1.Clinical effects of carboprost methylate suppositorites in general term vaginal delivery patients with postpartum hemorrhage

Objective To analysis the effect of misopmstol,oxytocin nasal spray and carboprost methylate suppositorites on estrogen,nitric oxide (NO),nitric oxide synthase (NOS) and clinical efficacy in patients with general term vaginal delivery.Methods A total of 600 patients with postpartum hemorrhage were randomly divided into group A,group B and group C,200 cases in each group.Group A was given oxytocin nasal spray,per nostril 0.1 mL,twice a day,group B was given misoprostol,200 μg sublingual,group C received carboprost methylate suppositorites 1 mg,anus.After treatment,the serum levels of estradiol (E2),nitric oxide (NO) and nitric oxide synthase (NOS),bleeding volume,clinical efficacy,adverse drug reactions were compared in three groups.Results After treatment,the total effective rates in group A,B and C were 76.50% (153/200 cases),86.00% (172/200 cases),90.50% (181/200 cases),total effective rate of group C significantly higher than group A (P < 0.05).After treatment,estradiol levels of group A,B,C were (14.75 ±2.09),(17.63 ±2.15),(21.82 ±3.09) nmol · L-1.Levels of NO in group A,B,C were (107.83 ± 11.21),(92.42 ± 10.17),(79.25 ±9.89) nmol · L-1.Levels of NOS in group A,B,C were (25.64 ±3.11),(22.56±3.05),(19.63 ±2.28) nmol · L-1(P <0.05).And 2 h bleeding volume were (712.43 ± 40.85),(632.43 ± 42.77),(580.74 ± 37.64) mL in group A,B,C;2-24 h bleeding volume were (138.92 ± 17.86),(126.94 ± 12.93),(103.85 ± 11.13) mL in group A,B,C(P <0.05).The adverse drug reactions in group A were nasal irritation,nasal bleeding,incidence of adverse drug reactions was 1.50% (3/200 cases).The adverse drug reactions in group B were mild nausea,vomiting,dizziness,incidence of adverse drug reactions was 2.50% (5/200 cases).The adverse drug reactions in group C were diarrhea,nausea and vomiting,incidence of adverse drug reactions was 2.00% (4/200 cases),with no statistical differences (P > 0.05).Conclusions Compared with the treatment of misoprostol and oxytocin nasal spray,the carboprost methylate suppositorites can significantly improve the serum E2 level,reduce serum NO and NOS levels in patients with postpartum hemorrhage and reduce bleeding.
2.Effect of cytochrome P450 2C19 gene polymorphism on clopidogrel antiplatelet therapy

Objective To investigate the effect of cytochrome P450 2C19 (CYP2C19) gene polymorphism on the antiplatelet therapy of clopidogrel in patients who underwent percutaneous coronary intervention (PCI).Methods Fifty-five patients who underwent PCI,as well as twenty-three healthy subjects,were enrolled in this study.The genotypes of the samples were identified by gene chips hybridization.The contents of soluble P-selectin (CD62p) and platelet glycoprotein GP Ⅱ b/Ⅲ a fibrinogen receptor(PAC-1) in patients who underwent PCI were detected by flow cytometry (FCM).The expressions of platelet activation markers were calculated according to interquartile range method.Results Seven patients with CYP2C19 * 2/* 2 genotypes were poor metabolizers,while twenty-eight patients with CYP2C19 * 1/* 2,CYP2C19 * 1/ * 3 were intermediate metabolizers,twenty patients with CYP2C19 * 1/ * 1 were extensive metabolizers.In intermediate metabolism (* 1/* 2),intermediate metabolism (* 1/* 3) and poor metabolism group,the CD62p expressions were (6.73 ± 5.69) %,(10.94 ± 9.80) %,(14.35 ± 6.24) %,(16.80 ± 13.65) %,respectively,statistically significant differences were found when compared with the control group;The PAC-1 expressions were (1.06 ± 0.69) %,(2.10 ± 4.09) %,(2.37 ± 3.15) %,(2.89 ± 2.75) %,and there were significant differences in CD62p and PAC-1 among the groups(P <0.05).The genotypes of extensive metabolism was high in the response to clopidogrel,while those of intermediate metabolism were moderate and poor metabolism were low.Conclusion Intermediate metabolism genotype patients could be adjusted to the dose of clopidogrel or the use of other antiplatelet drugs,and the poor metabolism genotype patients should be used other antiplatelet drugs.

A new hexaketide acid esterified by the 17-hydroxyl group of 16,17-dihydroxycyclooctatin, namely 17-[16,17-dihydroxycyclooctatinyl]-hexaketide ester (1), a member of the group of rare bacterial diterpenes with a fused 5-8-5 ring system was isolated from strain Streptomyces sp. SR107. The structure was determined on the basis of its spectral data (H NMR, C NMR, H-H COSY, HSQC, HMBC, NOESY, IR and HR-ESI-MS). The antibacterial activity was also evaluated in this paper.

4.Initial evolution research for design and process accuracy of one type of domestic computer aided design soft and computer aided manufacture

Objective To evaluate the design and manufacture accuracy of a domestic computer aided design(CAD) and computer aided manufacture(CAM) system,and to compare it with similar foreign products.Methods Thirty models of posterior-teeth-single-crown preparations were collected,and STL data of these preparations was collected by Denmark 3Shape scanner.Three copings were made for each preparation,the one designed and manufactured using commercial CAD/CAM system (3Shape CAD software and Wieland T1 CAM equipment) was assigned into control group T0,the one designed and manufactured using domestic CAD software (developed by Peking University School and Hospital of Stomatology and Nanjing University of Aeronautics and Astronautics) and Wieland T1 CAM equipment was assigned into experimental group TCAD for design accuracy evaluation,and the one designed and manufactured using 3Shape CAD software and domestic CAM equipment(developed by Peking University School and Hospital of Stomatology,Tsinghua University and ShanDong XinHua Incorporated Company of medical apparatus and instruments) was assigned into experimental group TCAM for manufacture accuracy evaluation.Finally,the marginal fitness were compared and evaluated by using 3D & Profile measurement microscope laser.Results The marginal fitness of TCAD was 27.98 (19.10,46.57) μm in buccal,32.67 (20.65,50.82) μm in lingual,27.38 (22.53,52.61) μm in mesial,29.50 (22.68,53.65) μm in distal; of TCAM was 21.69(15.87,30.21) μm in buceal,18.51(13.50,22.51) μm in lingual,19.15(15.42,26.89) μm in mesial,22.77(18.58,32.15) μm in distal; and there were no statistical differences compared with T0 [20.16(17.16,48.00) μm in buccal,21.51(17.05,28.31) μm in lingual,23.54(17.89,30.04) μm in mesial and 23.94(17.93,28.19) μm in distal] except lingual data of TCAD.Conclusions The design and machining precision of this domestic CAD/CAM system is at the same level of those comparable foreign products.
5.Left ventricular functional changes of stunned myocardium during perioperation of coronary artery bypass graft surgery

ObjectiveTo evaluate the functional changes of stunned myocardium before and after coronary artery bypass graft(CABG) treatment,and clear the meaning of revascularization which CABG has brought to patients with diffused vascular changes.MethodsA total of 36 patients with 99% diffused coronary artery stenosis in left anterior descending branch underwent non-pump CABG treatment in the Department of Cardiothoracic Surgery the First affiliated hospital of Harbin Medical University.Real-time three-dimensional echocardiography (RT3DE) was repeatedly performed 1 week before operation and 10 days,1 month,and 3 months after CABG.Regional diastolic volumes,systolic volumes,ejection fractions,regional stroke volume to global diastolic volume and the values of abnormal segments before and after CABG were studied.ResultsOne week before operation and 10 days,1 month and 3 months after CABG,the differences of volumes between groups in the last phases of diastole and systolic were statistically significant in anterior wall basement segment,anterior septal basement segment,anterior wall intercalary segment,anterior septal intercalary segment,anterior wall of apex cordis and septation of apex cordis(F =3.51,3.55,4.08,4.05,2.98,3.01,all P < 0.05; F =4.51,4.55,4.08,3.00,2.96,2.99,all P < 0.05).The values of the six segments mentioned above,3 months after operation[(6.74 ± 1.23),(6.64 ± 1.21),(6.02 ± 1.10),(5.95 ± 1.09),(5.82 ± 1.06),(5.10 ± 0.93)ml; (2.74 ± 0.50),(2.69 ± 0.49),(2.51 ± 0.46),(2.32 ± 0.42),(2.36 ± 0.43),(2.03 ± 0.37)ml] were compared with those of 1 week before operation[(8.33 ± 1.52),(8.20 ± 1.50),(7.43 ± 1.36),(7.36 ± 1.34),(7.19 ± 1.31),(6.29 ± 1.15)ml; (4.94 ± 0.90),(4.85 ± 0.88),(4.53 ± 0.83),(4.18 ± 0.76),(4.25 ± 0.78 ),(3.65 ± 0.67)ml],the differences were statistically significant (all P < 0.05); the differences between groups in regional ejection fractions,regional-global ejection fractions were statistically significant(F =4.56,4.88,4.28,3.15,2.93,2.88,P < 0.01 or < 0.05; F =5.56,5.28,4.98,5.15,3.03,2.78,P < 0.01 or < 0.05).Compared with 1 week before the operation, 1 month after the operation in regional ejection fractions,10 days,1 month in regionalglobal ejection fractions after the operation,4 segments of them were significantly improved(all P < 0.05) and 3 months after operation,all the 6 segments had been improved significantly(all P < 0.05).The maximum volume of the sum of group difference of the 6 segments and the 4 segments in the last phase of diastole was statistically significant(F =2.58,5.81,P < 0.05 or < 0.01 ),and the summation began to decrease 10 days after the operation.The values of 3 months after operation[ (36.27 ± 1.10),(25.35 ± 1.16)ml] were compared with that of 1 week before operation[ (44.80 ± 1.36),(31.32 ± 1.43)ml ] the difference was statistically significant (all P< 0.05).The maximum volume summafion comparisons of 6 segments and 4 segments in the last phase of systolic had statistical significance(F =5.77,5.57,all P < 0.01 ),and 10 days after the operation,the summation began to decrease.The values of 1 month[(16.4 0 ± 0.48),(11.58 ±0.51 )ml],and 3 months after operation[ (14.65 ± 0.45),(10.26 ± 0.46)ml],were compared with those of 1 week before operation[ (26.40 ± 0.80),(18.50 ± 0.84)ml],the differences were statistically significant (all P < 0.05).ConclusionsStunned myocardium can be improved through CABG in myocardium systolic,diastole function and ejection fractions of the relevant segments and all of this have proved that patients undergoing CABG revascularization can improve the heart function of the ischemic area.
6.Twenty six cases of human Fasciola gigantica infection in Dali, Yunnan province

Objective The aim of the study was to investigate the epidemic situation,clinical symptom,diagnosis and epidemiological characteristics of human Fasciola gigantica infection in Dali,Yunnan province.It will also provide a scientific basis for fasciolosis control and prevention.Methods Epidemic data were collected and patient's clinical signs and symptoms were studied.Serum soluble antigen of Fasciola gigantica of patients and part of family members and health people in the same village was detected using enzyme-linked immunosorbent assay (ELISA) and the eggs of Fasciola gigantica in stool were observed under microscope.Sequencing and PCR amplification of Fasciola gigantica eggs had been done.Sequencing results were analyzed using basic local alignment search tool (BLAST) program of the U.S.National Center for Biotechnology Information (NCBI) and the similarity of the two in the sequence of nucleic acid was compared.Furthermore,patients were experimentally given orally therapeutic doses of Triclabendazole 10 mg·kg-1·d-1 daily for 2 days,and kept in the hospital for observation for one week.Moreover,host and vector were investigated in the surrounding ditches of Dali prefecture and Limnaea peregra snail samples were collected.All the snails were squashed by glass sheet in order to detect the cercarie.Cow dung and sheep manure was collected in the Limnaea peregra distribution environment,and the eggs in the feces were checked by microscope after washing and precipitation.Results All the 26 patients had a continued hyperpyrexia with distinct alimentary system symptoms of nausea,vomiting,stomachache,abdominal distension as well as hepatomegaly,sensitive to percussion,different levels of liver damage detected by CT.All the patients had an eaten history of raw Herba Houttuyniae and other aquatic plants,and the course of the disease was similar,with the same epidemiological characteristics.ELISA detection was used in the 26 patients,family members and other healthy population,the results of all the 26 patients were positive(100.0%,26/26) ; the positive rates of the 57 family members and other health people of the same village were 31.6% (18/57) and 17.1% (6/35),respectively.The results of sequencing and BLAST program showed that the pathogen was Fasciola gigantica with the similarity between 99%-100%.PCR amplification also confirmed that the eggs were Fasciola gigantica eggs with an approximately 1000 bp band on agarose gel.After treatment with Triclabendazole,body temperature of the patients dropped to normal and symptoms improved markedly.Moreover,329 Limnaea peregra snails were collected including 5 ones with redia and one-tailed cercariae which were preliminary identified as the larva of Fasciola gigantica.There were also eggs of Fasciola gigantica detected in one stool of cattle and one of goat.Conclusions Eating raw food is the leading cause of the onset of the disease.Triclabendazole is the drug of choice to treat Fasciolasis.Health education should be strengthened by government and disease prevention and control departments in order to make the local residents to understand the potential hazard of eating raw aquatic vegetable and drinking unboiled water,which is the key to prevent the occurrence of the disease.
7.Effects of hyperbaric oxygen on TF、TNF-α in the rabbit model of steroid-induced avascular osteonecrosis of femoral head

Objective To investigate dynamic changes in serum TF and TNF-α in the rabbit model of steroid-induced avascular osteonecrosis of femoral head ( SANFH) and also to explore the mechanism of SANFH, as well as effects of hyperbaric oxygen ( HBO) on SANFH. Methods Seventy-eight New Zealand male rabbits were randomly divided into 3 groups:the normal control (group N) (7 animals), the model group (group M) (41 animals) and the HBO group (group H) (30 animals). The model group was subdivided into the immediate model group (the M0 group) (10 animals), the two-week model group (the M2 group) (10 animals), the four-week model group (the M4 group) (10 animals) and the six-week model group (the M6 group) (11 animals). The HBO group was further divided into the 2-week HBO therapy group (HBO2) (7 animals), the 4-week HBO therapy group (HBO4) (11 animals) and the 6-week HBO therapy group (HBO6) (12 animals). Through injection of endotoxin and methyl-prednisolone, rabbits in the group HBO2, HBO4 and HBO6 received HBO therapy 1 hour daily from the second day of the experiment. The durations of HBO therapy were 2 weeks ( HBO2), and 4 weeks respectively. The animals were sacrificed after blood samples were taken at respective blood collection time. Then, levels of TF, TNF-α in the serum were measured and the histological changes in the femoral heads were observed. Results Levels of TF and TNF-α in group M0 increased significantly, when compared with those of group N (P <0. 05 or P <0.01), while for the HBO subgroups the expression of TF and TNF-α measured at the same time points all decreased, when compared with that of the model subgroups (P<0. 05 or P <0.01). To elaborate, TF levels in group M2 and M4 were much higher than those in group HBO2 and HBO4 ( P<0. 01 ). TF level in group M6 was higher than that in group HB06 ( P < 0.05). TNF-α in group M0 also increased significantly, when compared with that in group N( P <0.01). TNF-α levels in group M2 and M6 were also much higher than those in group HBO2 and HBO6 ( both P <0. 01 ). TNF-α in group M4 was higher than that in group HBO4 (P<0.01). Histological examination revealed that tissues of the femoral heads in group N were normal, osteonecrosis and thrombus could be noted in group M2 and M4, hyperplasia fibrosis could be found in group M6, and osteonecrosis in HBO2 and HB04 groups seemed less severer than that in M2 and M4 groups, no thrombi in HBO2, HBO4 groups were noted, and growth of new bones were detected in HBO4 and HBO6. Conclusions The levels of TF and TNF-α levels increased in the rabbit model of SANFH, inducing blood coagulation. Thrombosis at the femoral heads was one of the causes of SANFH. HBO therapy could inhibit the release of TF and TNF-α, thus improving the abnormality of blood coagulation and enhancing treatment of osteonecrosis.

9.A computer aided design approach of all-ceramics abutment for maxilla central incisor

Objective To establish the computer aided design (CAD) software platform of individualized abutment for the maxilla central incisor. MethodsThree-dimentional data of the incisor was collected by scanning and geometric transformation. Data mainly included the occlusal part of the healing abutment, the location carinae of the bedpiece, the occlusal 1/3 part of the artificial gingiva's inner surface,and so on. The all-ceramic crown designed in advanced was "virtual cutback" to get the original data of the abutment's supragingival part. The abutment's in-gum part was designed to simulate the individual natural tooth root. The functions such as "data offset", "bi-rail sweep surface" and "loft surface" were used in the process of CAD. ResultsThe CAD route of the individualized all-ceramic abutment was set up. The functions and application methods were decided and the complete CAD process was realized. Conclusions The software platform was basically set up according to the requests of the dental clinic.
10.Effects of hyperbaric oxygen on IL19,IL20 in the rabbit model of steroid -induced avascular osteonecrosis of femoral head

Objective To investigate effects of HBO on serum IL19 and 1L20 in the animal model of steroid - induced avascular osteonecrosis of femoral head. Methods Seventy-eight healthy New Zealand male rabbits were randomly divided into 8 groups: group N ( the control group),group M2,M4,M6 ( the model groups) and the HBO therapy groups (HBO group 2,HBO group 4,HBO group 6). The animal model of steroid - induced avascular osteonecrosis of femoral head was developed by injections of endotoxin and methylprednisolone. Rabbits in the HBO group 2,HBO group 4,HBO group 6 received HBO therapy 1 hour a day,and the duration of HBO treatment were 2 weeks ( HBO group 2),4 weeks ( HBO group 4) and 6 weeks ( HBO group 6) respectively. Contents of IL19,IL20 in serum were measured and histological changes in the femoral heads were observed as well. Results IL19 in group M0 increased significantly,when it was compared with that of group N(P<0.05);IL19 in group M2 and M4 were much higher than those in group HBO group 2,HBO group 4 (P <0.05,P < 0.01 ). IL20 in group M0 also increased significantly,when compared with that of group N( P <0.05 );IL20 in group M2 and M4 were much higher than those in HBO group 2 and HBO group 4 as well( P < 0.01 ). Histological examination of the tissues of the femoral head revealed osteonecrosis and inflammatory cells in M2 and M4 groups,and lighter osteonecrosis together with less inflammatory cells in HBO group 2 and HBO group 4 groups than in M2,M4 groups. Conclusions Contents of IL19,IL20 elevated in the rabbit model of steroid-induced avascular osteonecrosis of femoral head. HBO therapy could inhibit release of IL19 and IL20,alleviate inflammatory response and decrease the extent of osteonecrosis.
