1.Geraniin attenuates isoproterenol-induced cardiac hypertrophy by inhibiting inflammation, oxidative stress and cellular apoptosis

Geraniin, a polyphenol derived from the fruit peel of Nephelium lappaceum L., has been shown to possess anti-inflammatory and antioxidant properties in the cardiovascular system. The present study explored whether geraniin could protect against an isoproterenol (ISO)-induced cardiac hypertrophy model. Mice in the ISO group received an intraperitoneal injection of ISO (5 mg/kg) once daily for 9 days, and the administration group were injected with ISO after 5 days of treatment with geraniin or spironolactone. Potential therapeutic effects and related mechanisms analysed by anatomical coefficients, histopathology, blood biochemical indices, reverse transcription-PCR and immunoblotting. Geraniin decreased the cardiac pathologic remodeling and myocardial fibrosis induced by ISO, as evidenced by the modifications to anatomical coefficients, as well as the reduction in collagen I/III á1mRNA and protein expression and cross-sectional area in hypertrophic cardiac tissue. In addition, geraniin treatment reduced ISO-induced increase in the mRNA and protein expression levels of interleukin (IL)-6, IL-1β and tumor necrosis factor-α, whereas ISO-induced IL-10 showed the opposite behaviour in hypertrophic cardiac tissue.Further analysis showed that geraniin partially reversed the ISO-induced increase in malondialdehyde and nitric oxide, and the ISO-induced decrease in glutathione, superoxide dismutase and glutathione. Furthermore, it suppressed the ISO-induced cellular apoptosis of hypertrophic cardiac tissue, as evidenced by the decrease in Bcell lymphoma-2 (Bcl-2)-associated X/caspase-3/caspase-9 expression, increase in Bcl-2 expression, and decrease in TdT-mediated dUTP nick-end labeling-positive cells.These findings suggest that geraniin can attenuate ISO-induced cardiac hypertrophy by inhibiting inflammation, oxidative stress and cellular apoptosis.

6.Three-dimensional finite element analysis of lumbar disc herniation under different body positions

Objective To campare biomechanical effects of different postural compression techniques on three-dimensional model of lumbar disc herniation(LDH)by finite element analysis.Methods Lumbar CT image of a 48-year-old female patient with LDH(heighted 163 cm,weighted 53 kg)was collected.Mimics 20.0,Geomagic Studio,Solidwords and other software were used to establish three-dimensional finite element model of LDH on L4,5 segments.Compression techniques under horizon-tal position,30° forward bending and 10° backward extension were simulated respectively.After applying the pressure,the ef-fects of compression techniques under different positions on stress,strain and displacement of various tissues of intervertebral disc and nerve root were observed.Results L4,5 segment finite element model was successfully established,and the model was validated.When compression manipulation was performed on the horizontal position,30° flexion and 10° extension,the annular stress were 0.732,5.929,1.286 MPa,the nucleus pulposus stress were 0.190,1.527,0.295 MPa,and the annular strain were 0.097,0.922 and 0.424,the strain sizes of nucleus pulposus were 0.153,1.222 and 0.282,respectively.The overall displace-ment distance of intervertebral disc on Y direction were-3.707,-18.990,-4.171 mm,and displacement distance of nerve root on Y direction were+7.836,+5.341,+3.859 mm,respectively.The relative displacement distances of nerve root and interverte-bral disc on Y direction were 11.543,24.331 and 8.030 mm,respectively.Conclusion Compression manipulation could make herniated intervertebral disc produce contraction and retraction trend,by increasing the distance between herniated interverte-bral disc and nerve root,to reduce symptoms of nerve compression,to achieve purpose of treatment for patients with LDH,in which the compression manipulation is more effective when the forward flexion is 30°.
7.Role and mechanisms of probiotics in regulating the ROS/JNK signaling pathway in the pathogenesis of Metabolic fatty liver disease

Objective This study was aimed to investigate the impact of probiotics on regulating the ROS/JNK signaling pathway their underlying mechanism of action in the treatment of metabolic associated fatty liver disease.Methods Male C57BL/6 mice were randomly divided into three groups:control,probiotics,and model groups.Serum levels of ALT,AST,TC,and TG were detected.Moreover,the pathological changes of the liver and ileum tissues were observed by hematoxylin and eosin(H&E)staining,and the content of reactive oxygen species(ROS)in liver tissues was determined.In addition,the levels of D-lactic acid and serum and small intestine diamine oxidase were measured to evaluate the effects of probiotics on the intestinal tract of MAFLD mice.Results Mice in the probiotic group were treated with probiotic intervention,compared with model group,mice hepatic lobule structure was complete,hepatic steatosis in cells and inflammatory cells infiltration were relieved.After C57BL/6 mice were fed the MCD diet,the body weight of the model group was significantly lower than that of the control group,showing poor mental state.After 4 weeks of probiotics treatment,the body weight of mice in the probiotics group was higher than that in the model group,and their mental state was better than that in the model group.Biochemical analysis showed that the levels of ALT,AST,TC,TG,D-lac,serum and small intestinal DAO and ROS in liver in the model group were considerably higher than those in the control group.After 4 weeks of probiotics intervention,serum ALT,AST,TC,TG,D-lac,DAO and ROS levels decreased compared with the model group(P<0.05).Western blot analysis showed that the expression levels of p-JNk,Caspase-3,Bax in the model group were up-regulated,and the expression level of intestinal tight junction protein ZO-1 was down-regulated compared with the control group.After 4 weeks of probiotics intervention,the expression levels of p-JNK,caspase-3,Bax in the probiotics group were down-regulated compared with that in the model group,while the expression level of intestinal tight junction protein ZO-1 was up-regulated compared with that in the model group(P<0.05).PCR analysis showed that the expression levels of Bax and Caspase-3 in the model group were significantly increased,while the expression levels of Bax and Caspase-3 in the probiotic group were significantly decreased compared with those in the model group(P<0.05).Immunohistochemistry analysis showed that the expression levels of P-JNk,Caspase-3 and Bax in the liver of mice in the probiotic group were lower than those in the model group(P<0.05).The pathological results in our investigation showed that the ileum mucosa in the control group was intact,and the intestinal villi were complete and neatly arranged,without any infiltration of inflammatory cells.However,in the model group the ileum mucosal structure was seriously damaged,with loose and disorderly villi arrangement.Importantly,the ileum mucosa in the probiotic group had fewer injuries and the villi were arranged more neatly.Besides,the ileum tissue of the control group had the lowest Chiu's score range(0-1),whereas that of the model group was 3-4,and the one of the probiotic group was 1-2.Conclusion The increased expression of P-JNk,ROS,Caspase-3 and Bax in MAFLD indicates that it is associated with the onset of MAFLD.The use of probiotics can improve the histological and serological indicators of MAFLD.Western blot,PCR and immunohistochemical results confirmed that probiotics can regulate the expression of factors related to ROS/JNK signaling pathway,reduce oxidative stress and inhibit cell apoptosis to achieve the goal of treating MAFLD.

Neck dissection(ND)is one of the main treatment methods for oral and maxillofacial malignancies.Although ND type is in con-stant improvement,but intraoperative peal-pull-push injury of the accessory nerve,muscle,muscle membrane,fascia and ligament induced shoulder syndrome(SS)is still a common postoperative complication,combined with the influence of radiochemotherapy,not only can cause pain,stiffness,numbness,limited dysfunction of shoulder neck and arm,but also may have serious impact on patient's life quality and phys-ical and mental health.At present,there is still a lack of a systematic evaluation and rehabilitation management program for postoperative SS of oral and maxillofacial malignant tumors.Based on the previous clinical practice and the current available evidence,refer to the relevant lit-erature at home and abroad,the experts in the field of maxillofacial tumor surgery and rehabilitation were invited to discuss,modify and reach a consenusus on the etiology,assessment diagnosis,differential diagnosis,rehabilitation strategy and prevention of SS,in order to provide clinical reference.
9.Study of economic burden of healthcare-associated infection caused by hemorrhagic stroke based on propensity score matching and generalized linear model

Objective To study the economic burden caused by healthcare-associated infection(HAI)in patients with hemorrhagic stroke.Methods Patients with hemorrhagic stroke in a tertiary first-class hospital from January 1,2021 to December 31,2022 were surveyed retrospectively.Data on demographic characteristics,clinical informa-tion,and hospitalization expenses were collected.According to the occurrence of HAI,patients were divided into the HAI group and control group.The length of hospital stay,increase in hospitalization expense,and hospital eco-nomic burden of the HAI group and control group were studied by propensity score matching(PSM)method and generalized linear model method.Results A total of 688 patients were included in the study,with 266 cases experi-encing HAI and a HAI incidence of 38.66%.After propensity score matching,199 patients in the HAI group were successfully matched.Compared with the control group,the median length of hospital stay in the HAI group doub-led,increasing by 16 days(Z=11.779,P<0.001);the median hospitalization expense increased by 34 597.42 Yuan,with an increase of 85%(Z=6.299,P<0.001).Based on the generalized linear model method,length of hospital days attributed to HAI increased by 1.24 times,hospitalization expense increased by 76%(both P<0.001).Except surgical expenses,the HAI group had higher single medical expenses than the control group(all P<0.05).Economic burden to hospital caused by HAI was 541 900 Yuan.Conclusion HAI significantly increases the economic burden of hemorrhagic stroke patients and hospitals,and prolongs the length of hospital stay.Clinical staff should enhance the awareness on infection control,reduce the incidence of HAI,and save medical resources.
10.Experimental validation of machine learning identification of KDELR3 as a signature gene for osteoarthritis hypoxia

BACKGROUND:Hypoxia is strongly associated with the development and progression of osteoarthritic chondrocyte injury,but the specific targets and regulatory mechanisms are unclear. OBJECTIVE:A machine learning approach was used to identify KDEL(Lys-Asp-Glu-Leu)receptor 3(KDELR3)as a characteristic gene for osteoarthritis hypoxia and immune infiltration analysis,to provide new ideas and methods for the treatment of osteoarthritis. METHODS:The osteoarthritis-related datasets were downloaded from the GEO database and the GSEA website to obtain hypoxia-related genes.The osteoarthritis datasets were batch-corrected and immune infiltration analyzed using R language,and osteoarthritis hypoxia genes were extracted for differential analysis.Differentially expressed genes were analyzed for GO function and KEGG signaling pathway.Weighted correlation network analysis(WGCNA)and machine learning were also used to screen osteoarthritis hypoxia signature genes,and in vitro cellular experiments were performed to validate expression and correlate immune infiltration analysis using the datasets and qPCR. RESULTS AND CONCLUSION:(1)8492 osteoarthritis genes were obtained by batch correction and principal component analysis,mainly strongly associated with immune cells such as Macrophages M2 and Mast cells resting;200 hypoxia genes were also obtained,resulting in 41 osteoarthritis hypoxia differentially expressed genes.(2)GO analysis involved mainly biological processes such as response to nutrient levels and glucocorticoids;cellular components such as lysosomal lumen and Golgi lumen;and molecular functions such as 14-3-3 protein binding and DNA-binding transcriptional activator activity.(3)KEGG analysis of osteoarthritis hypoxia differentially expressed genes was associated with signaling pathways such as PI3K-Akt,FoxO,and microRNAs in cancer.(4)The characteristic gene KDELR3 was obtained after using WGCNA analysis and machine learning screening.(5)The gene expression of KDELR3 was found to be higher in the test group than in the control group in the synovium(P=0.014)but lower in the meniscus(P=0.024)after validation by gene microarray.(6)In vitro chondrocyte assay showed that the expression of KDELR3 was higher in cartilage than in the control group(P=0.005),while KDELR3 was closely associated with Macrophages M0(P=0.014)and T cells follicular helper(P=0.014).Using a machine learning approach,we confirmed that KDELR3 can be used as a hypoxic signature gene for osteoarthritis and may intervene in osteoarthritis pathogenesis by improving hypoxia,expecting to provide a new direction for better treatment of osteoarthritis.
