1.Clinical efficacy evaluation of unilateral and bilateral puncture vertebral body shaping in the treatment of osteoporotic vertebral compression fractures

Objective:To compare the clinical efficacy of unilateral puncture and bilateral puncture vertebroplasty in osteoporotic compression fracture patients.Methods:A regression comparative analysis was performed using case-control study to collect clinical data of 372 patients with osteoporosis compression fractures admitted to Juxian People′s Hospital from January 2022 to July 2023, including 213 males and 159 females, and the age was (59.8±6.2) years old, aged range of 50 to 70 years old. 194 patients treated with unilateral puncture vertebroplasty were included in the unilateral group, and 178 patients treated with bilateral puncture vertebroplasty were included in the bilateral group. The intraoperative indicators, vertebral changes before surgery and 6 months after surgery, and the occurrence of adverse reactions after surgery of the two groups of patients were compared. The visual analogue scale (VAS), activities of daily living (ADL) score, oswestry disability index (ODI), and quality of life (QOL) scale were used to compare the pain degree, activity ability, improvement of functional disability, and changes in quality of life and clinical efficacy of the two groups of patients before the operation and at 3 and 6 months after the operation. SPSS 21.0 software was conducted to analyze data. Measurement data with normal distribution were represented as mean±standard deviation( ± s), and the comparison between groups was conducted using the t-test. The comparison of count data were represented as n(%) and was conducted by χ2 test or Fisher exact probability. Between-group comparisons of different time intervals were performed with repeated-measures analysis of variance test. Results:The surgical time, cement dosage, bleeding amount, and fluoroscopy of patients in the unilateral group were (29.86±5.87) min, (2.58±0.37) mL, (22.65±4.83) mL, and 12 times, respectively, and (42.63±4.55) min, (7.12±0.61) mL, (50.31±5.84) mL, 25 times in the bilateral group, respectively. The difference between the two groups was statistically significant ( P<0.05). Preoperatively, the anterior height ratio of the vertebral body, sagittal concavity angle, and kyphosis angle of the affected vertebra in the unilateral group were (59.83±5.55)%, 13.04°±1.14°, and 18.93°±2.56°, respectively, while in the bilateral group, there were (60.28±5.79)%, 13.08°±1.13°, and 18.67°±2.69°, respectively. After the operation, the unilateral group were (90.20±5.52), 3.95°±0.57°, and 11.03°±1.21°, respectively. The bilateral group were (90.58±6.00), 3.99°±0.59°, and 10.91°±1.14°, respectively. Comparison of data before and after surgery between the two groups, the difference was statistically significant ( P<0.05). The overall complication rate of unilateral group was 13 cases (6.70%), and the overall complication rate of bilateral group was 26 cases (14.61%). The difference between the two groups was statistically significant ( P=0.013). The VAS, ADL, ODI, and QOL of the unilateral group were (3.49±1.10), (66.10±13.02), (18.22±4.81) and (62.10±10.00) points, the two-sided groups were (5.49±1.13), (56.19±10.27), (24.76±5.81) and (52.13±9.56) points, respectively. The differences between the two groups were statistical significance ( P<0.05), the overall treatment efficiency of patients in the unilateral group was 90.72%, and the overall treatment efficiency of patients in the bilateral group was 83.15%, and the difference between the two groups was statistically significant( P<0.05). Conclusions:Unilateral puncture vertebroplasty has the advantages of short operation time, less bone cement usage, low blood loss, fewer fluoroscopy times, low complication rate, better postoperative pain relief and functional recovery, and high overall treatment efficiency in the treatment of patients with osteoporotic compression fractures. It has obvious advantages over bilateral puncture vertebroplasty.
2.Evaluation of a newly designed basket in non-radiation endoscopic lithotomy for common bile duct stones (with video)

Objective:To evaluate a newly designed basket fit for digital cholangioscope in non-radiation endoscopic lithotomy for common bile duct stones.Methods:Seventy-eight patients who underwent non-radiation endoscopic removal of common bile duct stones at Zhongda Hospital Southeast University between February and October 2021 were enrolled in the randomized controlled trial, and were randomly assigned into the new basket group ( n=35) and the conventional basket group ( n=43) by drawing lots. Procedures of cholangioscope-based biliary exploration and stone removal by baskets, volume of water irrigation, time lengths of biliary exploration, stone removal and whole endoscopic procedure, and adverse events were compared. Results:Compared with the conventional basket group, the new basket group needed less biliary explorations (1.08±0.28 VS 2.30±0.51, t=-12.535, P<0.001), smaller volume of water irrigation (46.14±11.89 mL VS 78.62±10.09 mL, t=-13.052, P<0.001), and shorter time of biliary exploration and stone removal (9.69±2.97 min VS 12.67±2.51 min, t=-4.815, P<0.001) and whole endoscopic procedure (30.17±7.19 min VS 33.44±6.69 min, t=-2.076, P=0.041). The incidence of adverse events was not significantly different between the two groups [5.7% (2/35) VS 11.6% (5/43), χ2=0.826, P=0.363]. Conclusion:This newly designed basket can facilitate digital cholangioscope-guided non-radiation endoscopic removal of common bile duct stones with the advantages of shorter operation time, less exploration time of choledochoscopy and smaller volume of water irrigation.
3. Application of meticulous anatomy skills with straight bipolar electric coagulation forceps in thyroid surgery

Objective:
To compare the efficacies of the two techniques of "micro-hemostasis and micro-cutting" with straight bipolar electrocoagulation forceps and traditional clamp-ligation for hemostasia in thyroid surgery.
Methods:
A total of 228 patients who underwent surgical treatment for thyroid neoplasms in our hospital between January 2015 and December 2018 were retrospectively analyzed, including 50 males and 178 females, aged 23-68 years old. Of those, 150 cases as electric knife group received traditional thyroid surgery between January 2015 and December 2018 and 78 cases as bipolar electrocoagulation group received thyroid surgery by using the technique of bipolar electrocoagulation with meticulous anatomy between January 2018 and December 2018. The total operation time, single operation time, intraoperative hemorrhage, postoperative drainage volume on the first day, postoperative hoarseness and hypocalcemia were compared between the two groups. SPSS 16.0 was used to analyze the data.
Results:
The total operation time and intraoperative hemorrhage in the bipolar electrocoagulation group were significantly lower than those in the electric knife group ((59.33±18.29)min
4.Expression and clinical significance of lncRNAANRILin glioma tissues

[Abstract] Objective: To investigate the expression and clinical significance of lncRNAANRIL in glioma tissues. Methods: 129 cases of glioma tissues and 25 cases of normal brain tissues as control were collected from patients treated in Sichuan Provincial People’ s Hospital from January 01, 2012 to December 30, 2016. Real-time quantitative PCR was used to detect the mRNA expression of lncRNAANRIL; and the relationship between lncRNAANRIL expression and sensitivity of patients to temozolomide as well as the clinical prognosis of glioma patients were analyzed. Results: Compared with control group, the expression of lncRNAANRIL in 129 cases of glioma tissues was significantly increased ([8.730±0.336] vs [1.090±0.137], t=9.957, P<0.01). The expression of lncRNAANRIL in WHO Ⅰ-Ⅱ grade patients was significantly lower than that of patients at grade Ⅲ-Ⅳ ([4.198±0.260] vs [10.550±0.291], t=13.03, P< 0.01). lncRNA ANRIL expression was significantly correlated with WHO stage,the sensitivity to temozolomide(TMZ)and survival status(all P<0.05), but not associated with gender, age, KPS score and tumor size (all P>0.05). Moreover, Kaplan-Meier analysis demonstrated that decreased lncRNAANRIL expression contributed to significantly longer overall survival ([29.17±0.64] vs [13.54±0.74] months, P<0.01) and recurrence-free survival time ([9.08±0.56] vs [15.88±0.83] months, P<0.01). Univariate and multivariate analysis also indicated that lncRNAANRIL expression, WHO stage and chemosensitivity could be independent prognostic markers for glioma (P<0.05). Conclusion: Higher pathological grade of glioma patients indicates higher lncRNA ANRIL expression and shorter survival time. lncRNAANRIL is involved in the occurrence and development of glioma, and can be used as a molecular marker for the diagnosis and prognosis of glioma.
5.Influence of parental compliance on therapeutic effect of children with epilepsy

Objective To investigate the influence of parental compliance on the therapeutic effect of children with epilepsy.Methods Two hundred and sixty children with epilepsy and their parents admitted to the First People's Hospital of Wenling from December 2013 to June 2016 were enrolled,and the classical Morisky medication adherence questionnaire (MMAS-8) was applied to evaluate the compliance of parents for treatment of their children with epilepsy;after the patient taking drug for 3 days,fasting venous blood was collected in the morning,the concentration of the blood drug was tested and the influence of parent compliance on the blood drug concentration of the child with epilepsy was evaluated.Results In 260 patients,122 cases took karma form,and 138 cases took valproate orally.There were parents with good medication compliance in 130 cases (50%),medium medication compliance 80 cases (30.76%) and poor adherence to the doctor order in 50 cases (19.23%).In cases using medication irregularly,there were 26 cases sometimes without taking any drug (10.0%),17 patients' medication being interrupted (6.54%) and 10 cases having excessive medication (3.85%);no relationships were found between parental compliance and each of the following items,family role,occupation and age (all P > 0.05);and the compliance was related to gender,indicating that women's good compliance level was higher than that of males';the education level was positively proportional to the compliance,and the compliance of parents with senior high school or above degree was higher than those with primary school and junior secondary school levels (83 cases vs.9 cases,38 cases,both P < 0.01).Under situation of parents with poor compliance,their children had blood drug concentration higher or lower than proper range of drug level (high in 22 cases,low in 41 cases,higher than the result in good compliance 0 cases and 17 cases respectively),thus seriously affected the safety and efficacy of the treatment;the patients' frequency of irregular medication in parents' good compliance group was significantly lower than that in parents' poor compliance group [3.08% (4/130) vs.72.0% (36/50),P < 0.05].Conclusion To improve the therapeutic effect of epileptic children,their parental good cooperation is necessary.
6.Relationship between TF,TFPI and severity in patients with diabetic cerebral in-farction

Objective:To explore the relationship between tissue factor (TF),tissue factor pathway inhibitor (TFPI) and the severity in patients with diabetic cerebral infarction.Methods: 226 patients with diabetic cerebral infarction were included into the study,National Institute of Health Stroke Scale ( NIHSS) was used to evaluate the severity of CIS.The single factor analysis and multiple regression analysis were used to explore the relationship between TF , TFPI and the severity.Results: The concentrations of TF,TFPI,TF/TFPI,cholesterol and triglyceride in the NIHSS≤12 group were significantly lower than that in the NIHSS>12 group ( P<0.05);the NIHSS was significantly positive correlate with TF (r=0.354,P=0.012),TFPI (r=0.302,P=0.027),TF/TFPI (r=0.410,P=0.000),cholesterol (r=0.364,P=0.006) and triglycerides (r=0.334,P=0.018);Multiple linear regression analysis showed that the TF , TFPI, TF/TFPI, cholesterol were independent risk factors of the severity in patients with diabetic cerebral infarction.Conclusion:The level of TF and TFPI could reflect the severity in patients with diabetic cerebral infarction according to the NIHSS.
7.Reforming the practice teaching of human anatomy

Objective To explore the practice teaching model of human anatomy through re-forming the practice teaching of system anatomy and regional anatomy. Methods Three classes of clinical medicine (5-year program) were randomly selected as reform experimental classes and the other 3 classes of the same grade were used as control classes. The teaching content of practice teach-ing was reformed in the experimental classes. In the system anatomy practice teaching, the teacher controlled the teaching process and developed the theme and requirements of each experimental class while the student completed the experiment developed by the teacher. In the regional anatomy practice teaching, students' interest in learning was stimulated by the content , which was related with the surgery, and the teacher guided students to complete the operation. In control classes, the forms of student observation as well as teacher answering questions and helping students were adopted. The student grading was divided into theoretical knowledge examination and specimen assessment and the achievement was analyzed by using a Simple Analysis of Variance (ANOVA) in SPSS 16.0. Results Compared with those of control classes, the scores of system anatomy and regional anatomy in experi-mental classes were significantly increased (80.7±5.8) vs. (67.4±17.4), (85.1±4.6) vs. (62.6± 12.4) and the theoretical scores of system anatomy were also improved significantly. But no significant difference in theoretical scores of regional anatomy was observed. Conclusions The teaching mode based on the practice teaching highlights the academic characteristics of the human anatomy, increas-es students' interest in learning and improves the effectiveness of practice teaching in human anatomy.

9.THE INFLUENCE OF JIAOGULAN EXTRACT ON PLATELET AGGREGATION AND ARACHIDONATE METABOLISM IN RABBITS

jiaogulan ( Gynostemma pentaphyllum) extract (0.25~2g/L. final concentration) obviously inhibited the platelet aggregation & TXB2 release induced by arachidonic acid ( AA ) in vitro, and the ID50 of inhibiting TXB2 release was 0.28g/L. With the dose of 35mg/kg i. v. , it evidently inhibited platelet aggregation at 10 & 20 min after injection, and also markedly decreased TXB2 released by platelets during 10~40 min after injection, especialy within 20 min. Jiaogulan extract (0.25~4g/L) did not influence 6-keto-PGF1? release of aorta thoracalis of rabbits in vitro. Enzyme immunoassay was used to measure the content of TXB2 & 6-keto-PGF1? in the experiments. The above results suggest that Jiaogulan extract may decrease TXA2/PGI2 ratio.
10.Regulatory Action of Astragalus Saponins and Buzhong Yiqi Compound on Synthesis of Nicotinic Acetylcholine Receptor Antibody in Vitro for Myasthenia Gravis

According to the differentiation of symptamcomplex theory of Spleen Deficiency on myasthenia grvis, with our clinical experience of treatment for some years. Astragalus Saponins and Buzhong Yiqi compound were selected to make phannaceutic experiment for peripheral blood mononuclear cells culture of 14 samples of 10 myasthenia gravis patients. Two controlled groups were given with the treatment of dexamethasone and 60Co respectively. The results showed that liters of nAchR-Ab in cells culture superatants were significantly decreased by watersoluble Astragalus saponias (418.8-2328 fmol/rnl- 0 fmol/ml in 6 cases; 1143 -1235 fmol/ml - 43-157 fmol/ml in 2 cases). Buzhong Yiqi compund could enhance the immunoregulatory action.
