1.Comparison of sleep EEG power spectral density between depressive episode patients and schizophrenia patients with suicidal behavior

BackgroundPatients with depressive episode and schizophrenia have a high risk of suicide. The sleep electroencephalogram power spectral density characteristics of patients with depressive episode accompanied by suicidal behavior and those with schizophrenia may be different, but there is currently a lack of direct comparative studies on these two groups of patients. ObjectiveTo compare the sleep electroencephalogram power spectral density between depressive episode and schizophrenic patients with suicidal behavior, in order to provide references for exploring predictive indicators of suicidal behavior. MethodsFrom June 2018 to December 2020, 20 patients with depressive episode and 20 patients with schizophrenia who had committed suicide within the past month and were treated at the outpatient department of Shenzhen Kangning Hospital were selected. All of them met the diagnostic criteria for depressive episode or schizophrenia as defined in the International Classification of Diseases, tenth edition (ICD-10). Using a random sampling method, 20 volunteers with matching gender and age to the patient groups were selected from the Cuiping community in Shenzhen as the control group. The subjective sleep of the patients was evaluated using the Insomnia Severity Index (ISI), the Dysfunctional Belief and Attitude about Sleep (DBAS), the Disturbing Dreams and Nightmare Severity Index (DDNSI), and the Epworth Somnolence Scale (ESS). The objective sleep of the patients was assessed using polysomnography. The sleep electroencephalogram was filtered and the power spectral density of the brain wave was analyzed and processed for all the subjects. The subjective and objective sleep conditions of the two patient groups were compared, and the sleep electroencephalogram power spectral density of the patient groups and the control group were also compared. ResultsA comparison of subjective and objective sleep conditions between patients with depressive episode accompanied by suicidal behavior and patients with schizophrenia accompanied by suicidal behavior showed no statistically significant differences (P>0.05). Comparisons of sleep electroencephalogram power spectral density in the W stage (average power of α wave, total power of δ wave, average power of δ wave, average power of θ wave), N1 stage (average power of β wave, total power of α wave, total power of δ wave), N2 stage (total power of α wave, average power of α wave, total power of δ wave, average power of δ wave), N3 stage (average power of α wave, average power of δ wave), and R stage (total power of α wave, average power of α wave, total power of δ wave, average power of δ wave) between patients with depressive episode accompanied by suicidal behavior, patients with schizophrenia accompanied by suicidal behavior, and the control group showed statistically significant differences (P<0.05 or 0.01). The total power of δ wave in the W stage and the average power of β wave and δ wave in the N1 stage were higher in two patient groups were higher than those of the control group. The total power of α wave and the average power of α wave in the N2 stage were lower than those of the control group, while the average power of δ wave was higher than that of the control group. The average power of α wave in the N3 stage of both patient groups were lower than that of the control group, while the average power of δ wave was higher than that of the control group. The total power and average power of α wave in the R stage were lower than those of the control group, while the total power and average power of δ wave were higher than those of the control group. All the differences were statistically significant. Patients with depressive episode accompanied by suicidal behavior had higher average powers of α wave, δ wave, and θ wave in the W stage compared with the control group, while the total power of α wave in the N1 stage was lower in the former group. All these differences were statistically significant (P<0.05). ConclusionThe depressive episode patients accompanied by suicidal behavior have highly overlapping sleep electroencephalogram abnormal patterns with those of schizophrenia patients, mainly manifested as a general decrease in α wave power (N2, N3, R stage) and a general increase in δ wave power (W, N1, N2, N3, R stage) as well as β wave power in N1 stage. At the same time, patients with depressive episode accompanied by suicidal behavior also show specific changes, including an increase in the average power of α and θ waves during the wakefulness period (W stage), and a decrease in the total power of α wave in N1 stage. 2.Correlation between choreiform symptoms of hepatolenticular degeneration and caudate nucleus atrophy on brain magnetic resonance imaging

Objective To quantitatively analyze the volumetric characteristics of each subregion of the basal ganglia in patients with hepatolenticular degeneration (also known as Wilson disease ,WD) using brain magnetic resonance imaging (MRI) and brain segmentation technology, to explore the specific imaging findings of choreiform symptoms, and to assess the clinical value of caudate nucleus atrophy as an imaging indicator for this symptom. Methods A retrospective analysis was performed for 40 WD patients with choreiform symptoms and 40 patients without choreiform symptoms from June 2023 to June 2025, and clinical indicators were compared between the two groups. In addition, the two groups were compared in terms of the volume of the basal ganglia after estimated total intracranial volume (eTIV) correction, and the correlation between the volume of differential brain regions and the chorea subscale score of Unified Wilson's Disease Rating Scale (UWDRS) was explored. Results There were no significant differences in baseline data between the two groups. UWDRS scores showed that the choreiform group had a higher neurological function score (P=0.005), a significantly higher chorea subscale score (P<0.01), and a lower hepatic function score (P<0.01). The choreiform group had a significantly smaller caudate nucleus volume than the non-choreiform group (P<0.001), suggesting severe subregional atrophy, and in contrast, the choreiform group had a significant increase in thalamus volume (P=0.002). Caudate nucleus volume ratio was significantly negatively correlated with chorea subscale score in the choreiform group (P<0.001). Conclusion Caudate nucleus atrophy is a specific imaging finding of choreiform symptoms in WD patients, and a quantitative analysis of caudate nucleus volume is expected to become an objective imaging indicator for assessing the severity of choreiform symptoms and monitoring disease progression in WD.
3.Mechanism of emodin improving cardiac hypertrophy in mice based on p38/ERK pathway

Aim Mouse model of myocardial hypertro-phy was established via intraperitoneal injection of iso-proterenol(ISO)in mice.This approach allows for an in-depth investigation into the pharmacological effects and mechanisms of action of emodin,offering novel in-sights and directions for the improvement of myocardial hypertrophy.Methods The mice were randomly di-vided into the following groups:control group(CON),emodin group(EMO),MAPK activator control group(EMO+Ani),model group(ISO),treatment group(ISO+EMO),and activator intervention group(ISO+EMO+Ani).After treatment with emodin and inter-vention with MAPK activator,the heart weight ratio and cardiac size of each group were observed.Hematoxy-lin-eosin(HE)staining was used to observe the patho-logical changes in cardiac tissue,and kits were utilized to measure the levels of GSH,LDH,and MDA in the serum.Western blot was employed to detect the protein expression levels of inflammatory and oxidative factors,as well as p-p38,p-ERK,p38,and ERK in cardiac tis-sue.Results Emodin can significantly inhibit the production of myocardial inflammatory and oxidative factors induced by ISO,thereby effectively alleviating the degree of myocardial hypertrophy and fibrosis.Af-ter the p38/ERK signaling pathway was specifically ac-tivated by farnesol,the improvement effect of emodin on myocardial hypertrophy was weakened.Further comparison revealed that,compared with the myocardi-al hypertrophy pathological model group,the pathologi-cal protein expression levels in the farnesol-treated group showed no significant difference,and were even higher in some indicators.Conclusion Emodin can effectively inhibit the release of inflammatory factors and improve the state of oxidative stress by modulating the p38/ERK signaling pathway,thereby exerting an ameliorative effect on myocardial hypertrophy.
4.Design and implementation of an outcome-based education-based online + offline course integrating medical specialty and humanistic spirit

The "minimally-invasive and non-invasive diagnosis and treatment of the most important tumor in gynecology", which is an online + offline first-class undergraduate course in Chongqing, China, was used as an example to demonstrate how to explore the integration of medical specialty with humanistic spirit in the design and construction of courses for medical students, and to strengthen the early cultivation of medical humanistic spirit. Based on the concept of outcome-based education, innovative course content and teaching modes were designed to guide students to learn the history of focused ultrasound therapy technology, understand the concept of minimally-invasive and non-invasive medicine, and incorporate the literacy cultivation of "always having compassion". This study explored the importance of incorporating humanistic spirit into medical education and provides new ideas and concepts for the cultivation of physicians with humanity and compassion.
5.Research progress on protective effect of cannabidiol on central nervous system diseases

Cannabidiol(CBD),a natural,non-toxic,non-psy-choactive cannabinoid extracted from Cannabis sativa L,has a wide range of pharmacological activities,including anti-inflam-matory,antioxidant,immunomodulatory,analgesic,anti-tumor,anti-convulsant,anxiolytic effects.Recent studies have indica-ted that CBD may have potential neuroprotective effects in vari-ous models of central nervous system diseases.This paper re-views the experimental research progress of CBD in central nerv-ous system diseases and its possible neuroprotective mecha-nisms,aiming to provide references for its clinical application and new drug development.
6.Protective effect of stereotactic neuroendoscopic hematoma evacuation on neurological function in hypertensive intracerebral hemorrhage patients

Objective To compare the effects of stereotactic endoscopic hematoma evacuation on elderly patients with hypertensive intracerebral hemorrhage.Methods A total of 220 patients with hypertensive intracerebral hemorrhage in the basal ganglion admitted to our hospital from January 2020 to December 2023 were retrospectively recruited.According to their surgical treat-ment,they were divided into an observation group(stereotactic endoscopic hematoma evacuation,n=100)and a control group(craniotomy hematoma removal surgery,n=120).Postoperative recovery was observed and compared between the two groups.Results There was no significant difference in operation time between the observation group and the control group(94.56±14.75 min vs 94.03±14.50 min,t=0.268,P=0.789).No statistical differences were observed in the NIHSS score before operation between the two groups(P=0.058),but the observation group obtained significantly lower NIHSS scores in 3 and 6 months after surgery(5.90±4.02 vs 9.23±3.47,P=0.000;4.54±2.56 vs 6.50±3.07,P=0.000).There were no significant differences in GOS score and incidence of postoperative complications between the two groups(P>0.05).Conclusion Stereotactic endoscopic hematoma evacuation can improve postoperative neurological function in patients with hypertensive intracerebral hemorrhage in the basal ganglia region.
7.Effects of Bushen Tongfu Prescription on gut microbiota and LPS/TLR4/NF-κB pathway in APP/PS1 mice based on"microorganism-intestine-brain axis"

Aim To explore the mechanism of Bushen Tongfu prescription(BSTF)regulating gut microbiota and improving learning and memory ability of Alzhei-mer's disease(AD)model mice.Methods APP/PS1 mice were administered by BSTF for eight weeks.The spatial learning and memory ability of mice were detected by Morris water maze.The changes in the gut microbiota abundance and diversity of mice were detec-ted by 16S rDNA technology.The morphological chan-ges of hippocampus were observed by HE staining.The expression of the activation of microglia in hippocampus was detected by immunofluorescence.The expressions of pro-inflammatory factors of TLR4,NF-κB and IL-6 in brain tissue were detected by Western blot.Results Compared with the model group,the escape latency and swimming path were shortened,the times of target crossings after removing the platform increased in BSTF groups(P<0.05),the α-diversity increased and the abundance of gut microbiota was regulated,the mor-phological structure and pathological damage of hipp-ocampal cells were improved,the co-expression of iN-OS/Iba1 decreased and the co-expression of Arg1/Iba1 increased significantly(P<0.01),the transformation of M1 to M2 of microglial cells was promoted,and the expression of TLR4,NF-κB and IL-6 protein decreased significantly.Conclusions BSTF can improve the learning and memory ability of APP/PS1 mice,and its mechanism may be related to regulating the gut micro-biota and the LPS/TLR4/NF-κB pathway,inhibiting the activation of pro-inflammatory microglia,reducing inflammation in hippocampus and improving the patho-logical damage of hippocampal cells.
8.Epidemiological Analysis of Pathogens in Acute Respiratory Infections During the 2023-2024 Autumn-Winter Season in Beijing:A Case Series of 5556 Patients at Peking Union Medical College Hospital

Objective To analyze the epidemiological characteristics of acute respiratory infections(ARIs)during the autumn-winter season in Beijing,providing evidence for the prevention,control,diagnosis,and treatment of ARIs.Methods A convenience sampling method was employed,enrolling patients who visited Peking Union Medical College Hospital(PUMCH)between September 2023 and February 2024 due to ARIs.Na-sopharyngeal swabs were collected,and real-time fluorescence quantitative PCR was used to detect six common respiratory pathogens[influenza A virus(FluA),influenza B virus(FluB),human rhinovirus(HRV),Myco-plasma pneumoniae(MP),respiratory syncytial virus(RSV),and adenovirus(ADV)],as well as SARS-CoV-2 infection.The distribution patterns of pathogen infections were analyzed.Results A total of 5556 eligible patients were included.The overall positivity rate for the six common respiratory pathogens was 63.7%,with sin-gle-pathogen positivity at 54.0%,dual-pathogen positivity at 8.9%,and triple or more pathogen positivity at 0.7%.The predominant pathogens detected were FluA(16.1%)and RSV(15.7%),followed by ADV(11.1%),MP(11.1%),HRV(10.0%),and FluB(10.0%).No significant difference in overall pathogen positivity was observed between genders.However,significant differences were found between autumn and winter(x2=34.617,P<0.001)and among pediatric,young/middle-aged,and elderly patients(x2=422.38,P<0.001).Specifically,MP(x2=8.647,P=0.003),FluA(x2=131.932,P<0.001),and HRV(x2=174.199,P<0.001)exhibited significantly higher positivity rates in autumn than in winter,whereas FluB was more prevalent in winter(x2=287.894,P<0.001).In pediatric patients,MP,RSV,HRV,and ADV positivity rates were significantly higher than in young/middle-aged and elderly patients(all P<0.001),whereas FluB was more common in young/middle-aged patients(both P<0.001).The positivity rates of the six common respiratory pathogens significantly declined during the SARS-CoV-2 epidemic period,exhibiting an asynchronous seasonal pattern.Conclusions The prevalence of respiratory pathogens in Beijing is associated with age and season.Tar-geted preventive measures should be implemented in different seasons and for key populations.

10.Diagnostic value of small field of view DWI-MRI in assessing benign and malignant pulmonary nodules

Objective:To explore the diagnostic value of small field of view ZooMit diffusion weighted imaging(Z-DWI)sequence of magnetic resonance imaging(MRI)in assessing benign and malignant pulmonary nodules.Methods:A total of 60 patients with pulmonary nodules who underwent MR examination at the First Affiliated Hospital with Nanjing Medical University from January 2021 to December 2022 were prospectively collected.All patients underwent conventional DWI(C-DWI)and Z-DWI.The signal to noise ratio(SNR),contrast to noise ratio(CNR),and apparent diffusion coefficient(ADC)of them were measured and analyzed.Two physicians with more than 8 years of experience in diagnosing pulmonary nodules implemented subjective scores for the image quality.Then,the ADC values of benign and malignant nodules between two kinds of images were compared.Results:The scores of Z-DWI images quality in terms of artifact,display of anatomical detail,lesion clarity,and image distortion were respectively(3.53±0.52),(3.27±0.70),(2.87±0.64),and(2.93±0.71),which were all higher than those of C-DWI images,and the differences were statistically significant(t=-4.68,-3.56,-2.44,-2.87,P<0.05).The SNR and CNR values of Z-DWI sequence were respectively(123.4±39.9)and(124.9±24.7),all of which were higher than those of C-DWI sequence,and the differences were statistically significant(t=-8.63,-7.09,P<0.05).The ADC values of C-DWI images of benign and malignant nodules were respectively(1.53±0.27)and(1.09±0.22),and the difference of that between them were statistically significant(t=9.88,P<0.001).The ADC values of Z-DWI images of benign and malignant nodules were respectively(1.39±0.18)and(0.99±0.19),and the difference of that between them was statistically significant(t=7.30,P<0.001).The ADC values of benign and malignant nodules of C-DWI images were higher than those of Z-DWI images,and the differences were statistically significant(t=3.40,2.13,P<0.05).Conclusion:Z-DWI can improve the image quality of benign and malignant pulmonary nodules,which has important clinically diagnostic significance in assessing the benign and malignant natures of pulmonary nodules.
