Objective:To explore the reliability of intraoperative fluoroscopy at lateral 10° and 30° Brodén views in addition to the standard lateral and axial calcaneal views in the lateral decubitus position to assess the quality of articular reduction in calcaneal fractures of Sanders types Ⅱ and Ⅲ.Methods:A retrospective study was conducted to analyze the clinical data of the 74 patients who had been treated at Department of Foot and Ankle Surgery, The People’s Hospital of Linyi for unilateral closed calcaneal fractures of Sanders type Ⅱ or Ⅲ from January 2024, to August 2024. According to the different methods of intraoperative fluoroscopy, the patients were divided into a precision group and a conventional group. In the precision group of 39 cases, intraoperative fluoroscopy was conducted at lateral 10° and 30° Brodén views in the surgery for calcaneal fractures in addition to the standard lateral and axial calcaneal views in the lateral decubitus position; in the conventional group of 35 cases, intraoperative fluoroscopy was conducted only in the standard lateral and axial calcaneal views in the surgery for calcaneal fractures. All patients were treated by traction assisted by external fixation, minimally invasive prying reduction through the tarsal sinus incision, and three-dimensional framework internal fixation. The 2 groups were compared in terms of frequency of intraoperative fluoroscopy; preoperative and postoperative B?hler angles, Gissane angles, and calcaneal varus angles; screw protrusions (more than 2 mm beyond the medial cortex) of the posterior articular surface screw, sustentaculum tali screw, and anterior tuberosity screw; skin irritation symptoms due to the main nail tail (protruding cortex > 1 mm); step-off of the posterior articular surface (more than 2 mm) and wide gap of the posterior articular surface (more than 2 mm). Results:There were no statistically significant differences in the preoperative general data between the 2 groups, indicating comparability ( P>0.05). Both groups showed significant postoperative improvements in X-ray B?hler angle, Gissane angle, and calcaneal varus angle compared with the preoperative values ( P<0.05). There were no statistically significant differences in postoperative X-ray B?hler angle, Gissane angle, or calcaneal varus angle between the 2 groups ( P>0.05). There was no statistically significant difference in the frequency of intraoperative fluoroscopy between the 2 groups either ( P>0.05). The precision group had significantly fewer cases of screw protrusion (more than 2 mm beyond the medial cortex) of the posterior articular surface screw, sustentaculum tali screw, and anterior tuberosity screw, skin irritation symptoms due to the main screw tail (protruding cortex>1 mm), step-off of the posterior articular surface (more than 2 mm), and wide gap of the posterior articular surface (more than 2 mm) on the postoperative CT three-dimensional reconstruction compared with the conventional group ( P<0.05). Conclusion:In surgery for calcaneal fractures of Sanders types Ⅱ and Ⅲ, intraoperative fluoroscopy at lateral 10° and 30° Brodén views in addition to the standard lateral and axial calcaneal views in the lateral decubitus position provides stable and reliable intraoperative monitoring of B?hler angle, Gissane angle, calcaneal varus angle, reduction of the posterior articular surface of the calcaneus and the positions and lengths of implants.

Objective:To investigate the effects and mechanism of very long chain acyl-CoA dehydrogenase(VLCAD)on prolifera-tion,invasion and metastasis of oral squamous cell carcinoma cells. Methods:The expression levels of VLCAD and glucose trans-porter 1(GLUT1)in human normal oral epithelial cell line(HOEC cells)and human oral squamous cell carcinoma line(SCC25 cells)were determined by Western blot, and the expression levels of VLCAD and GLUT1 in oral squamous cell carcinoma patients and their correlation with pathological stages and prognosis were analyzed in the TCGA database.SCC25 cell lines with VLCAD over-expression and knockdown were constructed and the expression level of VLCAD and GLUT1,cell proliferation, migration, invasion ability, lactate dehydrogenase content and lactate production level were detected respectively. Results:The expression level of VL-CAD in SCC25 cells was significantly lower than that in HOEC cells. The expression level of GLUT1 in SCC25 cells was significantly higher than that in HOEC cells. There was a significant negative correlation between the two expression levels(P<0.05).VLCAD overexpression significantly inhibited GLUT1 expression and aerobic glycolysis in SCC25 cells, and also inhibited cell proliferation, migration and invasion.VLCAD knockdown effectively reversed the antitumor effects of overexpression mediated by VLCAD(P<0.05).Conclusion:VLCAD inhibits proliferation, invasion, and metastasis of oral squamous cell carcinoma SCC25 cells by down-regulating GLUT1 and inhibiting its mediated aerobic glycolysis.

China Pharmacy 2024;35(8):972-979
OBJECTIVE To systematically review the pharmacoeconomic evaluation literature about proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitors for the prevention of cardiovascular disease in patients with hypercholesterolemia, and to provide a reference for clinical treatment, health decision-making and future follow-up research. METHODS Retrieved from English and Chinese databases such as PubMed and CNKI, the pharmacoeconomic literature about PCSK9 inhibitors (evolocumab and alirocumab) for the prevention of cardiovascular disease in patients with hypercholesterolemia was collected from the establishment of the database to October 8, 2023. The quality of the included literature was assessed with Consolidated Health Economic Evaluation Reporting Standards 2022 (CHEERS 2022) scale. The descriptive analysis was performed for basic information, model structure, related parameters, sensitivity analysis and the results of included studies. RESULTS & CONCLUSIONS A total of 29 literature were included, with overall good quality. The evaluation mainly adopted the Markov model from the healthcare system, payer and societal perspective. The effectiveness and utility data mainly came from the previous studies; the direct cost was mainly considered with a discount rate of 1.5%-5.0% per year, while the willingness-to-pay threshold was often set at 1-3 times the gross domestic product per capita. Most health output indicators were measured in life years and quality-adjusted life years. The sensitivity analysis of most studies demonstrated the robustness and the main influential factor was the drug cost. Most Chinese studies showed that PCSK9 inhibitor was not cost-effective for the prevention of cardiovascular disease in patients with acute coronary syndrome, myocardial infarction and atherosclerotic cardiovascular disease. It was cost-effective to use PCSK9 inhibitors for the prevention of cardiovascular disease only in specific groups, such as patients with triple vessel disease, patients with newly diagnosed acute coronary syndrome and low-density lipoprotein cholesterol≥100 mg/dL. Future research should refer to the CHEERS 2022 scale to improve the design, and strive to select large-scale, high-quality data to enhance the quality of reports and the transparency of health decisions, so as to more accurately assess the cost-effectiveness of PCSK9 inhibitors.
