1.Value of serum levels in predicting lower extremity deep vein thrombosis after hip joint surgery in the elderly

Objective To explore the value of serum D-dimer(D-D),fibrinogen(FIB),platelet(PLT),C-reactive pro-tein(CRP)and tissue plasminogen activator inhibitor(PAI)-1 levels in predicting lower extremity deep vein thrombosis(DVT)after hip joint surgery in the elderly.Methods A retrospective analysis was performed on 165 elderly patients with hip joint surgery admitted from February 2020 to May 2022,including 89 males and 76 females,aged from 60 to 75 years old with an average of(66.43±5.48)years,and there were 102 cases of femoral neck fracture and 63 cases of femoral head necrosis.Serum levels of D-D,FIB,PLT,CRP and PAI-1 tests were performed in all patients within 24 hours after admission,and the patients were divided into DVT group and non-DVT group according to whether they developed DVT.Results The levels of D-D,FIB,PLT,CRP,and PAI-1 in the DVT group were higher than those in the non-DVT group(P<0.001).Spearman analysis showed that DVT was positively correlated with PLT,CRP,D-D,FIB,and PAI-1 levels(r=0.382,0.213,0.410,0.310,0.353,all P<0.001).The results of binary Logistic regression analysis showed that D-D and PLT were independent factors affecting the occurrence of DVT(OR=0.038,0.960,P=0.032,0.011).The area under curve(AUC)of D-D,FIB,PLT,CRP,PAI-1,and the five combined predictions for DVT were 0.843,0.692,0.871,0.780,0.819,and 0.960,respectively.The AUC of the five combined predictions was higher than that of the single prediction(P<0.05).Conclusion D-D,FIB,PLT,CRP and PAI-1 are effective in predicting DVT after hip surgery in the elderly,and the combined prediction of the five factors has higher effi-cacy.
2.Study on the clinical effect of meridian massage in the treatment of lumbar disc herniation

Objective To observe the clinical efficacy of meridian massage in the treatment of lumbar disc herniation(LDH).Methods Between July 2020 and April 2023,82 patients with lumbar disc herniation were selected,including 58 males and 24 females,aged from 23 to 55 years old with an average of(43.76±6.64)years old.According to the different treatment methods,they were divided into observation group and control group with 41 cases in each group.The control group was treated with routine treatment,and the observation group was treated with meridian massage on the basis of routine treatment.In the control group,there were 30 males and 11 females;aged from 22 to 52 years old with an average of(42.27±9.34)years old;the Body mass index(BMI)ranged from 19 to 28 kg·m-2 with an average of(23.82±1.08)kg·m-2;the course of disease ranged from 0.5 to 3.0 years(2.40±0.48)years.There were 28 cases in L4,5 segment and 13 cases in L5S1 segment.In the observation group,there were 28 males and 13 females;the age ranged from 19 to 54 years old(42.19±9.26)years old;the BMI ranged from 18 to 29 kg·m-2 with an average of(23.73±1.15)kg·m-2;the course of disease ranged from 0.6 to 2.8 with an average of(2.56±0.45)years;there were 26 cases in L4,5 segment and 15 cases in L5S1 segment.Visual analogue scale(VAS),Oswestry disability index(ODI),M-JOA score and TCM syndrome score were measured before and after 3 courses of treatment,and the clinical efficacy was evaluated by the standard of curative effect evaluation.Results After treatment,VAS[(3.24±1.45)vs(4.46±0.64)],ODI[(11.45±1.98)%vs(17.21±2.74)%]and TCM symptom score[(2.03±0.27)vs(3.99±0.54)]of the observation group were lower than those of the control group.The score of M-JOA[(23.43±2.61)vs(19.37±1.62)]increased(P<0.05).The scores of VAS,ODI andTCM symptoms in the observation group were lower than those in the control group,while the scores of M-JOA were higher than those in the control group(P<0.05).Conclusion Meridian massage is effective in the treatment of LDH,which can effectivelyrelieve low back pain,improve clinical symptoms and increaselumbar function,which is worthy of clinical promotion.
3.Feasibility Analysis of Establishing Combat Readiness Blood Bank Based on Low Titer Group O Whole Blood and Group A Plasma

Objective:To explore the feasibility of establishing combat readiness blood bank with low titer group O whole blood and group A plasma.Methods:The Galileo automatic blood analyzer was used to detect the titers of IgM anti-A and anti-B antibodies in the samples of group O blood donors and IgM anti-B titer in the samples of group A blood donors.Group O blood donors with antibody titers below 128 were selected and included in the mobile blood bank for combat readiness,group A plasma with anti-B titer lower than 128 and group O whole blood with antibody titers below 128 were included in the combat readiness entity blood bank.Results:A total of 1 452 group O blood donors were selected,and the anti-A/B antibody titers were detected.Both antibody titers were distributed below 512,and both peak values of sample distribution were at titer 4.The proportion of samples with titers>128 for both antibodies was relatively low.There was a significant positive correlation between the titers of the two antibodies(r=0.383),and the proportion of samples with IgM anti-A titer higher than IgM anti-B titer was relatively high.1 335(91.94%)group O blood donors with IgM anti-A and anti-B antibody titers<128 could be included in the mobile blood bank.The anti-B titer of group A blood was detected in 512 cases and the results showed that as the antibody titer increased,the proportion of blood donors gradually decreased.99.8%of group A blood donors had anti-B antibody titer less than 128,and only one case did not meet the inclusion criteria.Conclusion:The proportion of group O blood donors whose whole blood meet the low antibody titer standard is high,and almost all plasma of group A blood donors meet the low titer standard,which improves the blood supply rate in emergencies.

This video presents a case of L4–5 unstable spondylolisthesis treated with full-endoscopic transforaminal lumbar interbody fusion (Endo-TLIF), emphasizing the GUARD (Glider Used as a Rotary Device) technique for nerve root protection. This innovative approach involves controlled rotation of the cage glider before cage insertion to minimize the risk of nerve root injury, a significant complication in Endo-TLIF procedures. The GUARD technique, validated in previous cadaveric studies, provides enhanced safety during cage insertion by protecting the nerve root. A 48-year-old woman with a 3-year history of progressive low back pain and bilateral lower extremity radiculopathy (right-sided predominance) was diagnosed with L4–5 unstable spondylolisthesis and spinal stenosis. After failure of conservative management, she underwent uniportal full-endoscopic facet-resecting transforaminal lumbar interbody fusion using the GUARD technique. Postoperatively, the patient experienced significant symptomatic improvement and resolution of radiculopathy, without any intraoperative nerve root injury or postoperative neurological deficits. This case demonstrates the effectiveness of the GUARD technique in reducing neurological complications and improving patient outcomes.
5.Reducing Postoperative Neurological Complications in Uniportal Full-Endoscopic Lumbar Interbody Fusion: Efficacy of the GUARD Technique Combined With Delayed Ligamentum Flavectomy

Objective:
Uniportal full-endoscopic transforaminal lumbar interbody fusion (FE-TLIF) carries a unique risk of nerve traction and abrasion injury during cage insertion. This study aims to evaluate the clinical efficacy of the GUARD technique and delayed ligamentum flavectomy in reducing postoperative radicular pain and neurapraxia in patients undergoing uniportal FE-TLIF.
Methods:
A retrospective analysis was conducted on 45 patients with an average age of 53.9±12.4 years who underwent either FE facet-sparing TLIF (FE fs-TLIF) or FE facet-resecting TLIF (FE fr-TLIF). Patients were divided into 2 groups: the sentinel group (21 patients) using traditional sentinel pin techniques, and the GUARD group (24 patients) using the GUARD technique with delayed ligamentum flavectomy. Patient-reported outcomes included the visual analogue scale (VAS) for leg and back pain, and Oswestry Disability Index. Complication rates, including incidental durotomy, postoperative neurapraxia, and hematoma, were also documented.
Results:
Postoperative radicular pain in the legs was significantly reduced at 6 weeks in the GUARD group compared to the sentinel group (VAS: 2.201 vs. 3.267, p=0.021). The incidence of postoperative neurapraxia was markedly lower in the GUARD group (0% vs. 19%, p=0.047). Both groups showed similar improvements in disc height, segmental lordosis, and lumbar lordosis at the 1-year follow-up, with no significant differences in endplate injury or fusion rates.
Conclusion
The GUARD technique and delayed ligamentum flavectomy significantly enhance patient safety by reducing postoperative radicular pain and neurapraxia without incurring additional costs. These techniques are easy to learn and integrate into existing surgical workflows, offering a valuable improvement for surgeons performing FE-TLIF procedures.

Objective To construct a hierarchical inventory model of emergency supplies applicable to uncertain environments in order to provide decision support for emergency responses to infectious disease outbreak.Methods An emergency supplies hierarchical model was constructed with the zigzag uncertainty distribution,which was combined with an economic order quantity(EOQ)model to establish an emergency supplies hierarchical inventory model.An acute respiratory infectious disease event in W city was used as an example to verify the practicality and effectiveness of the model proposed.Results The model was able to accurately classify emergency supplies in most cases,and the inventory quantity determined with the model was close to the demand levels.Conclusion The model proposed is practical and effective in emergency responses to acute respiratory infectious diseases,which facilitates the requirements prediction for emergency supplies and enhances the capacity and efficiency of emergency responses.[Chinese Medical Equipment Journal,2024,45(5):67-74]
