1.Syndrome Differentiation and Treatment of Atrial Fibrillation based on the Holistic View of "Spleen-Vessels-Heart-Spirit"

Based on the holistic view of "spleen-vessels-heart-spirit" system, this article explores the pathogenesis and progression of atrial fibrillation. It is proposed that the onset of atrial fibrillation is due to failure of the spleen to transport and disharmony of blood vessels; phlegm and blood stasis obstructing the collaterals and damage to the heart structure are the basis of its pathogenesis; the unclear mind and disorder of body and spirit are the causes of its progression. Based on the characteristics of pathological changes in different stages of the disease, it is proposed that early treatment should focus on restoring the middle jiao, clearing and promoting blood vessels, using modified Yigong Powder (异功散); during the progression of the disease, treatment should remove blood stasis and phlegm, nourish heart and protect the pulse, using self-prescribed modified Mengshi Tongmai Decoction (礞石通脉汤); meanwhile, calming mind and stabilizing palpitations, and regulating spirit should be sequentially incorporated, with self-prescribed Jiazao Ningmai Decoction (甲枣宁脉汤) or Shenying Dingji Decoction (参英定悸汤) and modified as appropriate. Clinical treatment should focus on the whole disease course of atrial fibrillation, implementing stage-based treatments to enable early intervention and holistic regulation.
2.Stage-Based Intervention in Atherosclerosis Using the "Attacking,Supplementing,Dispersing,Dissipating" Method Based on the Accumulation Syndrome Theory

Atherosclerosis is a complex pathological condition resulting from lipid deposition, chronic inflammatory responses, and fibrosis, with a prolonged disease course and multifactorial etiology. Based on the traditional Chinese medicine (TCM) theory of accumulation syndrome, atherosclerosis can be classified under this category, with its pathogenesis involving phlegm, blood stasis, deficiency, and accumulation. This paper proposed a stage-based intervention strategy using the four therapeutic principles of "attacking, supplementing, dispersing, dissipating", and divided into six stages based on the pathological progression, including the stage of accumulation before formation, the stage of accumulation already formed, the stage of nucleus accumulation, the stage of nucleus accumulation decay, the stage of nucleus accumulation consolidation, and the stage of severe stenosis of nucleus. At different stages, the intervention focuses on reinforcing healthy qi and consolidating the root, tonifying the kidneys and spleen, dispersing and removing turbidity, removing phlegm stagnation, promoting qi circulation, dispersing accumulations and removing stasis, attacking accumulation and expelling stasis, directing the turbid downward and dispersing accumulation, and treatment would be adjusted based on specific symptoms, which provides a theoretical framework for the prevention and treatment of atherosclerosis with TCM.
3.Treatment of Paroxysmal Atrial Fibrillation by Deficiency-Excess Pattern Identification Based on “Palpitations Caused by Wind Pathogen”

Paroxysmal atrial fibrillation can be attributed to the category of xinji (palpitations) and zhangchong (severe palpitations) in traditional Chinese medicine, and its onset has the characteristics of urgency, change, and movement, which is similar to the characteristics of diseases induced by wind pathogen. It is believed that the internal movement of wind pathogen runs through the whole course of this disease, and palpitations due to wind as the direct pathogenesis. Palpitations caused by wind pathogen showed different characteristics of deficiency and excess pattern. In the acute exacerbation period, excess wind is the main cause of disease. For excessive heat generating wind, the treatment is to clear the liver and extinguish wind by self-modified Lingxia Qinggan Decoction (羚夏清肝汤); for blood stasis generating wind, the treatment is to remove blood stasis and stop wind by self-modified Yandan Limai Decoction (延丹理脉汤); for phlegm-heat accumulation with wind, the treatment is to dissolve phlegm and eliminate wind by self-modified Lianlou Danxing Decoction (连蒌胆星汤). In the prolonged recovery period, deficiency wind is more common. For stirring of wind due to yin deficiency, the treatment is to nourish yin and extinguish wind by self-modified Zaoshao Zhenzhu Deoction (枣芍珍珠汤); for spleen deficiency generating wind, the treatment is to strengthen spleen and nourish wind by self-modified Shenying Dingji Deoction (参英定悸汤). Clinical prescriptions closely follow the characteristics of wind, weigh the changes of deficiency and excess, tailor with the patterns, and regulate qi and blood of the zang-fu organs, in order to extinguish wind and arrest convulsion.
4.An investigation of bacillary dysentery outbreaks in three schools in Ankang

Objective:
To investigate risk factors and epidemiological characteristics of bacillary dysentery outbreaks in three schools, and to provide scientific basis for the prevention and control of the epidemic in the future.
Methods:
Case definition was established. All suspected, possible and confirmed cases of all students and faculty members from 3 schools (A, B, C) were selected for epidemiological investigation. Control group was used for case-control analysis, and relevant samples were collected for laboratory testing.
Results:
A total of 132 cases were found in 3 schools, all of which were from students, with the incidence rate of 17.74%. The morbidity in kindergarten A was 20.00%, in center primary school B it was 21.74%, and in junior middle school C it was 11.61%. Cohort studies and casecontrol studies suggested that schools are exposed places and that washing hands with raw water in schools was possible risk factor [OR(95%CI) =4.50(1.01-20.11)]. Nine stool samples were tested in laboratory, among which 8 were positive for Shigella(88.99%), and Shigella was detected in the end nodes of school s pipeline network, the water samples from canteen bucket, and the floor drains of sewer pipe.
Conclusion
The bacillary dysentery outbreaks in 3 schools was caused by Shigella, which may be due to fecal contamination of domestic water in 3 schools before the start of the school year. It is suggested to strengthen the management of centralized water supply and construction in rural areas, intensify the supervision at all levels, and sanitation and disinfection before school opens at all levels.
5.A new type of anatomical locking plate for split fractures of humeral greater tuberosity

Objective To evaluate the clinical outcomes of split fractures of humeral greater tuberosity treated by our self-designed new type of anatomical locking plate.Methods From September 2012 to February 2017,23 patients were treated for acute split fracture of the humeral greater tuberosity using our self-designed new type of anatomical locking plate.They were 13 males and 10 females with a mean age of 52.8 years (range,from 25 to 81 years).Of them,6 were beyond 60 years old, 12 had comminuted fracture,10 were complicated with glenohumeral dislocation,and 12 with rotator cuff tear.The patients were evaluated clinically with Constant-Murley score,visual analog scale (VAS),range of motion and complications at the last follow-ups.Results This series were followed up for 12 to 30 months (mean,23.2 months).All the fractures healed after an average time of 10.6 weeks (range,from 8 to 12 weeks).Their mean Constant-Murley Score was 92.1 points (range,from 70 to 100 points),giving an excellent and good rate of 95.7% (22/23);their VAS scores averaged 0.8 points (range,from 0 to 4 points).Their forward flexion averaged 160.6°,abduction 157.8°,external rotation 46.4°,and internal rotation up to the T11 level,respectively.Their complications rate was 17.4% (4/23).One case of axillary nerve injury,one case of relapse of glenohumeral dislocation at sports,and 2 cases of stiff shoulder were observed.Conclusion Split fractures of the humeral greater tuberosity can be successfully treated with our new type of anatomical locking plate which serves as a new alternative treatment.
6.Study on Myocardial Infarction of Xinfukang Oral Liquid on Myocardial Mitochondrial Proteomics in Rats with Heart Failure

This paper aimed at investigating the effects of Xinfukang Oral Liquid on mitochondrial proteomic alterations in rats with heart failure after myocardial infarction and exploring its possible mechanisms. Heart failure models were established by ligating the left anterior descending coronary artery of SD rats. Rats were randomly divided into sham group, model group, Xinfukang group. 8 weeks after drug intervention, the mitochondrial proteomic alterations of myocardial tissue were detected by two-dimensional gel electrophoresis (2-DE) and matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF-MS) . Furthermore, expression levels of part of the differently expressed proteins were verified by western blot. Compared with model group, 20 differentially expressed protein spots were detected in Xinfukang group, 13 of which showed increased protein expression and 7 decreased; 13 differentially expressed protein spots were successfully identified by MALDI-TOF-MS. Bioinformatics analysis showed that these differential proteins were mainly associated with energy metabolism, stress reaction, oxidative damage, cyto-skeleton, cell differentiation and proliferation. Western blot results showed that the expression of Stress-70 protein and Nucleophosmin increased and the expression of ATP-αdecreased in model group. The expression of Stress-70 protein and Nucleophosmin decreased and the expression of ATP-αincreased in Xinfukang group, which shows the same results in proteomics. Xinfukang Oral Liquid can partly adjust proteomic alterations of myocardial mitochondrial in HF rats, and its intervention mechanism may involve improving energy metabolism, reliving stress reaction and oxidative damage, as well as regulating cell differentiation and proliferation. The results of comparative proteomic analysis performed by using2-DE and MALDI-TOF-MS are acurate, stable and reliable.
7.Clinical research of serum CysC combined with APACHE Ⅱ score in predicting acute kidney injury in patients with sepsis

Objective To investigate the clinical value of serum cystatin C (sCysC) and APACHE Ⅱ score in predicting diagosis and prognosis of acute kidney injury(AKI) in patients with sepsis. Methods In this study, we prospectively enrolled 138 adult patients with sepsis who had been admitted to the mixed ICU of Xiaolan Hospital of Southern Medical University during March 2015 to January 2016. According to the Kidney Disease Improving Global Outcomes (KDIGO) criterion, the patients were divided into non-AKI group and AKI group (including mild AKI and severe AKI). The receiver operating characteristic(ROC) curve and the area under curve(AUC) were used to evaluate these indexes' capability of detecting septic AKI and its prognosis. Results In this study,72 patients (52.2%) developed AKI. The levels of sCysC and APACHE Ⅱ score were significantly higher in AKI than in non-AKI (P<0.05). In total, 33 patients (23.9%) developed severe AKI. The levels of sCysC and APACHE Ⅱscore were significantly higher in severe AKI than in non-AKI and mild AKI (P<0.05) . Combination of sCysC and APACHE Ⅱ score predicted AKI and severe AKI after ICU admission with a higherAUC value (0.880&0.930) than each biomarker alone. In this cohort, in-hospital mortality was 19.6%and renal replacement therapy rate was 9.4%,which were strikingly higher in AKI group than non AKI group (P<0.05). Conclusions sCysC is a novel indexes for predicting AKI and its prognosis in patients with sepsis. Combinating with APACHE Ⅱ score can further improve its predictive performance of AKI detection and short-term prognosis.
8.Determination of halometasone and its pharmacokinetics study in rabbit plasma by LC-MS/MS

The test article could be monitored in rabbit plasma when following single dermal topical administration of Halometasone Cream at 1 g/100 cm2 to rabbits by u-singavalidatedLC-MS/MSassay.Results Calibra-tion curve was linear over the concentration range of0. 02~20 μg·L-1 in rabbit plasma.For low,medi-um,high concentration of QC solutions,the intra-and inter-day precision was in the range of 3. 72% ~7. 87%, and the accuracy was within 99. 1% to 103%. The pharmacokinetic parameters in rabbits were as follows:Tmax,Cmax,AUC0-t,T1/2 was (7. 38 ± 1. 06)h,(1. 16 ±0. 527)μg·L-1,(18. 8 ±7. 23)h·μg·L-1 ,(13. 8 ±3. 70)h,respectively.Conclusions ThisLC-MS/MSanalysismethodhashighsensitivi-ty,and sample processing method is simple,which has been rigorously validated.The method could be suc-cessfully applied to the pharmacokinetic study of halo-metasone after skin administration of Halometasone Cream to rabbits.
9.Correlation analysis between organizational safety atmosphere and work enthusiasm among junior nurses in ICU

Objective To explore the influence of organizational safety atmosphere on work enthusiasm among junior nurses in ICU,so as to provide reference for improving job engagement and work quality of junior nurses in ICU. Methods In December 2016,213 junior nurses in ICU from 12 comprehensive class Ⅲhospitals in Nanning,Guilin and Liuzhou in Guangxi were selected and surveyed by the Safety Organizing Scale(Chinese version)and Questionnaire on Work Enthusiasm Appraisal. Influence of organizational safety atmosphere perceived by junior nurses in ICU on their work enthusiasm was analyzed. Results For the junior nurses in ICU,scores of safety organizing were(38.60±7.12)points,and scores of work enthusiasm were (57.04±8.23);differences of junior nurses' work enthusiasm scores under different organizational safety atmosphere were statistically significant(F=10.908,P< 0.001). Work enthusiasm and its dimensions were positively related to organizational safety atmosphere(P<0.05). Conclusions The more safety organizing investment is,the higher work enthusiasm is of junior nurses in ICU.
10.Effect of colon cancer cell-derived IL-1α on the migration and proliferation of vascular endothelial cells.

OBJECTIVETo explore the effect of colon cancer cell-derived interleukin-1α on the migration and proliferation of human umbilical vein endothelial cells as well as the role of IL-1α and IL-1ra in the angiogenesis process.
METHODSWestern blot was used to detect the expression of IL-1α and IL-1R1 protein in the colon cancer cell lines with different liver metastatic potential. We also examined how IL-1α and IL-1ra influence the proliferation and migration of umbilical vascular endothelial cells assessed by PreMix WST-1 assay and migration assay, respectively. Double layer culture technique was used to detect the effect of IL-1α on the proliferation and migration of vascular endothelial cells and the effect of IL-1ra on the vascular endothelial cells.
RESULTSWestern blot analysis showed that IL-1α protein was only detected in highly metastatic colon cancer HT-29 and WiDr cells, but not in the lowly metastatic CaCo-2 and CoLo320 cells.Migration assay showed that there were significant differences in the number of penetrated cells between the control (17.9±3.6) and 1 ng/ml rIL-1α group (23.2±4.2), 10 ng/ml rIL-1α group (31.7±4.5), and 100 ng/ml rIL-1α group (38.6±4.9), showing that it was positively correlated with the increasing concentration of rIL-1α (P<0.01 for all). The proliferation assay showed that the absorbance values were 1.37±0.18 in the control group, and 1.79±0.14 in the 1 ng/ml rIL-1α group, 2.14±0.17 in the 10 ng/ml rIL-1α group, and 2.21±0.23 in the 100 ng/ml rIL-1α group, showing a positive correlation with the increasing concentration of rIL-1α(P<0.01 for all). IL-1ra significantly inhibited the proliferation and migration of vascular endothelial cells (P<0.01). The levels of VEGF protein were (1.697±0.072) ng/ml, (3.507±0.064)ng/ml and (4.139±0.039)ng/ml in the control, HUVECs+ IL-1α and HUVECs+ HT-29 co-culture system groups, respectively, showing a significant difference between the control and HUVECs+ 10 pg/ml rIL-1α groups and between the control and HUVECs+ HT-29 groups (P<0.01 for both).
CONCLUSIONSOur findings indicate that colon cancer cell-derived IL-1α plays an important role in the liver metastasis of colon cancer through increased VEGF level of the colon cancer cells and enhanced vascular endothelial cells proliferation, migration and angiogenesis, while IL-1ra can suppress the effect of IL-1α and inhibit the angiogenesis in colon cancer.
