Breast magnetic resonance imaging (MRI) is the most sensitive method available for detecting breast cancer, and its use in clinical practice is on the rise. Preoperative breast MRI plays a vital role in assessing the full extent of disease and identifying additional lesions in the contralateral breast that conventional imaging may overlook. These unexpected findings can lead to significant changes in treatment approaches and may ultimately affect long-term outcomes for patients. Despite this, the value of preoperative breast MRI for early-stage breast cancer patients remains a contentious issue. This review evaluates the existing literature on the influence of preoperative breast MRI on clinical outcomes in this patient population.

Purpose: In 2024, the South Korean government proposed an essential medical care package, including the expansion of medical college admissions, which faced strong opposition from the medical community. Consequently, residents resigned, increasing the workload of the remaining staff and putting public health at risk. This study analyzed changes in the hospitalization patterns of trauma patients during this political conflict period using single-center data from the National Health Insurance Service Ilsan Hospital. Methods: Data from the entire year 2023 and from March to August 2024 were used for analysis, excluding January and February 2024 when the conflict escalated. The selection criteria included patients with trauma who visited the emergency room, and comparisons were made between the periods before and during the medical conflict. Results: In 2023, the total number of patients with trauma were 1,182 (an average of 98.5 per month). From March to August 2024, the number dropped to 204 (34.2 per month), reflecting a significant reduction in the monthly average number of patients with trauma. Despite the overall decline, the number of severe trauma cases remained relatively stable, indicating a shift toward more critical patient care. Emergency department length of stay decreased significantly from 295 min in 2023 to 187 min in 2024. Conclusion The ongoing strain on emergency and support department risks leads to an irreversible collapse if critical points are reached. To address this, localized trauma centers are needed to relieve the burden on larger medical institutions and improve the efficiency of trauma care systems during crises.

Advancements in perioperative management and surgical techniques have led to an increased frequency of complex surgical procedures and emergency operations, particularly among elderly, and high-risk patients. Consequently, the incidence of severe complications such as intra-abdominal infections has risen, and in some cases, there is progression to refractory septic shock, a life-threatening condition unresponsive to typically effective source control and intensive treatments. Extracorporeal blood purification therapies, including CytoSorb, polymyxin B hemoperfusion (PMX-HP), and AN69-Oxiris, have emerged as adjunctive treatments for refractory septic shock. They remove excessive cytokines and circulating endotoxins, mitigating immune dysregulation, and improving outcomes. CytoSorb filters primarily remove cytokines based on molecular weight, whereas PMX-HP filters remove endotoxin.AN69-Oxiris combines cytokine and endotoxin removal with renal support functionality. Despite their promise, these modalities exhibit limitations such as cost considerations and variability in efficacy. PMX-HP demonstrates superior endotoxin clearance, making it preferable for severe endotoxemia, while AN69-Oxiris offers cost-effective solutions for mild endotoxemia, particularly in patients requiring renal replacement therapy. However, the absence of standardized protocols, and limited direct comparisons hinders widespread adoption. Evidence-based criteria and tailored strategies are essential to optimize the clinical application of blood purification therapies, and improve outcomes for patients with refractory septic shock.

Background and Objectives: Certain medications are associated with ototoxicity. This study assesses drug-induced ototoxicity in South Korea by analyzing the Korean national health data. Subjects and Methods: Hospital records of National Health Insurance members from 2009 to 2016 were reviewed. Data were compared between patients with and without hearing loss (HL). Individuals with HL were identified as having a primary diagnosis code for sensorineural HL or another type of HL in at least one outpatient or inpatient record according to the International Classification of Diseases-10. Results: The members in the HL group increased slightly from 0.8% to 1.0% relative to the total sample, compared with 99.2% to 99.0% among the controls. The proportion of males in the HL group ranged from 45.6% to 47.6%, compared with 48.4% to 48.8% among the controls. The proportion of those aged ≥65 years in the HL group increased from 34.1% to 41.4%, compared with 10.6% to 13.3% among the controls. Hypertension prevalence (24.7%-25.7%) in the HL group was higher than that in the control group (12%-12.6%). Diabetes prevalence in the HL group was 10.6%-12.3%, compared with 4.4%-5.9% among the controls. The use of proton pump inhibitor components increased, particularly esomeprazole magnesium trihydrate and rabeprazole sodium, whereas the usage of pantoprazole sodium sesquihydrate and revaprazan was high initially but declined subsequently. The usage of painkillers such as acetaminophen, loxoprofen sodium, and ibuprofen remained high, and antibiotics such as cephalosporins indicated the highest usage. However, the use of penicillin antibiotics such as amoxicillin decreased significantly. Anticancer agents showed relatively low usage compared with other drug categories, whereas antihistamines showed extremely high usage across all years, with a continual increase. Conclusions These correlations and the underlying mechanisms necessitate further investigation, as several medicines have been linked to an increased risk of HL.

Background and Objectives: This study aimed to investigate the potential impact of earphone use on the occurrence of otitis externa (OE) by conducting a survey. Subjects and Methods: This study included 517 participants. The questionnaire comprised 22 items divided into three categories; general information, earphone use, and OE. The questionnaire was designed using Google Forms and distributed through a notice posted on a hospital bulletin board. This notice included a QR code that enabled any interested individual to participate, and the responses were collected automatically. Results: The proportion of respondents who had experienced OE at least once was 21%. Among them, 33.7% experienced OE within the preceding month. For “adhering to the recommendation usage time,” individuals who responded “likely” had a 0.440 (odds ratio=0.440, 95% confidence interval=0.195-0.993) times higher likelihood; they were at lower risk of experiencing OE compared to those who responded “very unlikely.” Conclusions Our findings suggest that proper earphone use can reduce the risk of OE. Despite the lack of a significant relationship between earphone type, physical activity, and cleaning practices, continued adherence to these factors is recommended to prevent OE.

Background and Objectives: This study aimed to investigate the effects of hearing aids (HAs) on reducing symptoms of dizziness in patients with hearing loss (HL) and dizziness. Subjects and Methods: A pre- and post-intervention study was conducted between February 2022 and July 2022 at the outpatient clinic of a tertiary hospital in Seoul, South Korea. Ten participants (four males and six females) with HL and dizziness were recruited. Participants were screened using pure-tone audiometry and visual analog scale scores at baseline. Selected participants underwent the Modified Clinical Test of Sensory Interaction on Balance (mCTSIB) and Korean Dizziness Handicap Inventory (K-DHI) in the unaided state. Thereafter, participants were provided HAs and instructed to use them for at least 6 hours every day. At the 1-month follow-up, all participants were assessed in the aided state using the same tests as at baseline. All tests were performed bilaterally. Results: In the mCTSIB, a significant difference was observed in the foam surface-eyes closed score before (median=2.35) and after (median=2.2) HA use (p=0.049). No significant differences were observed in the K-DHI scores before and after HA use. Conclusions HA use may improve the static and subjective perception of balance in patients with HL and dizziness. Future research should explore the benefits of HAs under various balance conditions such as dynamic balance and gait.

Acute facial nerve palsy, particularly Bell’s palsy, is a common neurological disorder with an annual incidence of 20-30 cases per 100,000 individuals. It is characterized by sudden or gradual facial muscle palsy and is caused by viral reactivation, inflammation, or ischemia of the facial nerve. Prognosis varies widely, depending on the severity of nerve damage and timeliness of the intervention. Steroid therapy remains the cornerstone of Bell’s palsy treatment because it reduces inflammation and facilitates recovery. Early administration, preferably within 72 hours of symptom onset, considerably improves outcomes. However, the efficacy of combination therapy remains controversial. Current guidelines recommend oral steroids as the primary treatment for Bell’s palsy and suggest the selective use of antiviral agents in severe cases or when viral involvement is strongly suspected. For severe facial palsy, such as Ramsay Hunt syndrome or varicella-zoster virus-induced cases, combination therapy may improve outcomes and reduce sequelae; however, high-quality evidence is limited. Steroid therapy is the main treatment of Bell’s palsy and antiviral therapy can be added in severe cases to improve prognosis. Additional research is required to develop standardized guidelines, concerning the use of antiviral therapies in conjunction with steroids.

Background: Osteoporosis is a significant public health issue in aging populations. Despite advances in pharmacotherapy, underdiagnosis and undertreatment remain prevalent even in patients with recent fractures. This study examined 20-year trends (2002– 2022) in anti-osteoporotic medication (AOM) usage in South Korea. Methods: Data from the Korean National Health Information Database were retrospectively analyzed. The study included individuals aged ≥50 years and analyzed prescription trends, medication adherence, measured by the medication possession ratio (MPR), and treatment initiation rates post-fracture. The AOMs examined included bisphosphonates, selective estrogen receptor modulators, denosumab (DMAB), teriparatide (TPTD), and romosozumab (ROMO). Results: Over two decades, AOM use has shifted significantly from oral to injectable formulations, with injectables surpassing oral medications in 2020; from 397,440 prescriptions in 2016 to 1,162,779 in 2022. Prescriptions for DMAB surged following its approval as a first-line therapy in 2019, increasing 2.65 times from 217,606 in 2019 to 575,595 in 2022. The MPR improved from 35.4% in 2003 to 73.2% in 2021, with females demonstrating higher adherence than males. Post-fracture treatment rates increased from 31.1% in 2006 to 39.9% in 2021 but remained below 50%. Vertebral fractures had the highest treatment initiation rates, while anabolic agents, such as TPTD and ROMO were underprescribed despite their efficacy. Conclusions This 20-year analysis highlights significant progress in osteoporosis management in South Korea, including a shift towards injectable therapies and improved adherence. However, the persistent undertreatment of high-risk patients underscores the requirement for enhanced access to anabolic agents, clinician education, and policy reforms to optimize post-fracture care.

Purpose: The incidence of breast cancer in older females is increasing with increased life expectancy. This study analyzed tumor characteristics and oncological outcomes in patients aged ≥ 70 years compared to patients in a younger postmenopausal group, in conjunction with their adherence to treatment guidelines. Methods: Patients aged ≥ 50 years, newly diagnosed with breast cancer, were divided into two age categories: ≥ 70 years and 50–69 years. All patients underwent curative surgery at Korea University Guro Hospital between January 2009 and December 2019. Clinical data on tumor subtype, histopathological grade, and clinical stage, along with treatment details were collected. Disease-free survival, distant recurrence-free survival, and breast cancer-specific survival rates were determined. Results: Of 1,199 patients, 166 (13.8%) were ≥ 70 years at the time of surgery. The diseasefree, distant recurrence-free, and breast cancer-specific survival rates were significantly lower in patients aged ≥ 70 years (p < 0.05). In a subgroup analysis, human epidermal growth factor receptor 2-positive tumors were the only subtype with a statistically significant difference in survival outcomes, and adherence to the guidelines was strongly linked to a better prognosis. Conclusion Patients aged ≥ 70 years had lower disease-free, distant recurrence-free, and breast cancer-specific survival rates compared to younger postmenopausal patients aged 50–69 years. With the continuous increase in life expectancy and advances in healthcare, it is critical to optimize treatment strategies for older patients with breast cancer to improve survival outcomes and enhance their quality of life.

Purpose: Patients undergoing breast surgery may experience chronic postoperative pain in the breasts, upper extremities, and axillary regions, and no established methods for preventing this pain are available at present. This study aimed to investigate whether coaptation of the transected intercostal nerve can prevent the development of neuropathic and chronic breast pain after mastectomy in implant-based breast reconstruction. Methods: A prospective, double-blind, randomized controlled trial was conducted by dividing patients who underwent implant-based breast reconstruction after mastectomy into a control group without nerve coaptation and an experimental group with nerve coaptation.Patient clinical information was collected, and a survey using the pain and quality of life scale was conducted at 6 and 12 months after surgery. Results: Fifteen patients completed the study, including seven in the control group and eight in the experimental group. The two groups showed no significant differences in terms of clinical factors. The experimental group exhibited lower Short-Form McGill Pain Questionnaire scores than the control group at 6 and 12 months postoperatively, with a statistically significant difference at 6 months. Numerical Rating Scale and Present Pain Intensity scores for both groups were in the “no to mild” range throughout the study period, with no statistically significant differences between the groups. Although the difference in the BREAST-Q™ results did not reach statistical significance, the experimental group showed an improvement in the quality of life. Conclusion Intercostal nerve coaptation after mastectomy in implant-based breast reconstruction may facilitate initial nerve recovery. Although trial results are needed to fully determine the clinical impact, our findings support the ongoing scientific and clinical efforts to use this technique.
