Colorectal inflammatory cancer transformation poses a major risk to patients with colitis. Patients with chronic intestinal inflammation have an approximately 2-3 folds increased risk of developing colorectal cancer (CRC). Unfortunately, there is currently no effective intervention available. Huangqin decoction (HQD), a well-known traditional Chinese medicine (TCM) formula, is frequently clinically prescribed for treating patients with colitis, and its active ingredients have effective antitumour efficacy. Nonetheless, the mechanism of HQD-mediated prevention of colorectal inflammatory cancer transformation remains unclear. A strategy integrating metagenomic, lipidomic, and messenger RNA (mRNA) sequencing analysis was used to investigate the regulatory effects of HQD on the gut microbiome, metabolism and potential mechanisms involved in colorectal inflammatory cancer transformation. Our study revealed that HQD suppressed colorectal inflammatory cancer transformation, which was associated with enhanced intestinal barrier function, decreased the inflammatory response, and regulation of the gut microbiome. Notably, cohousing experiments revealed that the transfer of the gut microbiome from HQD-treated mice largely inhibited the pathological transformation of colitis. Moreover, gut microbiome transfer from HQD-treated mice primarily resulted in the altered regulation of fatty acid metabolism, especially the remodeling of arachidonic acid metabolism, which was associated with the amelioration of pathological transformation. Arachidonic acid metabolism and the key metabolic enzyme arachidonic acid 12-lipoxygenase (ALOX12) were affected by HQD treatment, and no obvious protective effect of HQD was observed in Alox 12 -/- mice, which revealed that ALOX12 was a critical mediator of HQD protection against colorectal inflammatory cancer transformation. In summary, multiple omics analyses were applied to produce valuable data and theoretical support for the application of HQD as a promising intervention for the transformation of inflammatory CRC.

IgA vasculitis is a common childhood vasculitis disorder. Its primary clinical manifestation is cutaneous purpura, which is often accompanied by gastrointestinal and joint symptoms.Renal involvement can present as hematuria or proteinuria. This condition is recurrent and protracted, significantly affecting the health of children. The pathogenic factors in pediatric IgA vasculitis are diverse."Wind-toxin damaging collaterals"is presented as the core pathogenesis based on clinical practice and theoretical exploration. This concept essentially entails the latent attack of wind-toxin,collateral damage leading to blood extravasation, and healthy qi deficiency and lingering toxins, with the disease primarily located in the collateral vessels. The disease course is divided into three stages:acute, lingering, and recovery.Clinical practice should adhere to the pathogenesis principles and apply stage-based pattern differentiation and treatment. The acute stage involves wind-toxin attacking collaterals with dampness-heat accumulation.Treatment focuses on dispersing wind, clearing heat, and eliminating dampness to expel wind-toxin from the muscular exterior. The self-prescribed Qufeng Xiaodian Formula is a frequently selected modified formula. The lingering stage features latent wind-toxin in the collaterals and congealed accumulations in the kidney collaterals.Treatment aims to resolve wind-toxin,disperse stasis, and eliminate accumulation to remove densely accumulated wind-toxin.A modified Taohong Siwu Decoction is selected. The recovery stage involves deficient healthy qi with persistent wind assault and lingering toxins.Emphasis should be placed on cultivating the fundamental, strengthening the root, and nourishing yin,as well as supporting the healthy qi to dispel wind and expel toxins, while conditioning the body to prevent recurrence.A modified Guomin Decoction is selected.Clinical application emphasizes the combination of characteristic medicines, such as wind medicines to expel pathogens, insect medicines to identify collaterals, and vine medicines to unblock the meridians,to enhance the effects of identifying wind, resolving toxins, unblocking collaterals, and dispersing congealed accumulations. This study aims to systematically elaborate on the stage-based treatment strategy from the perspective of"wind-toxin damaging collaterals,"providing a theoretical basis and clinical practice reference for the traditional Chinese medicine diagnosis and treatment of pediatric IgA vasculitis.

Objective To identify potential quality risks of Xiaoer Huatan Zhike Granules(Pediatric Phlegm-Resolving and Cough-Relieving Granules)through National Drug Sampling and Inspection, evaluate their overall quality, and propose improvements to the quality standards. Methods Samples from 34 manufacturers were tested using official standards.Exploratory methods were developed to enhance risk analysis, including thin-layer chromatography(TLC)identification, content uniformity testing, quantitative assays, and screening for unauthorized additives(e.g.,colorants).Results All batches met the official standards(100.0%compliance rate). However, deficiencies were observed:most manufacturers lacked controls for ephedrine hydrochloride content uniformity and quantification, omitted key ingredient identifications(e.g.,herbal components),and exhibited insufficient TLC specificity(e.g.,TLC of Ipecac tincture showed only one spot instead of multiple alkaloid-specific spots).Exploratory studies revealed risks in total emetine/cephaeline content, morroniside A/platycodin D content, ephedrine hydrochloride uniformity, and unauthorized colorant addition. Conclusions The overall quality of this variety is good, but the enterprise should improve the quality control measures throughout the entire process and establish stricter internal control standards; The official standards, unchanged for approximately 30 years, fail to address current regulatory requirements. The statutory quality standards need to be revised and improved.Qualitative or quantitative indicators for all drug flavors in the prescription should be added, and the content uniformity inspection of ephedrine hydrochloride should be increased.

The main pathogenesis of chronic cough in children is the disorder of ascending and descending of qi movement caused by healthy qi deficiency and pathogenic qi retention. The deficiency of lung, spleen, and kidney is the root of the disease, and the retention of phlegm-fluid, food accumulation, and fire from constraint is the branch pathogenesis of the disease. In the treatment, we should reinforce and tonify healthy qi, dispel pathogen and regulate qi, with Yupingfeng Powder (玉屏风散) as the basic prescription. For lung qi deficiency syndrome, modified Yupingfeng Powder could be used for supplementing lung to consolidate the exterior; for lung and spleen qi deficiency syndrome, modified Yupingfeng Powder plus Shenling Baizhu Powder (参苓白术散) could be used for supplementing lung and fortifying the spleen, treating with both supplementation and transformation; for lung kidney qi deficiency syndrome, modified Yupingfeng Powder combined with Suzi Jiangqi Decoction (苏子降气汤) could be used for supplementing lung and replenishing kidneys, absorbing qi to the root. All the above prescriptions could combine the method of dispelling phlegm, promoting digestion and guiding out food stagnation, soothing the liver and draining fire to remove the solid pathogens, in order to treat the root and branch simultaneously, and the cough will stop if the ascending and descending of qi movement recover as usual.

Aim To investigate the role and mecha-nism of CXC chemokine receptor 3(CXCR3)in hepa-toblastoma(HB).Methods The expression of CX-CR3 was detected by immunohistochemical and West-ern blot in 16 cases of HB tissue and adjacent normal liver tissue. The HB cells(Huh-6 and HepT1)were transfected with Con-shRNA,CXCR3-shRNA1,and CXCR3-shRNA2,respectively,and then divided into the Con-shRNA group,CXCR3-shRNA1 group, and CXCR3-shRNA2 group.Cell proliferation was detected by CCK-8 assay and EdU staining.Cell migration and invasion were detected by scratch and Transwell as-says.The expressions of β-catenin,c-Myc,cyclin D1,MMP-7 and MMP-9 were detected by Western blot. The tumor formation and tumor volume in each group were assessed using nude mouse xenograft tumor model, while the expressions of MMP-9 and Ki67 in tumor tissue were examined by immunohistochemistry. Results The expression of CXCR3 was up-regulated in HB tissue(P<0.01).Compared to the Con-shR-NA group, the viability, proliferation, migration and invasion of Huh-6 and HepT1 cells in the CXCR3-shR-NA1 and CXCR3-shRNA2 groups were reduced(P<0.01),the expressions of the Wnt/β-catenin signaling pathway related proteins were attenuated(P<0.01),the tumor grew slowly and the volume was significantly reduced(P<0.01),and the expressions of MMP-9 and Ki67 in tumor tissue decreased(P<0.01).Con-clusions Downregulation of CXCR3 hinders the pro-liferation and migration of HB cells, potentially as-cribed to the attenuation of Wnt/β-catenin signaling regulation.
