Objective To verify the radiation shielding effects of a self-developed protective shield for infant bedside X-ray radiography, and measure the dose distribution in the indoor radiation field with and without the shield, and to provide guidance for the radiation protection of infants sharing the room. Methods A MUX-100J mobile medical diagnostic X-ray machine was used as the radiographic device to simulate the exposure conditions of infant bedside X-ray radiography. The ambient dose equivalent rates at measurement points in eight directions on the irradiation plane in the indoor radiation field were measured with and without the protective shield using an AT1121 X/γ dose rate meter under the conventional condition of 56 kV and the maximum single exposure time. Results During infant bedside X-ray radiography, the dose levels in all directions on the irradiation plane within 150 cm from the scatterer center were essentially symmetrically distributed. The ambient dose equivalent rate at 100 cm from the scatterer center on the irradiation plane was (8.01-8.28)×103 μSv/h without the protective shield and 48-84 μSv/h with the shield. At various distances from the scatterer center in all directions on the irradiation plane, the doses measured with the protective shield ranged from 0.34% to 14.12% of those measured without the shield, with an average of 1.85%. Conclusion The dose levels in the indoor radiation field were significantly reduced with the protective shield during infant bedside X-ray radiography. The use of the protective shield can effectively reduce the radiation risk for individuals near the radiographic site within the same room.

OBJECTIVE:External fixators and plate internal fixation are commonly used treatments for comminuted distal radius fractures, each with its own advantages and disadvantages in clinical practice. To systematically evaluate the clinical efficacy and safety of external fixator and plate internal fixation in the treatment of comminuted distal radius fractures, and to provide a basis for the development of guidelines for the diagnosis and treatment of distal radius fractures with integrated traditional Chinese and Western medicine. METHODS:PubMed,Web of Science,Embase,Cochrane Library,China National Knowledge Infrastructure,China Biomedical Literature Database,VIP,andWanFang databases were systematically searched to include randomized controlled trials on external fixators and internal plate fixation for the treatment of comminuted distal radial fractures published from October 2013 to October 2023. The literature was screened according to the inclusion and exclusion criteria. Review Manager was used for literature quality evaluation and meta-analysis.RESULTS:(1) Eight articles were included, including 4 in Chinese and 4 in English, with a total sample size of 648 cases, including 328 cases in the external fixation stent group and 320 cases in the internal fixation plate group. (2) At 3 months after operation, the internal fixation plate group was superior to the external fixation stent group in the range of dorsal extension, palmar flexion and supination. At 12 months after operation, the grip strength, palmar inclination, palmar flexion, pronation and supination in the internal fixation plate group were better than those in the external fixation stent group. The postoperative infection in the internal fixation plate group was better than that in the external fixation stent group, and there was no statistical difference in other outcome indicators.CONCLUSION:Eight evidences showed that in the choice of treatment for comminuted distal radius fracture, both external fixation stent and incision plate internal fixation had good therapeutic effect, and plate internal fixation was better than other factors. However, for some special patients with highly severe comminuted distal radius fractures, poor bone quality, severely contaminated open fractures, and soft tissue swelling that did not allow incision surgery, external fixation was the first choice. The results of this study have limitations, and more high-quality,large-sample,multi-center randomized controlled trials are needed in the future, emphasizing the observation of long-term efficacy and other secondary indicators, and supplementing and optimizing the current research results.

Although immunotherapy has made a breakthrough in the treatment of cancer, the emergence of drug resistance has limited be-nefits in most patients, and reversing immunotherapy resistance remains a hotly debated and unresolved issue. The tumor microenviron-ment(TME)comprises a large number of T lymphocytes.During the biological processes of proliferation, activation, and the effect of tumor-infiltrating lymphocytes(TILs),TILs face severe local metabolic stress, resulting in metabolic reprogramming to meet the markedly increased energy demand and biosynthesis requirements. This process of adaptive changes in the metabolic pattern is closely related to the pheno-type and function of TILs,affecting immunotherapy efficacy and mediating immunotherapy resistance. In recent years, with the study of TIL metabolism and the search for specific metabolic regulatory points, targeting the TIL metabolic pathway could restore the tumor-killing func-tion and reverse immunotherapy resistance. This review explores the relationship between the metabolic reprogramming of TILs and im-mune resistance, and provides a new strategy for reversing immune resistance.

Objective:To compare the differences in the prevalence of mild micro-hepatic encephalopathy (MHE) among patients with cirrhosis by using the psychometric hepatic encephalopathy score (PHES) and the Stroop smartphone application (Encephal App) test. Methods:This prospective, multi-center, real-world study was initiated by the National Clinical Medical Research Center for Infectious Diseases and the Portal Hypertension Alliance and registered with International ClinicalTrials.gov (NCT05140837). 354 cases of cirrhosis were enrolled in 19 hospitals across the country. PHES (including digital connection tests A and B, digital symbol tests, trajectory drawing tests, and serial management tests) and the Stroop test were conducted in all of them. PHES was differentiated using standard diagnostic criteria established by the two studies in China and South Korea. The Stroop test was evaluated based on the criteria of the research and development team. The impact of different diagnostic standards or methods on the incidence of MHE in patients with cirrhosis was analyzed. Data between groups were differentiated using the t-test, Mann-Whitney U test, and χ2 test. A kappa test was used to compare the consistency between groups. Results:After PHES, the prevalence of MHE among 354 cases of cirrhosis was 78.53% and 15.25%, respectively, based on Chinese research standards and Korean research normal value standards. However, the prevalence of MHE was 56.78% based on the Stroop test, and the differences in pairwise comparisons among the three groups were statistically significant (kappa = -0.064, P < 0.001). Stratified analysis revealed that the MHE prevalence in three groups of patients with Child-Pugh classes A, B, and C was 74.14%, 83.33%, and 88.24%, respectively, according to the normal value standards of Chinese researchers, while the MHE prevalence rates in three groups of patients with Child-Pugh classes A, B, and C were 8.29%, 23.53%, and 38.24%, respectively, according to the normal value standards of Korean researchers. Furthermore, the prevalence rates of MHE in the three groups of patients with Child-Pugh grades A, B, and C were 52.68%, 58.82%, and 73.53%, respectively, according to the Stroop test standard. However, among the results of each diagnostic standard, the prevalence of MHE showed an increasing trend with an increasing Child-Pugh grade. Further comparison demonstrated that the scores obtained by the number connection test A and the number symbol test were consistent according to the normal value standards of the two studies in China and South Korea ( Z = -0.982, -1.702; P = 0.326, 0.089), while the other three sub-tests had significant differences ( P < 0.001). Conclusion:The prevalence rate of MHE in the cirrhotic population is high, but the prevalence of MHE obtained by using different diagnostic criteria or methods varies greatly. Therefore, in line with the current changes in demographics and disease spectrum, it is necessary to enroll a larger sample size of a healthy population as a control. Moreover, the establishment of more reliable diagnostic scoring criteria will serve as a basis for obtaining accurate MHE incidence and formulating diagnosis and treatment strategies in cirrhotic populations.
