1.Effectiveness of posterior single-level osteotomy with 360° release and correction in treatment of osteoporotic vertebral compression fractures with moderate to severe kyphosis.

OBJECTIVE:
To explore the effectiveness of posterior single-level osteotomy with 360° release and correction for the treatment of osteoporotic vertebral compression fractures (OVCF) complicated with moderate to severe kyphosis.
METHODS:
A retrospective analysis was conducted on 11 patients with OVCF complicated with moderate to severe kyphosis between January 2022 and March 2023. There were 4 males and 7 females with an average age of 57 years ranging from 47 to 69 years. The disease duration ranged from 3 to 15 months, with an average of 7 months. Fracture segments included T 11 in 3 cases, T 12 in 5, L 1 in 2, and L 2 in 1. The T value of lumbar spine bone density was -5.0 to -2.0, with an average of -3.5. The preoperative neurological function was grade E according to Frankel grading. The Pfirrmann classification of the intervertebral disc above the injured vertebra was grade Ⅲ in 8 cases and grade Ⅳ in 3 cases. All patients underwent posterior single-level osteotomy with 360° release and correction. The operation time, intraoperative blood loss, hospital stay, and postoperative complications were recorded. Thoracolumbar local kyphosis Cobb angle, the mean height of the functional spinal unit (FSU), the sagittal vertical axis (SVA), and the sagittal index (SI) were measured. The visual analogue scale (VAS) score and Oswestry disability index (ODI) were used to evaluate the improvement of pain and function before operation, at 1 month after operation, and at last follow-up.
RESULTS:
The operation successfully completed in all patients, and there was no obvious complication. The operation time ranged from 100 to 190 minutes, with an average of 153 minutes, and the intraoperative blood loss ranged from 200 to 800 mL, with an average of 468 mL. The length of hospital stay was 8-14 days (mean, 12 days). All patients were followed up 6-24 months, with an average of 12.4 months. At last follow-up, all the 11 patients had bony fusion in the osteotomy area, and there was no displacement or subsidence of the Cage, no complication such as internal fixation failure or pseudarthrosis formation was found. The Cobb angle of local thoracolumbar kyphosis, the mean height of FSU, SVA, and SI significantly improved immediately after operation and at last follow-up when compared with preoperative ones, and the VAS score and ODI also significantly improved at 1 month after operation and at last follow-up ( P<0.05); there was no significant difference in above indexes between the two time points after operation ( P>0.05).
CONCLUSION
Posterior single-level osteotomy with 360° release and correction is an effective surgical method for treating OVCF complicated with moderate to severe kyphosis, with definite early effectiveness.

2.Treatment of Pediatric Eczema in Cold Regions from the Theory of Latent Dryness

[Objective]Based on the theory of latent pathogen pathogenesis,to explore the etiology and pathological evolution of eczema in children from cold regions,providing insights for the treatment of pediatric eczema with traditional Chinese medicine(TCM).[Methods]Through a literature review of medical classics such as Huangdi Neijing,Shanghan Lun,Fuxie Xinshu and Linzheng Zhinan Yi'an,the etiology,pathogenesis and therapeutic methods of latent dryness contributing to pediatric eczema were analyzed.This analysis integrated the pathogenesis mechanisms of latent pathogens,climatic characteristics of cold regions and the constitutional traits of children in these areas.And a case was added for proof.[Results]The dry and cold regional climate and a constitution characterized by chronic Yin deficiency are critical factors in latent dryness-induced pediatric eczema.The formation of latent dryness arises either from lingering exogenous dryness pathogens or endogenous dryness transformation due to body fluid depletion,compounded by the body's failure to achieve timely recuperation.During acute exacerbation,treatment should focus on symptom management by using acrid-dispersing herbs that do not impair Yin.In the latent phase,therapy should target the root cause by strengthening the lung-defensive system,regulating constitution and harmonizing emotional states to eliminate latent dryness and maintain the robustness of healthy Qi.The attached test case was of wet sore(blood deficiency and wind dryness syndrome),which was treated by nourishing Yin and moistening dryness,to remove wind and stop itching,and achieved satisfactory curative effect.[Conclusion]Latent dryness is a significant etiological factor in pediatric eczema,influenced by climatic and constitutional elements.Treatment should prioritize symptom control during acute phases and emphasize reinforcing healthy Qi,cultivating primordial Qi,and expelling latent pathogens during remission to reduce recurrence.This approach aligns with TCM principles of addressing urgency versus chronicity and highlights the importance of region-specific and constitution-guided therapies.
3.Wang Youpeng's Experience in the Diagnosis and Treatment of Children with Henoch-Sch?nlein Purpura in Cold Regions

Henoch-Sch?nlein purpura(HSP)is a common vascular inflammatory disease in children,which is easy to recur and seriously affects the physical and mental health of children.Based on the cold climate and the characteristics of children's constitutions,Professor Wang Youpeng proposes that the pathogenesis is based on the invasion of external pathogens,and spleen deficiency and loss of circulation,yin-yang imbalance,and defensive qi instability are the key points of the pathogenesis.Professor Wang establishes the characteristic therapy of"three clear and one tonic",which includes the elimination and discharge method to clear dampness and heat,the dual solution method of external and internal to clear excessive heat,the method of cooling blood and dissipating blood stasis to clear stasis spots,and the method of strengthening the body and consolidating the essence to tonify deficiency.This therapy is based on"combining prescriptions to increase efficacy"and takes into account both deficiency in root and excess in superficiality.It not only focuses on the excessive syndromes of heat,blood stasis and dampness in the acute stage,but also focuses on the deficiency of healthy qi in the remission stage,which can provide references for the syndrome differentiation and treatment for HSP in children in cold regions.
4.Exploration on the Mechanism of Yizhu Wendan Decoction in Treating Eczema Based on GEO Database Combined with Network Pharmacology and Experimental Verification

Objective To explore the mechanism of Yizhu Wendan Decoction in treating eczema through GEO database combined with network pharmacology and experimental verification.Methods TCMSP,BATMAN-TCM and ETCM databases were used to screen the active components of Yizhu Wendan Decoction.Disease target information related to eczema was collected through GEO database.The drug-component-target network and PPI network were constructed by intersections of active component targets and disease targets.GO and KEGG pathway enrichment analyses were performed using DAVID database.CCK-8 method was used to screen out the optimal intervention concentration of freeze-dried powder of Yizhu Wendan Decoction.HaCaT cells were divided into control group,model group,Yizhu Wendan Decoction low concentration group,Yizhu Wendan Decoction high concentration group,si-IL-17RA group,si-IL-17RA+Yizhu Wendan Decoction low concentration group,si-IL-17RA+Yizhu Wendan Decoction high concentration group,Dexamethasone group,si-IL-17RA+Dexamethasone group.Each group was given relevant intervention.The expressions of chemokines and inflammatory factors were detected by qPCR.EdU and Annexin V-FITC/PI double staining were used to detect cell proliferation and apoptosis.Western blot was performed to detect the expressions of proteins related to apoptosis,skin barrier and IL-17 signaling pathway.Results By using databases,180 active components of Yizhu Wendan Decoction were obtained.Combined with GEO database microarrays related to eczema(GSE6012 and GSE57225),8 potential targets of Yizhu Wendan Decoction in the treatment of eczema were obtained.KEGG enrichment pathway mainly involved IL-17 signaling pathway,lipid and atherosclerotic,TNF signaling pathway,fluid shear stress and atherosclerotic,etc.When Yizhu Wendan Decoction freeze-dried powder concentration was 100 μg/mL,cell viability was the strongest.Yizhu Wendan Decoction could significantly inhibit the mRNA expressions of chemokines and inflammatory factors CCL17,CCL22,IL-1β,TNF-α,IL-6,IFN-γ,and increase the mRNA expression of IL-4 in eczema.It promoted the proliferation of HaCaT cells,increased the protein expression of Bcl-2,and reduced the protein expressions of Bad and Cleaved Caspase-3,thus inhibiting HaCaT cells apoptosis;promoted the protein expressions of FLG and LOR,and reduced the expression of MMP9,MMP1,CCL2,FOSL1,IL-17RA proteins in IL-17 signaling pathway.Conclusion Yizhu Wendan Decoction can treat eczema with multiple components,multiple pathways and multiple targets,promote the proliferation of HaCaT cells,inhibit their apoptosis,and restore the skin barrier.Its mechanism may be related to inhibiting the activation of IL-17 signaling pathway.
5.Serum proteomics analysis of pediatric corona virus disease 2019 with encephalopathy

Objective:To investigate the differences in protein profile expression in serum samples from children with corona virus disease 2019(COVID-19)related encephalopathy and to explore the underlying mechanisms.Methods:From December 1,2022 to January 31,2023,28 children with COVID-19 who were admitted to the Department of Pediatric Intensive Medicine at Shandong Provincial Hospital Affiliated to Shandong First Medical University were collected,including 21 patients with encephalopathy(COVID-19 with encephalopathy group) and seven patients without encephalopathy(COVID-19 without encephalopathy group).Three children from each group were selected for serum proteomic analysis using tandem mass spectrometry labeling proteomics technology.Proteins were considered significantly different if the fold change was >1.2 or <0.8,with P<0.05.Bioinformatics analysis,including Gene Ontology and Kyoto Encyclopedia of Genes and Genomes Pathway Enrichment were performed on differentially expressed proteins.Protein-protein interaction networks were analyzed using the STRING database.Selected proteins were further validated by enzyme-linked immunosorbert assay. Results:A total of 41 differentially expressed proteins were identified between the two groups.Among these,14 proteins were upregulated and 27 proteins were downregulated in COVID-19 patients with encephalopathy compared to those without encephalopathy.Bioinformatics analysis revealed that these proteins were primarily enriched in critical signaling pathways,including complement and coagulation regulation,neutrophil degranulation and activation,and platelet degranulation.Enzyme-linked immunosorbert assay validation confirmed significant differences in key coagulation-regulating proteins(von willebrand factor upregulated,serpin family F member 2 downregulated in COVID-19 patients with encephalopatly)between the two groups.Conclusion:Coagulation dysfunction may play a role in the development of COVID-19 associated encephalopathy in children,providing valuable insights for future research.

10.Application of extracorporeal membrane oxygenation therapy in acute poisoning

Severe acute poisoning can lead to respiratory and circulatory failure which can endanger the lives of patients,conventional treatment is limited,and the prognosis is poor.In recent years,extracorporeal membrane oxygenation has developed rapidly,which opens up a new way of treatment for acute poisoning.This paper reviewed the application of extracorporeal membrane oxygenation in various types of acute poisoning,in order to improve the awareness of clinical doctors on the application of this method and improve the success rate of treatment.
