% （16/50） in the control group （P＜0.01）. The treatment group also showed significantly lower TCM symptom scores， VAS scores， nodule size， WBC count， CRP level， and SII （P＜0.05 or P＜0.01）. During follow-up， the surgical intervention rate in the treatment group was 5.77% （3/52）， lower than 14.00% （7/50） in the control group， with a statistically significant difference （P＜0.01）. No significant abnormalities were observed in safety indicators before and after treatment in either group. ConclusionCXD effectively reduces nodule size and alleviates symptoms such as redness and pain in patients with acne mastitis of liver meridian heat accumulation type， improves TCM symptom scores， enhances overall clinical efficacy， and demonstrates good safety.
