1.Risk factors for PICC central line-associated bloodstream infection in cancer patients

OBJECTIVE To analyze the risk factors for peripherally inserted central catheter(PICC)central line-as-sociated bloodstream infection in cancer patients and explore corresponding management strategies.METHODS The data of 52 chemotherapy patients with PICC central line-associated bloodstream infection(PICC-CLABSI)ad-mitted to the Department of Gastroenterology of the First Affiliated Hospital of Air Force Medical University be-tween May 2021 and May 2023 were collected retrospectively,and the patients were classified as the infection group.Additionally,183 chemotherapy patients without PICC-CLABSI during the same period were included as the non-infection group.Clinical data from both groups were collected.Logistic regression analysis was used to identify risk factors for PICC-CLABSI in cancer patients,and receiver operating characteristic(ROC)curves were employed to assess the accuracy of predictive variables.RESULTS A total of 57 pathogens were isolated from 52 patients with PICC-CLABSI,including 15 strains of Escherichia coli,12 strains of Klebsiella pneumoniae,9 strains of Pseudomonas aeruginosa and 9 strains of Staphylococcus aureus.Diabetes(OR=2.694),catheter in-dwelling time≥30 d(OR=7.146),number of chemotherapy(OR=6.617),maintenance frequency of once per week(OR=2.803)and maintenance method(OR=6.289)were identified as risk factors for PICC-CLABSI(P<0.05).The area under the curve for the combined prediction of PICC-CLABSI was 0.904,with a sensitivity of 0.750 and a specificity of 0.907.CONCLUSIONS Diabetes,catheter indwelling time and number of chemotherapy are risk factors for PICC-CLABSI,while a maintenance frequency of once per week and the maintenance method are protective factors.Close attention should be paid to the assessment of risk factors after catheter placement,and targeted anti-infection strategies should be implemented.
2.Clinical features and prognosis of patients with primary sclerosing cholangitis:An analysis of 107 cases

Objective To describe the clinical features of patients with primary sclerosing cholangitis(PSC)in China based on a nationwide multicenter patient cohort,and to investigate the risk factors for prognosis.Methods A retrospective cohort study was conducted among the patients with a confirmed diagnosis of PSC based on the electronic medical record system of seven grade A tertiary hospitals across the country,and related data were extracted.The Mann-Whitney U test was used for comparison of continuous data between groups,and the chi-square test was used for comparison of categorical data between groups.The Kaplan-Meier method was used to estimate liver transplant-free survival,and the log-rank test was used for comparison of survival rate between PSC patients with different features.The Cox regression model was used to identify independent risk factors for the prognosis of PSC patients and the interactions between key factors.Results A total of 107 patients were enrolled,among whom 55.6%(55/99)had large-duct PSC and 29.0%(31/107)had comorbidity with inflammatory bowel disease(IBD).The positivity rate of anti-neutrophil cytoplasmic antibody(ANCA)was 32.9%(24/73),and 50.0%(40/80)of the patients had an increase in IgG/IgM.The median symptom-to-diagnosis interval was 1 year(<1-4.0),and 38.3%(41/107)of the patients had progressed to decompensated cirrhosis at the time of diagnosis.The median liver transplant-free survival time was 114 months(95%confidence interval[CI]:62-166),with a 5-year survival rate of 65.7%.The multivariate analysis showed that an increase in total bile acid(TBA)(hazard ratio[HR]=1.006,95%CI:1.002-1.010,P=0.001)and a prolonged symptom-to-diagnosis interval(HR=1.252,95%CI:1.059-1.480,P=0.009)were independent risk factors for prognosis.The interaction analysis showed that compared with the female patients with TBA<50 μmol/L,both male and female patients with TBA≥50 μmol/L had a significant increase in the risk of liver transplantation or death(male:HR=16.563,95%CI:2.103-130.449,P<0.001;female:HR=17.009,95%CI:2.113-136.934,P<0.001),and compared with the patients with an age of<45 years and a TBA level of<50 μmol/L,the patients with an age of≥45 years and a TBA level of≥50 μmol/L had a significant increase in the risk of liver transplantation or death(HR=10.729,95%CI:1.325-86.859,P=0.026).Compared with the female patients with an symptom-to-diagnosis interval of≤2 years,the male patients with a symptom-to-diagnosis interval of>2 years had an increased risk of liver transplantation or death(HR=4.825,95%CI:1.725-13.644,P=0.003),and compared with the patients with an age of<45 years and a symptom-to-diagnosis interval of≤2 years,the patients with an age of<45 years and a symptom-to-diagnosis interval of>2 years had an increased risk of liver transplantation or death(HR=4.983,95%CI:1.366-18.173,P=0.015).Conclusion Compared with the reports from Western countries,large-duct PSC is also the main type of PSC in China,but with a relatively low proportion,and there is also a relatively low proportion of patients with IBD or positive ANCA.An increase in TBA and a prolonged symptom-to-diagnosis interval are independent risk factors for prognosis,with significant interactions with age and sex.This suggests that early screening and intervention should be enhanced to improve prognosis.
3.Effect of Wenshen Tongluo Zhitong formula on mouse H-type bone microvascular endothelial cell/bone marrow mesenchymal stem cell co-culture system

BACKGROUND:Bone relies on the close connection between blood vessels and bone cells to maintain its integrity.Bones are in a physiologically hypoxic environment.Therefore,the study of angiogenesis and osteogenesis in hypoxic environment is closer to the microenvironment in vivo. OBJECTIVE:To explore the influence of Wenshen Tongluo Zhitong(WSTLZT)formula on H-type bone microvascular endothelial cell/bone marrow mesenchymal stem cell co-culture system in hypoxia environment and its related mechanism. METHODS:Enzyme digestion method and flow sorting technique were used to isolate and identify H-type bone microvascular endothelial cells.Mouse bone marrow mesenchymal stem cells were isolated and obtained by bone marrow adhesion method.H-type bone microvascular endothelial cell/bone marrow mesenchymal stem cell hypoxic co-culture system was established using Transwell chamber and anoxic culture workstation.WSTLZT formula powder was used to intervene in each group at a mass concentration of 50 and 100 μg/mL.The angiogenic function of H-type bone microvascular endothelial cells in the co-culture system was evaluated by scratch migration test and tube formation test.The osteogenic differentiation ability of bone marrow mesenchymal stem cells in the co-cultured system was evaluated by alkaline phosphatase staining and alizarin red staining.The protein and mRNA expression changes of PDGF/PI3K/AKT signal axis related molecules in H-type bone microvascular endothelial cells in the co-cultured system were detected by Western Blotting and q-PCR,respectively. RESULTS AND CONCLUSION:(1)Compared with the normal oxygen group,the scratch mobility and new blood vessel length of H-type bone microvascular endothelial cells were significantly higher(P<0.05);the osteogenic differentiation capacity of bone marrow mesenchymal stem cells was higher(P<0.05);the expression of PDGF/PI3K/AKT axis-related molecular protein and mRNA increased(P<0.05)in the hypoxia group.(2)Compared with the hypoxia group,scratch mobility and new blood vessel length were significantly increased in the H-type bone microvascular endothelial cells(P<0.05);bone marrow mesenchymal stem cells had stronger osteogenic function(P<0.05);the expression of PDGF/PI3K/AKT axis-related molecular proteins and mRNA further increased(P<0.05)after treatment with different dose concentrations of WSTLZT formula.These findings conclude that H-type angiogenesis and osteogenesis under hypoxia may be related to the PDGF/PI3K/AKT signaling axis,and WSTLZT formula may promote H-type vasculo-dependent bone formation by activating the PDGF/PI3K/AKT signaling axis,thereby preventing and treating osteoporosis.

Recent research progress into the use of Chinese medicine has demonstrated good therapeutic effects for increasing numbers of Chinese medicines for immune system diseases.Atopic dermatitis(AD)is an inflammatory disease characterized by type 2 immunity,and research into its pathogenesis and therapeutic immunopharmaceuticals has result ed in various different types of animal models.This review summarizes the existing animal models of AD and their immune-related characteristics,with the aim of providing appropriate references for the selection of future research models related to AD.
6.Differentiation and treatment of acute gouty arthritis from the perspective of six climatic exopathogens

Acute gouty arthritis is an inflammatory disease characterized by redness,swelling,heat,and pain in the joints.It mostly occurs at the distal ends of the limbs,especially in the toe joints.The six climatic exopathogens,which include wind,cold,summer-heat,dampness,dryness,and fire(heat),are the general term for exogenous pathogenic factors.This article explores the pathogenesis and treatment approaches of acute gouty arthritis from the perspective of the six climatic exopathogens.All the six climatic exopathogens can lead to the occurrence of acute gouty arthritis,among which"fire"is the most closely associated with this disease.Wind-pathogenic factor accumulates without dissipation and transforms into fire internally;cold-pathogenic factor congeals without warmth and transforms into fire after long-term stagnation;when wind and dampness interact and cannot circulate,they stagnate and transform into fire;dry-pathogenic factor accumulates internally without release,transforms into fire,and consumes yin;fire stagnates and intensifies when it cannot be discharged.It can be seen that the six climatic exopathogens are the inducing factors for acute gouty arthritis,and"fire"is the key factor and the general summary of the disease occurrence.Therefore,it is proposed that the core therapeutic principle is clearing heat-fire.Simultaneously,combined with clinical practice,methods are supplemented,such as expelling wind,dredging collaterals and relieving pain;warming yang,dispelling cold and eliminating arthralgia;drying dampness,strengthening the spleen and facilitating joint function;and moistening dryness,nourishing yin and dredging collaterals.Acute gouty arthritis is not simply treating based on the theory of fire.By tracing back to the root cause,treating both the symptoms and the root cause,it is hoped that this can provide a theoretical basis for the clinical treatment of acute gouty arthritis from the perspective of the six climatic exopathogens and broaden the therapeutic ideas.
7.Mechanism of Jiawei Simiao powder on monosodium urate crystal-induced RAW264.7 cell inflammation via miR-146a regulation of the Toll-like receptor 4/nuclear factor-κB signaling pathway

Objective To investigate the effect of Jiawei Simiao powder on the Toll-like receptor 4(TLR4)/nuclear factor-κB(NF-κB)signaling pathway through miR-146a in monosodium urate(MSU)crystal-induced RAW264.7 macrophage inflammation models,and to explore its anti-inflammatory mechanism.Methods A cell model of gouty arthritis was established by inducing RAW264.7 cells with MSU crystals.Cells were divided into control,model,Jiawei Simiao powder,and colchicine groups and cell viability was assessed using the CCK-8 method.Levels of interleukin(IL)-1β,IL-6,and tumor necrosis factor(TNF)-α were measured by enzyme-linked immunosorbent assay.Expression levels of miR-146a miRNA and TLR4,myeloid differentiation factor 88(MyD88),TNF receptor-associated factor 6(TRAF6),and NF-κB p65 mRNA were detected by quantitative real-time polymerase chain reaction amplification technology,and protein expression levels of TLR4,MyD88,TRAF6,and phospho(p)-NF-κB p65 were evaluated by Western blot.Results miR-146a expression was significantly decreased in the model group compared with the control group(P<0.01),while mRNA and protein expression levels of TLR4,MyD88,and TRAF6,and protein expression of p-NF-κB p65,IL-1β,IL-6,and TNF-α were significantly increased(P<0.01).In contrast,miR-146a expression was significantly increased(P<0.01)and mRNA and protein expression of TLR4,MyD88,and TRAF6,and p-NF-κB p65 protein expression(P<0.01),as well as IL-1β,IL-6,and TNF-α levels(P<0.05)were significantly decreased in the Jiawei Simiao powder and colchicine groups.Conclusions Jiawei Simiao powder may alleviate MSU crystal-induced macrophage inflammatory responses by upregulating miR-146a,suppressing TLR4/NF-κB signaling pathway activation,and decreasing secretion of inflammatory factors.
8.Evaluation of Left and Right Ventricular Myocardial Strain in Hypertensive Heart Disease Using Magnetic Resonance Tissue Tracking Technology

Purpose To explore the characteristics of left and right ventricular myocardial strain in hypertensive heart disease(HHD)assessed by magnetic resonance feature tracking technique,and to assess the effect of HHD on right ventricular function.Materials and Methods A retrospective analysis was performed on 40 patients with HHD at the General Hospital of Ningxia Medical University from January 2022 to August 2023.Among these 40 HHD patients,19 presented with a left ventricular ejection fraction(LVEF)of≥50%,and 21 presented with an LVEF of<50%.Additionally,25 healthy controls were enrolled.All subjects underwent cardiac magnetic resonance imaging.The cine sequence images were imported into cardiac post-processing analysis software(QIR suite software).The left ventricular radial strain(LVRS),left ventricular circumferential strain(LVCS),left ventricular longitudinal strain(LVLS)and right ventricular longitudinal strain(RVLS)were measured,and left and right ventricular strain parameters of the three groups were analyzed statistically.Results The absolute values of LVRS,LVCS,LVLS and RVLS in the HHD group with LVEF<50%were lower than those in the normal control group(t=7.48,-12.69,-8.19,-6.32,all P<0.001)and those in the HHD group with LVEF≥50%(t=5.97,-8.88,-4.50,-4.90,all P<0.001);the area under the curve values of LVRS,LVCS,LVLS and RVLS for distinguishing HHD from normal controls were 0.804,0.785,0.829,0.832,respectively,and the cut-off values of LVRS,LVCS,LVLS and RVLS were 22.9%,-10.0%,-11.9%and-18.7%,respectively.Conclusion Magnetic resonance feature tracking technique can detect left and right ventricular myocardial dysfunction in HHD at an early stage and can provide a new basis for evaluating cardiac function.
9.Analysis of Animal Models of Myasthenia Gravis Based on Its Clinical Characteristics in Chinese and Western Medicine

Myasthenia gravis(MG)is an autoimmune disease characterized primarily by skeletal muscle weakness and,in severe cases,respiratory involvement.Western medical treatment predominantly relies on immunosuppressants,but long-term administration often leads to notable side effects.In contrast,traditional Chinese medicine(TCM)offers the advantage of multi-target interventions.However,the pathogenesis of MG has not been fully elucidated,and the establishment of animal models that accurately reflect the clinical characteristics of both Chinese and Western medicine is essential for mechanism research and new drug development.This paper systematically reviews the etiology and pathogenesis,diagnostic criteria,and progress of animal model research for MG from both Chinese and Western medicine perspectives.In Western medicine,the pathogenesis of MG is closely related to genetic susceptibility,environmental factors,and autoantibody-mediated postsynaptic membrane damage.In TCM,MG is classified under the category of"flaccidity syndrome",attributed to congenital deficiencies and acquired malnourishment.Western diagnostic criteria involve a combination of clinical symptoms,fatigue testing,serum antibody assays,and electrophysiological evaluation.In contrast,TCM diagnosis emphasizes the integration of primary and secondary symptoms with tongue and pulse pattern differentiation.Currently available animal models mainly include experimental autoimmune myasthenia gravis(EAMG)and passive transfer myasthenia gravis(PTMG).The Toredo acetylcholine receptor(AChR)induced EAMG model aligns well with Western diagnostic criteria,but poorly matches secondary symptoms in TCM.The synthetic AChR peptide model is widely used,but shows low conformity with TCM syndromes.Models induced by muscle-specific tyrosine kinase(MuSK),low-density lipoprotein receptor-related protein 4(LRP4),and transgenic models demonstrate high innovation but exhibit low clinical conformity.Evaluation of these models requires integration of behavioral,electrophysiological,and immunological indicators.However,a systematic framework for modelling TCM syndromes is still lacking.Future research should integrate TCM-based etiological modelling methods with the Western pathological mechanisms to construct disease-syndrome combination models.Additionally,it is crucial to establish a TCM syndrome evaluation system based on"validation by prescription",as well as to improve the scientific rigor and practicality of animal models by the incorporation of emerging technologies.This review provides a theoretical foundation for optimizing MG animal model design,advancing the research on the combination of Chinese and Western medicine,and supporting efficacy assessment and mechanism exploration of Chinese herbal prescriptions.
