1.Impact of industrial and job changes among fishermen on schistosomiasis control following fishing ban and withdrawal in Poyang Lake areas

Objective To investigate the industrial and job changes among fishermen and their impact on economic incomes and schistosomiasis control following fishing ban and withdrawal in Poyang Lake areas, so as to provide insights into formulation of the schistosomiasis control strategy in these areas. Methods One township was selected as the study area from each of Duchang County, Poyang County and Yugan County around the Poyang Lake areas from May to June, 2023, and at least 500 fishermen at ages of 60 years and less were sampled from each township. Questionnaires were allocated to all subjects via the “Questionnaire Star” platform, including demographics, ratio of profits from different production activities before and after fishing ban and withdrawal, contact with infested water before and after fishing ban and withdrawal, and changes in household economic income before and after fishing ban and withdrawal. Subjects’ schistosomiasis examination and treatment data, and development of schistosomiasis hepatic fibrosis and advanced schistosomiasis were collected from 2016 to 2023. The changes in economic incomes, contact with infested water, sero-prevalence of human schistosomiasis, and proportions of schistosomiasis examinations and treatments were compared among study subjects. Results A total of 2 449 fishermen with an average age of (49.55 ± 8.86) years were investigated, including 1 654 full-time fishermen (67.54%) and 795 part-time fishermen (32.46%). The ratios of profits from fishing activities reduced from 88.15% and 34.82% before fishing ban and withdrawal to < 0.01% and 0.03% after fishing ban and withdrawal among full-time and part-time fishermen, while the ratios of profits from other production activities increased. The proportion of full-time fishermen who thought a reduction or a large reduction in their economic incomes was 79.93% (1 322/1 654), which was higher than that (66.79%, 531/795) of part-time fishermen (χ2 = 50. 307, P < 0.001), and the proportion of part-time fishermen who thought minor changes in their economic incomes was 15.60% (124/795), which was higher than that (4.11%, 68/1 654) of full-time fishermen (χ2 = 96.274, P < 0.001). There was a reduction in the contact with infested water among fishermen following fishing ban and withdrawal, and the proportions of respondents that had no contact, a large reduction in contact and a reduction in contact with infested water were 14.74% (361/2 449), 57.78% (1 415/2 449) and 26.05% (638/2 449), respectively. In addition, the sero-prevalence of schistosomiasis, and proportions of schistosomiasis examinations and treatments decreased from 9.79%, 87.14%, and 97.80% in 2016 to 4.38%, 53.16%, and 71. 51% in 2023 among fishermen in Duchang County, Poyang County and Yugan County (χ2 = 6.944, 865.194, 238.953, all P values < 0.01), and the prevalence of schistosomiasis liver fibrosis and advanced schistosomiasis was 42.64% (826/1 937) and 6.08% (149/2 449) among study subjects, respectively. Conclusions Industrial and job changes among fishermen are beneficial for schistosomiasis control after fishing ban and withdrawal in Poyang Lake areas; however, there is an increased difficulty for management of fishermen due to an increase in number of migrant labors and other factors.
2.Study on anti-atherosclerosis mechanism of blood components of Guanxin Qiwei tablets based on HPLC-Q-Exactive-MS/MS and network pharmacology

The analysis presented here is based on the blood components of Guanxin Qiwei tablets, the key anti-atherosclerosis pathway of Guanxin Qiwei tablets was screened by network pharmacology, and the anti-atherosclerosis mechanism of Guanxin Qiwei tablets was clarified and verified by cell experiments. HPLC-Q-Exactive-MS/MS technique was used to analyze the components of Guanxin Qiwei tablets into blood, to determine the precise mass charge ratio of the compounds, and to conduct a comprehensive analysis of the components by using secondary mass spectrometry fragments and literature comparison. Finally, a total of 42 components of Guanxin Qiwei tablets into blood were identified. To better understand the interactions, we employed the Swiss Target Prediction database to predict the associated targets. Atherosclerosis (AS) disease targets were searched in disease databases Genecard, OMIM and Disgent, and 181 intersection targets of disease targets and component targets were obtained by Venny 2.1.0 software. Protein interactions were analyzed by String database. The 32 core targets were selected by Cytscape software. Gene Ontology (GO) enrichment analysis and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway analysis were performed in DAVID database. It was found that the anti-atherosclerosis pathways of Guanxin Qiwei tablets mainly include lipid metabolism and atherosclerosis and AGE-RAGE signaling pathway in diabetic complications and other signal pathways. In recent years, with the continuous development and increasing maturity of interventional techniques, interventional treatment for congenital heart disease (CHD) has been progressively disseminated to county- and city-level hospitals in China. Concurrently, the standardized management of adult CHD (particularly patent foramen ovale) and the lifelong management of complex CHD are gaining increasing clinical attention, while the emergence of new techniques and products continuously advances the discipline. This article aims to review the new progress made in the field of interventional treatment for congenital heart disease in China during 2024. It specifically reviews and analyzes the following key aspects: (1) annual statistics on interventional closure procedures for CHD; (2) recent insights into patent foramen ovale closure; (3) advances in transcatheter pulmonary valve replacement; (4) interventional treatment and lifelong management strategies for complex CHD; (5) new interventional techniques for acquired heart disease; and (6) the application of artificial intelligence in CHD management. Through the synthesis and discussion of these topics, this article seeks to provide a detailed analysis of the current landscape of interventional treatment for CHD in China and project its future development trends.
4.Exploring anti-inflammatory mechanism of urolithin A based on miRNA155-5p-mediated MAPK/NF-κB pathway

Aim To explore anti-inflammatory mecha-nism of urolithin A(Uro A)based on the miRNA155-5p-regulated MAPK/NF-κB pathway in a lipopolysac-charide(LPS)-stimulated RAW264.7 inflammatory cell model.Methods The mRNA expression of miR-15-5p,p38 MAPK,JNK and ERK was detected by transfection of overexpression of miR-155-5p-mimics and miR-NC into LPS-induced RAW264.7 cells,and the miR-15-5p,p38 MAPK,JNK,and ERK mRNA expressions in the cells were detected by RT-qPCR.The effect of Uro A on cell viability was detected by MTT assay.The NO content in cell supernatant was detected by Griess assay.The cell supernatant PGE2,IL-6,IL-1 β and TNF-α expression levels were detec-ted by ELISA.The iNOS,COX-2,TLR4,and the MAPK/NF-κB pathway-related protein expression lev-els were detected by Western blot.Results The p38 MAPK,JNK and ERK mRNA expression levels were significantly higher in the miRNA155-5p overexpres-sion group than those in the miR-NC group(P<0.01).Compared with the model group,Uro A signif-icantly inhibited miRNA155-5p,NO,PGE2,TNF-α,IL-1β,and IL-6 levels(P<0.01)as well as de-creased the expression levels of iNOS and COX-2 pro-teins(P<0.01)in a concentration-dependent man-ner.Moreover,Uro A could significantly inhibit the expression levels of TLR4 protein and the expression levels of phosphorylation levels of p38 MAPK,JNK,NF-κB p65,and IκBα proteins in LPS irritated RAW264.7 cells(P<0.05).However,Uro A did not show a significant inhibitory effect on ERK1/2 pro-tein phosphorylation.Conclusion Uro A could signif-icantly inhibit LPS-induced inflammatory response,and its mechanism may be related to miRNA155-5p-mediated TLR4/MAPK/NF-κB signaling pathway.
5.Digital study on proximal clavicle anatomical plate based on 3D printing technology

Objective To explore feasibility of 3D metal printing technology combined with virtual design proximal clavicle anatomical plate.Methods A 52-year-old male healthy volunteer was retrospectively selected to design proximal clavicle anatomical plate system by using Mimics15.01,NX12.0 and other software.STL data were input into 3D printer to print 1∶1 clavicle model and proximal clavicle anatomical plate.The fit of the plate was tested in vitro and the accuracy of screw position was evaluated by imaging.Printing time of model,nail path design and fabrication time of the anatomical plate at proximal clavicle were recorded.Results The 3D metal printing proximal clavicle anatomical plate fitted well to clavicle model,orienta-tion of proximal clavicle locking screw was accurate,and X-ray and CT scan showed the screw position was good.Printing time of model,the time of nail path design,and the time of making anatomical plate of proximal clavicle were 120,15 and 300 min respectively.Conclusion The proximal clavicular anatomical plate system based on 3D metal printing technology could achieve good lamination of proximal clavicular fracture plate and precise screw placement,providing a new and accurate surgical method for the treatment of the proximal clavicular fracture.
6.Finite element analysis of anatomic plate fixation for proximal clavicular fractures

Objective To explore establishment and finite element analysis of personalized proximal clavicular anatomical plate screw fixation model.Methods A 40-year-old male healthy volunteer was selected and the finite element analysis modules of 3D reconstruction software Mimics 15.01,Hypermesh 2019 and Abaqus 2020 were used.The finite element model of anatomic plate at the proximal clavicle was established,and a vertical load of 250 N was applied to the distal end of long axis of clavicle about 15 mm,then the overall structure,plate and screw displacement cloud image,Mises stress distribution were ob-served.Results The displacement distribution of the overall structure shows the maximum displacement was distributed on the distal clavicle.Under the four conditions of normal upper limb weight,longitudinal clavicle fracture,oblique fracture and shoulder impact violence during fall,longitudinal clavicle fracture and oblique fracture,the maximum displacement were 1.04 mm,1.03 mm,1.35 mm and 1.33 mm,respectively.The displacement cloud map of titanium alloy steel plate showed the largest displacement was distributed near the distal clavicular bone,and the maximum displacement were 0.89 mm,0.88 mm,1.10 mm and 1.09 mm,respectively.The displacement cloud map of titanium alloy screw showed the largest displacement was distribut-ed at the root of the distal screw,and the maximum displacement were 0.88 mm,0.87 mm,1.08 mm and 1.06 mm,respectively.Mises stress distribution showed the maximum stress was mainly distributed on titanium alloy plates and screws,and the stress on the clavicle was very small.Mises stress distribution cloud showed the maximum Mises stress was distributed at the second row of screw holes near the clavicle,and the maximum Mises stress were 673.1,678.1,648.5,654.4 MPa,respectively.The maximum stresses of titanium alloy screws were 414.5,417.4,415.8 and 419.7 MPa,respectively.Conclusion The biomechan-ical changes of personalized proximal clavicular anatomical plates are demonstrated by using 3D finite element method to pro-vide biomechanical data for personalized proximal clavicular anatomical plates.
7.Analysis of factors influencing postoperative delirium in patients with Stanford Type A aortic dissection

Objective:To explore the incidence of postoperative delirium (POD) in patients with Stanford Type A aortic dissection and analyze the influencing factors and predictive efficacy of these factors for POD.Methods:Totally 237 patients with Stanford Type A aortic dissection who were treated at Beijing Anzhen Hospital, Capital Medical University, from May 2020 to February 2021 were selected by convenience sampling. Data were collected using a general information questionnaire, the Confusion Assessment Method for the Intensive Care Unit (CAM-ICU), the Pittsburgh Sleep Quality Index (PSQI), the State-Trait Anxiety Inventory (STAI), and the Chinese version of the Critical Care Pain Observation Tool (CPOT). Logistic regression analysis was employed to identify factors influencing POD, and receiver operating characteristic (ROC) curves were used to evaluate the predictive value of each influencing factor and determine the optimal cutoff values.Results:A total of 237 questionnaires were distributed, with 232 valid questionnaires returned, yielding a recovery rate of 97.89% (232/237). Among the 232 patients, 37 developed delirium, resulting in an incidence rate of 15.95% (37/232). Logistic regression analysis revealed that carotid artery involvement, preoperative anxiety scores, postoperative pain scores, and length of ICU stay were influencing factors for POD in patients with Stanford Type A aortic dissection ( P<0.05). ROC curve analysis indicated that the area under the curve ( AUC) values for preoperative anxiety scores, postoperative pain scores, length of ICU stay, and carotid artery involvement in predicting POD were 0.924, 0.927, 0.954, and 0.718, respectively. Conclusions:The incidence of POD in patients with Stanford Type A aortic dissection is relatively high. Preoperative anxiety, postoperative pain, length of ICU stay, and carotid artery involvement are key factors influencing the occurrence of POD in these patients.
8.Antimicrobial resistance and resistance gene carriage by Salmonella bacteria in environmental sewage in Guangzhou

This study explored the antibiotic resistance and resistance gene carriage of 140 Salmonella strains isolated from environmental sewage in Guangzhou city between March 1,2022,and November 30,2022.The micro broth dilution method was used to select 17 antibiotics for susceptibility testing.According to whole genome sequencing results,the CARD resistance database was used to obtain corresponding resistance genes.High antimicrobial resistance rates above 80%were observed a-gainst ampicillin,tetracycline,streptomycin,chloramphenicol,and cotrimoxazole.The intermediation rate of polymyxin E and ciprofloxacin exceeded 60%.The multiple drug resistance status was severe,and the rate of multiple drug resistance was as high as 92.86%.The strains carried multiple types of resistance genes,particularly for aminoglycosides,with a carriage rate as high as 92.68%.The resistance of Salmonella in environmental wastewater in Guangzhou to one or more drugs was severe,and the overall multi-drug resistance rate gradually increased over time.The resistance spectrum was diverse,and the resistance mechanism,mediated by mobile genetic elements such as re-sistance genes,was found to be the main cause of resistance to one or more drugs.
9.Thin corpus callosum and"lynx ear sign":A report of a family of hereditary spastic paraplegia type 11

To report a case with hereditary spastic paraplegia type 11(SPG11)in association with typical thin corpus callosum(TCC)and"Ears-of-the-lynx sign"on MRI imaging.The patient was a 13-year-old boy.The main symptoms are walking instability and falling easily.Over the period of one year,the symptoms gradually progressed when accompanied by poor handwriting and a decrease in learning ability.The parents are not related.Brain MRI shows a thin corpus callosum,and high symmetric signals in the anterior horn of the lateral ventricles on T2 and Flair sequence.WES detected two heterozygous mutations in the SPG11 gene,NM_025137:c.2073delT and c.257+5G>A,respectively from the parents.The proband was finally diagnosed with SPG11.Brain MRI found that TCC and"lynx ear sign"are highly sensitive and specific for the diagnosis of SPG11.The patients with spastic paraplegia should be considered the possibility of SPG11.
10.Association of Triglyceride Glucose-Derived Indices with Recurrent Events Following Atherosclerotic Cardiovascular Disease

Background:
Triglyceride glucose (TyG) and TyG-body mass index (TyG-BMI) are reliable surrogate indices of insulin resistance and used for risk stratification and outcome prediction in patients with atherosclerotic cardiovascular disease (ASCVD). Here, we inserted estimated average glucose (eAG) into the TyG (TyAG) and TyG-BMI (TyAG-BMI) as derived parameters and explored their clinical significance in cardiovascular risk prediction.
Methods:
This was a population-based cohort study of 9,944 Chinese patients with ASCVD. The baseline admission fasting glucose and A1C-derived eAG values were recorded. Cardiovascular events (CVEs) that occurred during an average of 38.5 months of follow-up were recorded. We stratified the patients into four groups by quartiles of the parameters. Baseline data and outcomes were analyzed.
Results:
Distribution of the TyAG and TyAG-BMI indices shifted slightly toward higher values (the right side) compared with TyG and TyG-BMI, respectively. The baseline levels of cardiovascular risk factors and coronary severity increased with quartile of TyG, TyAG, TyG-BMI, and TyAG-BMI (all P<0.001). The multivariate-adjusted hazard ratios for CVEs when the highest and lowest quartiles were compared from low to high were 1.02 (95% confidence interval [CI], 0.77 to 1.36; TyG), 1.29 (95% CI, 0.97 to 1.73; TyAG), 1.59 (95% CI, 1.01 to 2.58; TyG-BMI), and 1.91 (95% CI, 1.16 to 3.15; TyAG-BMI). The latter two showed statistical significance.
Conclusion
This study suggests that TyAG and TyAG-BMI exhibit more information than TyG and TyG-BMI in disease progression among patients with ASCVD. The TyAG-BMI index provided better predictive performance for CVEs than other parameters.
