1.Comparison of the therapeutic effects of defocus incorporated multiple segments with orthokeratology lenses on controlling axial length growth of myopia in adolescents

AIM: To observe and analyze the clinical effect of defocus incorporated multiple segments(DIMS)and orthokeratology lenses on controlling axial length(AL)growth of myopia in adolescents.METHODS: A retrospective study was conducted on 120 adolescent myopia patients(120 eyes)who visited our hospital from April 2021 to April 2023(using data from the main eye for study). According to the patient's preference, they were divided into DIMS group and orthokeratology group, with 60 cases(60 eyes)in each group. Followed-up for 1 a, the changes in spherical equivalent refraction(SER)and AL before and after wearing lenses were measured.RESULTS: The general data of adolescents in the two groups were comparable. There were significant differences between two groups in SER before and after wearing glasses(Ftime=123.700, Ptime<0.001; Fgroup=7.499, Pgroup=0.007; Finteraction=3.580, Pinteraction=0.029). The difference of AL between the two groups in comparison of time and interaction before and after wearing glasses was statistically significant(Ftime=40.100, Ptime<0.001; Finteraction=5.830, Pinteraction=0.003), there was no statistically significant difference between groups(Fgroup=0.008, Pgroup=0.927). After wearing lenses for 6 mo and 1 a, there was a statistical difference between the two groups in the changes of SER and AL(all P<0.01). Among them, the changes in SER and AL in the orthokeratology lenses group were smaller than those in the DIMS group.CONCLUSION: The orthokeratology lenses have a better effect than the DIMS on controlling the AL growth in myopic adolescents.
2.Isolation and application value of Yersinia enterocolitica phage strain in Yunnan Province

Objective:To isolate Yersinia enterocolitica phage and study its biological and genomic characteristics, and explore its application value. Methods:Phages were isolated from cecum of Apodemus chevrieri in the plague focus of wild rodent in Lijiang employing Yersinia enterocolitica (CMCC52301) as the host strain. After purification, proliferation and concentration, the morphology was observed by transmission electron microscope, and the host spectrum of the phage was determined by double-layer plate method. Phage DNA was extracted and sequenced, and the raw sequences data were assembled and visually analyzed, also performed genome functional annotation, phage classification status, phylogenetic tree, genome collinearity analysis, etc. Results:One strain of Yersinia enterocolitica phage was isolated and named vB_Yen_YN301-151, which had a head and a contractile tail. The phage only lysed Yersinia enterocolitica, and could not lyse other tested strains. And at a lysis temperature of 37 ℃, more transparent plaques were observed compared to at a lysis temperature of 21 ℃. The gene structure of this phage was double-stranded DNA, with a genome length of 51 176 bp and a GC content of 46.96%, and 95 genes were predicted to encode 15 structural morphological related proteins, 9 proteins involved in replication and metabolism, 3 DNA assembly functional proteins, 1 phage lysis related protein, and 67 hypothetical proteins. It was identified that the phage belong to the Drexlerviridae family of the Caudoviricetes class, clustered with vB_YenM_534, vB_Yen_X1, vB_YenM_281, and vB_YenM_531 in the phylogenetic tree and exhibited good collinearity with PY100, vB_YenM_ 25, and vB_Yen_X1. Conclusions:The successfully isolated vB_Yen_YN301-151 is a novel phage belonging to the Drexlerviridae family, and its gene encoded proteins are mostly hypothetical proteins. It only lyses Yersinia enterocolitica, with host specificity and has the potential to be used as diagnostic phage for screening Yersinia enterocolitica.
3.Characteristics of cognitive empathy and affective empathy in pupils with high levels of autistic traits

6.Correlation of the steady-state minimal concentration with AUC24/MIC of vancomycin and analysis of risk factors for treatment failure in pediatric patients

OBJECTIVE To assess the correlation between the steady-state minimal concentration (cmin) and 24 h area under the drug concentration-time curve (AUC24)/minimal inhibitory concentration (MIC) ratio (AUC24/MIC) of vancomycin in pediatric patients, and analyze independent risk factors for treatment failure. METHODS Data of hospitalized children treated with vancomycin and receiving therapeutic drug monitoring in our hospital from January 2021 to July 2024 were retrospectively collected and divided into success group and failure group according to whether the treatment was successful or not. Spearman correlation analysis was used to analyze the correlation between cmin and AUC24/MIC of vancomycin, and one-way and multifactorial Logistic regression analyses were used to screen the independent risk factors for vancomycin treatment failure. RESULTS A total of 59 children were included, with 41 in the success group and 18 in the failure group. Compared with the failure group, AUC24/MIC of vancomycin was significantly higher in the success group (P=0.038), but there was no statistically significant difference in the cmin of the two groups (P>0.05); cmin of vancomycin was significantly positively correlated with AUC24/MIC (r=0.499, P<0.001), but it has a certain efficacy in predicting the achievement of the AUC24/MIC standard (≥400) (area under the receiver operator characteristic curve=0.696), with an optimal cutoff value of 6.05 mg/L determined by the Youden index. The efficacy of AUC24/ MIC in predicting treatment failure was superior to cmin (areas under the receiver operator characteristic curve were 0.671 vs. 0.523, P were 0.038 vs. 0.684), with higher sensitivity (83.3% vs. 66.7%). Hypoproteinemia and AUC24/MIC≤369.1 were independent risk factors for vancomycin treatment failure (P<0.05). The incidence of nephrotoxicity was 3.4%. CONCLUSIONS There is a significant positive correlation between cmin and AUC24/MIC of vancomycin in pediatric patients; hypoproteinemia and AUC24/MIC≤369.1 are independent risk factors for vancomycin treatment failure in children.
7.Auricular electroacupuncture for post-stroke dysphagia in pharyngeal phase: a randomized controlled trial.

OBJECTIVE:
To observe the clinical efficacy of auricular electroacupuncture for post-stroke dysphagia in the pharyngeal phase.
METHODS:
Eighty-two patients with post-stroke dysphagia in the pharyngeal phase were randomized into an auricular electroacupuncture group (41 cases) and a swallowing electrical stimulation group (41 cases, 1 case dropped out). In the auricular electroacupuncture group, electroacupuncture was applied at auricular points, i.e. Xin (CO15) and Yanhou (TG3), using disperse-dense wave, in frequency of 2 Hz/10 Hz, 30 min a time. In the swallowing electrical stimulation group, swallowing electrical stimulation was delivered for 30 min a time. Both groups were treated once daily for 4 weeks. The functional oral intake scale (FOIS) grade, as well as the hyolaryngeal complex displacement, the pharyngeal constriction rate (PCR) and the pharyngeal delay time (PDT) under video fluoroscopic study of swallowing (VFSS) were observed before and after treatment, and the clinical efficacy was evaluated in the two groups.
RESULTS:
Compared before treatment, the FOIS grade was improved (P<0.01), the forward and upward displacement amplitude of hyoid bone and thyroid cartilage was increased (P<0.05), and the PCR and PDT were decreased (P<0.05) after treatment in the two groups. After treatment, compared with the swallowing electrical stimulation group, the FOIS grade was superior (P<0.01), the upward displacement amplitude of hyoid bone and thyroid cartilage was larger (P<0.05) and the PCR and PDT were lower (P<0.05) in the auricular electroacupuncture group. The total effective rate was 85.4% (35/41) in the auricular electroacupuncture group, which was higher than 62.5% (25/40) in the swallowing electrical stimulation group (P<0.05).
CONCLUSION
Auricular electroacupuncture can effectively trigger pharyngeal initiation and improve post-stroke dysphagia in the pharyngeal phase.

8.Construction and practice of drug traceability code management system in the outpatient pharmacy of a children’s hospital

OBJECTIVE To investigate the construction and practice of a drug traceability code management system in pediatric hospitals, providing a reference for promoting drug traceability code collection in healthcare institutions. METHODS Taking the outpatient pharmacy of our hospital as the research subject, a drug traceability code management system was constructed through the upgrade of the hospital information system (HIS), process optimization, and human-machine collaboration mechanism. The PDCA (plan-do-check-act) cycle management method was applied to continuously optimize this system. Based on operational data from March 2024 to February 2025, the changes in the collection rate of drug traceability codes were analyzed, and the differences in the average patient pickup time, the average pharmacist dispensing time, and the dispensing error rate were compared before and after the implementation of the system. RESULTS In the initial period of trial operation of the drug traceability code management system(June 2024), the collection rate of drug traceability codes was 57.17%, which subsequently improved to 93.52% by February 2025 following process optimization. Compared with the pre-implementation period (March-May 2024), there was no significant difference (P>0.05) in the average patient pickup time during the stable run-in period (August-October 2024); the overall average pharmacist dispensing time increased significantly (P<0.001), but the clinical significance of this increase (0.42 s) was limited; stratified analyses showed a significant increase in the average pharmacist dispensing time for prescriptions involving chronic disease multidrug combinations ([ 23.29±6.83) s vs. (17.87±3.64 ) s, P<0.001]; the dispensing error rate was reduced from 0.13‰ to 0.03‰ (P=0.038). CONCLUSIONS By adopting the strategy of “system reconstruction-process reengineering-human-machine collaboration”, our hospital has successfully established a drug traceability code management system. While complying with national regulatory requirements, we have maintained service efficiency and reduced the medication dispensing error rate.
9.Measurement and evaluation of personal attenuation rating before and after the training of the noise-exposed workers wearing foam earplugs

Objective:To measure and compare the difference of personal attenuation rating (PAR) of the workers wearing foam earplugs before and after the training, and to evaluate the effect of ear protector wearing training on the noise protection.Methods:In February 2023, 94 workers exposed to noise in a machinery manufacturing factory were selected as subjects. The production noise in the workplace was measured and subjects were trained to wear earplugs. The PAR values of wearing 3M 1110 foam test earplugs were measured and recorded before and after the training by using the fit testing of hearing protection device. The differences between the actual PAR values with nominal values and the noise attenuation values in related standards were compared, and the protective effect of hearing protection device before and after training was evaluated.Results:The average age of the subjects was (36.76±11.48) years old, the average length of service was (16.34±11.64) years, and the average exposure time to noise was (15.67±11.64) years. The noise detection results of the subjects' posts were ranged from 80.1 to 94.3 dB (A). The results of subjects wore 3M 1110 foam test earplugs for fit testing showed that the binaural PAR value after training was (19.3±6.4) dB (A), which was significantly higher than that before training (11.1±7.4) dB (A) ( t=13.31, P<0.001). After training, 11 people (11.70%) could reach the corrected noise reduction value (NRR value), 26 people (27.66%) could reach the standard of single noise reduction value (SNR value) ×0.6, and 84 people (89.36%) could reach the standard of (NRR-7) /2. The under protection rate of hearing protectors after training (7.45%) was significantly lower than that before training (45.74%), and the difference of different protection levels before and after training was statistically significant (χ 2=40.83, P<0.001) . Conclusion:It is suggested that enterprises should use the fit testing instead of nominal value estimation to evaluate the noise reduction effect of hearing protection device. Special training on the selection and use of hearing protection device should be strengthened, so as to ensure that workers wear them correctly and improve the protective effect of hearing protection device.
