1.Analysis of xenobiotics in colon and immune tissues of ulcerative colitis mice after administration of Sini San by LC-MS

Dysregulation of immune response is currently recognized as one of the important pathological factors in ulcerative colitis (UC). Based on the confirmation that the Sini San (SNS) can significantly improve the colon inflammation induced by dextran sulfate sodium sulfate (DSS) in mice, the present work systematically studied the xenobiotics in the colon and mesenteric lymph nodes, spleen, and thymus of UC mice after administration of SNS by high-performance liquid chromatography-ion trap time-of-flight mass spectrometry (HPLC-IT-TOF-MS). The results showed that, in addition to the colon, some components and their metabolites in SNS could be distributed in immune tissues, and it was found that the quality of relatively low-abundance and weakly responsive components such as saikosaponin a, paeoniflorin, and glycyrrhizic acid had the characteristics of efficient transmission to the colon and lymphoid organs. These components were very likely to be the source of pharmacodynamic substances of SNS. The findings of this study lay a foundation for the study of the efficacy and molecular mechanism of the components against ulcerative colitis, and also provide a scientific basis for the rational clinical application of SNS, which is expected to promote the secondary development of its preparations.
2.Adrenocortical carcinoma with rhabdoid features: a case report and literature review

[Objective] To investigate and summarize the clinicopathological features, diagnosis, treatment and prognosis of adrenocortical carcinoma with rhabdoid features. [Methods] The clinical diagnosis and treatment of a case of adrenocortical carcinoma with rhabdoid features admitled to Department of Urology, Weifang People's Hospital were reported.The clinical manifestations, pathological features, diagnosis and prognosis of the disease were analyzed in combination with relevant literature. [Results] A 34-year-old male patient was admitted due to scrotal distension and pain that had persisted for 6 months.Imaging examination showed a huge soft tissue tumor in the left adrenal region of the retroperitoneum with compression displacement of the left kidney, leading to obstruction of venous return in the left spermatic vein, which in turn caused varicose veins.The levels of serum renin, angiotensin, aldosterone, cortisol, and catecholamine were within normal ranges.Surgical resection of the tumor was performed, and postoperative pathological examination revealed that the tumor tissue was predominantly composed of rhabdoid cells, exhibiting positive immunohistochemical staining for INI 1, Syn, Calretinin and Vimentin.Genetic testing did not identify any deletion of SMARCB1 and SMARCA4 mutations.Therefore, the diagnosis was adrenocortical carcinoma with rhabdoid features.At the current 20-month follow-up, no recurrence or metastasis was observed.A review of the literature found that only 7 cases of this disease had been reported. [Conclusion] Adrenocortical carcinoma with rhabdoid features is a rare disease, and a definitive diagnosis is dependent upon pathological examination.Surgical resection remains the primary treatment.Long-term follow-up is essential, and further research is needed to evaluate the impact of adjuvant therapy.
3.Incidence and risk factors of pneumonia in patients with thoracic tumors receiving radiotherapy combined with immunotherapy

Objective:To evaluate the incidence and risk factors of pneumonia in patients with thoracic cancer treated with immunotherapy combined with radiotherapy (RT).Methods:The clinic data of 265 patients with thoracic cancer who received RT and at least 2 cycles of immunotherapy at Renmin Hospital of Wuhan University between January 2020 and January 2024 were retrospectively analyzed. Patients were divided into 2 groups according to treatment sequence: 100 patients with sequential immunotherapy after RT, 165 patients with RT after immunotherapy, including sequential RT after immunotherapy (119 cases) and concurrent RT with immunotherapy (46 cases). The occurrence and grading of treatment-related pneumonia were determined by clinical symptoms, signs, and chest CT findings. The relationship between interval time window of different treatment methods and pneumonia was analyzed by Pearson correlation analysis. χ2 test was used for univariate analysis of risk factors, and logistic regression for multivariate analysis. Results:In the sequential immunotherapy after RT group, the incidence of pneumonia was 12.0% (12/100), with grade ≥2 pneumonia in 4.0% (4/100). The interval time between RT and immunotherapy, has a strong negative correlation with pneumonia incidence and grade ≥2 pneumonia incidence ( r=-0.88, -0.79; both P<0.001). When immunotherapy was initiated ≥7 weeks after RT, the incidence of pneumonia significantly decreased to 5% ( P=0.020), with no grade ≥2 cases. In the immunotherapy followed by sequential / concurrent RT cohort, the incidence of pneumonia was significantly higher in the concurrent subgroup compared with the sequential subgroup [65% (30/46) vs. 44.5% (53/119), P=0.027], and the comparison of the incidence of grade ≥2 pneumonia was the same [33% (15/46) vs. 14.3% (17/119), P=0.014]. The interval time window of RT after immunotherapy was strongly negatively correlated with the incidence of pneumonia and grade ≥2 pneumonia ( r=-0.77, -0.83; both P<0.001). When RT was initiated ≥4 weeks after immunotherapy, the incidence of pneumonia significantly decreased ( P<0.001). Pneumonia incidence differed significantly across RT target sites (intrapulmonary vs. extrapulmonary), total dose (<60 Gy vs. ≥ 60 Gy), and fractionation regimen (conventional vs. hypofractionation) ( P < 0.001, = 0.008, = 0.031), but was not associated with age, gender, smoking history, type of immune checkpoint inhibitor (anti-programmed death-1 vs. anti-programmed death ligand-1), or the number of prior immunotherapy cycles (all P > 0.05). Multivariate analysis identified RT within the lung and interval time of RT after immunotherapy ≤ 4 weeks as independent risk factors for the incidence of pneumonia (both P < 0.001). Conclusions:The interval window between RT and immunotherapy is a critical determinant of pneumonia incidence. Delaying immunotherapy for more than 7 weeks after RT, or delaying RT for more than 4 weeks after immunotherapy, can significantly reduce the risk of radiation-related pneumonia.
4.Epidemiological characteristics of newly diagnosed occupational diseases among workers from other provinces in Guangdong Province from 2006 to 2023

Objective:To investigate the epidemiological characteristics and source distribution of newly diagnosed occupational diseases among workers from other provinces in Guangdong Province from 2006 to 2023, providing a basis for occupational health risk assessment and government decision-making.Methods:In July 2024, through the Occupational Disease Report Card of the Occupational Disease and Health Hazard Monitoring Information System of the China Disease Prevention and Control Information System, the basic information of newly diagnosed occupational disease cases reported online in Guangdong Province from 2006 to 2023 was collected, and the epidemiological characteristics of new occupational diseases among workers from other provinces were described. The locations of employment units and their source provinces of different cases were analyzed. Counting data were described by component ratio (%) and compared by Pearson χ2 test. Results:From 2006 to 2023, 9763 cases of newly diagnosed occupational diseases among workers from other provinces in Guangdong Province were reported, accounting for 78.82% (9763/12387) of the total number of occupational diseases in the province. The cases of occupational diseases were mainly concentrated in the Pearl River Delta region (94.93%, 9268/9763) and the manufacturing industry (86.97%, 8491/9763), and the domestic enterprises accounted for 57.35% (5593/9753). Newly diagnosed occupational disease cases were mainly from province A in Central China (2767 cases, 28.34%), province A in Southwest China (2323 cases, 23.79%) and a province in South China (1070 cases, 10.96%) .Conclusion:The proportion of newly diagnosed occupational diseases among workers from other provinces in Guangdong Province is relatively high, and the cases are concentrated in the regions, industries and source provinces.
5.Molecular mechanisms of lung cancer induced by the insecticide lambda-cyhalothrin.

The inappropriate utilisation of the agricultural insecticide lambda-cyhalothrin (LCT) has the potential to result in residues that compromise food safety and human health. Respiratory exposure represents a major route of LCT contact in humans. Nevertheless, its deleterious effects on the respiratory system remain inadequately characterized. It is imperative to elucidate the potential relationship and mechanisms by which lung cancer, a significant malignant neoplasm of the respiratory system, is associated with exposure to LCT. The objective of this study is to utilise bioinformatics methodologies to screen and analyse the key target molecules affected by LCT in the occurrence of lung cancer, and their mechanisms of action. Specifically, network toxicology methods were employed to identify core targets of LCT-induced lung cancer. Subsequently, functional annotation to delineate associated cellular pathways, and finally, molecular docking to simulate binding modes between LCT and shared core targets. Core target screening identified 50 targets for large cell lung cancer, 54 for small cell lung cancer, 29 for lung squamous cell carcinoma, and 28 for lung adenocarcinoma, with EGFR, HSP90AA1, JUN, CCL2, MYC, CXCL8, and HSPA4 shared in all subtypes. Functional annotation revealed that LCT-triggered oncogenic pathways predominantly involved ubiquitination, chemotaxis, and tumor immune signaling. Molecular docking demonstrated spontaneous binding of LCT to core targets mediated by hydrogen bonds and π-cation interactions. These results establish a theoretical framework for evaluating LCT-associated risks of lung cancer and respiratory system damage.

6.Analyzing the characteristics of newly diagnosed occupational disease in Guangdong Province, 2019-2023

Objective To analyze the epidemiological characteristics of newly diagnosed occupational diseases in Guangdong Province from 2019 to 2023. Methods Data on newly diagnosed occupational diseases reported in Guangdong Province from 2019 to 2023 were collected from the national occupational disease network reporting system. The spectrum of occupational diseases and their distribution by region, industry, and population were analyzed. Results A total of 4 136 newly diagnosed occupational disease cases were reported in Guangdong Province from 2019 to 2023, showing an overall downward trend. Newly diagnosed cases were classified into eight categories and 53 types of occupational diseases. In terms of the number of cases, the top five categories were occupational diseases of the ear, nose, throat and oral cavity;occupational pneumoconiosis and other respiratory diseases; occupational diseases caused by physical factors; occupational chemical poisoning; and occupational tumors, accounting for 98.62% of all cases. The top ten specific disease types were occupational noise-induced deafness, occupational silicosis, occupational other pneumoconiosis, occupational chronic benzene poisoning, occupational heatstroke, occupational hand-arm vibration disease, occupational coal workers′ pneumoconiosis, occupational welders′ pneumoconiosis, occupational tumor (leukemia caused by benzene exposure), and occupational chronic n-hexane poisoning, accounting for 94.85% of all cases. Most of the cases were distributed in the Pearl River Delta region, accounting for 89.19%; as well as manufacturing industry, accounting for 84.89%. Male cases accounted for 87.02%. Most diagnoses occurred in individuals aged >40-60 years, accounting for 74.73%. Conclusion Newly diagnosed occupational diseases in Guangdong Province from 2019 to 2023 showed the following characteristics: concentration of categories and disease types, polarization of regional distribution, industry clustering, and population difference. The disease spectrum is evolving from a dual-disease predominance toward a multi-disease predominance.
7.Analyzing the characteristics of newly diagnosed occupational chemical poisoning in Guangdong Province, 2006-2023

Objective To analyze the epidemiological characteristics of newly diagnosed occupational chemical poisoning (OCP) cases in Guangdong Province from 2006 to 2023. Methods The basic information of newly diagnosed OCP cases in Guangdong Province from 2006 to 2023 was collected from the Occupational Disease Report Card of the occupational disease network reporting system in China. The epidemiological characteristics were analyzed by the descriptive epidemiological methods. Results A total of 2 201 newly diagnosed OCP cases were reported in Guangdong Province during 2006-2023, showing a fluctuating downward trend with an average annual decrease rate of 6.98%. Occupational acute chemical poisoning (OACP) and occupational chronic chemical poisoning (OCCP) accounted for 18.08% and 81.92% of cases, respectively. Among the OACP cases, the top five disease categories were other OCP, occupational acute 1,2-dichloroethane poisoning, occupational acute dimethylformamide poisoning, occupational acute organophosphate poisoning, and occupational acute hydrogen sulfide poisoning, accounting for 77.14% (307/398). Among the OCCP cases, the top five disease categories were occupational chronic benzene poisoning, occupational chronic n-hexane poisoning, occupational chronic lead poisoning, occupational chronic cadmium poisoning and occupational chronic mercury poisoning, accounting for 97.67% (1 761/1 803), with occupational chronic benzene poisoning being the most prevalent (67.22%). Newly diagnosed OCP cases were predominantly concentrated in the Pearl River Delta region (96.00%), particularly in Dongguan City, Shenzhen City, and Guangzhou City, accounting for 69.29%. The cases mainly occurred in the manufacturing industry, domestically funded enterprises, and small- and medium-sized enterprises, accounting for 93.82%, 54.98%, and 70.88%, respectively. Males accounted for 52.57% of cases, individuals aged <50 years accounted for 91.32%, and workers with <5 years of service accounted for 60.29%. There were significant differences in the distribution of gender, age and length of service between the OACP and OCCP cases (all P<0.01). Conclusion The newly diagnosed OCP cases in Guangdong Province demonstrates a pattern characterized by concentration in disease types, geographic regions, industries, enterprise types, and affected populations, along with a high level of social risk, which was summarized as the feature of "five areas of concentrations and one high-risk factor".
8.Anti-reflux proximal gastrectomy based on fluid dynamics theory: transverse fan-shaped groin formed

Objective:To explore the anti-reflux effect of transverse fan-shaped groin formed (TFGF) when the residual stomach is large (the small curvature ≥10 cm).Methods:Twenty-four patients with residual curvature ≥10 cm after PG were treated with TFGF continuously from Nov 2020 to Dec 2023 in this study. The cohort included 20 patients with Siewert type Ⅱ adenocarcinoma of the esophagogastric junction and 4 patients with achalasia of cardia. Reflux disease questionnaire score gastroesophageal reflux disease questionnaire score and gastroscopy Los Angeles score were used to evaluate the anti-reflux effect and postoperative nutritional parameters (hemoglobin, protein and body weight).Results:The reconstructive operation time of adenocarcinoma of the esophago-gastric junction and achalasia of cardia was (20.0±5.1) min and (18.3±4.7) min, respectively. reflux disease questionnaire, gastro-esophageal reflux disease questionnaire and gastroscopy indicated GERD in 3 patients and 8% in 2 patients at 24 months. At follow-up, one patient transitioned from grade B to grade C based on the Los Angeles score at 12 months; 24 months after surgery, one patient's gastroesophageal reflux symptoms resolved and the Los Angeles rating was downgraded from B to A. Nutritional indicators (hemoglobin, protein and body weight levels) all showed a downward trend at 6 months and an upward trend at 24 months.Conclusion:TFGF has the advantages of simple operation, and effective prevention of postoperative gastroesophageal reflux for patients with lesser residual curvature ≥10 cm after PG.
