1.Traditional Chinese Medicine Treats Esophageal Cancer via PI3K/Akt Signaling Pathway: A Review

Esophageal cancer (EC) is a highly prevalent malignant tumor in China. The phosphatidylinositol 3-kinase (PI3K)/protein kinase B (Akt) signaling pathway, as one of the key oncogenic pathways, can promote the cell cycle progression, proliferation, migration, and invasion, induce chemoresistance, and inhibit apoptosis and autophagy of EC cells. Traditional Chinese medicine (TCM), with the advantages of targeting multiple points with multiple components to delay cancer progression, can target the PI3K/Akt signaling pathway for EC treatment. This article preliminarily discusses the molecular mechanism and role of the PI3K/Akt signaling pathway in EC and elaborates on the specific targets and efficacy of TCM in treating EC through intervention in the PI3K/Akt signaling pathway in the past five years. TCM materials and extracts inhibiting the PI3K/Akt signaling pathway in EC include Borneolum, spore powder of Ganoderma lucidum without spore coat, extract of Celastrus orbiculatus, root extract of Taraxacum, and Bruceae Fructus oil emulsion. TCM active ingredients exerting the effect include flavonoids, terpenoids, saponins, phenols, polysaccharides, alkaloids, and other compounds. TCM compound prescriptions with such effect include Qige San, Huqi San, Xuanfu Daizhetang, Tongyoutang and its decomposed prescriptions, Liujunzi Tang, and Xishenzhi Formula. In addition, TCM injections such as Compound Kushen Injection and Kang'ai injection also inhibit the PI3K/Akt signaling pathway in EC. This paper summarizes the role of the PI3K/Akt signaling pathway in EC and the TCM interventions, aiming to provide reference for the research and clinical application of new drugs for EC.
2.Clinical study of Yinqi Guiyuan Acupuncture Combined with Repetitive Transcranial Magnetic Stimulation on the Treatment of Post-Stroke Fatigue:A Prospective Randomized Controlled Trial

Objective:To observe the clinical efficacy of Yinqi Guiyuan acupuncture combined with repetitive transcranial magnetic stimulation in the treatment of post-stroke fatigue(PSF).Methods:A prospective study was conducted,91 patients with PSF admitted to our hospital from May 2023 to October 2024 were included as the research subjects,and they were divided into control group(received conventional treatment combined with repetitive transcranial magnetic stimulation,n=45)and study group(received Yinqi Guiyuan acupuncture on the basis of the control group,n=46)according to random number table method.The clinical efficacy,inflammatory factors[C-reactive protein(CRP),interleukin-1[3(IL-1β),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)]and related scale score[pittsburgh sleep quality index(PSQI),Fugl-Meyer motor function assessment(FMA),fatigue severity scale(FSS),stroke specific quality of life scale(SS-QOL)]were compared between the two groups.Result:The total clinical effective rate of the study group were higher than that of the control group(P<0.05).The levels of serum CRP,IL-1β,IL-6 and TNF-α after treatment in the study group were lower than those in the control group(P<0.05).The PSQI and FSS score after treatment in the study group were lower than those in the control group,and the FMA and SS-QOL score were higher than those of the control group(P<0.05).Conclusion:The synergistic intervention of Yinqi Guiyuan acupuncture and repetitive transcranial magnetic stimulation has multiple therapeutic benefits for PSF patients,which can not only significantly relieve clinical symptoms(including sleep disorders,fatigue levels and motor function disorders),but also improve the overall quality of life,and its mechanism of action may be related to the regulation of inflammatory factor expression.
3.Practical research on nursing coordination training for rapid sequential intubation in children based on LSPPDM framework

Objective To investigate the practical effects of pediatric rapid sequence intubation(RSI)nursing coordination training based on the LSPPDM(learn,see,practice,prove,do,maintain)framework in order to provide evidence for optimizing pediatric RSI nursing training programs.Methods Nurses from the intensive care unit(ICU)of Children's Hospital,Fudan University during Feb 2023 and Jan 2024 were divided into the experimental group(n=35)and the control group(n=35)by block randomization.The experimental group received LSPPDM framework-based training,while the control group underwent conventional training with theoretical lectures and procedural demonstrations.Outcomes included training satisfaction,theoretical knowledge and procedural skill assessment scores,team collaboration compliance and RSI procedure time were compared between the two groups.Results The experimental group demonstrated significantly higher training satisfaction(123.80±2.04 vs.117.26±9.82,P<0.05),superior post-training theoretical knowledge and procedural skills(P<0.05),enhanced team collaboration compliance(P<0.05),and shorter RSI completion time(P<0.05)compared with the control group.Conclusion Pediatric RSI nursing coordination training based on the LSPPDM framework can effectively increase training satisfaction,promote theoretical and procedural skills and reduce completion time in nurses.
4.Analysis of risk factors and establishment of a prediction model for endometrial cancer in postmenopausal bleeding

Objective To establish a method for predicting the risk of endometrial cancer(EC)and endometrial atypical hyperplasia(AH)in women with postmenopausal bleeding(PMB)by collecting clinical data on routine medical history.Methods The clinical data of a total of 408 PMB patients admitted to Fuxing Hospital,Capital Medical University were consecutively collected in this retrospective study from December 2013 to December 2023.According to the results of endometrial pathology,patients were divided into case group and control group.EC and AH were included in the malignant group(case group)and the other endometrial pathologies were included in the non-malignant group(control group).Clinical data,including clinical history,high risk factors,and common gynecological ultrasound measurement indicators,were collected and studied by univariate and multivariate Logistic regression analysis.Results The mean age of 408 patients was(60.4±7.8)years.A total of 74 cases(18.1%)were in case group and 334 cases(81.9%)were in control group.Based on Logistic regression analysis,the best predictors of endometrial malignant lesions were selected to create a"LRDNT"(light bleeding,recurrent bleeding,diabetes,non-uniform echogenicity & thickness)model.LRDNT scores range from 0 to 22.The score of LRDNT ≥15 has the largest Yoden index,and the sensitivity to predict endometrial malignant lesions is 79.73%,the specificity is 80.84%,and the prediction accuracy is 80.64%.Conclusions The risk prediction model LRDNT,which combines clinical information and common gynecological ultrasound measurement indicators of PMB patients,can help clinicians classify patients at high and low risk of endometrial malignant lesions,and optimize the strategy of diagnosis and treatment.

China has become the country with the highest global burden of heart failure(HF).Despite the widespread use of prognostic-improving medications today,the mortality rate of HF remains high,reaching 13.7%at one year-particularly among patients with heart failure with reduced ejection fraction(HFrEF).HF interventional device therapy(structural intervention)targets the structural factors underlying HF,including atrial pressure,ventricular remodeling,and valvular intervention.It leverages the heart's intrinsic physiological properties and pathological progression mechanisms to deliver treatments through interventions without external active forces,achieving anatomical or functional repair.This field has emerged as a rapidly growing area and plays an increasingly critical role in HF management.This article provides a comprehensive review and summary of the latest advancements in HF and cardiomyopathy interventional therapy over the past year.It covers various novel technologies and products currently in the research phase,aiming to provide an in-depth analysis of the current status and future directions of HF interventional therapy,and further advance the development of this discipline.
6.Rational use of medical consumables based on difference analysis of consumption proportion of single diseases

Objective To analyze the the differences in the use of medical consumables of single diseases,so as to provide ideas for the supervision of the rational use of medical consumables.Methods The case information of the patients discharged from some hospital in Hunan Province who underwent artificial femoral head replacement for femoral neck fracture,transurethral plasma electrosurgery for prostatic hyperplasia and percutaneous nephrolithotripsy for renal calculi from January 1,2022 to December 31,2023 were selected.Based on the results of the normality test,difference analyses were carried out over the consumption ratios of inpatients undergoing different procedures at different time periods and the consumption ratios treated by different physicians,and the percentile method was used to determine the recommended range for the consumption ratio based on the difference analysis results.SPSS 26.0 software was used for statistical analysis.Results The inpatients undergoing artificial femoral head replacement for femoral neck fracture at different periods had no significant difference in the consumption ratio(P>0.05);the inpatients going through transurethral plasma electrosurgery for prostatic hyperplasia had the consumption ratio in 2023 decreased by 5.22%when compared with that in 2022,with the difference being statistically significant(P<0.05);the inpatients undergoing percutaneous nephrolithotripsy for renal calculi had the consumption ratio at 2023 increased by 6.49%when compared with that at 2022,with the difference being statistically significant(P<0.05).There were no strong correlations between physician and inpatient consumption ratios for the three single-diseases(P<0.05),while the consumption ratios by different physicians for each single disease had differences.There were respectively 50%,40%and 50%of the physicians implementing artificial femoral head replacement for femoral neck fracture,transurethral plasma electrosurgery for prostatic hyperplasia and percutaneous nephrolithotripsy for renal calculi had the average consumption ratios higher than the average annual consumption ratios for the single diseases.The recommended ranges of the consumption ratio was[51.16%,63.89%]for artificial femoral head replacement for femoral neck fracture,[8.76%,10.77%]for transurethral plasma electrosurgery for prostatic hyperplasia and[26.40%,32.80%]for percutaneous nephrolithotripsy for renal calculi.Conclusion Under the premise of ensuring the quality of medical care,medical consumables can be scientifically and rationally supervised by setting a reasonable range,seizing the"vital few"and implementing joint management and control to reduce the consumption ratio.[Chinese Medical Equipment Journal,2025,46(7):87-91]
7.Construction of backpack-based field emergency medical rescue equipment system

Objective To establish a backpack-based field emergency medical rescue equipment system to enhance emergency rescue efficiency.Methods A backpack-based field emergency medical rescue equipment system was preliminarily constructed with the concept of medical treatment in echelons and prolonged field care(PFC)and the method of capability-based equipment need analysis;with the Delphi method 15 experts from relevant fields were invited to execute two rounds of questionnaire consultations,and the final equipment system was determined after the equipment varieties and quantities were revised based on the expert opinions.Results The response rate for the two rounds of expert questionnaire surveys was 100%.The experts'authority coefficients were 0.8734 and 0.87,respectively;Kendall coefficients were 0.232 and 0.345(both P<0.001),indicating statistically significant results.Ultimately,a backpack-based field emer-gency medical rescue equipment system comprising 15 backpacks and 158 individual pieces of equipment was established.Conclusion The established system demonstrates a certain degree of specificity and practicability,providing references for the equipment allocation and utilization of emergency medical rescue teams.[Chinese Medical Equipment Journal,2025,46(10):17-22]
8.Subchronic systemic toxicity of disposable plasma virus-inactivated blood transfusion sets

Objective To evaluate the subchronic systemic toxicity of disposable plasma virus-inactivated blood transfusion sets using hydroxyethyl starch(HES)130/0.4 sodium chloride injection as an extraction medium.Methods Firstly,40 Sprague Dawley(SD)rats including 20 male and 20 female ones were seleted and randomly enrolled into a sample group and a control group by sex,with 20 ones in each group.Secondly,instead of plasma HES 130/0.4 sodium chloride injection was used to leach disposable plasma virus-inactivated blood transfusion sets to prepare the test solution by simulating clinical application such as lighting,adsorption and filtration and storage.Finally,the test solution and HES 130/0.4 sodium chloride injection were injected into the tail vein of the SD rats at a dose of 20 mL/kg for 28 d in the sample group and in the control group respectively,and the subchronic systemic toxicity of disposable plasma virus-inactivated blood transfusion sets and the feasibility of using HES 130/0.4 sodium chloride injection as the extraction medium to assess their subchronic systemic toxicity were evaluated with clinical observation,body mass monitoring,clinical pathology examination,gross necropsy and histopathology examination.Results The sample group and control group had no significant differences in mortality rates,clinical observation results,body mass,gross necropsy results,hematological and coagulation examination results and organ weight(all P>0.05);blood biochemical examinations showed the male rats in the sample group had the cholesterol(CHO)values higher while the creatinine(CR)values lower than those in the control group,with the differences being statistically significant(both P<0.05)and the two indexes within the range of the laboratory's historical reference data,and other blood biochemical indexes were not significantly different(all P>0.05);the sample group had the spleen weight-to-body mass ratios of the female rates lower significantly than those in the control group(P<0.05),and the ratios of other organ weight to body mass had significant differences(all P>0.05);histopathology examination showed slight pathological changes in liver,spleen and kidney of female rats and in spleen and kidney of male rats in the sample group,and the female and male rats in the control group had similar pathological changes found in the sample group,which might be caused by HES metabolites.Conclusion Disposable plasma virus-inactivated blood transfusion sets prove to have no significant subchronic systemic toxicity,and its feasible to use HES 130/0.4 sodium chloride injection as the extraction medium to evaluate the subchronic systemic toxicity of disposable plasma virus-inactivated blood transfusion sets.[Chinese Medical Equipment Journal,2025,46(10):29-35]

Over 40%of critically ill patients will develop coagulopathy.Once critically ill patients are complicated with coagulopathy,the incidence of bleeding and mortality can increase by more than 4 times.Early identification of coagulopathy and accurate evaluation of coagulation function are essential for correcting coagulopathy as soon as possible.Therefore,Chinese Society of Thrombosis,Hemostasis and Critical Care,Chinese Medicine Education Association,together with Chinese People's Liberation Army Professional Committee of Critical Care Medicine updated the"Chinese expert consensus on standardized assessment of severe coagulopathy(2025 Edition)"on the basis of the"Consensus of Chinese experts on standardized evaluation of coagulation dysfunction in severe patients"formulated in 2022.This consensus includes four parts:classification and typing,etiology and mechanism,assessment methods,and diagnostic criteria of severe coagulopathy,with a total of 14 recommendations,aiming to provide corresponding guidance for clinical practice.
10.Shengmai Yin alleviates myocardial ischemia/reperfusion injury via inhibiting Calpains expression

Aim To investigate the protective effect of Shengmai Yin on myocardial ischemia/reperfusion in-jury(MI/RI)in vitro and in vivo and to unravel the underlying mechanism.Methods SD rats were divid-ed into the sham group,model group,and Shengmai Yin group(SM).Rat MI/RI model was established.Cardiac function,infarct area,pathological changes,cardiomyocyte apoptosis,macrophage infiltration,and serum cTnT and CK-MB levels were measured.The mRNA and protein expressions of Calpain-1 and Cal-pain-2 were assessed.The hypoxia/reoxygenation(H/R)model was constructed in H9c2 cells.The active ingredients of Shengmai Yin were screened using net-work pharmacology and verified by CCK-8.In the car-diomyocytes H/R model,Fluo-4 AM staining was used to detect the changes of Ca2+levels.Results Com-pared with model group,LVEF and LVFS of Shengmai Yin-treated rats increased,myocardial infarction area was reduced,while myocardial tissue injury was allevi-ated.Myocardial apoptosis rate and the number of macrophages were reduced.Similarly,cTnT and CK-MB levels decreased.In addition,the expression lev-els of Calpain-1 and Calpain-2 mRNA and protein de-creased in the SM treatment group.Under the H/R model,all the active ingredients of Shengmai decoction had protective effects on cardiomyocytes,and the treat-ment could reduce the level of Ca2+in cardiomyocytes.Conclusions Shengmai Yin has protective effects on MI/RI in rats.This effect may be related to the de-crease in Ca2+levels,as well as Calpain-1 and Calap-in-2 mRNA and protein expression.
