1.Application of metabolomics in anxiety disorders

Anxiety disorders are psychiatric conditions characterized by high prevalence,chronicity,and comorbidity.The World Health Organization has listed anxiety disorders as a cause of health-related disabilities.Metabolomics can be used to qualitatively and quantita-tively analyze metabolites in organisms using modern analytical techniques,comprehensively revealing the overall characteristics of meta-bolic changes in the body and providing novel insights into diseases.It is important to identify potential biomarkers associated with anxiety disorders by investigating perturbations in metabolites and metabolic pathways in patients with anxiety disorders.This review addresses the application of metabolomics in anxiety disorders to provide a basis for revealing their pathogenesis,early diagnosis,and efficacy evalu-ation.
2.Meta-analysis of external stent fixation and internal plate fixation for treatment of comminuted distal radius fractures

OBJECTIVE:External fixators and plate internal fixation are commonly used treatments for comminuted distal radius fractures,each with its own advantages and disadvantages in clinical practice. To systematically evaluate the clinical efficacy and safety of external fixator and plate internal fixation in the treatment of comminuted distal radius fractures,and to provide a basis for the development of guidelines for the diagnosis and treatment of distal radius fractures with integrated traditional Chinese and Western medicine. METHODS:PubMed,Web of Science,Embase,Cochrane Library,China National Knowledge Infrastructure,China Biomedical Literature Database,VIP,andWanFang databases were systematically searched to include randomized controlled trials on external fixators and internal plate fixation for the treatment of comminuted distal radial fractures published from October 2013 to October 2023. The literature was screened according to the inclusion and exclusion criteria. Review Manager was used for literature quality evaluation and meta-analysis.RESULTS:(1) Eight articles were included,including 4 in Chinese and 4 in English,with a total sample size of 648 cases,including 328 cases in the external fixation stent group and 320 cases in the internal fixation plate group. (2) At 3 months after operation,the internal fixation plate group was superior to the external fixation stent group in the range of dorsal extension,palmar flexion and supination. At 12 months after operation,the grip strength,palmar inclination,palmar flexion,pronation and supination in the internal fixation plate group were better than those in the external fixation stent group. The postoperative infection in the internal fixation plate group was better than that in the external fixation stent group,and there was no statistical difference in other outcome indicators.CONCLUSION:Eight evidences showed that in the choice of treatment for comminuted distal radius fracture,both external fixation stent and incision plate internal fixation had good therapeutic effect,and plate internal fixation was better than other factors. However,for some special patients with highly severe comminuted distal radius fractures,poor bone quality,severely contaminated open fractures,and soft tissue swelling that did not allow incision surgery,external fixation was the first choice. The results of this study have limitations,and more high-quality,large-sample,multi-center randomized controlled trials are needed in the future,emphasizing the observation of long-term efficacy and other secondary indicators,and supplementing and optimizing the current research results.
3.Study on protective effect of medical X-ray diagnosis equipment based on risk analysis of correlation

Objective:To formulate refined management strategies to improve the operation quality and protective effect of medical X-ray diagnosis equipment based on the risk analysis of the correlation of medical X-ray diagnostic equipment.Methods:The used 41 medical X-ray diagnosis equipment in Fuwai Hospital,Chinese Academy of Medical Sciences from January 2022 to December 2023 were selected.According to different management modes for equipment,they were managed respectively by the conventional management mode(20 units)and the management mode with risk analysis of the correlation(21 units).The risk grades of correlation of management mode with risk analysis of the correlation were classified on the basis of the probability of risk and the degree of harm in medical X-ray diagnosis equipment.The protection management of equipment was conducted from three aspects included the location of equipment,the staffs and subjects.A self-made questionnaire was used to investigate the satisfactions of engineers of using management for equipment,operators,and staffs of operating and maintaining equipment for the two kinds of management modes.The protection quality,failure rate of equipment,and qualification rate of protection of medical X-ray diagnosis equipment under different management modes were compared.Results:The quality scores of operation safety,regular inspection,maintenance and repair,and shielding protection for radiation of medical X-ray diagnosis equipment that adopted the management mode with risk analysis of the correlation were respectively(92.14±3.36),(91.47±2.41),(92.54±3.32)and(91.69±3.57)points,all of which were higher than those of the conventional management mode,and the differences of them were statistically significant(t=16.396,17.368,19.688,14.401,P<0.05).The failure rate of the 21 medical X-ray diagnosis equipment that adopted the management mode with risk analysis of correlation was 0,which was lower than 20.00%of the conventional management mode.The qualified rate of protection was 95.23%,which was higher than 70.00%of the conventional management mode,and the differences of them were statistically significant(x2=4.654,4.608,P<0.05).The satisfaction scores of engineers of using equipment,operators and staffs of operating and maintaining equipment for the protective effect of adopting the management mode with risk analysis of correlation were respectively(94.69±4.02),(93.21±4.36)and(92.54±3.47)points,all of which were higher than those of the conventional management mode,and the differences were statistically significant(t=16.902,11.291,12.020,P<0.05).Conclusion:The process-based protection management strategy on the basis of risk analysis of correlation for medical X-ray diagnosis equipment can prevent risks of medical X-ray diagnostic equipment,and improve the operation quality of the equipment,and reduce the operation risks of the equipment,and maximize the enhancement for management quality of medical X-ray diagnosis equipment.

7.Research progress on cognitive dysfunction and brain gray matter areas changes in patients with obstructive sleep apnea hypopnea syndrome

Obstructive sleep apnea hypopnea syndrome (OSAHS) is a sleep breathing disorder caused by obstruction of the upper airway during sleep from various causes. At present, the diagnosis and treatment of OSAHS are insufficient. OSAHS causes cognitive decline due to excessive oxidative stress and inflammatory response caused by sleep breathing disorder, and its alteration of the brain gray matter area may be related to cognitive dysfunction. This review investigates the correlation between cognitive dysfunction and brain gray matter areas changes in OSAHS, and elucidates the underlying mechanisms, which provide a theoretical basis for early clinical diagnosis and treatment.
8.Research progress of white matter lesions in patients with obstructive sleep apnea hypopnea syndrome

White matter lesions in patients with obstructive sleep apnea hypopnea syndrome (OSAHS) are common brain microstructural changes, the mechanism of which is still not clear, including decreased cerebral perfusion, oxidative stress, inflammatory damage, etc. At present, white matter lesions are mainly evaluated by magnetic resonance imaging. White matter lesions in patients with OSAHS are often manifested as cognitive dysfunction such as inattention, decreased executive ability and memory loss. Continuous positive airway pressure can relieve the white matter lesions and improve the cognitive function of some patients with OSAHS. Further study on the pathogenesis and early imaging characteristics of OSAHS white matter lesions is expected to provide targets and evidence for early intervention.
9.Deep transcranial magnetic stimulation based on H-coil in nervous system diseases

Deep transcranial magnetic stimulation (DTMS) is a novel non-invasive neural regulation technology developed on the basis of repetitive transcranial magnetic stimulation; it adopts the Hesed coil (H-coil), which can stimulate wider range of cerebral cortex and deeper brain regions without increasing the stimulation intensity; Currently, it has been applied to the treatment of depression and other mental diseases. In this paper, we review the research status of DTMS in Parkinson's disease, Alzheimer's disease, motor dysfunction after stroke, and aphasia.
10.Clinical effect and electroencephalographic oscillation changes of the repetitive transcranial magnetic stimulation and eszopiclone treatment of chronic insomnia disorder

Objective:To observe the clinical efficacy of repetitive transcranial magnetic stimulation (rTMS) combined with eszopiclone in the treatment of chronic insomnia disorder and its influence on brain electrical activity.Methods:Ninety patients with chronic insomnia were randomly divided into rTMS group, drug group and combination group, with 30 cases each group. The rTMS group was treated with bilateral dorsolateral prefrontal lobe (left 5 Hz 400 pulse, right 1 Hz 1 200 pulse), the drug group was treated with eszopiclone (3mg/d) and the combination group was treated with rTMS + eszopiclone. Continuous treatment for 2 weeks, Pittsburgh sleep quality index (PSQI), Hamilton Anxiety Scale (HAMA), Hamilton Depression Scale (HAMD) and changes in brain electricity activity (α, β, θ, δ) and sleep parameters were evaluated before treatment, after treatment and during follow-up. The SPSS generalized linear model statistical method was used to analyze the changes of each evaluation index.Results:Compared with before treatment, the PSQI score of the combination group decreased (7.2±1.7 vs 13.2±2.9), and the improvement was better than that of the rTMS group (9.2±2.5 vs 12.1±2.8) and the drug group (7.5±2.8 vs 11.4±2.9) ( P<0.05). Multiple comparisons results showed that combination group > drug group > rTMS group; and combination group > rTMS group > drug group during follow-up. After treatment and during follow-up, the HAMA and HAMD scores of the rTMS group and the combination group decreased. There was no statistical difference in the improvement rate between the two groups, but they were all higher than the drug group ( P<0.05). After treatment and during follow-up, the β power of the rTMS group and the combination group decreased, and the α power increased, but there was no statistical difference in the δ and θ power. The β, δ and θ power in the drug group increased ( P<0.05), but the α power had no statistical difference. Multiple comparisons results showed that the power of β, δ and θ bands in the rTMS group and the combination group were lower than the drug group, α power was higher than that in the drug group ( P<0.05). Compared with before treatment During follow-up, the sleep latency of the combination group and rTMS group was shorten, and the total sleep time, sleep efficiency, deep sleep (N3) and rapid eye movement (REM) increased( P<0.05), but there was no statistical difference in the changes of sleep parameters in the drug group. Conclusions:rTMS combined with Eszopiclone can significantly improve the sleep quality of patients with chronic insomnia, which is better than that of rTMS and Eszopiclone alone, and it can reduce cortical excitability by regulating brain electrical activity. It can be an ideal treatment for patients with chronic insomnia disorder.
