Objectives:To explore the relationship between plasma big endothelin-1(Big ET-1)level and left ventricular(LV)remodeling assessed by cardiac magnetic resonance(CMR)in patients with hypertrophic obstructive cardiomyopathy(HOCM).Methods:A total of 180 consecutive HOCM patients were enrolled from November 2013 to July 2015 in Fuwai Hospital,Chinese Academy of Medical Sciences.The baseline characteristics of the patients were collected and the plasma Big ET-1 levels were measured.According to LV remodeling index(LVRI),the patients were divided into severe remodeling group(LVRI>1.3 g/ml,n=119)and mild remodeling group(LVRI≤1.3 g/ml,n=61).The Pearson correlation analysis and multivariate logistic regression analysis were conducted to assess the correlations between plasma Big ET-1 level and indicators of LV remodeling.Results:Compared to the mild remodeling group,the plasma Big ET-1 level([0.84±0.30]pmol/L vs.[0.41±0.19]pmol/L),LVRI([1.7±0.3]g/ml vs.[1.1±0.1]g/ml),the extent of late gadolinium enhancement(LGE)([16.8±8.9]g vs.[7.0±6.6]g)were significantly higher in the severe remodeling group(all P<0.001).Pearson correlation analysis revealed that in patients with HOCM,plasma Big ET-1 level were positively correlated with left atrial anteroposterior diameter,interventricular septal thickness(IVS),LV mass(LVM),LVRI,LGE and LV outflow tract(LVOT)gradient and negatively correlated with LV ejection fraction(all P<0.05).Multivariate logistic regression analysis showed that LVM(OR=1.037,95% CI:1.019-1.056,P<0.001)and plasm Big ET-1 level(OR=11.461,95% CI:2.264-58.014,P=0.003)were the independent determinants for severe LV remodeling in HOCM patients.Conclusions:Among patients with HOCM,the plasma Big ET-1 level are markedly higher in those with severe LV remodeling and are positively associated with left atrial anteroposterior diameter,IVS,LVRI,LVM,LVOT gradient and LGE.Elevated Big ET-1 level and LVM are independent determinants of severe LV remodeling in HOCM patients.

Purpose: Inflammatory myofibroblastic tumor (IMT) is a rare mesenchymal malignancy that occurs primarily in children and adolescents. The clinical and pathological features of IMT in adult patients are not well understood. Materials and Methods: We retrospectively searched for records of adult patients with IMT at Fudan University Shanghai Cancer Center from 2006 to 2021. Clinicopathological data, treatments, and outcomes were collected and analyzed. Results: Thirty adult patients with IMT, mostly women (60.0%), were included. The median age of the patients was 38 (21-77). The most common primary site was abdominopelvic region (53.3%), followed by lungs (20.0%). Seven patients had an abdominal epithelioid inflammatory myofibroblast sarcoma (EIMS). The positivity rate of anaplastic lymphoma kinase (ALK) was 81.5% (22/27). Sixteen patients with advanced ALK-positive disease received crizotinib, with an objective response rate (ORR) of 81.3% and a disease control rate of 87.5%. The median progression-free survival was 20.8 months. EIMS was associated with more aggressive behavior; however, the prognosis was similar to that of non-EIMS patients after treatment with an ALK inhibitor. At a median follow-up time of 30 months (95% confidence interval [CI], 13.6 to 46.4), the 5-year overall survival was 77% (95% CI, 66 to 88) in all patients. Conclusion Adult IMTs appeared more aggressive, with a higher incidence of recurrence and metastases, and patients with EIMS had more aggressive cases. Treatment with ALK inhibitors resulted in a high ORR and a durable response, which suggested that ALK inhibitors could be used as a first-line treatment option in adult patients with ALK-positive advanced IMT.

Objective Perioperative myocardial infarction remains a severe complication in non-cardiac surgery and is one of the major causes of death.Cardiac troponin (cTn) Ⅰ elevation is associated with short-term and long-term mortality.The aim of the study was to assess the proportion rate of cTnⅠ elevation and its clinical characteristics among patients admitted for orthopaedic surgery with or without cardiovascular events.Methods This is a retrospective study including 27 744 patients aged 50 years or older who admitted for orthopaedic surgery from 2009-2015 in Beijing Jishuitan Hospital.Results Two hundred and sixty-five patients [age (71.7±9.9) years] had cTnⅠ level＞ 0.04 μg/L with 66％ (175 patients) of them being female.Among them,59 patients were isolated troponin rise (ITR) (n=59),13 were preoperative acute myocardial infarction (AMI),and 193 were postoperative AMI.The proportion of postoperative AMI was 0.69％.Those patients were more likely to have a history of coronary artery disease or hypertension.Non-ST-segment elevation myocardial infarction (NSTEMI) was more common (93.3％) than ST-segment elevation myocardial infarction in these patients.Most of them did not experience ischemic symptoms.Totally 76.7％ of the AMI occurred within 3 days of surgery;and the in-hospital mortality rate was 10.4％.Conclusions Perioperative elevation of troponin is common in patients undergoing orthopaedic surgery.Most postoperative AMI were NSTEMI and with absent or atypical ischemia symptoms.Monitoring troponin levels and electrocardiograph in at-risk patients is needed to find most of the AMI.

Objective: To explore the effect of apelin on angiotensin II (Ang II)-induced cardiomyocyte hypertrophy and intracellular signal transduction mechanism in experimental rats.
Methods: The cardiomyocyte from 1 to 3 days neonatal rats were cultured with Ang II to induce the cardiomyocyte hypertrophy, and the cells were treated by apelin at different concentrations. The [3H] Leucine incorporation, cardiomyocyte surface area and total protein expression were analyzed to evaluate the degree of cardiomycyte hypertrophy. The protein expressions of intracellular BNP, β-MHC, nuclear factor 3 of activated T cells (NFATc3), calcineurin, phospho-calcineurin, calmodulin kinase II (CaMK II) and phospho-CaMK II were assessed by Western blot analysis. The mRNA expressions of BNP andβ-MHC were examined by RT-PCR.
Results: Apelin may inhibit Ang II induced cardiomyocyte hypertrophic response in a dose-dependent manner, the maximum inhibition was achieved at Ang II 1000 nmol/L. Meanwhile, apelin may inhibit Ang II-induced elevations of intracellular resting free calcium level, mRNA expressions of BNP andβ-MHC, protein expressions of NFATc3, phospho-calcineurin, CaMK II and phospho-CaMK II in a dose-dependent manner.
Conclusion: Apelin may inhibit Ang II-induced cardiomyocyte hypertrophy in experimental rats which might be related to Ca2+-dependent calcineurin signal pass ways.
