1.Efficacy of modified Wendan Decoction combined with antipsychotic drugs in the treatment of schizophrenia and its impact on patients' cognitive function: a Meta-analysis

BackgroundSchizophrenia, as a common chronic mental disorder, although second-generation antipsychotic drugs have shown significant efficacy in alleviating positive symptoms, the widespread cognitive dysfunction among patients remains a challenge in clinical treatment. Traditional Chinese medicine has unique advantages in the treatment of mental disorders. However, the current clinical research on the combination of Wendan Decoction and antipsychotic drugs for schizophrenia varies in quality, and there is a lack of systematic reviews evaluating its effects on cognitive improvement and safety. ObjectiveTo systematically evaluate the effects of modified Wendan Decoction combined with antipsychotic drugs on schizophrenia symptoms and cognitive improvement, providing evidence-based support for the clinical application of Wendan Decoction. MethodsLiterature searches were conducted in China National Knowledge Infrastructure (CNKI), Wanfang Data, VIP Information, China Biomedical Literature Service System, China Clinical Trial Registry, PubMed, Web of Science, the Cochrane Library, and Embase to collect randomized controlled trials (RCTs) of modified Wendan Decoction combined with antipsychotic drugs for the treatment of schizophrenia. The search period was from the establishment of the databases to March 19, 2026. The quality of the included literature was evaluated using the Cochrane 6.3. Meta-analysis was performed using RevMan 5.3 software. ResultsA total of 11 studies were included, involving 1 130 patients with schizophrenia. Among them, 566 cases were in the study group (receiving modified Wendan Decoction combined with antipsychotic drugs), and 564 cases were in the control group (receiving antipsychotic drugs only). Meta-analysis showed that the effective rate of improvement in psychotic symptoms in the study group was higher than that in the control group (RR=1.21,95% CI: 1.15–1.27, P<0.01). In terms of psychotic symptoms, the Positive and Negative Symptom Scale (PANSS) positive symptom score (MD=-3.69, 95% CI: -5.87–-1.51, P<0.01) and PANSS total score (MD=-9.20, 95% CI: -11.80–-6.59, P<0.01) of the study group were lower than those of the control group. In cognitive function assessments, the Mini-Mental State Examination (MMSE) score (MD=2.51, 95% CI: 1.33–3.68, P<0.01) and the Loewenstein Occupational Therapy Cognitive Assessment (LOTCA) score (MD=11.85, 95% CI: 2.55–21.15, P=0.010) of the study group were higher than those of the control group, and the Wisconsin Card Sorting Test (WCST) score was lower than that of the control group (MD=-9.34, 95% CI: -12.57–-6.11, P<0.01). The levels of brain-derived neurotrophic factor (BDNF) (SMD=1.34, 95% CI: 0.63–2.05, P<0.01) and nerve growth factor (NGF) (MD=6.94, 95% CI: 4.00–9.89, P<0.01) of the study group were higher than those of the control group. In terms of safety, there was no statistically significant difference in the incidence of adverse reactions between the two groups (RR=0.60, 95% CI: 0.31–1.18, P=0.14). ConclusionThe modified Wendan Decoction combined with antipsychotic drugs may be more effective than antipsychotic drugs alone in improving positive symptoms and cognitive function in patients with schizophrenia, and it also exerts a favorable neurotrophic regulatory effect. 2.An exploratory study on the identification of early esophageal cancer and non-cancerous lesions based on Lab color space

ObjectiveTo explore the clinical application value of CIE 1976 L*a*b*(Lab) color space in the differential diagnosis of early esophageal cancer and non-cancerous lesions. MethodsWe selected the endoscopic images of patients with esophageal lesions confirmed by pathology who underwent white light imaging endoscopy (WLI) and narrow band imaging endoscopy (NBI). Five regions of interest (ROI) were selected respectively from the mucosa of the lesion area and the mucosa of the surrounding normal area for labeling. The Lab color space parameters were extracted and counted, and the color difference values(ΔE*)were calculated. ResultsA total of 213 eligible patients were included for analysis in the study. In WLI and NBI modes, there were differences in mucosal color between the early esophageal cancer group and the non-cancer group (P<0.05). Compared with WLI mode, NBI mode could significantly increase the color difference between early esophageal cancer and non-cancerous lesions (P<0.05). The lightness component value (L* value) of the early esophageal cancer lesion area was lower than that of the non-cancerous lesion area, and this color difference was more significant in the NBI mode (P<0.05). In WLI mode, there was no significant difference in yellow-blue component value (b* value) between the mucosa of early esophageal cancer and non-cancerous lesions. However, in the NBI mode, the b* value of the mucosa in the non-cancerous lesion area was higher than that in the early esophageal cancer lesion area (P<0.05). On the red-green axis, the mucosa of the early esophageal cancer and non-cancerous lesions was red in WLI mode and green in NBI mode. There was no significant difference in red-green component value (a* value) between the two groups. ConclusionThere are color differences between early esophageal cancer and non-cancerous lesions under WLI and NBI. The color of early esophageal cancer is darker under WLI, and the color of non-cancerous lesions is yellower under non-magnified NBI mode. Lab color space is helpful to identify early esophageal cancer and non-cancerous lesions.
3.Effect and Mechanism of Angelicae Sinensis Radix-Polygonati Rhizoma Herb Pair in Treatment of Simple Obesity

ObjectiveTo preliminarily explore the active components and target pathways of Angelicae Sinensis Radix-Polygonati Rhizoma (ASR-PR) herb pair in the treatment of simple obesity through network pharmacology and molecular docking, and to verify and investigate its mechanism of action via animal experiments. MethodsThe chemical constituents and targets of ASR and PR were predicted using the Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP). Targets related to simple obesity were identified by retrieving the GeneCards, Online Mendelian Inheritance in Man (OMIM), Pharmacogenomics Knowledgebase (PharmGKB), and DisGeNET databases. The intersection of drug and disease targets was used to construct an active component-target network using Cytoscape software. This network was imported into the STRING database to construct a protein-protein interaction (PPI) network, and topological analysis was conducted to identify core genes. Gene Ontology (GO) analysis and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analysis and mapping were performed using the DAVID database and the Microbioinformatics platform. AutoDock 1.5.7 software was used to perform molecular docking between the top five active components and core targets. An animal model of simple obesity was established by feeding C57BL/6J mice a high-fat diet. The mice were administered ASR (2.06 g·kg-1), PR (2.06 g·kg-1), or ASR-PR (4.11 g·kg-1) for 10 weeks, while the model group received an equal volume of purified water by gavage. After the administration period, the mice were sacrificed to measure body fat weight and serum levels of total cholesterol (TC), triglycerides (TG), high-density lipoprotein (HDL), and low-density lipoprotein (LDL). Hematoxylin-eosin (HE) staining was used to observe histopathological sections of liver and adipose tissue. Serum levels of leptin, interleukin-6 (IL-6), and tumor necrosis factor-α (TNF-α) were determined by enzyme-linked immunosorbent assay (ELISA), and the mRNA expression levels of epidermal growth factor receptor (EGFR) and signal transducer and activator of transcription 3 (STAT3) in liver tissue were detected by real-time quantitative polymerase chain reaction (Real-time PCR). ResultsNetwork pharmacology and molecular docking results indicated that the treatment of simple obesity by ASR-PR may involve the regulation of protein expression of core targets EGFR and STAT3 by its main components MOL009760 (Siberian glycoside A_qt), MOL003889 (methyl protodioscin_qt), MOL009766 (resveratrol), MOL006331 (4′,5-dihydroxyflavone), and MOL004941 (baicalin), thereby modulating the PI3K/Akt and JAK/STAT signaling pathways. The animal experiment results showed that compared with the normal group, the model group had significantly increased body weight, body fat weight, and serum levels of TG, TC, TNF-α, IL-6, and leptin (P<0.01). EGFR mRNA expression was significantly elevated (P<0.05), while STAT3 mRNA expression was significantly decreased (P<0.01). Histological analysis revealed disordered hepatic architecture in the model group, with pronounced lipid vacuoles, cytoplasmic loosening, lipid accumulation, and steatosis. Adipocytes in white adipose tissue (WAT) and brown adipose tissue (BAT) of the model group exhibited markedly increased diameters, reduced cell counts per unit area, and irregular morphology. Compared with the model group, the ASR-PR group significantly reduced body weight, body fat weight, serum TC, IL-6, TNF-α, leptin levels, and EGFR mRNA expression (P<0.01). TG levels were also significantly decreased (P<0.05), while STAT3 mRNA expression was significantly increased (P<0.01). Histopathological improvements included reduced size and number of hepatic lipid vacuoles and restoration of liver cell morphology toward that of the normal group. The diameter of adipocytes significantly decreased, and the number of adipocytes per unit area increased. ConclusionASR-PR may regulate the expression of key target proteins such as EGFR and STAT3 via its core active components, modulate the PI3K/Akt and JAK/STAT signaling pathways, repair damaged liver and adipose tissues, and thereby alleviate the progression of obesity in mice.

5.Treatment of rheumatoid arthritis of wrist using Ilizarov wrist joint distraction technique: a case report.

OBJECTIVE:
To report the clinical experience of using Ilizarov wrist joint distraction technique in the treatment of a case of rheumatoid arthritis of the wrist.
METHODS:
In January 2019, a 49-year-old female patient with rheumatoid arthritis of the left wrist, complicated by ulnar impaction syndrome, was admitted for treatment. Preoperatively, the active range of motion of the left wrist was as follows: extension 0°-flexion 0°, pronation 65°-supination 35°, and grip strength of 4.0 kg. The visual analogue scale (VAS) score was 9, and the Cooney wrist function score was 15, indicating poor function. As conservative treatment failed to achieve symptom relief, Ilizarov wrist joint distraction surgery was performed. Postoperatively, joint distraction was applied at 2 mm increments on postoperative days 2 and 7, in 4 separate sessions.
RESULTS:
Postoperative X-ray film examination at 7 days revealed a distraction of 3.6 mm in the affected wrist joint compared to the contralateral side. The external fixator was removed 2.5 months postoperatively. At 22 months postoperatively, X-ray film and MRI examinations revealed that the joint space of the left wrist had returned to near-normal, with significant reduction in joint effusion and synovial proliferation. The active range of motion of the left wrist improved to extension 15°- flexion 30°, pronation 90°-supination 90°, with a grip strength of 18.0 kg. The wrist pain VAS score decreased to 0, and the Cooney wrist function score improved to 90, indicating excellent function. At 50 months postoperatively, follow-up X-ray film, MRI, and functional assessments showed the results similar to those at 22 months.
CONCLUSION
Ilizarov wrist joint distraction may be a viable treatment option for rheumatoid arthritis of the wrist.

6.3D Pulse Image Detection and Pulse Pattern Recognition Based on Subtle Motion Magnification Technology.

To address the problem of large reconstruction errors in 3D pulse signals caused by excessively small out-of-plane displacement of the contact membrane in the existing traditional Chinese medicine fingertip tactile binocular vision detection technology, this study proposes a 3D pulse image detection method based on subtle motion magnification technology and explores its application in pulse pattern recognition. Firstly, a 3D pulse image detection system based on binocular vision to obtain pulse image signals is developed as experimental data. Then, the phase motion video magnification algorithm is used to amplify the original signals, and the amplified signals are reconstructed in three dimensions to obtain 3D pulse signals. On this basis, nine features are extracted from the 3D pulse signals and features selection is performed using a two-sample Kolmogorov-Smirnov test. Finally, machine learning algorithms such as decision trees and random forests are used to identify the five types of pulse conditions: deep pulse, intermittent pulse, flooding pulse, slippery pulse, and rapid pulse. The experimental results show that compared to the methods without subtle motion magnification technology, the proposed method significantly improves waveform clarity, amplitude stability, and periodic regularity. Meanwhile, the average accuracy in pulse pattern recognition reaches 96.29%±0.26%.

7.Dendritic cells immunotargeted therapy for atherosclerosis.

Atherosclerosis, a chronic inflammatory disease, is markedly influenced by both immune and inflammatory reactions throughout its progression. Dendritic cells, as pivotal antigen-presenting entities, play a crucial role in the initiation of immune responses and the preservation of immunological homeostasis. Accumulating data indicates that dendritic cells are present in healthy arteries and accumulate significantly in atherosclerotic plaques. Novel immunotherapeutic approaches and vaccination protocols have yielded substantial clinical advancements in managing chronic inflammatory diseases, with dendritic cell-centric modalities emerging for atherosclerotic management. In this review, we delineate the essential functions and underlying mechanisms of dendritic cells and their subsets in the modulation of atherosclerotic inflammation and immune responses. We underscore the immense promise of dendritic cell-based immunotherapeutic strategies, including vaccines and innovative combinations with nanotechnological drug delivery platforms for atherosclerosis treatment. We also discuss the challenges associated with dendritic cell immunotherapy and provide perspectives on the future direction of this field.

Acute ischemic stroke (AIS) is a cerebrovascular disease characterized by high morbidity, disability, and mortality, posing a significant threat to human health. Endovascular treatment has now been established as a key method for AIS management, in which stent retrievers that can mechanically remove blood clots play a key role in this technique. In recent years, stent retrievers have evolved in complexity and functionality to improve the ability of clot removing and surgical safety. However, the present instruments still have limitations on treatment efficiency, vascular adaptability, and operational precision, posing an urgent need for innovation in the design of stent retrievers. This paper systematically reviewed the structural features and working principles of AIS stent retrievers from the perspective of efficacy evaluation metrics, historical development, recent advancements in stent retrieval technology, and future prospects.

9.The application effect of modified intermittent horizontal mattress suture technique in anastomosing intimal stratified artery

Objective:To investigate the efficacy of a modified intermittent horizontal mattress suture technique in arterial anastomosis with intimal dissection during free flap transplantation.Methods:A retrospective analysis was conducted on patients with limb wounds who underwent free flap transplantation with the modified intermittent horizontal mattress suture technique at the First People’s Hospital of Pingyuan County from September 2016 to November 2023. After debridement, free flap transplantation was performed. Under microscopic examination, recipient arteries with intimal dissection were identified, and proximal trimming failed to resolve the delamination. The modified intermittent horizontal mattress suture technique was applied to anastomose the dissected recipient arteries to donor arteries, while veins were conventionally horizontal mattress sutured. Postoperative flap viability, complications, and patient satisfaction were evaluated.Results:Fourteen patients were enrolled, including 8 males and 6 females, aged (51±14) years old (range: 18-71 years). Injuries involved the hand (5 cases) and foot (9 cases), with wound sizes ranging from 7 cm×8 cm to 22 cm×18 cm. Flaps included anterolateral thigh perforator flaps (11 cases) and groin flaps (3 cases), sized 8 cm×9 cm to 24 cm×20 cm. Seven patients had intimal dissection of the dorsalis pedis artery, with anastomoses performed to the descending branch of the lateral circumflex femoral artery (LCFA) in 5 cases, and to the superficial circumflex iliac artery in 2 cases. Intimal dissection of the medial plantar artery was identified in 2 cases, both of which were anastomosed to the descending branch of LCFA. And 5 cases had intimal dissection of the radial artery, with anastomoses performed to the LCFA in 3 cases, to the transverse branch of LCFA in one case, and to the superficial circumflex iliac artery in one case. The patients were followed up for (11.6±7.6) months (range: 3-30 months) after surgery. Eleven flaps survived completely, with partial flap margin necrosis in one case, in another case necrosis of one-third of the flap because of arterial insufficiency, both cases healed with dressing changes. The third patient resumed smoking and subsequently fell from the bed, resulting in necrosis of two-thirds of the flap five days post-discharge (12 days postoperatively). After debridement of the necrotic part of the flap, a split-thickness skin graft was applied for repair. Thirteen flaps exhibited no ulceration, with satisfactory texture and appearance, these patients expressed satisfaction. One patient with skin graft infection achieved scar healing.Conclusion:The modified intermittent horizontal mattress suture technique ensures reliable anastomosis of arteries with intimal dissection in free flap surgery, improving success rates and yielding favorable outcomes with high patient satisfaction.

BACKGROUND:Bone tissue,as a highly mineralized and structurally complex connective tissue,plays a pivotal role in maintaining the form and function of living organisms.Traditional imaging techniques have been insufficient to meet the demands for in-depth observation of bone.The advent of tissue clearing technology has enabled researchers to more clearly observe the intricate structures within bones,such as trabecular bone,bone marrow cavities,and neural and vascular networks interacting with bone tissue.This has opened new perspectives for research and clinical applications in orthopedics.OBJECTIVE:To comprehensively summarize the applications of tissue clearing technology in the field of orthopedics and to explore its potential in the study of bone tissue structure and function,elucidation of disease mechanisms,and innovation in treatment strategies.Additionally,it provides an outlook on innovative directions.METHODS:The review encompassed a search of the CNKI and PubMed databases using search terms"tissue clearing,tissue optical,bone"in English and"tissue clearing,bone,bone defect"in Chinese,combined with Boolean logic to optimize the search strategy.The inclusion criteria were scholarly articles and dissertations directly related to tissue clearing technology and orthopedic research,excluding literature with weak relevance,duplication,or incomplete data.A total of 82 articles were finally analyzed,focusing on the efficacy of clearing techniques,their scope of application,and their contributions to orthopedic research.RESULTS AND CONCLUSION:(1)Tissue clearing technology has demonstrated unique advantages in various domains,including the analysis of bone structure,the study of bone metabolism,the pathological characteristics of bone tumors,the process of fracture healing,the mechanisms of bone infection,and the evaluation of biocompatibility of bone grafts and implant materials.(2)The application of this technology has not only accelerated the progress of basic research in the field of orthopedics but also provides new strategies and methods for clinical treatment,indicating a broad application prospect in orthopedic research.
