1.Study on Graded Quantitative Diagnosis of Lung Qi Deficiency Syndrome in Chronic Obstructive Pulmonary Disease Based on Latent Class Analysis Combined with Hidden Structure Model

ObjectiveTo clarify the graded quantitative diagnostic characteristics of lung qi deficiency syndrome in chronic obstructive pulmonary disease (COPD) based on latent class analysis combined with a hidden structure model. MethodsClinical data, including the four diagnostic methods of traditional Chinese medicine (TCM), were collected from 745 COPD patients with lung qi deficiency syndrome. Latent class modeling was performed using R 4.1.2 software, and each patient was classified into one of three severity categories (mild, moderate, or severe) based on probabilistic parameterization, parameter estimation, and model fitting. A database was established for different severity levels of lung qi deficiency syndrome. Based on this, Lantern 5.0 software was used to construct hidden structure models for mild, moderate, and severe lung qi deficiency syndrome, and syndrome differentiation rules were developed through comprehensive clustering. ResultsA latent class model was constructed using 28 symptoms and signs with a frequency greater than 10%. Considering TCM theory and model simplicity, the optimal model was determined when the number of latent classes was three, categorizing lung qi deficiency syndrome into mild (298 cases), moderate (164 cases), and severe (283 cases). Hidden structure models were separately developed for each severity level, and syndrome differentiation rules were established. A comparison of common symptoms in the syndrome differentiation rules for mild and moderate lung qi deficiency syndrome showed no statistically significant differences in diagnostic values and weights (P>0.05), leading to their combined analysis and the development of a unified syndrome differentiation rule. Value and weight of quantitative diagnosis of mild-to-moderate lung qi deficiency syndrome were as followed: shortness of breath (diagnostic value 9.3, diagnostic weight 86.92%), dyspnea on exertion (8.2, 76.64%), low voice and reluctance to speak (6.7, 62.62%), poor appetite (4.0, 37.38%), loose stools (4.0, 37.38%), weak cough sound (2.9, 27.10%), wheezing (2.3, 21.50%), fatigue (1.8, 16.82%), spontaneous sweating (1.7, 15.89%), susceptibility to colds (1.6, 14.95%), swollen tongue (1.4, 13.08%), teeth marks on the tongue edge (1.2, 11.21%), deep pulse (1.6, 14.95%), with a diagnostic threshold of 10.3. Value and weight of quantitative diagnosis of severe lung qi deficiency syndrome were as followed: weak cough sound (15.1, 61.13%), soreness and weakness of the waist and knees (12.6, 51.01%), shortness of breath (11.1, 44.94%), low voice and reluctance to speak (8.3, 33.60%), frequent nocturia (6.1, 24.70%), spontaneous sweating (3.7, 14.98%), susceptibility to colds (3.5, 14.17%), teeth marks on the tongue edge (7.8, 31.58%), pale tongue body (1.9, 7.69%), white tongue coating (5.5, 22.27%), thin pulse (1.5, 6.07%), with a diagnostic threshold of 23.7. ConclusionThe combination of latent class analysis and a hideen structure model effectively clarified the graded quantitative diagnostic characteristics of lung qi deficiency syndrome, providing a reference for the quantitative diagnosis of other fundamental syndromes in TCM.
2.Diagnostic value of miR-128-3p, SIRT1, and AMPK in patients with type 2 diabetes mellitus comorbid with nonalcoholic fatty liver disease

ObjectiveTo investigate the expression levels of miR-128-3p, SIRT1, and AMPK in the peripheral blood of patients with type 2 diabetes mellitus (T2DM) comorbid with nonalcoholic fatty liver disease (NAFLD), as well as the role of miR-128-3p in predicting NAFLD in T2DM patients. MethodsA total of 80 patients with T2DM who were hospitalized in The First Affiliated Hospital of Anhui University of Chinese Medicine from September 2022 to August 2023 were enrolled and divided into T2DM group with 40 patients and NAFLD group with 40 patients, and according to the NAFLD fibrosis score (NFS), the patients were further divided into progressive liver fibrosis group with 16 patients and non-progressive liver fibrosis group with 64 patients. General data and biochemical parameters were collected; quantitative real-time PCR was used to measure the mRNA expression levels of miR-128-3p, SIRT1, and AMPK in peripheral blood, and Western blot was used to measure the protein expression levels of SIRT1 and AMPK. The independent-samples t test was used for comparison of normally distributed data between two groups, and the Mann-Whitney U test was used for comparison of data with skewed distribution between two groups; the chi-square test was used for comparison of categorical data between two groups. The logistic regression analysis was used to identify the influencing factors for the presence of NAFLD and progressive liver fibrosis, and the receiver operating characteristic (ROC) curve analysis was used to determine the optimal cut-off value of miR-128-3p for predicting NAFLD. ResultsThere were significant differences between the NAFLD group and the non-NAFLD group in body mass index, fasting plasma glucose, glycated hemoglobin, fasting insulin, fasting C-peptide, alanine aminotransferase (ALT), aspartate aminotransferase, gamma-glutamyl transpeptidase, alkaline phosphatase, fibronectin, triglycerides, high-density lipoprotein cholesterol, total triiodothyronine (TT3), Homeostasis Model Assessment of Insulin Resistance (HOMA-IR), and NFS (all P<0.05). Compared with the non-NAFLD group, the NAFLD group had a significantly higher mRNA expression level of miR-128-3p in peripheral blood (t=-8.765, P<0.001) and significant reductions in the mRNA and proteins expression levels of SIRT1 and AMPK (P<0.001). There were significant differences between the progressive liver fibrosis group and the non-progressive liver fibrosis group in age, ALT, free triiodothyronine, TT3, superoxide dismutase, and miR-128-3p (all P<0.05). The logistic regression analysis showed that miR-128-3p was an independent risk factor for the development of NAFLD (odds ratio [OR]=8.221, 95% confidence interval [CI]: 2.735 — 24.714, P<0.001) and progressive liver fibrosis (OR=1.493, 95%CI: 1.117 — 1.997, P=0.007). The ROC curve analysis showed that miR-128-3p had an area under the ROC curve of 0.890 (95%CI: 0.829 — 0.950), with an optimal cut-off value of 13.165, a sensitivity of 89.3%, and a specificity of 72.7%. ConclusionThere is an increase in the expression of miR-128-3p in peripheral blood of T2DM patients with NAFLD, while there are reductions in the expression levels of SIRT1 and AMPK, suggesting that miR-128-3p has a certain diagnostic value in identifying NAFLD and liver fibrosis in such population.
3.Development,reliability and validity of nurses'knowledge,attitude and practice questionnaire on medication management for patients with dysphagia

Objective To develop nurses'knowledge,attitude and practice questionnaire on medication management for patients with dysphagia,and test its reliability and validity.Methods Based on the evidence-based summary of the best evidence of medication management for patients with dysphagia,guided by the the-ory of knowledge,attitude and practice,the basic dimensions and item pool of the questionnaire were deter-mined through group discussion,Delphi expert consultation and pre-investigation.In order to revise the ques-tionnaire,437 nurses from 10 tertiary hospitals in Jiangsu Province were conveniently selected for investigation,and the reliability and validity of the questionnaire were tested according to the survey results.Results The nurses'knowl-edge,attitude and practice questionnaire on medication management for patients with dysphagia included 43 items in three dimensions.The three dimensions were analyzed by exploratory factors,and six common factors with characteristic roots>1 were extracted.Two factors were extracted from the knowledge dimension,and the cumulative variance contribution rate was 74.958%,One factor was extracted from the attitude dimen-sion,and the cumulative variance contribution rate was 77.655%.Three factors were extracted from the prac-tice dimension,and the cumulative variance contribution rate was 72.274%.The factor load of each item was 0.618-0.902,Cronbach's α coefficient of the total questionnaire was 0.949,and the test-retest reliability was 0.909.The overall content validity coefficient of the questionnaire was 0.922,and the content validity coeffi-cient for each item was 0.800-1.000.Conclusion The nurses'knowledge,attitude and practice questionnaire on medication management for patients with dysphagia developed in this study has good reliability and validi-ty,and could be used as an effective tool to evaluate the status quo of nurses'medication management for pa-tients with dysphagia.
4.Perioperative management strategy and efficacy of the left ventricular assist device HeartCon in the treatment of adult patients with end-stage heart failure

Objective:To discuss the perioperative management strategy of the HeartCon left ventricular assist device in the treatment of adult patients with end-stage heart failure and evaluate the effectiveness of blood pump.Methods:Ten consecutive patients with end-stage heart failure treated with the LVAD HeartCon at the Department of Cardiovascular Surgery, Jiangsu Province Hospital from July 2021 to July 2023 were enrolled in this study. The clinical data and follow-up results were retrospectively analyzed. Blood pump parameter, cardiac function classification, liver and kidney function, coagulation, myocardial markers, N-terminal pro-B-type natriuretic peptide, von Willebrand factor antigen, echocardiography, cardiothoracic ratio, and 6-minute walking distance test (6MWT) were evaluated before and after implantation of LVAD in 30, 60, 90, 180, 360, 540, and 720 days. EQ-5D-5L questionnaire was used to evaluate the quality of life.Results:There were 9 males and 1 female with a mean age of (53.7±9.7) years. All patients survived. Renal insufficiency occurred in 1 patient and recurrent aseptic granuloma occurred in 1 patient. There were no significant differences in aspartate aminotransferase (AST), alanine aminotransferase (ALT), lactate dehydrogenase (LDH), serum creatinine (Scr), blood urea nitrogen (BUN), right ventricular fractional area change (FAC) and tricuspid annular plane systolic excursion (TAPSE) between pre-operation and 30, 60 and 90 days post-operation( P>0.05). The levels of N-terminal pro-B-type natriuretic peptide (NT-proBNP), von Willbrand factor (vWF) antigen, left ventricular end-diastolic diameter (LVDd) and cardiothoracic ratio decreased significantly on the 30th, 60th and 90th day after operation ( P<0.05). The left ventricular ejection fraction (LVEF), 6MWT and EQ-5D-5L scores were significantly increased at 30, 60 and 90 days after operation compared with those before operation ( P<0.05). Conclusion:With the left ventricular assist device HeartCon, the cardiac function and quality of life of patients were significantly improved within 3 months after operation, and no serious complications were observed, proving that the device is safe and effective.
5.Prospective Comparison of FOCUS MUSE and Single-Shot Echo-Planar Imaging for Diffusion-Weighted Imaging in Evaluating Thyroid-Associated Ophthalmopathy

Objective:
To prospectively compare single-shot (SS) echo-planar imaging (EPI) and field-of-view optimized and constrained undistorted single-shot multiplexed sensitivity-encoding (FOCUS MUSE) for diffusion-weighted imaging (DWI) in evaluating thyroid-associated ophthalmopathy (TAO).
Materials and Methods:
SS EPI and FOCUS MUSE DWIs were obtained from 39 patients with TAO (18 male; mean ± standard deviation: 48.3 ± 13.3 years) and 26 healthy controls (9 male; mean ± standard deviation: 43.0 ± 18.5 years). Two radiologists scored the visual image quality using a 4-point Likert scale. The image quality score, signal-to-noise ratio (SNR), contrast-tonoise ratio (CNR), and apparent diffusion coefficient (ADC) of extraocular muscles (EOMs) were compared between the two DWIs. Differences in the ADC of EOMs were also evaluated. The performance of discriminating active from inactive TAO was assessed using receiver operating characteristic curves. The correlation between ADC and clinical activity score (CAS) was analyzed using Spearman correlation.
Results:
Compared with SS EPI DWI, FOCUS MUSE DWI demonstrated significantly higher image quality scores (P < 0.001), a higher SNR and CNR on the lateral rectus muscle (LRM) and medial rectus muscle (MRM) (P < 0.05), and a non-significant difference in the ADC of the LRM and MRM. Active TAO showed higher ADC than inactive TAO and healthy controls with both SS EPI and FOCUS MUSE DWIs (P < 0.001). Inactive TAO and healthy controls did not show a significant ADC difference with both DWIs. Compared with SS EPI DWI, FOCUS MUSE DWI demonstrated better discrimination of active from inactive TAO (AUC:0.925 vs. 0.779; P = 0.007). The ADC was significantly correlated with CAS in SS EPI DWI (r = 0.391, P < 0.001) and FOCUS MUSE DWI (r = 0.645, P < 0.001).
Conclusion
FOCUS MUSE DWI provides better images for evaluating EOMs and better performance in diagnosing active TAO than SS EPI DWI. The application of FOCUS MUSE will facilitate the DWI evaluation of TAO.
6.Correlation between biomarker EHD3 and immune cell infiltration in gastric cancer

【Objective】 To investigate the prognostic role of EHD3 and its association with immune cell infiltration in gastric cancer. 【Methods】 In this study, EHD3 expression was analyzed using RNA sequencing data from Cancer Genome Atlas (TCGA). Differential analysis, functional enrichment, immune infiltration, immune checkpoint exploration, and clinical baseline data analysis were performed. Independent prognostic factors were identified using univariate and multivariate Cox regression analysis; a column diagram model was developed and evaluated using C-index and calibration diagrams. 【Results】 EHD3 was significantly upregulated in STAD and functional enrichment analysis showed that EHD3 expression was associated with immune response, with most immune cells and immune checkpoints positively correlated with their expression. Cox regression showed that EHD3 was an independent prognostic factor in STAD patients (HR=2.112, 95% CI: 1.340-3.327, P=0.001). 【Conclusion】 EHD3 is considered to be a novel prognostic biomarker for STAD patients, and this study provides a potential therapeutic target for STAD treatment.
7.Allgrove syndrome: A case report and literature review

Objective:To discuss the clinical manifestations, diagnosis and treatment of a patient with Allgrove syndrome.Methods:Retrospective analysis of the clinical data of a patient with Allgrove syndrome admitted to Gansu Provincial People's Hospital on August 8, 2018, and review of relevant literature.Results:The patient, female, 20 years old, presented with dysphagia for 10 years and aggravated for 4 years. No tears since childhood. The upper digestive tract barium meal showed that lower esophageal stenosis, a "beak" shaped change. Esophageal manometry suggested multiple peristalsis failures. Laboratory tests showed low plasma cortisol and high ACTH. The clinical diagnosis was Allgrove syndrome. POME was performed to improve dysphagia, and hormone replacement therapy was given. The symptoms were improved after long-term follow-up.Conclusion:Allgrove syndrome is a multisystem disease. The possibility of Allgrove syndrome should be vigilant when suspicious symptoms appear. If necessary, gene detection is helpful for early diagnosis.Treatment, which focuses on the appropriate management of the manifested signs and symptoms, requires the mutual assistance of multiple disciplines.
8.Targeting a novel inducible GPX4 alternative isoform to alleviate ferroptosis and treat metabolic-associated fatty liver disease.

Metabolic-associated fatty liver disease (MAFLD), which is previously known as non-alcoholic fatty liver disease (NAFLD), represents a major health concern worldwide with limited therapy. Here, we provide evidence that ferroptosis, a novel form of regulated cell death characterized by iron-driven lipid peroxidation, was comprehensively activated in liver tissues from MAFLD patients. The canonical-GPX4 (cGPX4), which is the most important negative controller of ferroptosis, is downregulated at protein but not mRNA level. Interestingly, a non-canonical GPX4 transcript-variant is induced (inducible-GPX4, iGPX4) in MAFLD condition. The high fat-fructose/sucrose diet (HFFD) and methionine/choline-deficient diet (MCD)-induced MAFLD pathologies, including hepatocellular ballooning, steatohepatitis and fibrosis, were attenuated and aggravated, respectively, in cGPX4-and iGPX4-knockin mice. cGPX4 and iGPX4 isoforms also displayed opposing effects on oxidative stress and ferroptosis in hepatocytes. Knockdown of iGPX4 by siRNA alleviated lipid stress, ferroptosis and cell injury. Mechanistically, the triggered iGPX4 interacts with cGPX4 to facilitate the transformation of cGPX4 from enzymatic-active monomer to enzymatic-inactive oligomers upon lipid stress, and thus promotes ferroptosis. Co-immunoprecipitation and nano LC-MS/MS analyses confirmed the interaction between iGPX4 and cGPX4. Our results reveal a detrimental role of non-canonical GPX4 isoform in ferroptosis, and indicate selectively targeting iGPX4 may be a promising therapeutic strategy for MAFLD.
9.Celastrus orbiculatus Extract Promotes Apoptosis of Gastric Cancer Cells by Destroying Mitochondrial Structure

ObjectiveTo study the inhibitory effect of Celastrus orbiculatus extract (COE) on gastric cancer cells, to clarify the specific mechanism of COE promoting the apoptosis of gastric cancer cells by affecting the mitochondrial structure and function, and to provide an experimental basis for the further development and clinical application of C. orbiculatus. MethodBrdu staining combined with flow cytometry and Annexin V-fluorescein isothiocyanate (AnnexinV-FITC) staining combined with flow cytometry were employed to detect the effects of COE (20, 40, 80 mg·L-1) on the proliferation and apoptosis of gastric cancer cells, respectively. The changes in mitochondrial membrane potential were detected with JC-1 mitochondrial membrane potential assay kit. The expression of apoptosis-associated proteins including B-cell lymphoma-2 (Bcl-2), B-cell lymphoma-xL (Bcl-xL), Bcl-2-associated X (Bax), and cysteine aspartutespecific protease-3 (Caspase-3) in gastric cancer cells was determined by Western blot. Transmission electron microscopy was employed to detect changes in the mitochondrial microstructure of gastric cancer cells exposed to COE. Western blot was employed to measure the expression of mitochondrial marker proteins [superoxide dismutase 1 (SOD1), voltage-dependent anion channel (VDAC), prohibitin 1 (PHB1), and heat shock protein 60 (HSP60)] in gastric cancer cells. ResultCompared with the control group, COE (40, 80 mg·L-1) inhibited the proliferation and promoted the apoptosis of gastric cancer cells (P<0.05). Furthermore, COE reduced the mitochondrial membrane potential of gastric cancer cells. Compared with the control group, COE (20, 40, 80 mg·L-1) up-regulated the expression of Bax and Caspase-3 which promoted apoptosis of gastric cells (P<0.05, P<0.01), and COE at 40 and 80 mg·L-1 down-regulated the expression of Bcl-2 and Bcl-xL which inhibited the apoptosis of gastric cancer cells (P<0.01). The results of transmission electron microscopy showed that COE changed the microstructure of gastric cancer cells, which led to the appearance of vacuoles in the cell membrane and mitochondria and damaged the mitochondrial structure. Compared with the control group, COE (20, 40, 80 mg·L-1) changed the expression of mitochondrial marker proteins. Specifically, it up-regulated the expression of SOD1 involved in stress response (P<0.05, P<0.01) and down-regulated that of VDAC, PHB1, and HSP60 associated with mitochondrial stability and permeability (P<0.01). ConclusionCOE can significantly inhibit the proliferation and promote the apoptosis of gastric cancer cells. It may activate the mitochondrial apoptosis pathway by destroying the mitochondrial structure and function of gastric cancer cells.
10.Study on drying methods and harvesting time of Gynura divaricata leaf based on main active constituents

OBJECTIVE To determ ine the contents of main active constituents in Gynura divaricata leaf with different drying methods and at different harvesting time ,so as to confirm the best drying method and harvesting time. METHODS G. divaricata leaf with different drying methods [drying in the shade ,drying in the sun ,oven drying (60℃,70℃,80℃),microwave drying and freeze drying] and different harvesting time (March to October )were prepared. The content of water-soluble extract was determined by hot dip method. The contents of total flavonoids and polysaccharides were determined by ultraviolet-visible spectrophotometry. The content of astragalin was determined by HPLC. Analytic hierarchy process was used for comprehensive analysis. RESULTS The time of drying in the shade ,drying in the sun ,drying at 60 ℃,drying at 70 ℃,drying at 80 ℃, microwave drying and freeze drying were 7 d,5 d,8 h,5 h,3.5 h,6 min and 1 d respectively. The average contents of water-soluble extract in G. divaricata leaf were 55.98%,60.78%,52.33%,49.54%,46.87%,59.70% and 58.24%;those of total flavonoids were 3.27%,3.22%,1.99%,1.70%,1.31%,3.92% and 2.28%;those of polysaccharides were 4.70%,6.09%, 6.48%,5.45%,5.74%,5.76% and 7.15%;those of astragalin were 0.48%,0.46%,0.24%,0.23%,0.20%,0.48%,0.29%. The comprehensive score of microwave drying was the highest ,being 0.996 3. The average contents of water-soluble extract from March to October were 41.50%,40.57%,39.16%,40.65%,40.68%,43.30%,45.19% and 40.12%;those of total flavonoids were 2.24%,2.81%,3.87%,3.92%,3.82%,3.93%,3.66% and 3.25%;those of polysaccharides were 4.41%,4.61%, 4.98%,5.26%,5.75%,5.94%,5.32% and 4.47%;those of astragalin were 0.20%,0.21%,0.25%,0.26%,0.25%,0.24%, 0.25% and 0.21%,respectively. The comprehensive scores of samples collected from May to September exceeded 0.92,and the comprehensive score in August was the highest (0.988 6). CONCLUSIONS Microwave-dried Gynura divaricata leaf has the best quality ,and the best harvesting time is from May to September.
