1.Safety and efficacy of argon-helium cryoablation combined with targeted therapy and anti-programmed death-1 monoclonal antibody in treatment of patients with unresectable hepatocellular carcinoma aged 60 years or older

ObjectiveTo investigate whether anti-programmed death-1 (PD-1) monoclonal antibody can enhance the efficacy and safety of argon-helium cryoablation combined with targeted therapy in patients with unresectable hepatocellular carcinoma (uHCC) aged 60 years or older. MethodsA retrospective analysis was performed for the clinical data of 124 patients with advanced uHCC aged 60 years or older who were treated at The Fifth Medical Center of Chinese PLA General Hospital from January 2013 to September 2024. After propensity score matching, 57 patients received cryoablation combined with targeted therapy (double combination group), while 57 received cryoablation combined with targeted therapy and anti-PD-1 monoclonal antibody (triple combination group). The indicators for efficacy assessment included objective response rate (ORR), disease control rate (DCR), progression-free survival (PFS), overall survival (OS), and the incidence rate of adverse events. The Mann-Whitney U test was used for comparison of continuous data between two groups, and the chi-square test or the Fisher’s exact test was used for comparison of categorical data between two groups. The Kaplan-Meier method was used to plot survival curves, and the Log-rank test was used for comparison between groups. A Cox proportional-hazards regression model analysis was used to investigate the influencing factors for survival prognosis. ResultsThe triple combination group had a significantly higher ORR than the double combination group (59.6% vs 29.8%, χ2=9.083, P=0.003), while there was no significant difference in DCR between the two groups (87.7% vs 77.2%, χ2=1.516, P=0.218), and compared with the double combination group, the triple combination group had significantly longer median PFS (9.1 months vs 4.8 months, χ2=7.813, P=0.005) and median OS (26.1 months vs 13.6 months, χ2=14.199, P<0.001). The multivariate Cox proportional-hazards regression model analysis showed that triple combination treatment was an independent influencing factor for PFS (hazard ratio [HR]=0.52, 95% confidence interval [CI]: 0.35 — 0.78, P=0.001) and OS (HR=0.32, 95%CI: 0.20 — 0.51, P<0.001). There was no significant difference in the incidence rate of adverse events between the two groups (P>0.05). ConclusionTriple combination treatment with argon-helium cryoablation, targeted therapy, and anti-PD-1 monoclonal antibody can significantly improve survival benefits in uHCC patients aged 60 years or older, with a controllable safety profile.

Objective:To develop a treatment-related quality of life scale for Chinese patients with multiple myeloma (MM) and to test its reliability and validity.Methods:The initial scale was constructed through a literature search, Delphi expert correspondence, and cognitive testing. This study conducted a preliminary survey of 379 patients with MM and a formal survey of 865 patients from the hematology departments of 155 hospitals nationwide from February 2024 to March 2024. The final scale was obtained after conducting item analysis and reliability and validity tests on the initial scale.Results:The constructed scale contains 36 items covering six domains: physiological, psychological, social, treatment side effects, general health, and others. In the preliminary survey, the Cronbach’s alpha coefficient of each item ranged from 0.597 to 0.939, and the test-retest reliability was 0.747 ( P<0.001). Exploratory factor analysis extracted eight common factors with a cumulative variance contribution of 60.058%. In the formal survey, the Cronbach’s alpha coefficient of each item ranged from 0.484 to 0.930, and the test-retest reliability was 0.835 ( P<0.001). Confirmatory factor analysis revealed a comparative fit index of 0.750, a root-mean-square error of approximation of 0.090, and a root-mean-square residual of 0.067. Conclusion:The treatment-related quality of life scale for Chinese patients with MM designed in this study exhibited good reliability and validity, reflecting the impact of treatment on the quality of life of patients. This scale can provide a reference to clinicians for assessing the disease status of patients.
3.Application of a remote occupational health management model combining cluster management and individualized guidance in the treatment of pneumoconiosis

Objective:To explore the effects of the "group+individual" combined internet occupational health management model on the physiological indicators, pulmonary imaging changes and self-management ability of people with pneumoconiosis.Methods:In November 2022, patients diagnosed with pneumoconiosis in Baotou city from January 2012 to January 2022 were selected. Eighty cases exhibiting a decline in forcect expiratory volume in one second (FEV 1) and pulmonary imaging changes (primarily ground-glass opacity variations) were chosen as study subjects. These patients were randomly divided into a control group and an experimental group using the sealed envelope method, with 40 patients in each group. The control group received conventional outpatient follow-up and occupational health surveillance management, while the experimental group underwent a remote occupational health management model combining "group+individual" approaches over 12 months, with interventions administered once every 3 months. Comparative analysis was conducted on pre-and pos-management indicators between the two groups, including pulmonary function, pulmonary imaging changes, and self-management ability scores. Results:After occupational health management, the experimental group showed superior improvement in observed indicators (including pulmonary function and pulmonary imaging indicators) compared to the control group ( P<0.05). The self-management ability scores of the experimental group after occupational health management were also better than those of the control group (all P<0.05) . Conclusion:The remote occupational health management model combining group management and individualized guidance, when applied to patients with pneumoconiosis, is beneficial for improving their physiological indicators, pulmonary imaging indicators, and enhancing self-management abilities.
4.Analysis of human papillomavirus subtype distribution and cervical lesion characteristics in a cancer hospital in Shenzhen City

This study aims to retrospectively analyze the distribution of human papillomavirus (HPV) subtypes and cervical lesion characteristics in female patients at Shenzhen Cancer Hospital. Adopting a retrospective observation research method. Using convenience sampling, the study subjects were selected from women who visited the Shenzhen Hospital of the Chinese Academy of Medical Sciences Cancer Hospital from September 2020 to December 2023.HPV genotyping was performed on cervical exfoliated cells from 6 402 women using rapid nucleic acid hybridization. Additionally, thinprep cytologic test (TCT) was conducted on 845 HPV-positive samples. High-grade squamous intraepithelial lesions (HSIL) were further classified as cervical intraepithelial neoplasia grades 2 or 3 (CIN2/3) through histopathological examination via cervical biopsy or conization. Data were statistically analyzed using χ2 test and two-sided test. The results showed that HPV testing identified 1 205 HPV-positive cases, with an overall positivity rate of 18.8% (1 205/6 402). Among these, the positivity rate for high-risk(HR) HPV was 14.34%(918/6 402), low-risk(LR) HPV was 3.58%(229/6 402), and other HPV types accounted for 0.91% (58/6 402). The most prevalent high-risk HPV subtypes were HPV16, HPV52, HPV39, HPV58, and HPV53, with positivity rates of 3.16% (202/6 402), 2.92% (187/6 402), 1.34% (86/6 402), 1.30%(83/6 402), and 1.05%(67/6 402), respectively. HPV16 had the highest infection rate in cervical malignancies, CIN3, and CIN2/3 groups, with rates of 53.18%(92/173), 47.37%(27/57), and 34.78%(8/23), respectively. Conversely, HPV52 showed the highest infection rate in the atypical squamous cells(ASC), Low-grade squamous intraepithelial lesion(LSIL), and CIN2 groups, at 29.68%(19/64), 23.50%(43/183), and 37.93%(11/29), respectively. Among 845 HPV positive female patients, the pathological diagnosis was squamous cell carcinoma, accounting for 22.13% (187/845), and adenocarcinoma accounting for 1.42%(12/845). HPV infection rates increased with age: 16.39%(97/592) in the ≤30 age group, 14.46%(301/2 082) in the 31-40 group, 17.26%(318/1 842) in the 41-50 group, 23.98%(300/1 251) in the 51-60 group, and 29.76%(189/635) in those over 60. In conclusion,in the female patient population of the Shenzhen Hospital of the Chinese Academy of Medical Sciences Cancer Hospital, HPV16 is associated with cervical cancer and high-grade cervical lesions, while HPV52 is related to low-to moderate-grade cervical lesions. Enhancing HPV screening among adult women is essential for preventing cervical cancer and associated lesions.
5.The efficacy of treating tympanic membrane perforation under CIM-EES without tympanomeatal flap elevation

Objective To investigate the clinical efficacy of continuous irrigating mode of endoscopic ear sur-gery(CIM-EES)utilizing tragus cartilage-perichondrium without tympanomeatal flap elevation in repairing tympanic membrane perforation.Methods The date of 70 patients(70 ears)who underwent tympanic membrane repair under ear endoscopy from June 2020 to August 2023 were randomly selected and analyzed.They were divided into two groups according to the time of operation:CIM-EES group(observation group)of 44 cases(44 ears)and a conven-tional surgery group(control group)of 26 cases(26 ears).Both groups were repaired with tympanic membrane per-foration using the method of implanting tragus cartilage-perichondrium grafts under endoscope without tympa-nomeatal flap elevation.After 6 months of postoperative follow-up,two groups were compared in terms of operation time,frequency of the endoscopic lens scrubbing,postoperative healing rate,and hearing improvement outcomes.Results The average operation time of the observation group and the control group was 37.50±4.81 minutes and 50.31±8.21 minutes respectively,and the average number of scrubbing the endoscope was 6.77±1.51 and 35.54±7.13 respectively,there was statistical difference significance between the two groups(P<0.01).All patients in the observation group were successfully repaired in the first stage,with the healing rate of 100%(44/44),and one patient in the control group had a postoperative small perforation that healed after secondary repair with the healing rate of 96.15%(25/26).There was no statistically significant difference between the two groups.The average 6 months postoperative air conduction threshold and air-bone conduction threshold of 0.5-4 kHz in the two groups improved compared to preoperative results(P<0.01),with no statistically significant difference between the two groups(P>0.05).Conclusion Endoscopic myringoplasty without tympanomeatal flap elevation under continuous irrigating mode has the advantages of high healing rate,short operation time,simple surgical operation,few post operation complications and good hearing improvement.
6.Association of serum exosomal miR-122-5p with the prognosis of hepatic confluent necrosis and fibrosis in patients with chronic hepatitis B

Objective To investigate the association of serum exosomal microRNAs(miRNAs)with hepatic inflammatory injury and histological outcomes in patients with chronic hepatitis B(CHB).Methods Peripheral serum samples were collected from six healthy adults and six patients with CHB,and size exclusion chromatography was used to extract exosomes.Small RNA sequencing and transcriptomic analysis were used to identify the serum exosomal miRNAs associated with liver inflammatory injury and fibrosis,and quantitative real-time PCR was used for validation in a mouse model of acute liver injury induced by lipopolysaccharide/D-galactosamine,a rat model of liver fibrosis induced by carbon tetrachloride,and 84 CHB patients undergoing liver biopsy twice before and after treatment.The independent-samples t test was used for comparison of normally distributed continuous data between two groups;an analysis of variance was used for comparison between multiple groups,and the Tukey test was used for further comparison between two groups.The Mann-Whitney U test was used for comparison of non-normally distributed continuous data between two groups;the Kruskal-Wallis H test was used for comparison between multiple groups,and the Dunn test was used for further comparison between two groups.The chi-square test or the Fisher's exact test was used for comparison of categorical data between groups.The univariate and multivariate Logistic regression analyses were used to investigate influencing factors.Results Abnormal expression of serum exosomal miR-122-5p was observed in patients with CHB,and it was downregulated in patients with confluent necrosis and advanced fibrosis.In the mouse model of acute liver injury and the rat model of liver fibrosis,compared with the control group,the model group had a significant reduction in the expression level of miR-122-5p in the liver(P=0.048 and 0.014),and compared with the patients with mild liver injury,the patients with severe confluent necrosis and advanced fibrosis showed a significant reduction in the expression level of miR-122-5p in liver tissue(P<0.05).Among the 84 CHB patients,the patients with severe hepatic confluent necrosis or advanced liver fibrosis had a significantly lower expression level of serum exosomal miR-122-5p than those with mild liver injury(P<0.001 and P=0.003).The multivariate Logistic regression analysis showed that the expression level of miR-122-5p was an independent influencing factor for confluent necrosis(odds ratio[OR]=0.001,95%confidence interval[CI]:0.000-0.037,P=0.005)and liver fibrosis degree(OR=0.568,95%CI:0.331-0.856,P=0.019).In addition,compared with the patients with low expression of miR-122-5p,the patients with high expression of miR-122-5p before treatment had a significantly higher reversal rate of liver fibrosis after 72 weeks of antiviral therapy(64.3%vs 38.1%,P=0.029).Conclusion Serum exosomal miR-122-5p in CHB patients is closely associated with the progression of hepatic confluent necrosis and fibrosis,and the reduction in the expression level of miR-122-5p may aggravate hepatic confluent necrosis,promote the progression of fibrosis,and affect the histological outcome of CHB patients after antiviral therapy.
