1.Identification of diagnostic biomarkers for metastatic lymph nodes in oral squamous cell carcinoma using spatial metabolomics

Objective:To uncover alterations in the metabolic microenvironment of lymph node metastasis (LNM) in oral squamous cell carcinoma (OSCC) and identify potential metabolic biomarkers for the early diagnosis of LNM using desorption electrospray ionization mass spectrometry imaging (DESI-MSI) spatial metabolomics.Methods:Six OSCC patients with LNM, who underwent neck dissection surgery at the Department of Oral and Maxillofacial Surgery, Affiliated Hospital of Jiangsu University between October 2020 and October 2022, were enrolled. Matched metastatically involved (positive) and benign (negative) lymph node tissue samples were collected and analyzed using DESI-MSI. Univariate and multivariate statistical analyses were employed to identify differentially abundant metabolites. The diagnostic efficacy of these metabolites was evaluated using receiver operating characteristic (ROC) curve analysis. Finally, Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway analysis was performed to determine the implicated metabolic pathways.Results:A total of 62 and 29 differentially abundant metabolites were identified in the metastatically involved lymph nodes compared to benign lymph nodes under positive-ion mode and negative-ion mode, respectively. These metabolites were predominantly amino acids and lipids. Four metabolites common to both ionization modes were selected for ROC curve analysis: phenylalanine, phosphoethanolamine, histidine, and taurine. The area under the curve values were 0.861, 0.802, 0.729, and 0.722, respectively, indicating promising diagnostic performance. Metabolic pathway analysis revealed significantly heightened activity in Amino acid metabolism ( P=0.469) and Glycerophospholipid metabolism ( P=0.006) within the LNM microenvironment. Conclusions:This DESI-MSI-based study identified disruptions in amino acid and glycerophospholipid metabolism within OSCC metastatic lymph node tissues. The associated differentially abundant metabolites represent potential candidate molecules for diagnosing OSCC LNM.
2.Analysis of radioactivity monitoring results of atmospheric fallout in the area around Daya Bay Nuclear Power Plant, 2019-2022

Objective To analyze the activity concentrations of gross α, gross β, and radionuclides in atmospheric fallout around Daya Bay Nuclear Power Plant from 2019 to 2022, and provide foundational scientific data for the healthy development of nuclear energy. Methods Five monitoring sites were set up at different distances (1.2, 6.9, 12.4, 42.3, and 69.2 km) from Daya Bay Nuclear Island 1. Stainless steel sampling barrels were used to collect atmospheric fallout, with a monitoring cycle of three months. The collected samples were ashed using radiochemical method. Subsequently, the gross α and gross β radioactivity were quantified using a low-background α/β measurement instrument. The remaining ash samples were mixed with water and left to equilibrate for over three weeks before γ-nuclide analysis using a low-background, high-purity germanium γ-spectrometer. Results The gross α activity concentration in atmospheric fallout around Daya Bay Nuclear Power Plant from 2019 to 2022 averaged (25.3 ± 10.6) Bq/m2/season and ranged from 5.8 to 73.4 Bq/m2/season. The gross α activity concentrations in sampling sites #1 to #5 were 5.8-34.4, 11.9-35.2, 14.4-46.4, 7.2-73.4, and 13.1-43.1 Bq/m2/season, respectively. The gross β activity concentration averaged (50.5 ± 23.4) Bq/m2/season and ranged from 13.9 to 139.3 Bq/m2/season. The gross β activity concentrations of sampling sites #1 to #5 were 17.1-107.4, 17.6-87.5, 25.8-102.0, 13.9-139.3, and 23.4-99.2 Bq/m2/season, resprctively. The activity concentrations of 238U, 226Ra, 232Th, 40K, 210Pb, and 7Be in atmospheric fallout were < lower limit of detection (LLD)-4.2, < LLD-5.8, < LLD-6.3, < LLD-42.1, < LLD-514.0, and 35.7-

4.Efficacy of intraoperative sliding CT assisted deep brain stimulation of subthalaminal nucleus in short-term motor improvement in patients with Parkinson's disease under general anesthesia

Objective:To evaluate the efficacy of intraoperative sliding CT assisted deep brain stimulation of subthalaminal nucleus (STN-DBS) in short-term motor improvement and its influencing factors in patients with Parkinson's disease (PD) under general anesthesia.Methods:The cohort consisted of 48 PD patients accepted STN-DBS in Department of Neurosurgery, First Affiliated Hospital of Zhengzhou University from June 2021 to June 2023, including 21 patients accepted STN-DBS under general anesthesia and 27 patients accepted STN-DBS under local anesthesia. STN-DBS was performed in the CT operating room and intraoperative CT was used to confirm the electrode position. Regular program-controlled follow-up for over one year in Out-patient Clinic was applied after discharge. Operation time, complications, stimulus-related adverse events, unified Parkinson's disease rating scale (UPDRS)-Ⅲ score and UPDRS-Ⅲ improvement rate at medicine-off (Med-off) stage/DBS-on stage 1 year after operation, and UPDRS-Ⅲ score and UPDRS-Ⅲ improvement rate at medicine-on (Med-on) stage/DBS-on stage 1 year after operation, and levodopa equivalent daily dose (LEDD) and LEDD reduction were compared between the two groups. Spearman rank correlation analysis or Pearson correlation analysis were used to investigate the correlation between short-term motor improvement ([UPDRS-Ⅲ score at Med-off stage before operation-UPDRS-Ⅲ score at Med-off stage/DBS-on stage 1 year after operation]/UPDRS-Ⅲ score at Med-off stage before operation×100%) and clinical data in PD after STN-DBS under general anesthesia.Results:The operation time of general anesthesia group was (1.9±0.5) h, and that of local anesthesia group was (2.6±0.8) h, with significant difference ( P<0.05). During the 1-year follow-up, no significant difference in incidence of adverse reactions, UPDRS-Ⅲ score improvement rate at Med-on and Med-off stages, LEDD and LEDD reduction was noted between the two groups ( P>0.05). In the general anesthesia group, the short-term motor improvement after STN-DBS was negatively correlated with age or gender ( P>0.05), but positively correlated with maximum improvement rate of preoperative UPDRS-Ⅲ score ( r=0.840, P<0.001). Conclusion:For patients who cannot tolerate local anesthesia, DBS under general anesthesia is also safe and effective.
5.Five new meroterpenoids from Rhododendron anthopogonoides and their anti-inflammatory activity.

Five meroterpenoids, rhodonoids K-M (1-2), daurichromene E (3), and grifolins A-B (4-5), together with seven known compounds (6-12), were isolated from Rhododendron anthopogonoides. The chemical structures of these compounds were elucidated through comprehensive analysis of high-resolution electrospray ionization mass spectrometry (HR-ESI-MS), ultraviolet (UV), infrared spectroscopy (IR), and nuclear magnetic resonance (NMR) data. Their absolute configurations were determined by comparing experimental electronic circular dichroism (ECD) spectra with computed values. Notably, compounds 1 and 3 demonstrated significant inhibitory effects on lipopolysaccharide (LPS)-induced inflammation in RAW264.7 cells. These compounds markedly suppressed the mRNA expressions of inflammatory factors, including interleukin (IL)-1β, IL-6, and tumor necrosis factor-α (TNF-α) while also down-regulating the protein expressions of inducible nitric oxide synthase (iNOS) and cyclooxygenase-2 (COX-2).

7.Reflections on the role of technological innovation in facilitating high-quality development of hospital discipline construction

Objective:This study aims to explore how technological innovation can promote high-quality development of hospital discipline construction.Methods:The study elucidated the connotation, significance, and current status of high-quality development in discipline construction. Taking the Orthopedic Department of a tertiary hospital in Beijing as an example, it analyzed the problems and deficiencies in hospital discipline construction in China. Additionally, specific suggestions were put forward on how technological innovation can facilitate high-quality development of hospital discipline construction in China.Results:Strengthening technological innovation capacity, actively exploring collaborative technological innovation, optimizing research platform construction, enhancing talent cultivation and recruitment, and strengthening awareness of translating clinical research findings into practice were important approaches to achieving high-quality development of hospital discipline construction. These measures provided solid support for the high-quality development of hospitals.Conclusions:By enhancing the vitality of technological innovation and promoting the integration and coordinated development of disciplinary construction and technological innovation, it can assist in the high-quality development of medical disciplines in our country. Furthermore, it can accelerate the advancement of high-level first-class disciplinary construction, serving as an inexhaustible driving force for the cohesive high-quality development of Chinese hospitals.
8.Research of impacts on medical visit behaviors of patients due to the two reforms of the clinic-pharmacy-separation at a hospital in Beijing

Objective To study the impacts and the difference made by the pilot reform and comprehensive reform of clinic-pharmacy-separation on the medical visit behaviors of outpatients in Beijing.Methods Invoice information of outpatients′ fee payment during the December 2012 reform and April 2017 reform at the hospital was collected.Information of these patients at the same time as controls was collected to analyze patients′ fee categories, so as to study the effect on the behavior of these patients.Results The outpatient visits decreased by 5.29% as a result of the pilot reform.The ratio of medical insurance patients rose by 6.86%, and that of such patients seeking only prescriptions rose by 4.30%;number of outpatient visits decreased by 2.03% after the comprehensive reform, while the ratio of medical insurance outpatients dropped by 2.61%, and those seeking prescription only dropped by 2.54%.Conclusions The comprehensive reform of clinic-pharmacy-separation has influenced the outpatient′s medical visit behavior, evidencing a positive outcome of the healthcare reform.
9.Etiological classification of virus in adult community-acquired pneumonia in Yantai

To employ multiplex polymerase chain reaction (PCR) to classify the types of virus in 128 patients of community-acquired pneumonia and analyze the relationship between type of virus,age and seasons.The positive rate of viral pneumonia was 34.4% (44/128).In 44 virus positive patients,the rates of influenza A virus,rhinovirus,adenovirus,parainfluenza type 1,human metapneumovirus virus,influenza B virus,parainfluenza type 2,parainfluenza type 3 and enterovirus were 22.7% (10/44),15.9% (7/44),15.9% (7/44),9.1% (4/44),9.1% (4/44)6.8% (3/44),2.3% (1/44),2.3% (1/44) and 2.3% (1/44) respectively.And other 6(13.6%) patients were at least infected by one virus.The viral pneumonia had a high positive rate in spring and winter.And the age group of 20 to 29 years had a high positive rate.The major pathogen contributing to adult community-acquired pneumonia in Yantai were influenza A virus,rhinovirus and adenovirus.
