Objective To compare the performance and efficacy of prognostic models constructed by different machine learning algorithms in predicting the survival period of lung transplantation (LTx) recipients. Methods Data from 483 recipients who underwent LTx were retrospectively collected. All recipients were divided into a training set and a validation set at a ratio of 7:3. The 24 collected variables were screened based on variable importance (VIMP). Prognostic models were constructed using random survival forest (RSF) and extreme gradient boosting tree (XGBoost). The performance of the models was evaluated using the integrated area under the curve (iAUC) and time-dependent area under the curve (tAUC). Results There were no significant statistical differences in the variables between the training set and the validation set. The top 15 variables ranked by VIMP were used for modeling and the length of stay in the intensive care unit (ICU) was determined as the most important factor. Compared with the XGBoost model, the RSF model demonstrated better performance in predicting the survival period of recipients (iAUC 0.773 vs. 0.723). The RSF model also showed better performance in predicting the 6-month survival period (tAUC 6 months 0.884 vs. 0.809, P = 0.009) and 1-year survival period (tAUC 1 year 0.896 vs. 0.825, P = 0.013) of recipients. Based on the prediction cut-off values of the two algorithms, LTx recipients were divided into high-risk and low-risk groups. The survival analysis results of both models showed that the survival rate of recipients in the high-risk group was significantly lower than that in the low-risk group (P<0.001). Conclusions Compared with XGBoost, the machine learning prognostic model developed based on the RSF algorithm may preferably predict the survival period of LTx recipients.

Objective To comparatively evaluate the diagnostic value of metagenomic next-generation sequencing(mNGS)versus conventional microbial culture in spinal infections. Methods A cross-section design was conducted on 82 consecutive patients with suspected spinal infections treated between February 2022 and January 2024 at Jiangbei Branch of First Affiliated Hospital of Army Medical University(Third Military Medical University).Microbiological culture, histopathological examination, and mNGS results from infected specimens were analyzed.Clinical diagnosis, primarily based on clinical manifestations, laboratory tests and radiologic features combined with medical history, was defined as the gold standard, and then the diagnostic performance, including sensitivity and specificity, were compared between mNGS and microbial culture. Results Among the 82 patients, definitive microbiological evidence was identified in 70 cases, and mNGS demonstrated a significantly higher detection rate than microbial culture(64 vs 36 cases,78.05%vs 43.9%,P<0.05).mNGS also obtained obviously higher sensitivity, accuracy, and negative predictive value(NPV),and notably lower positive predictive value(PPV)when compared to conventional microbial culture(all P<0.05).When stratified by infection type,mNGS obtained significantly higher sensitivity and accuracy compared to microbial culture in tuberculous spinal infections(P<0.05).For non-tuberculous spinal infections,mNGS also showed superior sensitivity to microbial culture(P<0.05).Conclusion In patients with spinal infections,mNGS demonstrates a significantly higher pathogen detection rate than conventional microbial culture. This technique can provide early and broad-spectrum pathogenic microbiological evidence for spinal infection.
4.Research progress on the role of zinc finger proteins in the development of oral cancer
Journal of Prevention and Treatment for Stomatological Diseases 2025;33(5):425-432
Oral cancer is one of the most common malignancies in the head and neck regions. few patients benefit from current clinical therapy. Zinc finger proteins (ZNFs) are one of the largest transcription factor family proteins in the human genome. ZNFs bind to DNA, RNA, and proteins through their unique three-dimensional structure created by zinc ions to regulate gene transcription, RNA packaging, and protein folding. In recent years, the number of studies focused on the functional mechanism of ZNFs in regulating the progression of oral cancer has been increasing, with focuses on: ① ZNF677, ZNF460, ZNF154, ZNF132, ZNF281, Kaiso, and ZNF582, which regulate the invasion and metastasis of tumor cells; ② ZNF750 and PEST-containing nuclear protein (PCNP), which regulate the cell cycle; ③ ZNFs, which are involved in forming the tumor immune microenvironment, such as ZNF71 and myeloid zinc finger 1 (MZF1). For example, methylation modification modulates the reduction of ZNF677 in oral cancer and reduces the proliferation, migration, and invasion of oral cancer cells by inhibiting the protein kinase B/forkhead box O3a (AKT/FOXO3a) pathway; and ZNF460 promotes the proliferation, migration, and invasion of oral cancer cells by regulating microRNA-320a/alpha thalassemia/mental retardation, X-linked (ATRX) axis. In addition, ZNF750 inhibits the growth and metastasis of oral cancer by suppressing cell cycle transcription factor activity. Further, ZNF71 promotes the progression of oral cancer by reducing the infiltration of tumor immune cells. In this review, we will summarize the molecular mechanism, regulatory meshwork, and pro-tumor and anti-tumor roles of ZNFs in the pathogenesis of oral cancer. Our study may provide a new strategy for the diagnosis and treatment of oral cancer.

Objective:To evaluate the clinical efficacy of endoscopic treatment of superficial non-ampullary duodenal adenoma. Methods:A retrospective analysis was performed on the clinical data and follow-up information of patients diagnosed with superficial duodenal non-ampullary adenomas via preoperative endoscopy and treated endoscopically at Peking University First Hospital between January 2013 and January 2024. The overall en bloc resection rate, complete resection rate of the lesion, perioperative complications, and recurrence rates were evaluated. Patients were categorized into three groups based on their treatment modality: endoscopic mucosal resection (EMR)( n=46), endoscopic submucosal dissection (ESD)( n=16), and modified ESD (ESD with snare, ESD-S)( n=24). Comparative analyses were conducted to evaluate operative time, en bloc resection rate, and complete resection rate among the three groups. Results:Among 86 patients, the overall en bloc and complete resection rates were 87.2% (75/86) and 86.0% (74/86), respectively. No case of delayed bleeding was observed during the perioperative period. Intraoperative perforation occurred in two patients, both of whom improved following conservative management. Delayed perforation was noted in four patients, and three of them were successfully managed with surgical intervention, while one case was resolved after conservative treatment. During the follow-up period, local recurrence was identified in two patients. Following re-treatment with endoscopy and continuous surveillance, no further recurrence was observed. The operative times for the EMR group, ESD-S group, and ESD group were 4 (1-36) minutes, 25 (5-190) minutes, and 46 (5-150) minutes, respectively. Significant differences were observed in operative times among the three groups ( Hc=49.892, P<0.001). The en bloc resection rates for the EMR, ESD-S, and ESD groups were 80.4% (37/46), 91.7% (22/24), and 100.0% (16/16), respectively. The complete resection rates were 80.4% (37/46), 91.7% (22/24), and 93.8% (15/16) for the respective groups. Conclusion:Endoscopic treatment demonstrates favorable efficacy and safety for superficial non-ampullary duodenal adenoma. In addition to traditional EMR and ESD, ESD-S is also an effective procedure for endoscopic treatment of non-ampullary duodenal adenoma.

7.Research progress of ultrasonic measurement of optic nerve sheath diameter to assess intracranial pressure in children
Chinese Pediatric Emergency Medicine 2025;32(7):530-534
Children with high intracranial pressure can occur in a variety of neurological diseases,is an important cause of secondary brain injury, the disease has a high time sensitivity, and is easy to miss the best treatment time.Invasive monitoring of intracranial pressure has many complications and contraindications, which has promoted the clinical application of noninvasive monitoring of intracranial pressure. The optic nerve sheath has a special anatomical structure, and it is surrounded by cerebrospinal fluid, which is connected with the ventricular system of the brain. When intracranial pressure increases, the optic nerve sheath will be thickened.Ultrasound measurement of the optic nerve sheath diameter can provide non-invasive,real-time and repeated assessment of intracranial pressure. The diameter of optic nerve sheath in children was affected by race, age, sex, transverse diameter of eyeball, openness of fontanel, skull compliance, plasticity, disease course, interobserver variability and other factors.Although there are many influencing factors, compared with invasive monitoring, ultrasonic measurement of optic nerve sheath diameter may be a safe and non-invasive bedside tool to fill, which can evaluate intracranial pressure earlier and faster in real time, and have important role in children with neurocritical diseases.

Arson is classified as a violent crime, and often involves accelerants that significantly increase casualties and property damage. These accelerants, typically flammable liquids with low saturated vapor pressure, present volatile characteristic components in fire residues, making evidence preservation critical for accurate forensic analysis. In this study, the preservation performance of AMPAC(American KAPAC polyester sampling bags)and polyethylene(PE)evidence bags were evaluated under simulated field conditions(40 oC)through gas chromatography-mass spectrometry(GC-MS)technique,focusing on background interference, sealing integrity, and cross-contamination.The results demonstrated that AMPAC bags exhibited minimal background interference(C12?C14 alkanes detected after 3?7 days)and effective gasoline sealing, though slight losses of C2?C3 alkylbenzenes and trace cross-contamination were observed. In contrast,PE bags showed significant background hydrocarbons(C15?C19 alkanes)and pronounced gasoline component losses, including C2?C4 alkylbenzenes, naphthalenes, and indenes.Notably, severe cross-contamination of gasoline markers was detected in adjacent blank PE bags, with characteristic components such as C2?C4 alkylbenzenes, naphthalenes, methylnaphthalenes, dimethylnaphthalenes, indenes, indanes, methylindenes, and dimethylindenes identified, indicating substantial cross-contamination issues in PE evidence bags.Microstructural analysis revealed superior sealing in AMPAC bags attributed to their smooth, dense surface morphology compared to PE's rougher, porous structure. This study established a comprehensive evaluation framework for fire residue evidence containers, and found that PE bags were unsuitable for long-term gasoline evidence preservation due to compromised integrity. These findings provided critical references for forensic container selection in fire investigations.

Objective To investigate the potential mechanism of action of polygonatum odoratum in treating Alzheimer's disease through the utilization of network pharmacology and molecular docking techniques. Methods The methods employed include target screening,Gene Ontology(GO)function and Kyoto Encyclopedia of Genes and Genomes(KEGG)enrichment analysis, and molecular docking simulations to assess the binding interactions between the active compounds in polygonatum odoratum(POD)and the key target proteins associated with Alzheimer's disease.Subsequently, lipopolysaccharide(LPS)was used to induce an inflammatory cell model in BV2 microglial cells. After treating the cell model with POD extract for 24 hours, the cells were collected, and the expression of the target genes were detected by Real-time PCR.Results Eight active ingredients and 172 targets of POD were screened. The biological processes such as protein phosphorylation and signal transduction, protein binding and ATP binding were obtained by GO functional analysis.KEGG enrichment yielded PI3K/Akt,cAMP and other signaling pathways. The molecular docking result showed that the active ingredient of POD had well binding activity with epidermal growth factor receptor(EGFR),proto-oncogene tyrosine-protein kinase Src(SRC),tumor necrosis factor(TNF),STAT3.Through Real-time PCR experiments, the gene expressions of inducible nitric oxide synthase(iNOS),prostaglandin G/H synthase 2(PTGS2),interleukin(IL)-6,and IL-1β in the LPS-induced inflammatory cell model were significantly upregulated. After treating the inflammatory model with POD extract for 24 hours, the expression of TNF-α was significantly reduced, the expression of STAT3 was upregulated, there were no significant changes in the expressions of SRC and EGFR.Conclusion Network pharmacology suggests polygonatum odoratum's potential anti-Alzheimer's effects may be mediated through its interaction with targets such as EGFR,TNF,SRC,and STAT3.The experimental results suggest that polygonatum odoratum exerts an anti-inflammatory effect by acting on TNF-α,which may further alleviate the symptoms of Alzheimer's disease.

OBJECTIVES:
This study aimed to explore the expression of lysosomal-associated membrane protein 5 (LAMP5) and microRNA (miR)-302a-3p in oral squamous cell carcinoma (OSCC) and their functional mechanism on the invasion and metastasis of OSCC.
METHODS:
The expression of LAMP5 in OSCC and its sensitivity as a prognostic indicator were analyzed on the basis of The Cancer Genome Atlas database. Western blot, quantitative reverse transcription polymerase chain reaction, and cell immunocytochemistry were used to detect the expression of LAMP5 in OSCC tissues and cells. The effect of LAMP5 on the proliferation, migration, and invasion of OSCC cells was evaluated through cell counting kit-8, immunocytochemistry, migration, and invasion assays, respectively. The miRNA targeting prediction websites were used to predict the miR that regulates LAMP5 and verify the targeted regulatory effect of miR-302a-3p on LAMP5. The effect of LAMP5 knockdown on OSCC tumor growth was evaluated in a nude mouse tumorigenesis model.
RESULTS:
LAMP5 was highly expressed in OSCC tissues and cells. It showed high sensitivity in the early diagnosis of OSCC. LAMP5 knockdown significantly inhibited the proliferation, migration, and invasion of OSCC cells, whereas LAMP5 overexpression increased these cell activities. The expression of LAMP5 was regulated by miR-302a-3p. In vivo, LAMP5 knockdown significantly inhibited the growth of OSCC tumor.
CONCLUSIONS
LAMP5 promotes the malignant progression of OSCC by enhancing the proliferation, migration, and invasion of OSCC cells. The expression of LAMP5 is negatively regulated by miR-302a-3p.
