1.Study on the Improvement of Glucose-Lipid Metabolism in Type 2 Diabetes Mellitus by Compounding Seven-Flower Granules

Objective To investigate the ameliorative effect of Qihuakeli,a Hani formula,on glycolipid metabolism in type 2 diabetes mellitus by in vivo and in vitro experiments.Methods Rat liver mesenchymal stromal cells(BRL-3A)were inoculated in six-well plates and divided into blank,palmitic acid,fenofibrate,and Qihuakeli serum-containing 5.4,10.8,and 21.6 g/kg groups.Except for the blank group,the remaining groups were intervened with 0.2 mmol/L palmitic acid(PA)for 24 hour,and then added with drug-containing serum,and then continued to incubate for 24 hour.The proliferation rate of BRL-3A cells in each group was determined.Total cholesterol(T-CHO),low-density lipoprotein cholesterol(LDL-C),and high-density lipoprotein cholesterol(HDL-C)concentrations in the supernatant of each cell group were measured,cell culture medium was aspirated and discarded,triglyceride(TG)concentration in the cell lysate.The lipid content of the cells was determined by measuring and staining with red oil.Meanwhile,45 rats were taken and divided into blank group,model group,fenofibrate group(0.225 g/kg),Qihuakeli compound 5.4 g/kg group,and Qihuakeli compound 10.8 g/kg group,the blank group was given normal feed and the rest of the groups were given high-fat feed for 42 day.Beginning on the 43rd day,each group,except the blank group,was injected with a single intraperitoneal injection of Starting from the 43rd day,except the blank group,each group was given a one-time intraperitoneal injection of 0.25%streptozotocin(STZ)solution,and at the same time,the corresponding drugs were given by gavage for 14 day.The rats'weight gain and liver index were measured.Serum fasting blood glucose(FBG)and fasting insulin(FINS)were detected,and the insulin resistance index(ISI)was calculated.Serum free fatty acid(FFA)levels and tumor necrosis factor-α(TNF-α)in liver tissue were also detected.HE staining was used to detect pathological changes in the pancreas.Pathological changes were observed in the tissues,and islet α and β cell expression was detected by immunohistochemistry.Results Compared to the PA group,the accumulation rate of BRL-3A cells was significantly higher(P<0.01)in the 10.8 and 21.6 g/kg Qihuakeli-containing serum groups.The levels of T-CHO,LDL-C and TG in the 5.4 and 21.6 g/kg serum groups were significantly lower(P<0.05),and HDL-C levels significantly increased(P<0.05).Oil red staining results showed that lipids in the cytoplasm of the 5.4,10.8 and 21.6 g/kg.Qihuacel-containing groups significantly reduced.Compared to the model group,the body weight of the 10.8 g/kg group containing Qihuakeli granules increased significantly(P<0.05).The liver index of the 5.4 g/kg group containing Qihuakeli decreased significantly(P<0.05).The serum indices of FBG,FINS,FFA and insulin resistance of the 5 g/kg group containing Qihuakeli decreased significantly(P<0.05).In the 5.4,10.8 g/kg groups,all serum FBG,FINS,FFA and insulin resistance indices significantly reduced in the 5.4 and 10.8 g/kg Qihuakeli groups(P<0.05 or P<0.01).TNF-α levels were significantly reduced(P<0.01).HE staining showed that a small number of lymphocytes were scattered in the pancreatic ducts and perivascular area of the rats in the Qihuakeli 5.4 and 10.8 g/kg groups,the local vasodilatation was observed,the number of pancreatic islet cells and the area of islet cells significantly increased.Immunohistochemical study was further used.The results of immunohistochemistry showed that the area of pancreatic islet α-cells significantly reduced and the area of pancreatic islet β-cells significantly increased in Qihuakeli 5.4 and 10.8 g/kg groups.Conclusion Qihuakeli compound improved glucose-lipid metabolism in T2DM,probably by improving the function of pancreatic islet cells,increasing the sensitivity of insulin to blood glucose,improving insulin resistance,decreasing the secretion of insulin and glucagon,and thus lowering the level of fasting blood glucose.Meanwhile,by decreasing the content of TNF-α,inhibiting lipolysis in the body,and promoting the uptake of FFA by adipocytes,and further lowering the FFA.Thus,it regulates the levels of TG,T-CHO,HDL-C and LDL-C,improves the abnormalities of glucose and lipid metabolism,and alleviates T2DM.
2.Comparison of the efficacy of two potassium binders in the treatment of hyperkalaemia in patients with chronic kidney disease

Objective To compare the efficacy and safety of sodium zirconium cyclosilicate and calcium polystyrene sulphonate in the treatment of hyperkalemia in chronic kidney disease(CKD).Methods Hospitalized patients with CKD hyperkalemia admitted to Qingyuan People's Hospital from January 2022 to November 2023 were retrospectively selected as study objects,and were divided into sodium zirconium cyclosilicate group and calcium polystyrene sulphonate group according to different drug use.The two groups of patients were matched with 1:1 propensity score,and finally 43 pairs of data were successfully matched.The efficacy and incidence of adverse reactions were compared between two groups of patients after matching.Results After treatment,the serum potassium level in both groups was significantly lower than that before treatment(P<0.05),but there was no significant difference in serum potassium level between two groups(P>0.05).There was no significant difference in the total effective rate between two groups(χ2=1.242,P=0.537).After treatment,the blood sodium level in sodium zirconium cyclosilicate group was significantly higher than that before treatment(P<0.05),there was no significant difference in blood calcium level before and after treatment(P>0.05).There was no significant difference in serum sodium and serum calcium before and after treatment in calcium polystyrene sulphonate group(P>0.05).There was one patient with hypokalemia in each group,and no treatment related adverse reactions such as nausea,edema,constipation,abdominal bloating and diarrhea were observed,and no abnormal laboratory test results were found.Conclusion The clinical efficacy of sodium zirconium cyclosilicate and calcium polystyrene sulphonate in treatment of hyperkalemia in CKD patients is comparable,and the safety is good.
3.Real-time core competency assessment by mobile terminal in the standardized training of internal medicine residents

Objective:To assess the core competencies of internal medicine residents undergoing standardized residency training and to explore the effectiveness of core competency evaluation on mobile devices.Methods:The mobile formative evaluation module was developed based on the "Xueyiku" teaching management platform. From January to December 2023, clinical teachers were asked to evaluate 150 internal medicine residents based on the "Resident Core Competency Milestone Evaluation System in China Consortium of Elite Teaching Hospitals", and the results were analyzed using non-parametric tests.Results:Among the six core competencies of internal medicine residents, professionalism received the highest score, whereas teaching skill received a lower score (97.50 vs. 90.00; H=167.31, P<0.001). Second-year residents had significantly higher scores than first-year residents (93.00 vs. 90.00; P<0.001), but similar scores to third-year residents (93.00 vs. 93.00; P>0.05). In addition, there was no significant difference in score among residents with different medical education backgrounds ( P>0.05). Conclusions:More emphasis should be placed on improving the teaching skills of internal medicine residents, along with the implementation of tiered progressive training. The mobile core competency evaluation is an effective means for assessing the comprehensive skills of residents in a timely manner.

7.A national multi-center prospective study on the perioperative practice of enhanced recovery after surgery for choledochal cysts in children

Objective:To evaluate the safety and efficacy of enhanced recovery after surgery(ERAS) in the perioperative period of congenital choledochal cysts in children.Methods:This is a multicenter prospective randomized controlled study. The clinical data of 273 pediatric congenital choledochal cysts(CCC) patients who underwent surgery at 14 medical centers with complete follow-up data were collected through the medical data analysis platform. Among them, 123 cases in ERAS group were managed perioperatively in strict accordance with ERAS mode, and 150 cases in conventional group were managed according to traditional mode. The length of hospital stay,time to first farting, time to complete feeding, the incidence of complications, cost and readmission rate within 30 days,stress indexes and liver function were compared between the two groups.Results:Compared with the conventional group, median time to start farting (2.0 d vs. 3.0 d, P<0.001), median time to complete feeding (5.0 d vs. 7.0 d, P<0.001), median postoperative hospitalization time (6.0 d vs. 9.0 d, P<0.001),the median total length of stay(13.0 d vs. 15.0 d, P<0.001) were shorter,the median hospitalization cost (37,000 yuan vs.43,000 yuan P<0.001) was lower, and stress indexes recovered quickly. The incidence of postoperative hospital stay and readimission rate within 30 d were not statistically different between the two groups. Conclusion:It is safe and feasible to implement ERAS for children with CCC in the perioperative period, which can reduce stress response, speed up recovery,and save medical costs.
8.To Study the Mechanism of"Yajieshaba"Against Acute Alcoholic Liver Injury in Mice Based on 16S rDNA Technology

Objective To investigate the pharmacological effects of"Yajieshaba"on mice with alcohol-induced liver injury and to investigate the mechanism of the impact of"Yajieshaba"on the regulation of intestinal flora by 16S rDNA technology.Methods Healthy male KM mice were randomly divided into control,model,"Yajieshaba"low,medium,and high dose(0.39,1.17 and 3.51 g·kg-1)groups and Bifendatatum(2.93 mg·kg-1)groups,with eight mice in each group.After one week of pre-administration of"Yajieshaba",a mouse model of acute alcoholic liver injury was established by a single instillation of 56%alcohol,and the levels of AST and ALT in the serum of mice were measured,and the morphological changes of liver histology were observed by HE staining;secondly,faecal DNA was extracted from each group under aseptic operation,and 16S rDNA sequencing and differential analysis by alpha diversity and species composition at the phylum and genus levels were performed.Results The results showed that the biochemical indexes of liver function(ALT and AST)were significantly improved by"Yajieshaba",and the degree of liver damage was significantly reduced by HE staining.At the phylum level,it significantly decreased the abundance of Aspergillus and increased the quantity of Bacteroides;at the genus level,it significantly up-regulated the plenty of Bacteroides and Prevotella and downregulated a lot of Prevotella and Helicobacter.At the genus level,"Yajieshaba"significantly up-regulated the abundance of Bacillus spp.and Prevotella spp.and down-regulated the abundance of Prevotella spp.and Helicobacter spp.Conclusion"Yajieshaba"may play an anti-acute alcoholic liver injury effect by regulating the intestinal flora and metabolites.
9.Analysis of influencing factors of coagulation disorders caused by cefoperazone sodium and sulbactam sodium in patients with chronic renal insufficiency

Objective:To explore the influencing factors of coagulation disorders caused by cefoperazone sodium and sulbactam sodium in patients with chronic renal insufficiency.Methods:The medical records of adult patients with chronic renal insufficiency, who were hospitalized and treated with cefoperazone sodium and sulbactam sodium in the Department of Nephrology of the Affiliated Qingyuan Hospital, Guangzhou Medical University, Qingyuan People′s Hospital from January 2021 to December 2022, were collected. Patients who developed coagulation disorders related to cefoperazone sodium and sulbactam sodium were imputed as having an end-point event, and the occurrence of end-point events in these patients was analyzed descriptively. According to whether an end-point event occurred, patients were divided into end-point event group and non-end point event group. Univariate and multivariate logistic regression analysis were performed on the risk of end-point events.Results:A total of 121 patients with renal insufficiency were included in the analysis, including 76 males (62.8%) and 45 females (37.2%), aged (66±13) years. Among 121 patients, 39 (32.2%) had end-point events, and 6 (5.0%) had clinical bleeding. The results of univariate analysis showed that the differences in age, renal replacement therapy, and daily and total doses of cefoperazone sodium and sulbactam sodium of patients between 2 groups were statistically significant (all P<0.05). Multivariate logistic regression analysis was performed using the occurrence of end-point events as dependent variables, and age, renal replacement therapy, and daily and total doses of cefoperazone sodium and sulbactam sodium as independent variables. The results showed that only older age was an independent risk factor for the occurrence of end-point events (odds ratio=1.044, 95% confidence interval: 1.004-1.086, P=0.029). Conclusions:Patients with renal insufficiency have a higher risk of coagulation disorders in treatment with cefoperazone sodium and sulbactam sodium, and older age is an independent risk factor. Cefoperazone sodium and sulbactam sodium should be used cautiously in elder patients, and coagulation function monitoring should be strengthened in clinical use.
