1.Efficacy of endoscopic posterior rhinoplasty for treating post-radiotherapeutic atretorrhinia in nasopharyngeal carcinoma and influencing factors analysis of postoperative recurrence

Objective To investigate the effect of endoscopic posterior rhinoplasty in the treatment of post-radiotherapy atretorrhinia in the patients with nasopharyngeal carcinoma,and to analyze the postopera-tive recurrence influencing factors.Methods The clinical data in 60 patients with post-radiotherapy nasal atre-torrhinia due to nasopharyngeal carcinoma diagnosed and treated in this hospital from January 2019 to January 2023 were analyzed retrospectively.They were treated by endoscopic posterior rhinoplasty.The operation situ-ation and postoperative related complications occurrence were recorded.The visual arialogue scale (VAS) was used to evaluate the severity degrees of subjective symptoms before operation and on postoperative 1,3 months.The recurrence situation conducted the statistics.The univariate and multivariate logistic regression was used to analyze the influencing factors of postoperative recurrence.Results The surgical time in 60 cases was (65.12±5.23)min,and intraoperative blood loss volume was (25.02±1.46)mL.Eight cases developed recurrence after surgery.The VAS scores of subjective symptoms such as nasal congestion,rhinorrhea,mouth dryness and olfactory dysfunction on postoperative 1,3 months were decreased compared with before opera-tion (P<0.05).The multivariate logistic regression analysis revealed that postoperative nostril size,exposure of normal mucosa,use of mucoperiosteal flap and regular postoperative regular nasal endoscopic re-examina-tions were the independent influencing factors of postoperative recurrence in the patients with post-radiothera-py atretorrhinia after endoscopic posterior rhinoplasty treating nasopharyngeal carcinoma (P<0.05).Conclu-sion Using endoscopic posterior rhinoplasty for treating atretorrhinia after nasopharyngeal carcinoma radio-therapy has good effect,which could improve the subjective symptoms of the patients.The nostril size,normal mucosa exposure and regular re-examinations may affect the postoperative recurrence.
2.Cognitive function and performance of the crew during different legs of the long oceangoing voyage

Objective:To study the changes of the crew’s cognitive function and performance during long oceangoing voyages.Methods:The cognitive function and performance of the crew members of the " Mission Harmony 2018" were measured by the attention distribution instrument (EP708A), finger dexterity tester (EP109), and trail making test A and B at 1 month and 6 months after the Mission began, and the data were compared.Results:Comparing the data at 6 month after the Mission began with those at 1 month after the Mission began, the correct responses of the crew to the low-, middle-, and high-pitch sound were significantly increased ( P<0.01); the reaction time to middle-pitch sound and light was significantly reduced ( P<0.05); the wrong responses to low-pitch sound were significantly reduced ( P<0.01); the completion time of trail marking test A was significantly reduced ( P<0.05), while that of test B was significantly increased ( P<0.05); the time to finish finger dexterity task was significantly reduced ( P<0.01). Conclusion:The crew members had better cognitive function and performance in the late leg of the voyage than those in the early leg of the voyage.
3.Health and job burnout survey of the officers and sailors of "Mission Harmony 2018"

Objective:To survey the health condition and job burnout level of the officers and sailors on the hospital ship Peach Ark during the "Mission Harmony 2018".Methods:All types of officers and sailors of the "Mission Harmony 2018" on the hospital ship were surveyed by the physical and mental state questionnaire and the job burnout scale.Results:Among all, 96.7% of the officers and sailors deemed the Mission was of great significance and voluntarily joined the Mission. More than 70% of the officers and sailors had adapted themselves well to the environment on board. There were significant differences in the level of job burnout among the officers and sailors of different ages, identities, and academic qualifications ( P <0.05). In terms of job burnout level, there was no significant difference between the officers and sailors who are the only child in his/her family and those who are not, nor was there any significant difference due to different marital status ( P>0.05). However, the officers and sailors aged 30-49 had a higher level of job burnout than those aged 20-29; the officers and sailors with a bachelor’s degree or above had a higher level of job burnout than those with a high school degree or below; and the medical staff had a higher level of job burnout than the sailors, and the above differences were statistically significant ( P<0.05). During the "Mission Harmony 2018", family situation, physical health, and adaptability on board were the factors related to the job burnout level of officers and sailors ( P<0.05). The officers and sailors with more family worries, heavy family burdens, and poor health tended to have higher levels of job burnout. Conclusion:The physical and mental health of the officers and sailors on Mission Harmony should be paid close attention to, and their family troubles and difficulties should be known about in time. In particular, those who join long oceangoing mission for the first time and have difficult in adapting themselves to the environment on board should be provided regular psychological counseling service. Meanwhile, the medical staff should also be given more attention. A reasonable task assignment emphasizing the proper balance between work and rest can be helpful to reduce their job burnout level.
4.Practice and reflection of the hospital ship mental health service for "Mission Harmony 2018"

During "Mission Harmony 2018", the crew on the hospital ship were prone to various physiological and psychological reactions that affected their health. Based on the characteristics of the Mission, our own work experience on board, and the requirements for mental health support during the Mission, we summarized and reflected on our mental health support for the crew and the mental health service for the local people of the host countries. We believe that it is vital to support the mental health of the crew during a long oceangoing mission by formulating and carrying out psychological interventions and training according to the on-board environment and different legs of the voyage. We hope that this paper can provide a reference for the mental health service in future long oceangoing missions.
