Aim To investigate the potential role and related mechanisms of protein phosphatase 2Cm(PP2Cm)overexpression in atorvastatin-induced insu-lin resistance. Methods Male C57BL/6J mice,fibro-blast growth factor 21 knockout(FGF21-KO)mice,and wildtype(WT)mice were raised for 12 weeks to construct models.Groups included atorvastatin,con-trol,atorvastatin+PP2Cm overexpression(OE),FGF21-KO+vehicle,FGF21-KO+PP2Cm OE,WT+vehicle,WT+PP2Cm OE.Body weight, fasting blood glucose levels, fasting insulin levels, and intraperitoneal glucose tolerance tests(IPGTT)were measured in 4,8 and 12 weeks. The concentrations of branched-chain a-mino acids(BCAA)in cells, tissues and serum,as well as the mRNA and protein expression of BCAA cat-abolic enzymes, were determined by qRT-PCR,Western blot and ELISA after atorvastatin treatment.Further-more,the effects of PP2Cm overexpression on these in-dicators were explored, and the FGF21 was verified in vivo and in vitro. Results Atorvastatin induced insu-lin resistance in mice, altered insulin, glucose tolerance and increased BCAA levels.PP2Cm overexpression mitigated these changes. In the Atorvastatin+PP2Cm OE group,FGF21 mRNA,protein and concentration were all significantly upregulated.Regardless of PP2Cm overexpression, the knockout of FGF21 signifi-cantly increased BCAA expression levels, both fasting insulin and blood glucose levels were significantly high-er than those in WT group. Conclusions FGF21 may be an important regulator of PP2Cm involved in atorv-astatin-induced insulin resistance.PP2Cm overexpres-sion alleviates the effects of atorvastatin-induced insulin resistance by regulating FGF21.

Objective To evaluate the application of high-resolution magnetic resonance vessel wall imaging(HRMR-VWI)in identifying high-risk features of intracranial atherosclerotic plaques, and to analyze the correlation between plaque characteristics and stroke recurrence under varying degrees of stenosis. Methods The data from 368 patients with intracranial atherosclerotic stenosis(ICAS)across two centers were retrospectively analyzed.Based on the degree of stenosis, all patients were categorized into mild-to-moderate stenosis group(luminal stenosis<70%,n=155)and severe stenosis group(luminal stenosis≥70%,n=213).HRMR-VWI images and clinical information of the patients were collected and analyzed, and the culprit plaques were quantitatively analyzed.Univariate and multivariate logistic regression analyses were employed to identify the risk factors for stroke recurrence, and the predictive performance was evaluated using the area under the curve(AUC)of the receiver operating characteristic(ROC)curve. Results Higher normalized wall index(NWI)[odds ratio(OR)=1.082,95%confidence interval(CI)1.050-1.118,P<0.05]and the presence of intraplaque hemorrhage(IPH)(OR=1.843,95%CI 1.120-3.036,P<0.05)were risk factors for stroke recurrence in all patients. And these two factors were also significant in the mild-to-moderate stenosis group(NWI:OR=1.088,95%CI 1.009-1.186,P<0.05;IPH:OR=4.049,95%CI 1.227-16.065,P<0.05).A predictive model for stroke recurrence was constructed using the combination of IPH and NWI,with the best performance in the mild-to-moderate stenosis group(AUC=0.813,95%CI 0.723-0.906).Conclusion In patients with luminal stenosis<70%,the increase of NWI and the presence of IPH have been validated as significant and effective indicators for predicting stroke recurrence, demonstrating notable predictive performance. In contrast, among patients with luminal stenosis≥70%,the utility of plaque characteristics in predicting stroke recurrence is relatively lower, indicating that the correlation between plaque characteristics and stroke recurrence varies across different degrees of stenosis.

Respiratory viral infections such as influenza and respiratory syncytial virus infections continue to rapidly increase in patients worldwide.Host immune responses to respiratory viruses play a key role in the pathogenesis and clinical manifestations of the disease.Respiratory viruses not only activate antiviral immune responses, but also may lead to uncontrolled inflammatory re-sponses,characterized by significant release of pro-inflammatory cytokines in severely infected patients, resulting in lymphopenia, lymphocyte dysfunction, and abnormalities in immune cells such as neutrophils and macrophages. These respiratory virus-induced im-mune abnormalities may lead to microbial infection, septic shock, and severe multiorgan dysfunction. Therefore, clarifying the immu-nopathogenic mechanisms of patients with respiratory viral infections can guide clinical treatment and patient prognosis;in addition, rational regulation of the immune response of respiratory viruses in the host, including enhancing antiviral immunity while suppressing systemic inflammation, may be the key to successful treatment. This review mainly discusses the immunomodulation and related clini-cal treatment strategies for respiratory viral infections to help develop new therapeutic strategies for respiratory viral infections and pa-tient prognosis.
