1.A Case of Acquired Zinc Deficiency in a Patient with Ischemic Colitis.

Zinc deficiency is characterized by acrodermatitis enteropathica like eruption such as periorificial dermatitis, diarrhea and mental irritability. This syndrome occurs due to decreased supplementation, increased consumption or decreased bowel absorption of zinc. We report here on a case of acquired zinc deficiency in a patient who was receiving total parenteral nutrition due to ischemic colitis. She showed denuded patches on the perinasal, perioral and perineal area and tense bullae on both hands. She was suffering from diarrhea and mental irritability. Her blood zinc level was 4.9microng/dl. Subsequent intravenous zinc supplementation cleared up her clinical manifestations.

2.Nevus Sebaceus Associated with Secondary Tumors.

BACKGROUND: Frequent association of nevus sebaceus with other skin appendage tumors and with apocrine glands suggests that the nevus sebaceus may be derived from the primary epithelial germ. Syringocystadenoma papilliferum was found most frequently in the nevus sebaceus. A number of cases have been reported, but extensive studies on the secondary tumors of nevus sebaceus are rare in Korea. OBJECTIVES: The purpose of this study was to investigate characteristic clinical features in Korean patients with the secondary tumors of nevus sebaceus. MATERIALS & METHODS: Enrolled were total 136 patients confirmed as nevus sebaceus at the Chungnam National University Hospital, the Dankook University Hospital, and the Chungbuk National University Hospital from March 1993 to February 2003 for this study. And we reviewed the Korean Journal of Dermatology and the Annals of Dermatology for nevus sebaceus with secondary tumors. RESULT: The results were as follows; Total 45 cases were included from 23 males and 22 females. The male/female ratio was similar. Trichoblastoma(86% in female) and basal cell carcinoma(67% in female) were more frequent in female than in male. Mean age was 38.7 years(range 1.5 years to 71 years). All malignant tumors except basal cell carcinoma developed beyond 40 years of age. In the malignant tumors, 19(95%) of 20 cases developed in the scalp. However, in the benign tumors, 44(86.3%) of 51 cases developed in the scalp and 7(13.7%) of 51 cases in the face. Tumors of one type developed in 29 cases and tumors of more than two types developed in 16 cases. The total seventy eight tumors were composed of 23 malignant tumors and 55 benign tumors. Basal cell carcinoma and syringocystadenoma papilliferum were the most common tumor in each group respectively. Although trichoblastoma was not counted as the most common tumor, we suggest that it might be the most frequent tumor in secondary tumors associated with nevus sebaceus because differential diagnosis between trichoblastoma and basal cell carcinoma was so difficult that many cases of trichoblastoma were misdiagnosed as basal cell carcinoma.

4.Immunohistochemical Analysis for Basal Activation of NF-κB in Acral Lentiginous Melanoma.

BACKGROUND: Spontaneous basal activation that might be related to survival mechanism of tumor cells by allowing them to escape from apoptosis has been proven in some tumor cells, but it has not been evaluated in malignant melanoma tissue. OBJECTIVE: The purpose of this study was to evaluate basal activation of NF-κB using immunohistochemical analysis and demonstrate its clinical significance in cutaneous malignant melanoma tissue. MATERIALS AND METHODS: Twenty-five cases of acral lentiginous melanoma(ALM) from 20 patients were selected. Immunohistochemical analysis was performed to detect nuclear localization of classic NF-κB heterodimer, p50 and p65 in the formalin-fixed, paraffin-embedded tissues. RESULTS: In about 50% of cases, nuclear expression of NF-κB heterdimer, p50 and p65 was detected, 12(48%) and 13(52%) of 25 cases of ALM, respectively. However, the nuclear ex-pressing of p50 and p65 was not significant for tumor thickness or level of invasion in ALM. CONCLUSION: We demonstrated basal NF-κB activation in malignant melanoma, but we need to research further to demonstrate its clinical significance.

5.A Case of Adams-Oliver Syndrome which was Observed in Two Brothers.

Adams-Oliver syndrome is characterized by the presence of scalp aplasia cutis congenita with distal limb anomalies. Cutis mammorata and dilated scalp veins are additional frequent manifestations of the condition. In almost all reported patients with Adams- Oliver syndrome, the trait of inheritance was autosomal dominant. Only three reports suggested autosomal recessive inheritance characterized by multiple affected offsprings in unaffected parents. But a number of sporadic cases have been described. We have experienced a boy with this syndrome, who showed large congenital scalp defect with exposed dural membrane and bony defect beneath it, cutis mammorata on whole body, equinovarus, and no metatarsal ossification center of left foot. His brother also had these symptoms, but their parents were not affected. He was complicated by bacterial meningitis, and cured with antibiotics. The skin defect was closed spontaneously with atrophic scar. We report this case with the brief review of literature.

6.One Case of Neurofibromatosis Showing Elephantiasis Neurofibromatosa Treated by Surgical excision.

We present herein case of allergic contact dermatitis associated with implants. A 30 year-old male patient has had oozing, crusted, eczematous lesion on the right anterior tibia for five months after tibial osteosynthesis with plate and screw made of vitalliurn. He showed positive patch test, reaction to cobalt on second and fourth days. The skin lesion disappeared after removal of T-plate.

9.A Case of Dysplastic Melanocytic Nevus.

The dysplasitc melanocytic nevus(DMN) of the skin is an atypical-appearing melanocytic tumor characterized by intraepidermal atypical melanocytic preliferation. Clinically, DMN has a diameter greater than 5mm, ill-defined irregular borders, irregularly distributed pigmentation surrounded by erythems,. Histopat- hologically, there are numerous discrete nests of atypical melanocytes situated at the dermoepidermal junction of elongated rete ridges. In the dermis, concentric lamellar fibroplasia, lymphocytic infiltrates and neovascularization are noted. We report herein a case of DMN developed on the trunk of 31-year-old Korean male.
