1.Clinical Study on the Expression Levels of LINC02009 and LOC107984895 in Peripheral Lymphocytes of Patients with Atrial Fibrillation

Objective To investigate the expression levels of LINC02009 and LOC107984895 in peripheral lymphocytes of patients with atrial fibrillation and their clinical significance.Methods A total of 75 hospitalized patients with atrial fibrillation(50 with persistent atrial fibrillation and 25 with paroxysmal atrial fibrillation)from Kunming First People's Hospital between January 2023 and December 2023 were selected as study subjects,along with 50 normal control patients.Real-time quantitative PCR was used to detect the expression levels of LINC02009 and LOC107984895 in peripheral blood leukocytes of patients with atrial fibrillation.Logistic regression analysis was employed to assess the relationship between expression levels and risk factors for atrial fibrillation,and ROC curves were used to predict the diagnostic cut-off values for LINC02009 and LOC107984895 in diagnosing atrial fibrillation.Results There were statistically significant differences in baseline diseases such as hypertension and coronary heart disease,biochemical indicators such as Cr and BNP,and myocardial remodeling indicators such as LAd and LVEF between the AF(paroxysmal atrial fibrillation and persistent atrial fibrillation)group and the Normal group(P<0.05).The expression levels of LINC02009 and LOC 107984895 in the plasma of the atrial fibrillation group were significantly higher than those in the control group(P<0.05)and were negatively correlated with LVEF(P<0.05).The areas under the curve(AUC)of LINC02009 and LOC 107984895 in predicting atrial fibrillation were 0.967(95%CI:0.938~0.995)and 0.900(95%CI:0.838~0.963),respectively.The optimal cut-off values were 1.985 and 0.915,with sensitivities of 88%and 76%,respectively,and specificities of 94%and 90%,respectively.Conclusion LINC02009 and LOC 107984895 are independent risk factors for atrial fibrillation and have certain predictive value for the occurrence of atrial fibrillation.
2.Effect of curcumin-chitosan microspheres on inflammatory response in ulcerative colitis mice via TLR4/MyD88/NF-κB pathway

Objective:To investigate the effect of curcumin-chitosan microspheres on the inflammatory response in ulcerative colitis (UC) mice via the Toll like receptor 4 (TLR4)/myeloid differentiation factor 88 (MyD88)/nuclear factor-κB (NF-κB) pathway.Methods:BALB/c mice were induced to establish a UC model by drinking 5% dextran sulfate sodium (DSS) aqueous solution. According to the random number table method, the mice were randomly divided into model group, empty chitosan microsphere group, curcumin group, curcumin-chitosan microsphere group and curcumin-chitosan microsphere+lipopolysaccharide group, with 12 mice in each group. 0.5% carboxymethyl cellulose solution, 19 mg/ml empty chitosan microsphere solution, 12 mg/ml curcumin solution, 19 mg/ml curcumin-chitosan microsphere solution by gavage, 19 mg/ml curcumin-chitosan microsphere solution by gavage+3 mg/ml lipopolysaccharide solution by intraperitoneal injection were administered at an administration volume of 5 ml/kg. Another 12 BALB/c mice drank purified water were taken as the control group. The disease activity index (DAI) score and colon length of mice in each group were determined. The permeability of the intestinal mucosal barrier of mice in each group was evaluated by detecting the level of fluorescein isothiocyanate (FITC)-labelled dextran. Hematoxylin-eosin (HE) staining was used to detect the pathological morphology of the colon tissue in each group. Enzyme-linked immunosorbent assay (ELISA) was used to detect the levels of proinflammatory factors [interleukin (IL)-6, tumor necrosis factor-α (TNF-α), IL-1β, IL-18] in serum and colon tissue of mice in each group. The expression levels of TLR4/MyD88/NF-κB pathway related proteins in colon tissues of mice in each group were detected by Western blotting.Results:The DAI score [(0.42±0.49) points] of the curcumin-chitosan microsphere group was lower than that of the model, curcumin, empty chitosan microsphere, and curcumin-chitosan microsphere+lipopolysaccharide groups [(3.50±0.26), (2.33±0.41), (2.83±0.35), and (3.33±0.25) points], and the length of colon [(6.17±0.38) cm] was greater than that of the model, curcumin, empty chitosan microsphere, and curcumin-chitosan microsphere+lipopolysaccharide groups [(3.89±0.23), (5.13±0.34), (4.78±0.31), and (4.01±0.27) cm], and the differences were statistically significant (all P<0.05). The FITC-labelled dextran level in the curcumin-chitosan microsphere group [(2.51±0.70) μg/ml] was lower than that in the model, curcumin, empty chitosan microsphere, and curcumin-chitosan microsphere+lipopolysaccharide groups [(10.13±0.71), (5.47±0.63), (4.52±0.60), (9.63±0.58) μg/ml], and the differences were statistically significant (all P<0.05). The villus height [(160.47±11.34) μm] of the curcumin-chitosan microsphere group was higher than that of the model, curcumin, empty chitosan microsphere, and curcumin-chitosan microsphere+ lipopolysaccharide groups [(102.13±7.65), (134.26±9.42), (124.85±8.36), and (106.94±7.81) μm], the mucosal thickness [(253.89±18.38) μm] was higher than that of the model, curcumin, empty chitosan microsphere, and curcumin-chitosan microsphere+lipopolysaccharide groups [(171.86±12.51), (218.34±13.12), (204.75±14.36), and (175.23±12.74) μm], and the differences were statistically significant (all P<0.05). The serum levels of IL-6, TNF-α, IL-1β, and IL-18 in the curcumin-chitosan microspheres group were (25.58±5.43), (49.37±7.82), (69.85±15.74), and (43.01±7.69) ng/L, respectively, which were lower than those of the model group [(130.69±10.24), (172.14±11.76), (214.54±25.72), and (165.85±18.74) ng/L], the curcumin group [(56.87±6.82), (98.76±10.65), (125.43±18.67), and (94.64±16.85) ng/L], the empty chitosan microspheres group [(68.24±8.67), (113.53±12.13), (149.76±20.35), and (117.93±15.63) ng/L], the curcumin-chitosan microspheres+lipopolysaccharide group [(121.75±9.89), (163.85±10.86), (203.12±24.86), and (154.12±18.54) ng/L], and the differences were statistically significant (all P<0.05). The levels of IL-6, TNF-α, IL-1β, and IL-18 in colon tissue of mice in the curcumin-chitosan microsphere group were (16.94±4.02), (31.12±4.25), (39.13±6.73), and (41.23±7.50) ng/g respectively, which were lower than those of the model group [(92.63±7.21), (114.32±8.24), (142.37±13.41), and (165.85±18.74) ng/g], the curcumin group [(46.86±6.25), (67.95±6.84), (75.20±8.64), and (84.93±11.84) ng/g], and the empty chitosan microsphere group [(57.12±6.73), (79.31±7.76), (89.76±10.52), and (98.74±13.62) ng/g], the curcumin-chitosan microspheres+lipopolysaccharide group [(86.53±7.46), (105.43±7.93), (133.81±12.95), and (154.90±18.39) ng/g], and the differences were statistically significant (all P < 0.05). The relative expression levels of TLR4 and MyD88 proteins and phosphorylated NF-κB p65 (p-NF-κB p65)/NF-κB p65 (0.32±0.07, 0.38±0.09, and 0.09±0.03) in the colon tissue of curcumin-chitosan microspheres group were lower than those of the model group (1.34±0.13, 1.40±0.16, and 0.89±0.09), the curcumin group (0.73±0.09, 0.79±0.11, and 0.39±0.05), the empty chitosan microsphere group (0.82±0.11, 0.89±0.13, and 0.47±0.08), the curcumin-chitosan microsphere+ lipopolysaccharide group (1.31±0.12, 1.36±0.14, and 0.87±0.08), and the differences were statistically significant (all P<0.05). Conclusions:Curcumin-chitosan microspheres can alleviate inflammation, reduce colon tissue damage, and restore intestinal mucosal barrier function in UC mice, which may be achieved by inhibiting TLR4/MyD88/NF-κB signaling activation.

Background Prolonged awkward postures during occupational activities can lead to excessive musculoskeletal load on the wrist of workers and symptoms such as wrist pain or discomfort. Objective To survey the prevalence of wrist pain among workers in 10 key industries and analyze its correlation with wrist working postures. Methods By using stratified cluster sampling method, workers from 10 key industries, such as footwear manufacturing industry, shipbuilding manufacturing industry, and automobile manufacturing industry, were selected from seven regions in North China, East China, Central China, South China, Southwest China, Northwest China, and Northeast China. The demographic information, wrist working postures, pain in wrist of the workers were collected through a cross-sectional survey. Pearson χ2 test was used to compare prevalence by selected factors, trend χ2 test for between group comparison, and unconditional logistic regression models for the association of wrist working postures with wrist pain. Results There were 64052 workers enrolled in this survey, and 56286 provided valid questionnaires (the effective rate was 87.8%). According to the survey, the prevalence of wrist pain was 23.3% (13112/56286), and the industries with higher prevalences were footwear manufacturing (27.1%, 1927/7106), automobile manufacturing (24.9%, 5378/21560), and shipbuilding and related equipment manufacturing (24.4%, 850/3488) industries. Finger pinching (OR=2.09, 95%CI: 1.95-2.24), frequent wrist bending (OR=2.03, 95%CI: 1.92-2.15), fixed wrist bending (OR=1.77, 95%CI: 1.69-1.85), wrist on hard edge (OR=1.34, 95%CI: 1.28-1.40), and arms over shoulders (OR=1.11, 95%CI: 1.05-1.17) increased the risk of reporting wrist pain. Conclusion Awkward postures are related to wrist pain among workers in selected 10 key industries. The related factors are wrist on hard edge, frequent wrist bending, finger pinching, fixed wrist bending, and arms over shoulders.

5.Microcoils embolization for the treatment of intracranial wide-necked aneurysms under LVIS stent assistance in elderly patients

Objective To summarize operation experiences in applying microcoils embolization for the treatment of intracranial wide-necked aneurysms using the low-profile visualized intraluminal stent support (LVIS) in elderly patients.Methods We reviewed data of 47 elderly patients (48 aneurysms) with intracranial wide-necked aneurysms treated with LVIS stent-assisted microcoils embolization.Results Forty-seven aneurysms were embolized successfully,one stent was withdrawn because its midpiece failed to stretch.Instant Raymond classifications evaluation after the surgery showed Grade Ⅰ in 52.1% (25/48 aneurysms),Grade Ⅱ in 37.5% (18/48 aneurysms),and Grade Ⅲ in 8.3 % (4/48 aneurysms).Besides,the effective rate of aneurysms embolization was 91.5 %.During 3 to 12-month follow-up with digital subtraction angiography (DSA) in 36 patients (36 aneurysms),mRS was scored 0 (best effectiveness) in 28 cases,scored 1 in 4 cases,scored 2 in 3 cases,and scored 3 in 1 case,and the total favorable rate was 97%.Conclusions LVIS stent-assisted microcoils embolization for the treatment of intracranial wide-necked aneurysms in elderly patients may avoid or reduce the incidence of complication.
6.Clinical application of serum miR-122-5p and miR-486-5p in the diagnosis of hepatocellular carcinoma

Objective Explore the relative expression of miR-122-5p and miR-486-5p in the serum of Hepatocellular carcinoma ( HCC) patients and its clinical value .Methods Case-control study was used in this research.From June of 2016 to March of 2017,60 HCC patients who were hospitalized in Guangzhou General Hospital were selected as HCC group .It also selected 20 hepatitis patients ( hepatitis group ) , 20 cirrhosis patients ( cirrhosis group ) , 20 breast cancer patients ( breast cancer group ) , 20 gastric cancer patients(gastric cancer group)and 20 healthy controls (normal control group) for comparison.The relative expression of miR-122-5p and miR-486-5p was detected by real-time fluorescent quantitative PCR.The specificity and sensitivity of miRNAs for the diagnosis of HCC were analyzed by receiver operating characteristic ( ROC) , and the results were compared with the tumor marker AFP .The effect of miRNA on the diagnosis of hepatocellular carcinoma was evaluated by the area under the ROC curve , which was used to detect the diagnostic efficiency of liver cancer .SPSS22.0 statistical software was used for statistical analysis . The rank sum test was applied in the group comparison .Results Serum levels of miR-122-5p in HCC group, hepatitis group, cirrhosis group, breast cancer group, gastric cancer group and control group were 0.14(0.05-0.51),0.45(0.32-0.58),0.53(0.34-0.67),0.14(0.07-0.28),0.29(0.13-0.36) and 0.73 (0.63-0.95),respectively, and the miR-486-5p were 0.50(0.23-0.77),0.62(0.48-0.82),0.65(0.54-0.85),0.23(0.08-0.40),0.29(0.15-0.45)and 0.76(0.69-1.23).The serum levels of miR-122-5p in hepatitis group , cirrhosis group , HCC group were significantly lower in healthy control group , significance was found (U was 315.37,393.46,429.08, all P<0.01), and the serum levels of miR-486-5p in hepatitis group, cirrhosis group, HCC group were lower in healthy control group , significance was found ( U was 103.67,156.18,207.35, all P<0.05).When using one serum marker to diagnosis HCC , AFP had the highest sensitivity ( 73.7%) and miR-122-5p had the highest specificity ( 95%) .While combined two serum markers, AFP +miR-122-5p had the highest sensitivity and specificity (93%),and miR-122-5p +miR-486-5p had the highest specificity (70%), compared AFP +miR-122-5p to AFP, AUC difference was statistically significant(Z=3.02,P<0.01), while there was no significant difference in AUC with AFP +miR-486-5p, miR-122-5p +miR-486-5p to AFP(Z=1.57,1.39,all P>0.05).The sensitivity and specificity of the three markers were 96.5%and 55%respectively , and the area under the ROC curve was 0.891 (95%CI:0.818-0.964).The combination miR-122-5p, miR-486-5p and AFP were higher than the single test, compared with AFP, miR-122-5p, miR-486-5p, the AUC differences was statistically significant (Z=3.26, 3.72, 4.25, all P<0.01).Conclusion Serum miR-122-5p and miR-486-5p could be used as biological markers for the diagnosis of HCC .
7.The therapeutic effect of balloon-assisted coiling and stent-assisted coiling of ruptured intracranial aneurysms in the acute phase

Objective To investigate the perioperative complications and therapeutic effects of balloon-assisted coiling (BAC) and stent-assisted coiling (SAC) in patients with ruptured intracranial aneurysms in the acute phase. Methods Totally 91 patients with 91 intracranial ruptured aneurysms were treated with BAC or SAC in our hospital between January 2014 and December 2016. Among them, 37 patients were treated with BAC and 54 patients with SAC respectively. Of the two groups, the position distribution and shape of aneurysms, and the complications after procedures and the therapeutic effects were summarized and evaluated retrospectively using chi-square test. Results The width of the aneurysm neck was narrower in the BAC-treated group compared to the SAC-treated group (3.31±1.63 mm vs. 4.35±2.10 mm, P=0.01). The aneurysm body/neck ratio (B/N) was lower in the BAC-treated group than in the SAC-treated group (1.64 ± 0.46 vs. 1.35±0.66, P=0.025). The recurrence rate was higher in the BAC-treated group than that in the SAC-treated group (18.9% vs. 0.9%, P=0.005). There was no statistical difference in perioperative complication in both the BAC-treated group and SAC-treated group. However, 2 patients died due to the relative postoperative intracranial bleeding in the SAC-treated group. Better outcomes (Modified Rankin Score, mRS, 0-2) were achieved in the BAC-treated group compared to the SAC-treated group (94.6% vs. 88.9%, P=0.028) at the follow-up visit. Conclusions These findings suggested that there is no difference between the BAC-treated group and the SAC-treated group in the risk of complication. BAC can achieve a better prognosis,but it is more prone to relapse. The SAC method was more appropriate for wider neck aneurysms. It was also an option to coiling the aneurysm in BAC in acute phase firstly, followed by additional treatment in SAC during the follow-up period.
8.Interpleural block via costodiaphragmatic recesses in the treatment of 92 patients with functional dyspepsia

ObjectiveToexplorefeasibilityofinterpleuralblockviacostodiaphragmaticrecessesfortreating functionaldyspepsia(FD).Methods 88FDpatientsincludingthreesubtype:postprandialdistresssyndrome(PDS 24cases),epigastric pain syndrome(EPS 28 cases)and PDS overlapping EPS( 36 cases)were divided into two groups randomly according to FD subtype:treatment group and control group.The control group were treated with paroxetine, metoclopramide and omeprazole,PO,8 weeks per course of treatment;The treatment group were treated by interplural blocks via costodiaphragmatic recesses ( CDR ) on the basis of treatment of control group, sterile syringe needles of 20mL were inserted vertically by the methods of resistance lossing into CDR,the widest position of interplural space through superior borders of the ninth ribs of the affected sides in midaxillary lines.Weekly injections of 20 mL of anti-phlogistic and analgetic solution were given over a four-week period.VAS,SCL-90 and degree of postprandial distress were evaluated before treatment,at 1 week and 6 months after treatment.Results The patients of the two groups a-chieved pain,depression and anxiety and postprandial distress relief obviously after treatment,VAS and SCL-90 de-creased significantly.Before the treatment,the VAS scores of the two group were (7.66 ±1.14) points,(7.57 ± 1.18)points,respectively,which after the treatment were (1.13 ±0.33) points,(3.22 ±0.66) points;Before the treatment,the SCL-90 scores of the two group were (173.56 ±10.21) points, (174.50 ±11.18) points,respectively, which after the treatment were (106.52 ±9.91) points,(106.38 ±10.12) points.There were statistically significant differences between pretreatment and posttreament in the two groups(all P<0.05),and the treatment group was bet-ter and lasted longer than that of control group(all P<0.05).Conclusion Interplural block via CDRs for treating FD is a feasible,simple,safe,effective approach and can be provided on an outpatient basis.
9.Investigation on in-stent recurrent stenosis after stenting of the extra-and intracranial artery in the elderly

Objective To investigate the distribution of in-stent recurrent stenosis (1RS) after balloon-angioplasty and stenting in elderly cerebral ischemic patient and its correlative factors.Methods Totally 158 elderly cerebral ischemic patients with extra-and/or intracranial artery stenosis were treated with 172 stents in our hospital.The inhibition of platelet function was assessed by using the thrombus elasticity chart system in perioperative period,and the anti-platelet agents were adjusted correspondingly.150 patients were followed up by digital subtraction angiography (DSA) and the other 8 patients were followed up by CT angiography,during an average follow-up duration of 8.3 months.Results There were 11 (6.4%) IRS lesions,among which 10 lesions were at the vertebral artery origin and 1 lesion was at common carotid artery bifurcation.23.8% of IRS happened after vertebral artery origin stenting,and all in-stent recurrent stenosis were in the origin of non-dominant vertebral artery,whose IRS rate was 66.7%.Conclusions IRS are more common found in vertebral artery origin than in other parts of vertebral artery and are more common found in nondominant side than in dominant side.
