1.Dual-energy CT quantitative parameters predict the short-term treatment efficacy of second-generation epidermal growth factor receptor-tyrosine kinase inhibitors in lung adenocarcinoma

Objective To investigate the predictive value of dual-energy CT quantitative parameters for assessing the short-term treatment efficacy of second-generation epidermal growth factor receptor-tyrosine kinase inhibitors(EGFR-TKIs)in patients with advanced lung adenocarcinoma.Methods A total of 77 patients with advanced lung adenocarcinoma who received second-generation EGFR-TKIs treatment were retrospectively included.All patients underwent non-contrast and contrast-enhanced dual-energy CT scans.The patients were divided into effective group(45 cases)and ineffective group(32 cases).The clinical data and CT morpho-logical features of the patients were collected,and the iodine concentration(ICA,ICV),normalized iodine concentration(NICA,NICV),and spectral curve slope(kA,kV)in the arterial and venous phases were obtained.The predictive model was constructed using statistically significant intergroup differences,and the predictive performance of the model was evaluated via the area under the curve(AUC)of the receiver operating characteristic(ROC)curves,calibration curves,and decision curve analysis(DCA).Results Signifi-cant differences were observed in ICA 90 keV,ICA 150 keV,kA and NICA 90 keV,NICA 150 keV between the effective group and the ineffective group(P<0.05).Univariate and multivariate logistic regression analyses identified that ICA 90 keV and ICA 150 keV as independent predictors of the short-term treatment efficacy of second-generation EGFR-TKIs.The AUC of the logistic regression model was 0.96[(95%confidence interval(CI)0.87-0.97].Conclusion The quantitative parameters of dual-energy CT can predict the short-term therapeutic efficacy of EGFR-TKIs in advanced lung adenocarcinoma to a certain extent.

3.Influence of blood pressure level on optical coherence tomography angiography parameters in patients with essential hypertension

Objective To analyze the changes in optical coherence tomography angiography (OCTA) parameters in patients with essential hypertension,and to explore the effect of blood pressure on OCTA parameters. Methods A total of 164 patients with essential hypertension were selected and divided into controlled blood pressure group (n=92) and uncontrolled blood pressure group (n=72). OCTA examination was performed on the optic disc and macula of all patients, and the right eyes were selected for analysis. Results There were no significant differences in retinal nerve fiber layer (RNFL) thickness, radial peripapillary capillary (RPC) total vascular density, RPC total small vessel density, perifovea superficial capillary plexus (SCP) vascular density, and perifovea deep capillary plexus (DCP) vascular density between the two groups of patients. There were no significant differences in foveal avascular zone (FAZ) area, FAZ diameter, and fovea retinal thickness between the two groups of patients. The density of the parafovea SCP, parafovea DCP, and fractal dimension (FD) in the uncontrolled blood pressure group were significantly lower than those in the controlled blood pressure group (P<0.05). Multiple linear regression analysis showed that elevation of blood pressure was a independently related factor of reduced parafovea DCP density (P=0.026), while there was no correlation between the uncontrolled blood pressure and parafovea SCP density and FD level. Conclusions The blood pressure level is correlated with the parafovea DCP density, while has no correlation with other OCTA parameters in hypertension patients.
4.Short-segment pedicle screw fixation after vertebroplasty augmentation for stage Ⅲ Kümmell's disease with neurologic symptoms

Objective:To evaluate the outcomes of short-segment pedicle screw fixation after vertebroplasty augmentation using polymethylmethacrylate (PMMA) and laminectomy for stage Ⅲ Kümmell's disease with neurological symptoms.Methods:A retrospective study was conducted to analyze the clinical data of the 23 patients who had been treated at Department of Spinal Surgery, Beijing Jishuitan Hospital from January 2016 to August 2022 for single level stage Ⅲ Kümmell's disease with neurological symptoms by short-segment pedicle screw fixation after vertebroplasty augmentation using PMMA and laminectomy. There were 9 males and 14 females, with an age of (69.7±3.8) years. The visual analogue scale (VAS) for low back pain, Oswestry disability index (ODI) for low back pain, kyphotic Cobb angle of the fixed segment, and loss of the anterior vertebral body height were compared between preoperation, 2 weeks after operation, 2 years after operation, and the last follow-up. The improvements in American Spinal Injury Association (ASIA) impairment grading at the last follow-up compared to the preoperative levels, bone graft fusion rates at the last follow-up and complications were recorded.Results:All patients were followed up for (57.4±17.7) months after operation. The VAS pain scores [(2.2±0.3) points, (1.8±0.6) points and (1.6±0.5) points], ODIs (23.9%±4.5%, 21.6%±4.5% and 19.1%±3.8%), kyphotic Cobb angles of the fixed segments (12.2°±2.7°, 12.5°±2.6° and 12.8°±2.8°) and losses of the anterior vertebral body height (20.0%±3.4%, 20.2%±3.5% and 20.3%±3.5%) at 2 weeks after operation, 2 years after operation, and the last follow-up were significantly improved compared to the pre-operative values [(7.8±0.5) points, 79.7%±5.1%, 40.2°±6.2°, and 60.4%±14.2%, respectively] ( P<0.05). However, there were no significantly differences in the above values between 2 weeks after operation, 2 years after operation and the last follow-up ( P>0.05). Of the 2 patients with grade C, the ASIA grading at the last follow-up improved to grade D in one and to grade E in the other; of the 12 patients with grade D, the ASIA grading at the last follow-up improved to grade E in 11. The complications included 2 asymptomatic cases of bone cement leakage from the upper endplate of the affected vertebra, 1 asymptomatic case of bone cement leakage from the lower endplate of the affected vertebra, 1 asymptomatic case of bone cement leakage from the spinal canal, and 1 case of distant vertebral compression fracture. The last follow-up showed good bone graft fusion in all patients. Conclusion:In the treatment of stage Ⅲ Kümmell's disease with neurological symptoms, short-segment pedicle screw fixation after vertebroplasty augmentation using PMMA is a relatively minimally invasive, safe, and effective treatment, because it can alleviate back pain, improve neurological function and daily function, correct thoracolumbar kyphosis, restore vertebral height and reconstruct spinal stability without significant complications.

6.Screening of hepatoma-derived growth factor with highly expression in HCC bone metastasis and its cellular biological effects

Objective To investigate the expression of hepatoma-derived growth factor(HDGF)in hepatocellular carcinoma(HCC)and its effects on DNA damage,proliferation,migration,epithelial mesenchymal transition(EMT),and expression of osteoprotegerin(OPG)in HCC cells.Methods GEO database was analyzed to obtain the expression features of differential expressed mRNAs in liver tumors between HCC patients with bone metastasis and those without,and then HDGF was found as the most significantly differentially expressed gene.The mRNA expression of HDGF was verified in the HCC cells with varying metastatic capabilities by qRT-PCR.DNA damage levels in different groups of liver cancer cells were analyzed through y-H2AX immunofluorescence assay.The effects of HDGF on cell proliferation and migration were detected with CCK-8 assay,colony formation assay,and Transwell migration assay.Western blotting was employed to measure the expression of EMT-related proteins Vimentin and E-cadherin in HCC cells with various capacities of bone metastasis.Then qRT-PCR and Western blotting were performed to determine the differential expression of OPG in various groups of liver cancer cells.Results HDGF was highly expressed in HCC tissues with bone metastasis and in high-metastatic HCC cell lines.High expression of HDGF inhibited DNA damage repair.HDGF promoted the proliferation and migration of HCC cell lines.HDGF up-regulated the protein expression level of Vimentin while down-regulated that of E-cadherin(both P<0.05),and overexpression of HDGF down-regulated the expression of OPG.Conclusion HDGF is highly expressed in bone metastatic hepatocellular tumors and may promote bone metastasis by facilitating EMT,migration,and proliferation.
7.Roles of intestinal microecological preparations in intra-abdominal infection

Intra-abdominal infection is the second common infectious disease in intensive care unit and inhospital patients, with the mortality rate of 20%-30%. Advances in medicine have not improved the outcomes of patients with intra-abdominal infection, and the increasing multi-drug resistance organism may lead to a deterioration in the prognosis of patients with intra-abdominal infection. Gut microbiota dysbiosis and abdominal cavity infections show an interdependent and mutually aggravating relationship. Intestinal microecological preparations regulate gut flora and are potential therapeutic measures for intra-abdominal infections. The authors review the changes in gut flora during intra-abdominal infection, the effect of gut flora on the prognosis of intra-abdominal infections and the role of intestinal microecological preparations in intra-abdominal infections.
8.The Role of Exportin-5 in MicroRNA Biogenesis and Cancer.

MicroRNAs (miRNAs) are conserved small non-coding RNAs that play an important role in the regulation of gene expression and participate in a variety of biological processes. The biogenesis of miRNAs is tightly controlled at multiple steps, such as transcription of miRNA genes, processing by Drosha and Dicer, and transportation of precursor miRNAs (pre-miRNAs) from the nucleus to the cytoplasm by exportin-5 (XPO5). Given the critical role of nuclear export of pre-miRNAs in miRNA biogenesis, any alterations of XPO5, resulting from either genetic mutation, epigenetic change, abnormal expression level or posttranslational modification, could affect miRNA expression and thus have profound effects on tumorigenesis. Importantly, XPO5 phosphorylation by ERK kinase and its cis/trans isomerization by the prolyl isomerase Pin1 impair XPO5's nucleo-to-cytoplasmic transport ability of pre-miRNAs, leading to downregulation of mature miRNAs in hepatocellular carcinoma. In this review, we focus on how XPO5 transports pre-miRNAs in the cells and summarize the dysregulation of XPO5 in human tumors.

9.Clinical value of combined detection of tumor biomarkers in diagnosis , pathological typing and clinical staging in lung cancer

Objective To investigate the clinical value of combined detection of serum carcinoembryonic an-tigen(CEA) ,carbohydrate antigen-125 (CA125) ,cytokeratin 19 fragment antigen (CYFRA21-1) ,squamous cell carcinoma(SCC) antigen ,neuron-specific enolase(NSE) and plasma progastrin-releasing peptid(ProGRP) in the diagnosis ,pathological typing and clinical staging in lung cancer .Methods The serum CEA ,CA125 , CYFRA21-1 ,SCC ,NSE levels and plasma ProGRP level were detected by adopting the chemiluminescent mi-croparticle immunoassay method in 378 cases of lung cancer ,200 cases of benign lung diseases and 200 people undergoing healthy physical examination .Results The levels and positive rates of CEA ,CA125 ,CYFRA21-1 , SCC ,NSE and ProGRP in the patients with lung cancer were significantly higher than those in the patients with benign lung diseases and healthy control group ,the difference was statistically significant (P<0 .05);in the single index detection ,the biomarkers of highest positive rate in adenocarcinoma ,squamous cell carcinoma and small cell lung carcinoma were CEA ,CYFRA21-1 and SCC ,NSE and ProGRP respectively .The sensitivi-ty ,specificity ,accuracy ,negative predictive value and positive predictive value of combined detection of these 6 indexes were 92 .86% ,85 .00% ,88 .17% ,92 .64% and 85 .40% respectively ,except the specificity and positive predictive value were slightly decreased ,the levels of other indexes were higher than those of single index de-tection .The aresa under the receiver operating characteristic (ROC) curves of CEA ,CA125 ,CYFRA21-1 ,SCC ,NSE and ProGRP were 0 .775 ,0 .778 ,0 .891 ,0 .585 ,0 .710 and 0 .620 respectively .The area under ROC curves of the combined detection was 0 .950 .The positive rates of CA125 ,CYFRA21-1 ,NSE and the combined detection in the patients with stage Ⅲ ,Ⅳof lung cancer were obviously higher those in the patients with stageⅠand Ⅱof lung cancer ,the difference was statistically significant (P<0 .05);the combined detection obviously improved the positive rates for the diagnosis in the patients with different stages of lung cancer .Conclusion The combined detection of CEA ,CA125 ,CYFRA21-1 ,SCC ,NSE and ProGRP is conducive to improve the di-agnosis performance and early detection rate for lung cancer ,differentially diagnosing different pathological types of lung cancer and judge the clinical stage of lung cancer .The combined detection of 6 tumor biomarkers is an ideal diagnosis index of lung cancer .
10.Electrophysiological Properties of Frequency Domain in Rats' Skeletal Muscle

Objective To study the electrophysiological properties of rat skeletal muscle within the frequency from 100Hz to 100MHz.Methods The AC impedance of rat sural muscle tissues was measured using the Agilent 4294A impedance analyzer, then the electrophysiological properties of the normal rat sural muscle cells were established in the respect of cell dielectric spectroscopy, Cole-Cole plots, spectrum of loss factor and loss tangent.Results (1) The dielectric response of rat sural muscle cells was frequency-dependent, such as increase in frequency caused decrease in permittivity. At the same time the conductivity trended to arise; (2) The dielectric parameters(?_L,?_h,?_L,?_h,△?″_ max,△tg?_ max,f_ C1,f_ C2) cell fiber parallel to the applied electric field differed from that cell fiber perpendicular to the applied electric field; (3) The effect of alternating current electric field on rat sural muscle cells showed a dielectric dispersion phenomenon, representing two characteristic frequency: the 1 st frequency(f_ C1(∥)=f_ C1(⊥)=1.02 kHz) and 2 nd frequency(f_ C2(∥)=59.31 kHz,f_ C2(⊥)=380.04kHz). Conclusion The method for studying the electrophysiological properties of rat skeletal muscle cells was established in the respect of the frequency domain,providing a possibility for investigating muscle fatigue in the future.
