Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.
2.Study of safety and clinical outcomes of ommaya reservoir combined with external ventricular drainage for intraventricular hemorrhage

Objective To evaluate safety and clinical effect of Ommaya reservoir combined with external ventricular drainage for intraventricular hematoma(IVH),and to analyze the effects on neurological recovery,blood-brain-barrier(BBB)protection and the incidence of complications.Methods A total of 100 patients admitted to Yichun People's Hospital of Jiangxi Province for IVH and underwent surgical treatment from January 2021 to January 2023 were selected as the study subjects.Using a random number table,the patients were divided into control group(n=50)and treatment group(n=50).The treatment group received Ommaya reservoiv combined with external ventricular drainage,while the control group received conventional external ventricular drainage.The postoperative improvement rate,neurological recovery,BBB index,S100 calcium binding protein beta(neural)(S100β),myelin basic protein(MBP)levels,and hematoma clearance rate were compared between two groups.The incidence of complications in both groups was monitored over a 24-month follow-up period.Results At 3d,7d,and 14d post-surgery,the hematoma clearance rate in treatment group was higher than that in control group.At 3,12,and 24 months post-surgery,the patients of treatment group showed a higher improvement rate then that of control group,with better performance in National Institutes of Health stroke scale and Canadian stroke scale scores for neurological recovery,all differences being statistically significant(P<0.05).During follow-ups at 1,3,12,and 24 months post-surgery,the BBB index,S100β,and MBP levels in treatment group were all lower than those in the control group,with all differences being statistically significant(P<0.05).At 24 months post-surgery,the patients of treatment group had a lower incidence of complications than that of control group(P<0.05).Conclusion Ommaya reservoir combined with exteral ventricular drainage can significantly improve neurological function,protect the BBB,and show good long-term efficacy in follow-up.
3.Analysis of Positive Results of Anti-M Unexpected Antibody in Pediatric Inpatients in Central China

Objective:To analyze the positive rate and distribution of anti-M unexpected antibody in pediatric inpatients aged 0 to 14 years in central China.Methods:A total of 30 049 pediatric inpatients admitted to the Second Xiangya Hospital of Central South University,Wuhan Children's Hospital and Children's Hospital Affiliated of Zhengzhou University from May 2020 to August 2022 were enrolled in this study,and relevant clinical data were collected.Blood samples from the patients were tested for blood typing and screened for unexpected antibodies.For samples that screened positive for unexpected antibodies,identification was conducted using the identification panel to determine the specificity of the antibodies.The distribution and differences of anti-M antibodies in pediatric patients of different sexes,ages,blood groups,disease types,with or without a history of blood transfusion,and across different regions were analyzed.Results:Among 30 049 inpatients,the positive rate of unexpected antibodies was 0.91%(273/30 049),of which the positive rate of anti-M antibodies was 0.44%(131/30 049).The positive rate of anti-M antibodies in the neonates aged 0 to<1 month was 0.10%(5/4 881),and all of them were IgG antibodies from their mothers;The positive rate of anti-M antibodies for the group aged from 1 month to<1 year old was 0.23%(7/3 108),with no anti-M antibodies detected in patients aged 1-6 months;The positive rates of anti-M antibodies in the 1-4 years old group,5-9 years old group,and 10-14 years old group were 0.87%(88/10 064),0.38%(27/7 190),and 0.08%(4/4 806),respectively.The positive rate of anti-M antibodies in the 1-4 years old group was significantly higher than that of the other groups(P<0.001),and there were also statistical differences in the positive rate between the 5-9 years old group and the 0-<1 month and 10-14 years old groups(P<0.001).The prevalences of anti-M antibodies in ABO blood group A,B,O and AB were 0.32%(30/9 482),0.70%(58/8 293),0.32%(31/9 595)and 0.45%(12/2 679),respectively.The prevalence of anti-M antibodies in patients with blood group B was significantly higher than that in patients with blood groups A and O(P<0.05).The prevalences of anti-M antibodies in Hunan,Hubei and Henan was 0.18%,0.32%and 0.71%,respectively.The prevalence of anti-M antibodies in Henan was significantly higher than that in Hunan and Hubei(P<0.05),and the distribution showed obvious regional differences between the north and the south.There were no significant differences in the positive rate of anti-M antibodies between the children with different sexes,disease types,and with or without a history of blood transfusion(P>0.05).Conclusion:This study reveals the distribution pattern of anti-M antibodies in pediatric inpatients aged 0-14 years in central China,which has reference value for the research on unexpected red blood cell antibodies in Chinese children.

Thoracic anesthesia with spontaneous respiration represents a form of precision anesthesia meticulously customized to individual patients.Considering the more stringent requirements this anesthesia approach imposes on the regulation of respiratory function,the writing group of the"Consensus of Experts on the Management of Thoracic Anesthesia with Spontaneous Respiration"has formulated elaborate guidelines regarding indications and contraindications,preoperative evaluation,anesthesia implementation,common complications,and treatment strategies.This was accomplished by referencing relevant domestic and international literature and integrating it with actual clinical requirements.The objective is to standardize the rational application of this anesthesia method.

Postoperative infection of internal fixation of closed fractures the lower limbs in adults represents a devastating complication, characterized by diagnostic challenges, prolonged treatment duration and high disability rates. Current management of these infections faces multiple challenges, such as difficulties in early accurate diagnosis, and various controversies about the treatment plan, leading to poor overall diagnosis and treatment results. To address these issues, based on evidence-based medicine and principles with emphasis on scientific rigor, clinical applicability and innovation, the Trauma Branch of the Chinese Medical Association, Orthopedic Branch of the Chinese Medical Doctor Association, Orthopedics Branch of the Chinese Medical Association, and Trauma Orthopedics and Polytrauma Group of the Resuscitation and Emergency Committee of the Chinese Medical Doctor Association have collaboratively organized a panel of relevant experts to develop the Guideline for diagnosis and treatment of infection after internal fixation of closed lower limb fractures in adults ( version 2025). The guideline proposed 10 recommendations, aiming to provide a foundation for standardized diagnosis and treatment of postoperative infection in adults with closed lower limb fractures.

Paraplegia caused by spinal cord injury is a serious neurological complication, for which surgery is currently the main treatment method. Due to different surgical approaches, patients are usually expected to maintain a passive prone position for a long time or switch between the supine and prone positions. Affected by multiple factors such as neurogenic sensory disorders, pathological changes in muscle tone and operative duration, the risk of intraoperative acquired pressure injury (IAPI) is significantly increased. Current clinical prevention strategies for IAPI in these patients predominantly focus on localized pressure relief during positioning, lacking systematic, standardized comprehensive prevention protocols or evidence-based guidelines. To address it, Department of Nursing, Orthopedics Branch, China International Exchange and Promotive Association for Medical and Health Care, Spinal Trauma Professional Committee, Orthopedics Branch, Chinese Medical Doctor Association, Nursing Group of Spine and Spinal Cord Professional Committee of Chinese Association of Rehabilitation Medicine organized experts in relevant fields to formulate Guideline for the prevention of intraoperative acquired pressure injury in paraplegic patients with spinal cord injury ( version 2025), based on evidence-based medical evidence and latest research results and clinical practice at home and abroad. Eleven recommendations were put forward from the aspects of preoperative risk assessment, intraoperative prevention strategies, postoperative handover and monitoring, and supportive mechanisms for IAPI prevention, aiming to standardize the prevention measures and management strategies of IAPI in paraplegic patients with spinal cord injury and accelerate the recovery of patients and improve the therapeutic effect.
7.Clinical characteristics of 21 cases of nocardiosis and antimicrobial resistance of Nocardia strains in a hospital in Henan Province

Objective The clinical characteristics of 21 cases of nocardiosis were reviewed and antimicrobial resistance of Nocardia strains was analyzed in order to improve the accuracy of clinical diagnosis and treatment of nocardiosis.Methods Clinical data of patients diagnosed with nocardiosis in Zhoukou Central Hospital from 2019-2023 and the corresponding results of antimicrobial susceptibility testing were retrospectively analyzed to summarize the clinical characteristics and outcomes of patients.Results Overall,the 21 cases of nocardiosis included 9 males and 12 females,aged 2-91 years.Underlying disease was reported in 15 patients.Most common type of nocardiosis was pulmonary nocardiosis in 15 cases,followed by skin and soft tissue infection,pleurisy,lymphadenitis,and disseminated nocardiosis.Laboratory tests showed increased levels of WBC,neutrophils percentage,erythrocyte sedimentation rate,C-reactive protein,and procalcitonin.The 21 strains of Nocardia included 4 strains of Nocardia cyriacigeorgica,2 strains each of Nocardia brasiliensis,Nocardia abscessus,Nocardia asiatica,Nocardia otitidiscaviarum and Nocardia beijingensis,and 1 strain each of Nocardia puris,Nocardia asteroides,Nocardia farcinica,Nocardia pneumoniae,Nocardia amamiensis,and 2 strains of unclassified Nocardia.All of the Nocardia strains(100%)were susceptible to linezolid,amikacin,and trimethoprim-sulfamethoxazole,followed by various levels of susceptibility to cefotaxime,moxifloxacin,imipenem and ceftriaxone,and lower susceptibility rate to cefepime,minocycline,ciprofloxacin and clarithromycin.Antimicrobial susceptibility of Nocardia strains varied with different Nocardia species.Of the 21 patients,two were referred to other hospitals,another two died,two patients received unknown treatment,and the remaining 15 patients were improved after antibiotic treatment,including sulfonamides combined with other antibiotics in 11 cases,other antibiotics in 4 cases.Conclusions Immunocompromised patients or those with underlying diseases are more susceptible to nocardiosis.The clinical features are complex and diverse.Antimicrobial susceptibility of Nocardia strains varied with different Nocardia species.Accurate identification and antimicrobial susceptibility test are essential for prescribing effective antibiotic treatment.
8.Ruibin Agent versus mainstream large language models: A comparative study on medical literature comprehension with esophageal cancer as a case study

Objective To explore the application value of artificial intelligence in medical research assistance, and analyze the key paths to achieve precise execution of model instructions, improvement of model interpretation completeness, and control of hallucinations. Methods Taking esophageal cancer research as the scenario, five types of literature including research articles, case reports, reviews, editorials, and guidelines were selected for model interpretation tests. The model performance was systematically evaluated from five dimensions: recognition accuracy, format accuracy, instruction execution accuracy, content reliability rate, and content completeness index. The performance differences of Ruibin Agent, GPT-4o, Claude 3.7 Sonnet, DeepSeek V3, and DouBao-pro models in medical literature interpretation tasks were compared. Results A total of 15 studies were included, with 3 studies of each type. The five models collectively conducted 1 875 tests. Due to the poor recognition accuracy of the editorial type, the overall recognition accuracy of Ruibin Agent was significantly lower than other models (92.0% vs. 100.0%, P<0.001). In terms of format accuracy, Ruibin Agent was significantly better than Claude 3.7 Sonnet (98.7% vs. 92.0%, P=0.002) and GPT-4o (98.7% vs. 78.9%, P<0.001). In terms of instruction execution accuracy, Ruibin Agent was better than GPT-4o (97.3% vs. 80.0%, P<0.001). In terms of content reliability rate, Ruibin Agent was significantly lower than Claude 3.7 Sonnet (84.0% vs. 92.0%, P=0.010) and DeepSeek V3 (84.0% vs. 94.7%, P<0.001). In terms of content completeness index, the median scores of Ruibin Agent, GPT-4o, Claude 3.7 Sonnet, DeepSeek V3, and DouBao-pro were 0.71, 0.60, 0.85, 0.74, and 0.77, respectively. Conclusion Ruibin Agent has significant advantages in terms of formatted interpretation of medical literature and instruction execution accuracy. In the future, it is necessary to focus on optimizing the recognition ability of editorial types, strengthening the coverage ability of core elements of various types of literature to improve interpretation completeness, and improving content reliability through optimizing the confidence mechanism to ensure the rigor of medical literature interpretation.
10.Artificial intelligence in endoscopic diagnosis of esophageal squamous cell carcinoma and precancerous lesions.

Esophageal squamous cell carcinoma (ESCC) poses a significant global health challenge, necessitating early detection, timely diagnosis, and prompt treatment to improve patient outcomes. Endoscopic examination plays a pivotal role in this regard. However, despite the availability of various endoscopic techniques, certain limitations can result in missed or misdiagnosed ESCCs. Currently, artificial intelligence (AI)-assisted endoscopic diagnosis has made significant strides in addressing these limitations and improving the diagnosis of ESCC and precancerous lesions. In this review, we provide an overview of the current state of AI applications for endoscopic diagnosis of ESCC and precancerous lesions in aspects including lesion characterization, margin delineation, invasion depth estimation, and microvascular subtype classification. Furthermore, we offer insights into the future direction of this field, highlighting potential advancements that can lead to more accurate diagnoses and ultimately better prognoses for patients.
