OBJECTIVE:
To detect the expression of miR-99a-5p in myelodysplastic syndrome (MDS), to predict the target genes and to analyze its function by using bioinformatics.
METHODS:
The expression levels of bone marrow miR-99a-5p in MDS patients were detected by qRT-PCR, and the correlation of miR-99a-5p expression with clinical pathological characteristics, percentage of marrow blasts , chromosome karyotype and peripheral blood hemogram were analyzed. The target genes of miR-99a-5p were predicted by Targetscan, Miranda and Microcosm, and the intersection of the predicted results of 3 softwares was used as a potential target gene for miR-99a-5p.
RESULTS:
The expression level of bone marrow miR-99a-5p in MDS patients was significantly higher than that of healthy controls (P<0.01); According to the prognostic score of IPSS, MDS patients were divided into relatively low risk group (including low risk group and intermediate risk group 1) and relatively high risk group (including intermediate risk group 2 and high risk group). Analysis showed that the expression level of bone marrow miR-99a-5p in relatively low risk group was 2.40 times higher than that in healthy control group (P<0.01), the expression level of bone marrow miR-99a-5p in relatively high risk group was 6.66 times higher than that in healthy control group (P<0.01), the expression level of miR-99a-5p in relatively high risk group was 2.80 times higher than that in the relatively low risk group ( P<0.01) ; the expression level of bone marrow miR-99a-5p in t-MDS group was 6.35 times higher than that in healthy control group (P<0.001). There was a significant positive correlation between the expression level of miR-99a-5p and the percentage of marrow blasts (P<0.01), but the expression of miR-99a-5p did not correlate with chromosome karyotype and peripheral blood hemogram; In this study it was obtained that ST5 might participate in pathological mechanism of MDS by bioinformatic analyses and references.
CONCLUSION
The expression levels of miR-99a-5p is up-regulated in MDS patients, and its expression showed a rising tendency which may be involved in the pathogenesis of MDS by regulating target gene ST5.

To research the effection and probable mechanism for the total saponins of Panax japonicas(TPSJ) in mice on non-alcoholic fatty liver disease. Forty SPF male Kunming mice were randomily divided into four group:control group,NAFLD group, low-dose TPSJ treated group,high-dose TPSJ treated group. High-fatty and high-frutose-diet was applied to eatablish NAFLD model, and TPSJ (100 and 200 mg·kg⁻¹) in feeding were given for the TPSJ groups for 4 weeks. To collect the serum with liver and the ALT and TC of serum were monitored after 4 weeks. The hepatic histopathologic structure was observed by haematoxylin-eosin (HE) staining, RT-PCR and RT-qPCR was applied for the detection of miR-199-5p,VEGFa,HGF,c-Met and protein expression level was detected bv laser confocal microscope.Compared with control group, the level of serum ALT and TC in the model group was higher, the liver of the model group showed that hepatocytes display obvious lipid deposition. Then TPSJ treated showed that markedly improved histopathologic changes, decreased fatty deposition. In the meantime, the expression level of miR-199-5p was significantly decreased, thus the expression of HGF and c-Met were significantly increased. TPSJ play a role of prevention on fatty liver, the machanism maybe by blocking miR-199-5p targeted to c-Met signaling pathways in NAFLD.

AIM:To investigate the role of nitric oxide synthase (NOS) inhibitor asymmetric dimethylarginine (ADMA) in erectile dysfunction of diabetic rats.METHODS:Type 2 diabetic rat model was established by 4 weeks of high-fat diet plus a single intraperitoneal injection of streptozotocin and continued high-fat diet feeding for 8 weeks.Corpus cavernosum was isolated from the rats under anesthetization, and the endothelium-dependent relaxation response to acetylcholine (ACh) was tested in an organ chamber to reflect erectile function. The level of ADMA in serum was detected. The NOS activity, nitric oxide (NO) content and cyclic guanosine monophosphate (cGMP) content in corpus cavernosum were measured. The protein expression of ADMA-NOS-NO pathway-related molecules and phosphodiesteras 5 (PDES) in the corpus cavernosum was detected by Western blot.Superoxide dismutase activity and malondialdehyde content were analyzed to evaluate oxidative stress.RESULTS:Elevated blood glucose and lowered insulin sensitivity were observed in the diabetic rats, indicating that type 2 diabetic rat model was successfully established.Compared with control group, the relaxation response to ACh of corpus cavernosum from diabetic rats was significantly decreased, which was accompanied with the elevation of serum ADMA level and reduction of NOS activity,NO content and cGMP content in the corpus cavernosum. The protein expression of ADMA-generating enzyme protein arginine methyltransferase 1 was up-regulated,while ADMA-metabolic enzymes dimethylarginine dimethylaminohydrolases 1 and 2,and ADMA-targeting enzymes endothelial NOS and neuronal NOS were down-regulated.The protein expression of PDE5 was up-regulated,accompanied with an increase in oxidative stress in the corpus cavemosum of diabetic rats.Incubation of isolated corpus cavernosum from normal rats with NOS inhibitor ADMA induced the similar relaxation dysfunction of corpus cavemosum response to ACh and decreased NO and cGMP contents in diabetic rats.CONCLUSION:Elevated endogenous NOS inhibitor ADMA plays an important role in erectile dysfunction of diabetic rats. The underlying mechanism may be related to the reduction of NO production and the increase in oxidative stress.

Objective To explore the role of the changes in endogenous L-citrulline levels and arginases expression in erectile dysfunction of diabetic rats, and investigate the effect of L-citrulline incubation ex vivo on the erectile dysfunction. Methods Type 2 diabetic rats were induced by high fat diet plus a single intraperitoneal injection of small dose streptozotocin (30 mg/kg),followed by high fat diet for 8 weeks. The erectile function of rats was reflected by the relaxation response to acetylcholine (ACh) of corpus cavernosum detected by isometric tension measurement.Levels of endogenous nitric oxide inhibitor asymmetric dimethylarginine (ADMA) and cyclic guanosine monophosphate (cGMP) were measured by enzyme-linked immunosorbent assay (ELISA).The content of L-citrulline,nitric oxide (NO) and the activity of nitric oxide synthase (NOS) were determined with colorimetric method.Western blotting was applied to detect the protein expression of NOS,arginases and phosphodiesterase 5 (PEDS).Results The relaxation response to ACh of corpus cavernosum from diabetic rats was significantly lower than that of control rats (P < 0.01),indicating the erectile dysfunction of diabetic rats. This erectile dysfunction was companied by the decreases of L-citrulline levels in serum and corpus cavernosum as well as up-regulation of arginaseⅠ & Ⅱ expression in the corpus cavernosum of diabetic rats.Incubation with L-citrulline (3 mmol/L) for 45 min ex vivo improved the relaxation function of corpus cavernosum from diabetic rats (P < 0.05),while incubation with PED5 inhibitor silaenafil (3 μmoL/L) had no significant effect.Furthermore, the ADMA level was increased, while the NOS activity,cGMP and NO contents were decreased in corpus cavernosum of diabetic rats compared to control rats.Down-regulation of endothelial nitric oxide synthase (eNOS) and neuronal nitric oxide synthase (nNOS) protein expression as well as up-regulation of inducible nitric oxide synthase (iNOS) and PDE5 protein expression were also observed in the corpus cavernosum of diabetic rats. Conclusions The decreased endogenous L-citrulline levels and upregulated arginases expression was closely related to erectile dysfunction of diabetic rats, and the mechanism is associated to the up-regulation of arginase activity and decrease of NO synthesis in corpus cavernosum.L-citrulline incubation ex vivo exerted therapeutic effect on the relaxation dysfunction of corpus cavernosum from diabetic rats.

BACKGROUNDThere are limited data on longer-term outcomes (>5 years) for patients with unprotected left main coronary artery (ULMCA) disease who underwent percutaneous coronary intervention (PCI) in the drug-eluting stents (DES) era. This study aimed at comparing the long-term (>5 years) outcomes of patients with ULMCA disease underwent PCI with DES and coronary artery bypass grafting (CABG) and the predictors of adverse events.
METHODSAll consecutive patients with ULMCA disease treated with DES implantation versus CABG in our center, between January 2003 and July 2009, were screened for analyzing. A propensity score analysis was carried out to adjust for potential confounding between the two groups.
RESULTSNine hundred and twenty-two patients with ULMCA disease were enrolled for the analyses (DES = 465 vs. CABG = 457). During the median follow-up of 7.1 years (interquartile range 5.3-8.2 years), no difference was found between PCI and CABG in the occurrence of death (P = 0.282) and the composite endpoint of cardiac death, myocardial infarction (MI) and stroke (P = 0.294). Rates of major adverse cardiac and cerebrovascular events were significantly higher in the PCI group (P = 0.014) in large part because of the significantly higher rate of repeat revascularization (P < 0.001). PCI was correlated with the lower occurrence of stroke (P = 0.004). Multivariate analysis showed ejection fraction (EF) (P = 0.012), creatinine (P = 0.016), and prior stroke (P = 0.031) were independent predictors of the composite endpoint of cardiac death, MI, and stroke in the DES group, while age (P = 0.026) and EF (P = 0.002) were independent predictors in the CABG group.
CONCLUSIONSDuring a median follow-up of 7.1 years, there was no difference in the rate of death between PCI with DES implantation and CABG in ULMCA lesions in the patient cohort. CABG group was observed to have significantly lower rates of repeat revascularization but higher stroke rates compared with PCI. EF, creatinine, and prior stroke were independent predictors of the composite endpoint of cardiac death, MI, and stroke in the DES group, while age and EF were independent predictors in the CABG group.

OBJECTIVETo evaluate the effect of long-term therapy with entecavir and Fufang Biejia Ruangan tablet in patients with chronic hepatitis B (CHB)-associated fibrosis and explore the synergistic therapy that accelerates the reversion of liver fibrosis.
METHODSA total of 197 patients with CHB-associated fibrosis were recruited from Nanfang Hospital between June, 2010 and June, 2015. The patients were divided into two groups after matching for age, gender and liver stiffness measurement (LSM), namely group A (n=98) treated with Fufang Biejia Ruangan Tablet plus entecavir, and group B (n=99) to receive entecavir only. HBV DNA quantification, HBV serological indicators, blood biochemical indexes, and results of abdominal ultrasound and FibroScan were recorded every 12 weeks. FibroScan values were converted to Metavir staging.
RESULTSBoth groups showed significant decreases in serum levels of HBV DNA, alanine aminotransferase (ALT), and LSM value from baseline (all P<0.05). The median time to achieve Metavir fibrosis staging improvement were 72 weeks in group A and 96 weeks in group B (P<0.05), and the median time to achieve ALT and AST normalization were 12 and 24 weeks in Group A, respectively, significantly shorter than the time in group B (P<0.05). No significant difference was found between the two groups in HBV DNA undetectable rate and HBeAg seroconversion rate.
CONCLUSIONThe combination therapy with Fufang Biejia Ruangan tablet and entecavir produces a stronger efficacy than entecavir alone in the treatment of chronic hepatitis B patients with liver fibrosis, and Fufang Biejia Ruangan tablet shows an obvious hepatoprotective effect in these patients.
