1.Predictive value of serum levels of AnxA1,CLU and GAR for ASCVD

Objective:To explore the predictive value of serum levels of annexin A1(AnxA1),clusterin(CLU)and γ-glutamyltransferase/albumin ratio(GAR)for atherosclerotic cardiovascular disease(ASCVD).Methods:A total of 300 patients treated in Xi'an No.9 Hospital between October 2020 and March 2024 were selected.According to ASCVD diagnosis,patients were divided into ASCVD group(n=149)and no ASCVD group(n=151).The base-line data,serum levels of AnxA1,CLU and GAR were compared between two groups.The factors associated with ASCVD were analyzed by multivariate Logistic regression.Receiver operating characteristic(ROC)curve was used to analyze the predictive value of serum AnxA1,CLU,GAR and combined detection for ASCVD.Results:Com-pared to patients in the no ASCVD group,those in the ASCVD group had significantly higher proportions of family history of early onset coronary heart disease(CHD)(37.76%vs.5.48%),smoking(44.76%vs.23.97%),AnxA1[(1.98±0.39)μg/L vs.(1.05±0.27)μg/L],CLU[(71.61±9.88)mg/L vs.(30.53±6.52)mg/L]and GAR[(1.42±0.32)vs.(0.70±0.19)](P<0.001 all).Multivariate Logistic regression analysis showed that family history of early onset CHD(OR=3.665,95%CI 1.456~6.072),smoking(OR=2.023,95%CI 1.016~4.073),AnxA1(OR=3.378,95%CI 1.293~5.846),CLU(OR=4.382,95%CI 1.179~7.534)and GAR(OR=3.153,95%CI 2.032~4.787)were independent risk factors for ASCVD(P<0.001 all).ROC curve analysis in-dicated that the AUC of AnxA1,CLU,GAR alone and combined detection diagnosing ASCVD was 0.821(95%CI 0.766~0.871),0.859(95%CI 0.813~0.902),0.802(95%CI 0.751~0.862)and 0.945(95%CI 0.900~0.970)respectively,and the AUC of combined detection was significantly higher than those of single detection(Z=3.678,2.035,4.139,P<0.05 or<0.01).Conclusion:Serum levels of AnxA1,CLU and GAR significantly increase in ASCVD patients,and combined detection of above-mentioned indexes has good predictive efficacy for the occur-rence of ASCVD.
2.Current situation analysis and quality evaluation of guidelines and consensus in ovarian stimulation for in vitro fertilization

Objective:To systematically evaluate the current status and methodological quality of guideline and consensus in ovarian stimulation for in vitro fertilization (IVF), and to provide reference for the development of future guidelines and research. Methods:A systematic search was conducted in databases including PubMed, Embase, CNKI, Wanfang, Sinomed and relevant guideline websites from inceptions to October 2024. Data were extracted from the literature that met the inclusion criteria, and methodological quality was assessed using the Appraisal of Guidelines for Research and Evaluation Ⅱ (AGREE Ⅱ) tool. Descriptive analysis was performed with results presented in visual data charts.Results:A total of 23 studies were included, with 8 guidelines and 15 consensus. Totally 21 (91.3%) studies focused on the selection of ovarian stimulation protocols, with the majority concentrating on protocols for patients undergoing IVF treatment for the first cycle, and 12 (52.2%) studies focused on the selection of gonadotropin starting dosage. In terms of methodology, the average standardized scores in the 6 assessment domains of the AGREE Ⅱ tool were as follows: scope and purpose (85.1%), stakeholder involvement (66.6%), rigour of development (36.5%), clarity of presentation (70.1%), applicability (36.4%), and editorial independence (46.2%).Conclusion:Current recommendations in ovarian stimulation for IVF exhibit inconsistencies, and the methodological quality varies, which partially hinders the translation of evidence into clinical practice. Future research should further focus on the selection of ovarian stimulation protocols and gonadotropin starting dosage, promoting the update and refinement of guidelines and consensus to guide the precise treatment of ovarian stimulation.
3.Study on the correlation between vancomycin therapeutic drug monitoring and short-term and long-term prognosis in sepsis patients based on the MIMIC-Ⅳ database

Objective To explore the relationship between vancomycin therapeutic drug monitoring(TDM)and short-term and long-term prognosis in patients with sepsis.Methods Clinical data of sepsis patients treated in the intensive care unit were obtained from the MIMIC-Ⅳ database.Patients were divided into the TDM group and the non-TDM group based on the status of vancomycin TDM.A 1∶1 propensity score matching(PSM)was employed.The association between vancomycin TDM and the risk of 28 d and 90 d mortality of patients before and after PSM was analyzed using Kaplan-Meier survival curves and multivariate Cox regression models.Meanwhile,the relationship between vancomycin TDM and the risk of mortality within 28 d and 90 d was investigated.Results A total of 3,525 patients were included,with 2,474 in the TDM group and 1,051 in the non-TDM group.After PSM,848 patients were included in each group.The mortality rates at 28 d and 90 d in the TDM group were lower than those in the non-TDM group both before and after PSM(P<0.005).The Kaplan-Meier curve results showed that the survival rates at 28 d and 90 d in the TDM group were higher than those in the non-TDM group both before and after PSM(P<0.001).Multivariate COX regression analysis showed that vancomycin TDM was an independent protective factor against mortality risk at 28 d[HR=0.74,95%CI(0.65,0.84),P<0.001]and 90 d[HR=0.79,95%CI(0.70,0.89),P<0.001]in sepsis patients.Conclusion Vancomycin TDM reduces the short-term and long-term mortality risk in sepsis patients and is worthy of clinical promotion and application.

Objective To understand the current situation of healthcare-associated infection(HAI)in China,pro-vide data support and decision-making basis for formulating scientific and effective strategies for HAI prevention and control.Methods A nationwide cross-sectional survey on HAI was conducted among various types and levels of medical institutions in China according to a unified protocol of bedside surveys and case investigations.Results In 2024,a total of 5 736 medical institutions and 2 751 765 patients were surveyed.Among them,34 889 HAI cases were identified,with a prevalence rate of 1.27%.The number of HAI episodes was 38 032,and case prevalence rate was 1.38%.The prevalence rate of HAI in medical institutions in different regions of China ranged from 0.66%to 2.35%.Among medical institutions of different scales,those with a bed capacity of ≥900 had the high-est incidence of HAI,reaching 1.65%.The most common infection site was the lower respiratory tract(44.66%),followed by the urinary tract(12.94%),surgical site(9.32%),upper respiratory tract(7.02%),and bloodstream infection(5.78%).The top 3 departments with the highest HAI rates were the general intensive care unit(10.02%),department of neurosurgery(5.51%),and department(group)of hematology(5.34%).A total of 23 238 strains of HAI pathogens were detected,with 10 714 strains(46.10%)from lower respiratory tract speci-mens.The top 5 detected strains were Klebsiella pneumoniae(14.76%),Pseudomonas aeruginosa(13.33%),Escherichia coli(12.79%),Acinetobacter baumannii(9.23%),and Staphylococcus aureus(7.88%).231 944 pa-tients underwent class Ⅰ incision surgery were monitored,with 1 647 cases experienced surgical site infection,and the prevalence rate of surgical site infection was 0.71%.The number of patients who should undergo pathogen de-tection(patients receiving therapeutic and therapeutic combined prophylactic antimicrobial agents)was 715 179,while the actual number was 480 492,with a pathogen detection rate of 67.18%.425 225 patients received patho-genic detection before treatment,with a detection rate of 59.46%.Conclusion The overall HAI prevalence in Chi-na is lower,showing disparities among medical institutions of different regions and scales.Therefore,precise imple-mentation of measures is necessary for HAI prevention and control,with a focus on high-risk institutions and high-risk departments,key areas,and critical procedures.All levels of medical institutions should continuously reduce the incidence of HAI by strengthening monitoring,standardizing the use of antimicrobial agents,and reinforcing basic HAI prevention and control measures.
6.Buzhong Yiqitang Regulates Endoplasmic Reticulum Stress to Attenuate Cisplatin Resistance in Non-small Cell Lung Cancer via Nrf2/ROS Pathway

ObjectiveTo investigate the mechanism of Buzhong Yiqitang in attenuating cisplatin resistance in non-small cell lung cancer by observing the effects of Buzhong Yiqitang on endoplasmic reticulum stress-related molecules in human lung adenocarcinoma cells (A549) and cisplatin-resistant cells in human lung adenocarcinoma cells (A549/DDP) via the nuclear factor E2-related factor 2(Nrf2)/reactive oxygen species(ROS) pathway. MethodsThe serum containing Buzhong Yiqitang was prepared and A549 cells and A549/DDP cells were cultured. The cells were randomized into groups A (A549 cells+blank serum), B (A549 cells+20 mg·L-1 cisplatin+blank serum), C (A549 cells+20 mg·L-1 cisplatin+10% Buzhong Yiqitang-containing serum), D (A549/DDP cells+blank serum), E (A549/DDP cells+20 mg·L-1 cisplatin+blank serum), and F (A549/DDP cells+20 mg·L-1 cisplatin+10% Buzhong Yiqitang-containing serum). The cell counting kit-8 (CCK-8) method was used to detect the half maximal inhibitory concentration (IC50) of cisplatin. The protein levels of Nrf2 and p-Nrf2 were determined by Western blotting. The DCFH-DA fluorescent probe was used to measure the content of reactive oxygen species (ROS) in each group. The protein levels of glucose-regulated protein 78 (GRP78), activated transcription factor 6 (ATF6), and C/EBP-homologous protein (CHOP) were determined by Western blot. ResultsCompared with group B, group C showed a reduction in IC50 of cisplatin (P<0.05), which held true in group E compared with group F (P<0.05). Moreover, the IC50 of cisplatin to A549/DDP cells was higher than that to A549 cells before and after Buzhong Yiqitang intervention (P<0.05). Compared with group A, group B showed up-regulated protein levels of Nrf2 and p-Nrf2 (P<0.05). Compared with group B, group C showed down-regulated protein levels of Nrf2 and p-Nrf2 (P<0.05). Compared with group D, group E showed up-regulated protein levels of Nrf2 and p-Nrf2 (P<0.05), which, however, were significantly down-regulated in group F (P<0.05). The ROS content and the protein levels of GRP78, ATF6, and CHOP followed a descending trend of group C > group B > group A in A549 cells and group F > group E > group D in A549/DDP cells (P<0.05). Moreover, the ROS content and the protein levels of GRP78, ATF6, and CHOP in A549 cells were higher than those in A549/DDP cells before and after Buzhong Yiqitang intervention (P<0.05). ConclusionBuzhong Yiqitang may regulate endoplasmic reticulum stress via the Nrf2/ROS pathway to attenuate cisplatin resistance in non-small cell lung cancer.

8.Symptom burden among survivors with oropharyngeal cancer after radiotherapy

Objective:To investigate the prevalence and severity of symptom burden among long-term survivors of oropharyngeal cancer after radiotherapy, to identify core symptom clusters, and to explore their correlation with quality of life.Methods:A previous retrospective study was conducted by the Cancer Hospital, Chinese Academy of Medical Sciences on patients with oropharyngeal cancer who underwent radiotherapy between January 2010 and December 2020. Patients who were still alive as of December 2023 were further followed and analyzed. From December 2023 to August 2024, symptom burden and quality of life were assessed using the Chinese version of the MD Anderson Symptom Inventory–Head and Neck Module (MDASI-HN) and the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ). Exploratory factor analysis (principal component analysis with Promax rotation) were used to identify symptom clusters. Spearman correlation analysis was performed to explore the relationship between total symptom cluster scores and standardized domain scores of quality of life. Multivariate linear regression analysis was further employed to determine the relationship between identified symptom clusters and overall quality of life.Results:A total of 273 patients were included, with a median follow-up duration of 6.2 years (range: 3.5-14.5 years) and a median age of 61 years (range: 27-88 years) at follow-up. The top 5 incidence rates of symptom reported by patients were mucus problems in the mouth or throat (147 cases, 53.8%), dental or gum issues (143 cases, 52.4%), xerostomia (140 cases, 51.3%), difficulty swallowing or chewing (95 cases, 34.8%), and taste disturbance (79 cases, 28.9%). Among them, xerostomia was the most serious symptom. The most frequently reported interference was impact on work (including household chores) (55 cases, 20.1%). Exploratory factor analysis identified 3 symptom clusters: fatigue-nausea cluster, eating-voice cluster, and xerostomia-sleep cluster, all of which were significantly correlated with lower overall quality of life of patients (all P<0.001). Conclusion:Long-term survivors of oropharyngeal cancer after radiotherapy experience substantial symptom burden. The fatigue-nausea, eating-voice, and xerostomia-sleep clusters are the core symptom clusters impacting quality of life.

10.Qianggu Kangshu Formula attenuates osteoclast differentiation in rheumatoid arthritis by inhibiting the HIF-1α/BNIP3 autophagy signaling pathway.

OBJECTIVES:
To investigate the effect of Qianggu Kangshu Formula (QGKSF) for alleviating osteoclast differentiation in rheumatoid arthritis and the underlying mechanism.
METHODS:
RAW264.7 cells cultured under hypoxic conditions were treated with RANKL to induce osteoclast differentiation and incubated with normal rat serum or sera from rats medicated with methotrexate (MTX) or QGKSF at low and high doses. Cell viability, TRAP-positive multinucleated cells and F-actin ring formation in the treated cells were assessed with CCK-8 assay, TRAP staining and Phalloidin staining, respectively. Autophagy and autophagosomes in the cells were observed with MDC staining and transmission electron microscopy. ELISA was used to measure IL-6 and TNF-α levels in the culture supernatant, and the expressions of HIF-1α, BNIP3, Bcl-2, Beclin1, LC3-I, LC3-II, P62 and TRAP mRNAs and proteins were analyzed using RT-qPCR and Western blotting.
RESULTS:
In hypoxia- and RANKL-induced RAW264.7 cells treated with normal rat serum, significant increments of TRAP-positive cells and F-actin ring formation were observed with an enhanced autophagic fluorescence intensity and increased autophagosomes. Treatment of the induced cells with rat sera medicated with MTX and low- and high-dose QGKSF obviously reduced the TRAP-positive cells, F-actin rings and autophagosomes as well as the autophagic fluorescence intensity. RANKL treatment significantly increased IL-6 and TNF-α levels in RAW264.7 cells, which were obviously decreased by treatment with MTX- and QGKSF-medicated sera. RANKL also significantly increased the mRNA and protein expression levels of HIF-1α, BNIP3, Bcl-2, Beclin1, LC3 and TRAP and lowered P62 expressions, and these changes were effectively reversed by treatment with MTX- and QGKSF-medicated sera.
CONCLUSIONS
QGKSF attenuates RANKL-induced osteoclast differentiation in hypoxic RAW264.7 cells by inhibiting the HIF-1α/BNIP3 autophagy signaling pathway, suggesting its potential for treatment of bone destruction in rheumatoid arthritis.
